Hypoxia is encountered under many physiological and .pathophysiological situations. Although bothintermittent hypoxia （IH） and continuous hypoxia （CH） result in decreases in arterial 02, there are striking differences in the response of the physiological systems to CH and IH. Obstructive sleep apnea-hypopnea syndrome （OSAHS）, characterized by frequently recurrent intermittent hypoxia/reoxygenation （IH/ROX） as its obvious pathological trait, is clearly associated with an increased risk of cardiovascular disorders, particularly hypertension,1 and IH/ROX may be the prima pathological basis for OSAHS complicated hypertension.

Ultrasonography is first line, cost-effective modality in imaging of peripheral nerves. High frequency probes allow high resolution imaging of peripheral nerves located at relatively superficial location. USG can diagnose and localize various pathologies that involve the peripheral nerves such as traumatic, inflammatory, infective, neoplastic, and traumatic neuropathies. USG exceptionally has an important role to play in evaluation of patients with suspected nerve injury, as MRI may not differentiate neural contusion from nerve disruption. Neurapraxic injury is seen as swollen nerve with hypoechoic appearance.

Objective： As a vascular risk factor, carotid atherosclerosis is crucial to cognitive impairment. While carotid intima-media thickness, carotid artery plaque, and carotid stenosis can reflect carotid atherosclerosis in different stages, this review aimed to explore researches on the role of carotid intima-media thickness, carotid artery plaque, and carotid stenosis in the progress of cognitive impairment in nonstroke patients and tried to illustrate the possible mechanisms. Data Sources： We searched the PubMed database for recently published research articles up to July 2017, with the key words of ＂carotid atherosclerosis,＂＂carotid intima-media thickness,＂＂carotid plaque,＂＂carotid stenosis,＂＂nonstroke,＂ and ＂cognitive impairment.＂Study Selection： Articles were obtained and reviewed to analyze the role of carotid atherosclerosis such as carotid intima-thickness, carotid plaque, and carotid stenosis in the progress of cognitive impairment in nonstroke patients and the possible mechanisms. Results： In recent years, most studies proved that by evaluating carotid atherosclerosis with ultrasonography, carotid atherosclerosis accounts for the development of cognitive decline in nonstroke patients. Carotid atherosclerosis not only impairs the subtle general cognitive function but also decreases the specific domains of cognitive function, such as memory, motor function, visual perception, attention, and executive function. But, it is still controversial. The possible mechanisms of cognitive impairment in nonstroke patients with carotid atherosclerosis can be classified as systemic global cerebrovascular function, small-vessel diseases, and the mixed lesions. Conclusions： Carotid atherosclerosis can be used to predict the risk of cognitive impairment. Furthermore, diagnosing and treating carotid atherosclerosis at early stage might help clinicians prevent and treat vascular cognitive impairment in nonstroke patients.

Objective: To investigate whether different treatment methods have an impact on the quality of life and life span after fracture of patients with proximal femoral pathological fractures caused by advanced metastasis of highly malignant tumors. Methods: Karnofsky performance status (KPS) and visual analogue score (VAS) were counted at the time of admission and 2 months after the treatments. Survival analysis was implemented to compare the median survival time and 6-month survival rate of the 2 groups. Musculoskeletal score (MSTS) was used to evaluate limb function in the surgical group at 2 months after the treatment. Results: There was no significant difference in KPS score and VAS score between the two groups at the time of admission (p > 0.05). At 2 months after treatment, the KPS score of the surgical group was higher than that of the conservative group (P Conclusion: Surgical intervention can benefit patients with pathological fractures of the proximal femur due to metastasis of highly malignant tumors in terms of quality of life and survival. Local tumor resection and endoparasitic replacement, which can be tolerated by most patients, can effectively reconstruct the limb function of these patients and restore their self-care ability.

Objective Based on the observation that coagulation necrosis occurs in the majority of neonatal necrotizing enterocolitis （NEC） patients, it is clear that intestinal ischemia is a contributing factor to the pathogenesis of NEC. However, the published studies regarding the role of intestinal ischemia in NEC are controversial. The aim of this paper is to review the current studies regarding intestinal microcirculatory dysfunction and NEC, and try to elucidate the exact role of intestinal microcirculatory dysfunction in NEC. Data sources The studies cited in this review were mainly obtained from articles listed in Medline and PubMed. The search terms used were ＂intestinal microcirculatory dysfunction＂ and ＂neonatal necrotizing enterocolitis＂. Study selection Mainly original milestone articles and critical reviews written by major pioneer investigators in the field were selected. Results Immature regulatory control of mesentery circulation makes the neonatal intestinal microvasculature vulnerable. When neonates are subjected to stress, endothelial cell dysfunction occurs and results in vasoconstriction of arterioles, inflammatory cell infiltration and activation in venules, and endothelial barrier disruption in capillaries. The compromised vasculature increases circulation resistance and therefore decreases intestinal perfusion, and may eventually progress to intestinal necrosis. Conclusion Intestinal ischemia plays an important role through the whole course of NEC. New therapeutic agents targeting intestinal ischemia, like HB-EGF, are promising therapeutic agents for the treatment of NEC.

