Can patients with mild non-neoplastic lesions diagnosed at baseline screening be safely exempt from surveillance: evidence from multicenter community-based cohorts

Surveillance recommendations for gastric cancer（GC） in current guidelines focused on advanced precancerous lesions and were based on precise diagnosis of severity/extent of baseline lesions. We aimed to develop a less endoscopy-related equipment-dependent risk-stratification tool, and assessed whether mild-precursor-lesion patients can be safely exempt from surveillance. In the multicenter communitybased cohort, 75,051 participants receiving baseline endoscopy were enrolled during 2015–2017 and followed-up until 2021. Cumulative incidence rates（CIRs） of GC for precancerous-conditions were calculated by Kaplan-Meier method and compared by Log-rank tests. Mixedeffects Cox regression models were used to detect potential factors for progression towards GC. A risk score was calculated as counts of selected factors. An independent cohort, including 26,586 participants was used for external validation. During a median follow-up of 6.25 years, CIRs of GC were 0.302%, 0.436%, and 4.756% for normal group, non-neoplastic（atrophic gastritis/intestinal metaplasia） and neoplastic lesions（low-grade/high-grade dysplasia）, respectively(Ptrendnon-neoplastic lesions showed higher GC risks（adjusted HR=7.73, 95%CI: 4.29–13.92）, and their four-year CIR reached the one-year CIR of neoplastic lesions. Further categorizing non-neoplastic lesions by histological grade, both patients with moderate-to-severe lesions（aHR=3.07, 95%CI: 1.67–5.64） and high-risk patients with mild lesions（aHR=7.29, 95%CI: 3.58–14.86） showed higher risks. Consistent trends were observed in validation cohort. High-risk mild-precursor-lesion patients should receive surveillance within 3–5 years after baseline screening. Our study provides evidence on supplementing current guideline recommendations.

Willingness to pay for colorectal cancer screening in Guangzhou

AIM To measure the willingness to pay for colorectal cancer screening in Guangzhou, and to identify those factors associated with it. METHODS A face-to-face questionnaire survey for pre-screening population from free and non-free colonoscopy districts was used to collect information on demographic characteristics, health behaviours, the intention of the cancer screenings and willingness to pay for colorectal cancer screening. A total of 1243 participants who took part in the pre-screening for colorectal cancer in Guangzhou were collected in the study. Categorical data were compared using the χ~2 test to analyse significant differences. non-conditional logistic regression and multi-class logistic regression were also performed for multivariate analysis and to estimate the odds *** The percentage of participants willing to pay for colorectal cancer screening was 91.7%. "Unnecessary" was the dominant reason that participants gave for their unwillingness, accounting for 63.1%. Of those who were willing to pay, 29.2%, 20.7%, 14.8%, 13.0% and 22.4% of participants were willing to pay less than $100, $100-$199, $200-299, $300-$399 and more than $400, respectively. non-logistic regression analysis showed that respondents who were male, had a high level of education, were from the family with more children/older to raise, and accepted colorectal cancer screening were willing to pay for this screening. Multi-class logistic regression analysis showed that respondents with higher annual household income per capita, from government and private enterprises, government agency/institution and peasants, and less family medical expenditure were willing to pay *** Willingness to pay for colorectal cancer screening in Guangzhou is high, but the amount of willing to pay is not much.

Introduction:This study aims to analyze the potential impact of the meteorological environment and air pollutants on road traffic ***:Road traffic fatality data in Shandong Province from 2012 to 2021 were obtained from the Population Death Information Registration Management *** and air pollutant data for the same period were collected from the *** Oceanic and Atmospheric Administration and the Ecological Environment Monitoring Center of Shandong Province,***’s correlation and ridge regression were used to analyze the impact of the meteorological environment and air pollutants on road traffic ***:From 2012 to 2021,there were 163,863 road traffic fatality *** results of the ridge regression analysis showed that the daily average temperature was negatively correlated with total fatalities and passengers and positively correlated with pedestrians,nonmotorized drivers,and motorized *** daily minimum temperature was negatively correlated with total fatalities and positively correlated with motorized *** daily maximum temperature was positively correlated with both pedestrian and nonmotorized *** daily accumulated precipitation was negatively correlated with *** duration was positively correlated with both nonmotorized and motorized *** particulate matter(PM_(10))and nitrogen dioxide(NO_(2))were positively correlated with total fatalities,pedestrians,and nonmotorized *** dioxide(SO_(2))was positively correlated with total fatalities but negatively correlated with nonmotorized drivers,passengers,and motorized ***:Atmospheric factors associated with the occurrence of road traffic fatalities include air temperature,daily accumulated precipitation,sunshine duration,and air pollutants such as PM_(10),NO_(2),and SO_(2).

Cell adhesion plays pivotal roles in the morphogenesis of multicellular *** cells form several types of cell-to-cell adhesion,including zonula occludens(tight junctions),zonula adhaerens(adherens junctions),and macula adhaerens(desmosomes).Although these adhesion complexes are basically observed only in epithelial cells,cadherins,which are the major cell adhesion molecules of adherens junctions,are expressed in both epithelial and non-epithelial tissues,including neural tissues(Kawauchi,2012).The cadherin superfamily consists of more than 100 members,but classic cadherins.

Effectiveness and safety of Shenmai injection in treatment of shock:a Meta-analysis

OBJECTIVE:To evaluate the effectiveness and safety of Shenmai injection for ***: Randomized controlled trials(RCTs)that evaluated the therapeutic effect of Shenmai injection on shock(including septic shock, cardiogenic shock, hypovolemic shock, neurogenic shock and anaphylactic shock) were included in this analysis. The major electronic databases were searched until May 2015. The methodological quality of the trials was assessed according to the Cochrane Handbook. Review Manager 5.3 and Stata 12.0 software were applied for data ***: Thirty RCTs involving 2038 participants were included. The methodological quality of the trials was generally passable. The combined use of Shenmai injection and conventional medicine was significantly more effective at managing shock compared to conventional medicine alone in the outcomes of total effective rate [risk ratio(RR 1.25,95% confidence interval(CI) 1.18 to 1.31] and mortality rate [risk difference(RD)-0.10, 95% CI-0.17 to-0.02]. Likewise, improvements were observed in other metrics. Three trials reported adverse events, but no trial reported serious adverse ***: Our results indicated the potential effectiveness of Shenmai injection combined with conventional medicine treatment for shock. However, further rigorously designed trials are needed to collect and weigh up all the evidence for the use of Shenmai injection.
