Alpha1-antitrypsin deficiency is an autosomal recessive disease characterized by reduced serum levels of alpha1-antitrypsin(AAT)due to mutations in the SERPINA1 gene causing early onset pulmonary emphysema and,occasionally,chronic liver *** report an incidental finding of a novel null AAT allele,Q0Milano,consisting of a 17 nucleotides deletion in exon 3 of SERPINA1 gene,in an Italian child with persistently increased liver enzymes,a mild decrease in circulating AAT levels and without any pulmonary disease.Q0Milano variant results in an unfunctional protein lacking of AAT active site,as the resultant protein is truncated near PiS locus involved in AAT protein stability.

AIM: To analyze the impact of steroid maintenance on the outcomes in kidney transplant recipients stratified by induction agent ***: Patients who underwent first-time deceased donor kidney transplantation between 2000 and 2008 after receiving induction therapy with rabbitantithymocyte globulin(r-ATG), alemtuzumab or an interleukin-2 receptor blocker(IL-2B) and discharged on a calcineurin inhibitor(CNI)/mycophenolate mofetil(MMF)-regimen along with or without steroids were identified from the Organ Procurement and Transplant Network/United Network of Organ Sharing *** each induction type, adjusted overall and deathcensored graft as well as patient survivals were compared between patients discharged on steroid vs no steroid. Among r-ATG induced patients, analysis was repeated after splitting the group into low and high immune risk ***: Among the 37217 patients included in the analysis, 17863 received r-ATG(steroid = 13001, nosteroid = 4862), 3028 alemtuzumab(steroid = 852, no-steroid = 2176) and 16326 IL-2B(steroid = 15008, no-steroid = 1318). Adjusted overall graft survival was inferior(HR = 1.16, 95%CI: 1.06-1.27, P = 0.002) with similar death-censored graft survival(HR = 0.99, 95%CI: 0.86-1.14, P = 0.86) for steroid vs no-steroid groups in r-ATG induced patients. Both adjusted overall and death-censored graft survivals for steroid vs nosteroid groups were similar in alemtuzumab(HR = 0.92, 95%CI: 0.73-1.15, P = 0.47 and HR = 0.87, 95%CI: 0.62-1.22, P = 0.43 respectively) and IL-2B(HR = 1.05, 95%CI: 0.91-1.21, P = 0.48 and HR = 0.94, 95%CI: 0.75-1.18, P = 0.60 respectively) induced groups. Adjusted patient survivals were inferior for steroid vs nosteroid groups in r-ATG induced(HR = 1.31, 95%CI: 1.15-1.49, P < 0.001) but similar in alemtuzumab(HR = 1.02, 95%CI: 0.75-1.38, P = 0.92) and IL-2B(HR = 1.17, 95%CI: 0.97-1.40, P = 0.10) induced patients. Objective: The aim of the study is to identify whether Atypical Lymphocyte (AL), liver transaminases, and Glutathione Reductase (GR) can be used as potential biomarkers in the assessment of severity and thrombocytopenia in dengue. Methods: A cross-sectional analytical study was carried out on diagnosed dengue patients admitted to Nawaloka Hospital, Sri Lanka. Blood samples were taken from patients (n = 50) on the day of admission, 3rd and 5th day from admission for analysis of GR, aspartate transaminase, alanine transaminase, platelets, white blood cells, and Atypical Lymphocytes (AL). Results: GR level of all three measured stages had a higher area under the curve (>88%), high sensitivity and specificity compared to liver transaminases. A significant regression model represents on admission GR and AL levels as predictive variables to platelet levels in day 03 from admission (Day 3 Platelet level = 127155.3 - 383 * GR - 0.431 * AL). Conclusion: Liver transaminases, GR, and AL% can be considered as a profile of predictive biomarkers in early diagnosis of severity of dengue infection. The degree of thrombocytopenia can be predicted using on admission GR and AL% level in acute dengue viral infection.

Objective The correlation between various formative conditions and the pore characterizationsof the anodic alumina membrane is investigated to seek the optimal conditions for the formation of anodic aluminamembrane. Methods High purity aluminum foils are used as the starting materials. The anodization is conduc-ted in three types of electrolytes, 3% sulfuric acid, 5% sulfuric acid and 2. 7% oxalic acid, respectively, with dif-ferent voltages at for 48h. The characterizations of the pore size, the effective porosity and the pore porosity areobserved and determined by scanning electron microscopy. The hydraulic conductances of the membranes are meas-ured to confirm that the pores are open and evaluate the permselectivity of the membranes. Results The experi-mental result shows that the ordered pore arrays are obtained for oxidation under our experimental conditions. Withthe increasing of the voltage, the pore size and pore porosity increased significantly (P <0.05) , while the effectiveporosity decreased significantly (P <0.05) with the same electrolyte. The pore size formed in 3% sulfuric acid or5% sulfuric acid is much smaller than in 2. 7% oxalic acid as an electrolyte. The hydraulic conductance of anodicalumina membrane that formed under our experimental condition is higher than those of the membranes are availablecurrently used in clinical. Conclusion The results suggest that the optimal conditions for the formation of anodicalumina membrane that used in hemodialysis are in 3% or 5% sulfuric acid with 12. 5V to 17. 5V at for 48h.

Background: Impaired fasting glucose (IFG) is a prediabetic condition and is a high-risk state for developing diabetes and associated complications. The aim of this study was to explore the association of vitamin D with insulin secretory function among the IFG subjects. Materials and Methods: This was a prospective cross-sectional study conducted in the department of Biochemistry and Cell Biology, Bangladesh University of Health Sciences (BUHS), Dhaka, Bangladesh;from June 2016 to May 2017, on forty-six (46) IFG subjects. Serum blood glucose was measured by glucose-oxidase method, Fasting serum lipid profile (Total cholesterol, TG and HDL-c);liver enzyme like alanine aminotransferase (ALT) was measured by enzymatic-colorimetric method;Serum creatinine was measured by colorimetric kinetic method;Serum Insulin and vitamin D [25(OH)D] were measured by ELISA method;insulin secretory capacity (HOMA%B) and insulin sensitivity (HOMA%S) were calculated by Homeostasis Model Assessment (HOMA) using HOMA-CIGMA software. Results: Among 46 IFG subjects, 22 (47.8%) were male and rest 24 (52.2%) were female and their mean age was 40 (±8) years. Mean fasting serum glucose level of the study subjects was 6.33 (±0.23) mmol/l and mean postprandial serum glucose level was 7.23 (±0.41) mmol/l. Mean serum vitamin D level of the study subjects was 26.54 (±8.83) ng/ml. Mean HOMA%S, HOMA%B and HOMA%IR of the total study subjects were 48.34 (±16.70), 102.16 (±23.13) and 2.28 (±0.70) respectively. Insulin secretory capacity (HOMA%B) was significantly higher (p = 0.047) in the subjects having higher vitamin D [25(OH)D level ≥ 20 ng/ml] compared to the subjects having lower vitamin D [25(OH)D levels Conclusion: High prevalence of hypovitaminosis D exists among IFG subjects. Vitamin D is associated with insulin secretory function in IFG subjects. Population based prospective studies using larger sample size should be done to confirm the results.