Objective: To derive the Chinese medicine(CM) syndrome classification and subgroup syndrome characteristics of ischemic stroke patients. Methods: By extracting the CM clinical electronic medical records(EMRs) of 7,170 hospitalized patients with ischemic stroke from 2016 to 2018 at Weifang Hospital of Traditional Chinese Medicine, Shandong Province, China, a patient similarity network(PSN) was constructed based on the symptomatic phenotype of the patients. Thereafter the efficient community detection method BGLL was used to identify subgroups of patients. Finally, subgroups with a large number of cases were selected to analyze the specific manifestations of clinical symptoms and CM syndromes in each subgroup. Results: Seven main subgroups of patients with specific symptom characteristics were identified, including M3, M2, M1, M5, M0, M29and M4. M3 and M0 subgroups had prominent posterior circulatory symptoms, while M3 was associated with autonomic disorders, and M4 manifested as anxiety;M2 and M4 had motor and motor coordination disorders;M1 had sensory disorders;M5 had more obvious lung infections;M29 had a disorder of consciousness. The specificity of CM syndromes of each subgroup was as follows. M3, M2, M1, M0, M29 and M4 all had the same syndrome as wind phlegm pattern;M3 and M0 both showed hyperactivity of Gan(Liver) yang pattern;M2 and M29 had similar syndromes, which corresponded to intertwined phlegm and blood stasis pattern and phlegm-stasis obstructing meridians pattern, respectively. The manifestations of CM syndromes often appeared in a combination of 2 or more syndrome elements. The most common combination of these 7 subgroups was wind-phlegm. The 7 subgroups of CM syndrome elements were specifically manifested as pathogenic wind, pathogenic phlegm,and deficiency pathogens. We sought to identify the differences between adolescents left behind in their home villages/towns （LBA） and non-left behind adolescents （NLB） on subjective well-being and family functioning due to parental migration in south China. We used a stratified cluster sampling method to recruit middle school students in a city experiencing population-emigration in Jiangxi Province in 2010. Participants included adolescents from families with：（1） one migrant parent, （2） both parents who migrated, or （3） non-left behind adolescents （i.e., no migrant parent）. To determine predictors of subjective well-being, we used structural equation models. Adolescents left behind by both parents （LBB） were less likely to express life satisfaction （P = 0.038） in terms of their environments （P = 0.011） compared with NLB. A parent or parents who migrated predicts lower subjective well-being of adolescents （P = 0.051） and also lower academic performance. Being apart from their parents may affect family functioning negatively from an adolescent＇s viewpoint. Given the hundreds of millions of persons in China, many who are parents, migrating for work, there may be mental health challenges in some of the adolescents left behind.

BACKGROUND Eosinophilia is an increase of more than 0.5×109/L in the number of eosinophils;it is a systemic condition with an unknown etiology and is often accompanied by multiple impaired organ *** clinical manifestations of the disease are highly variable and diverse,rendering identification of the diagnosis challenging;hence,diagnosis and treatment are often *** few reports of this disease exist globally,especially with rare manifestations of cerebral venous sinus thrombosis and *** SUMMARY A 32-year-old woman with eosinophilia presented to the hospital with bilateral lower-limb edema as the first clinical manifestation,followed by an extensive maculopapular rash throughout the *** subsequently developed cerebral venous sinus thrombosis along with bilateral lower-limb deep vein *** weeks earlier,she had received a single course of antibiotics from a local hospital for a low-grade fever and sore *** various treatments were administered for anticoagulation,maintaining blood circulation,and relieving blood stasis,the lower extremity edema improved significantly;however,the patient’s eosinophil count gradually *** experienced cerebral venous sinus thrombosis,cerebral hemorrhage,and deep vein thrombosis of the lower limbs before being declared brain *** this case report,we have elaborated the diagnosis and management of deep vein thrombosis manifested as eosinophilia,thrombocytopenia,and elevated D-dimer *** Because proper diagnosis is challenging,clinical vigilance is required for patients with eosinophilia,as it can lead to thrombus formation.
