Objective: This study aims to analyze clinical, paraclinical and therapeutic aspects of sinusitis complications in the Otorhinolaryngology department at the teaching hospital of Gabriel Toure in Bamako. Materials and methods: This descriptive and longitudinal study was conducted on 12 months from March 2012 to February 2013 in 9 cases of sinusitis complications in the department of Otorhinolaryngology at the university Hospital Gabriel Toure in Bamako. Results: The average age of patients was 22.7 years;a median of 19 years within 6 men and 3 women with almost 12.9 days of care seeking duration. The factors of risky were the use of nonsteroid anti-inflammatory drugs NSAID (4 cases), maxillofacial trauma (1 case) and HIV (1 case). Facial pain was the major symptom, such as frontal oedema, rhinorrhea with pus shedding and nasal obstruction. Computer tomography was used for complications specification. Frontal sinusitis was the most encountered with subperiosteal abscess (44.4%). Streptococcus pneumoniae was identified in 3 cases. medical treatment associated to surgery was conducted on 7 patients. Two cases of death have been registered. Conclusion: The complicated sinusitis is an emergency and dangerous disease requiring specific care by a multidisciplinary staff. Much of complications are commonly encountered despite the antibiotic era.

The Management of Spontaneous Cerebrospinal Fluid Rhinorrhea in Patients at Risk for Increased Intracranial Tension

Introduction: The exact pathophysiology of spontaneous CSF rhinorrhea is not always understood in some patients. Idiopathic intracranial hypertension (IIH) is now recognized as one of the causes of spontaneous CSF leak in the neurosurgical and ent literature. Our aim was to set a management protocol for such cases according to the same setting intracranial tension (ICT). Methods: We prospectively managed patients with spontaneous CSF rhinorrhea who were admitted to our hospital between 1st of January 2014 and 31st of December 2017 with a prespecified treatment algorithm. Patients with a history of previous cranial or nasal surgery, trauma, skull base congenital malformations were excluded from the study. The patient’s demographics, clinical data, comorbidities, body mass index (BMI), first time or recurrent leakage and duration of the leak were collected. Results: 41 patients, 35 females and 6 males, presented with spontaneous CSF rhinorrhea with a mean BMI of 38 ± 4.16 Kg/m2. The mean pre-operative ICT manometry was 17.2 ± 5.9 cmH2O (range, 10 - 26 cmH2O). 43.9% of the patients were found to have an increased ICT (≥20 cmH2O) and underwent a permanent CSF diversion at the same setting of the endoscopic repair. None of the patients had a recurrence during the follow-up period. Conclusion: The prespecified treatment algorithm with measuring the ICT at the same setting of the endoscopic repair has a better result for control of spontaneous CSF rhinorrhea. This has led to no recurrence and decreased hospital stay.

Assessment of Haemostasis in Anaesthesia for Surgery at the Sylvanus Olympio university Hospital Center in Lomé

Title: Assessment of haemostasis in anaesthesia for surgery at the Sylvanus Olympio university Hospital Center in Lomé. Objectives: Evaluate the prescription of the preoperative haemostasis assessment. Methodology: This was a prospective descriptive and observational study which had taken place in the central operating room and in the operating room of the ent department at UHC SO of Lomé from January 1 to June 31, 2016. It had concerned all patients who had anaesthesia for scheduled surgery after pre-anesthetic consultation and the haemostasis assessment carried out according to the anaesthesia technique and the type of surgery. Results: Two hundred and sixty (260) patients underwent anaesthesia during the study period. The male sex predominated (60%), the age group 18 - 40 years predominated (50.4%). GA was more practiced (62.7%) followed by spinal anaesthesia (30.3%). Minor ent surgery was more performed (28%). ASA1 patients predominated (48.5%). The pre-anesthetic haemostasis assessment including platelet count, prothrombin rate, activated partial thromboplastin time and bleeding time was almost always done. The platelet count was achieved in all patients followed by the activated partial thromboplastin time (94%). No haemorrhagic complication related to a haemostasis disorder was observed in the perioperative period in anaesthesia than in surgery. Conclusion: The prescription of the pre-anesthetic haemostasis assessment should not be systematic. It must take into account the clinical history, the patient’s bleeding history during the anaesthesia consultation, the type of anaesthesia, the surgery planned and the age.

各种内源性、外源性和系统性因素均可影响脑血流的神经血管内单位,但又互为导致血管闭塞的独立因素。无论何种机制引起的脑缺血,其结局均相同,包括血管因素、星形胶质细胞、神经元和诱发神经炎症疾病谱的周围小胶质细胞。而神经炎症反应则为缺血级联反应中的重要环节,可引起神经细胞去极化和炎性细胞因子前体激活,继而细胞死亡。神经炎症反应可引起中枢神经系统急性能量衰竭、兴奋性毒性反应、电解质紊乱、离子通道功能障碍和氧自由基分泌增多等一系列反应,而又可以是其后果。若在阈值期内缺血状态不能得到改善,则可激活基质金属蛋白酶,导致脑组织广泛损害,而即早基因激活将致使神经体液微内环境和血脑脊液屏障破坏。脱氧和复氧均可造成卒中相关性损伤。缺血性脑损伤级联反应的病理过程还包括小胶质细胞和星形胶质细胞活化,释放细胞因子、氧自由基、神经毒性因子和神经营养因子等化学介质,进一步加重脑损伤。与此同时,中性粒细胞活化并通过细胞内黏附因子结合到内皮细胞表面,然后移行至缺血核心区,加剧脑组织损伤。单核细胞和巨噬细胞通过释放炎性细胞因子和转化为吞噬细胞而在缺血性脑损伤过程中发挥作用。针对神经炎症不同环节的治疗原则,是停止或减轻脑损伤程度,如抑制细胞内黏附分子(ICAMs)、血管细胞黏附分子(VCAMs)、中性粒细胞、小胶质细胞、主要组织相容性复合物(MHC)、细胞因子、趋化因子和自由基活性。上述针对脑缺血后神经炎症的各种治疗方法,动物实验已有可喜的结果,但临床试验未见成效,目前正在做进一步研究。然而,了解神经炎症的病理学机制,对今后开发新型神经保护治疗方法的开拓脑缺血治疗领域至关重要。本文旨在阐述缺血性卒中相关神经炎症反应的不同病理学机制。

BACKGROUND: Epigenetic silencing of the MGMT (O6-methylguanine-DNA methyltra nsferase) DNA-repair gene by promoter methylation compromises DNA repair and ha s been associated with longer survival in patients with glioblastoma who receive alkylating agents. METHODS: We tested the relationship between MGMT silencing i n the tumor and the survival of patients who were enrolled in a randomized trial comparing radiotherapy alone with radiotherapy combined with concomitant and ad juvant treatment with temozolomide. The methylation status of the MGMT promoter was determined by methylation-specific polymerase-chain-reaction analysis. RE SULTS: The MGMT promoter was methylated in 45 percent of 206 assessable cases. I rrespective of treatment, MGMT promoter methylation was an independent favorable prognostic factor (P ent confidence interval, 0.32 to 0.61). Among patients whose tumor contained a m ethylated MGMT promoter, a survival benefit was observed in patients treated wit h temozolomide and radiotherapy; their median survival was 21.7 months (95 perce nt confidence interval, 17.4 to 30.4), as compared with 15.3 months (95 percent confidence interval, 13.0 to 20.9) among those who were assigned to only radioth erapy (P=0.007 by the log-rank test). In the absence of methylation of the MGMT promoter, there was a smaller and statistically insignificant difference in sur vival between the treatment groups. CONCLUSIONS: Patients with glioblastoma cont aining a methylated MGMT promoter benefited from temozolomide, whereas those who did not have a methylated MGMT promoter did not have such a benefit.

Treatment of tiny intracranial aneurysms with guidewire manipulation

Background:Tiny false intracranial rupture aneurysms are particularly rare. It is challenged both to neurosurgery and endovascular ***:We present here five rare cases of perforator tiny aneurysms that were diagnosed based on DSA. These patients were found due to SAH, and they decided to treat these aneurysms with endovascular therapy. After numerous attempts, the coiling microcatheter failed to access the aneurysms. As a result, the aneurysms finally were treated with guidewire ***:Mean follow-up time was about 10.4 months. The outcomes of the 5 cases were all surprisingly excel ent. The patients were followed up with angiography and telephone till now. No revascularization of aneurysm was ***:This paper demonstrated for the first time to our knowledge that tiny false intracranial aneurysm may be treated with guidewire manipulation. While larger studies with long-term follow-up are required to validate these promising results, guidewire manipulation is a new approach worth considering when microcatheter cannot enter aneurysm.

Objective:To evaluate the efficacy of a novel coated stent in the treatment of intracranial ***:MEDLINE,EMBASE,and PubMed databases were searched for literature published between 1990 and April 2022 according to PRISMA *** studies with≥10 patients reporting successful implantation of Willis covered stent,therapeutic effect,complications,and postoperative follow-up were *** combined incidence and corresponding 95%confidence intervals were assessed using a generalized linear mixed method and random effects ***:Five studies(116 patients with pseudoaneurysms)were *** experimental groups in the selected studies showed a combined technical success rate of 81.03%(OR=18.31,95%CI=9.39-35.69,I^(2)=79%,Pent is feasible,safe,and effective in the treatment of intracranial pseudoaneurysm.
