Spectrum and risk factors for invasive candidiasis and non-Candida fungal infections after liver transplantation

Background Invasive fungal infections are an important cause of posttransplant mortality in solid-organ recipients. The current trend is that the incidence of invasive candidiasis decreases significantly and invasive aspergillosis occurs later in the liver posttransplant recipients. The understanding of epidemiology and its evolving trends in the particular locality is beneficial to prophylactic and empiric treatment for transplant recipients. Methods A retrospective analysis was made of recorded data on the epidemiology, risk factors, and mortality of invasive fungal infections in 352 liver transplant recipients. Results Forty-two （11.9%） patients suffered from invasive fungal infection. Candida species infections （53.3%） were the most common, followed by Aspergillus species （40.0%）. There were 21 patients with a superficial fungal infection. The median time to onset of first invasive fungal infection was 13 days, first invasive Candida infection 9 days, and first invasive Aspergillus infection 21 days. Fifteen deaths were related to invasive fungSI infection, 10 to Aspergillus infection, and 5 to Candida infection. Invasive Candida species infections were associated with encephalopathy （P=0.009） and postoperative bacterial infection （P=0.0003） as demonstrated by multivariate analysis. Three independent risk factors of invasive Aspergillus infection were posttransplant laparotomy （P=-0.004）, renal dysfunction （P=0.005） and hemodialysis （P=-0.001）. Conclusions The leading etiologic species of invasive fungal infections are Candida and Aspergillus, which frequently occur in the first posttransplant month. Encephalopathy and postoperative bacterial infection predispose to invasive Candida infection. Posttransplant laparotomy and poor perioperative clinical status contribute to invasive Aspergillus infection. More studies are needed to determine the effect of prophylactic antifungal therapy in high risk patients.

Objective:To investigate levels of autophagy in T cells and B cell of patients with systemic lupus erythematosus(SLE)and its clinical ***:68 SLE patients withought treatment within 4 weeks were enrolled in this *** accessed the levels of autophagy in T and B cells of 23 healthy controls and 68 patients before and after treatment by flow cytometry,and analyzed their correlations with serum levels of C3 and anti-ds DNA antibodies,SLEDAI score,et ***:Before treatment,a significantly increased levels of LC3-II was observed in SLE patients than healthy controls,the active group(SLEDAI score≥10)was significantly higher than the stable group(SLEDAI scorers=0.289,P<0.05),and the levels of C3 were positively correlated the levels of LC3-II in B lymphocyte(rs=0.371,P<0.01).After treatment for five days,levels of autophagy in T lymphocytes of SLE patients with good prognosis(SLEDAI score decreased≥4)significantly decreased(P<0.05);Also,three days after treatment,levels of autophagy in B lymphocytes of SLE patients with good prognosis were increased ***:Levels of autophagy in T and B lymphocytes of SLE patients were abnormal compared to healthy controls,and these changes were associated with disease ***,these changes are expected to be as indicators of disease activity and potential therapeutic targets in SLE.

BACKGROUND Angioimmunoblastic T-cell lymphoma(AITL), a unique subtype of peripheral Tcell lymphoma, has relatively poor outcomes. High-dose chemotherapy with autologous stem cell transplantation(ASCT) can achieve complete remission and improve outcomes. Unfortunately, subsequent T-cell lymphoma-triggered hemophagocytic lymphohistiocytosis(HLH) has a worse prognosis than B-cell lymphoma-triggered *** SUMMARY We here report a 50-year-old woman with AITL who achieved a favorable outcome after developing HLH 2 mo after receiving high-dose chemotherapy/ASCT. The patient was initially admitted to our hospital because of multiple enlarged lymph nodes. The final pathologic diagnosis, made on biopsy of a left axillary lymph node was AITL(Stage Ⅳ, Group A). Four cycles of the following chemotherapy regimen were administered: Cyclophosphamide 1.3 g, doxorubicin 86 mg, and vincristine 2 mg on day 1;prednisone 100 mg on days 1-5;and lenalidomide 25 mg on days 1-14. The interval between each cycle was 21 d. The patient received a conditioning regimen(busulfan, cyclophosphamide, and etoposide) followed by peripheral blood stem cell infusion. Unfortunately, she developed sustained fever and a low platelet count 17 d after ACST, leading to a diagnosis of HLH after ASCT. During treatment, she experienced thrombocytopenia and Pneumocystis carinii pneumonia. BACKGROUND Aberrant activation of phosphorylated form of glycogen synthase kinase-3β[pS9 GSK-3β(Serine 9 phosphorylation)] is known to trigger Wnt/β-catenin signal cascade but its clinicohistopathological implications in bladder carcinogenesis remain *** To investigate the diagnostic and prognostic relevance of expressions of pS9 GSK-3β, β-catenin and its target genes in the pathobiology of bladder *** Bladder tumor tissues from ninety patients were analyzed for quantitative expression and cellular localization of pS9 GSK-3β by immunohistochemical(IHC) staining. Real time-quantitative polymerase chain reaction and IHC were done to check the expression of β-catenin, Cyclin D1, Snail and Slug at transcriptome and protein level respectively. Clinicohistopathological variables were obtained from histology reports, follow up and OPD visits of *** of the markers were statistically correlated with these variables to determine their significance in clinical setting. Results were analysed using SPSS20.0 *** Aberrant(low or no membranous/high nuclear/high cytoplasmic) expression of pS9 GSK-3β was noted in 51% patients and found to be significantly associated with tumor stage and tumor grade(P = 0.01 and 0.04; Mann Whitney U test).Thirty one percent tumors exhibited aberrant co-expression of pS9 GSK-3β andβ–catenin proteins and showed strong statistical association with tumor stage,tumor type, smoking/tobacco chewing status(P = 0.01, 0.02 and 0.04, MannWhitney U test) and shorter overall survival probabilities of patients(P = 0.02;Kaplan Meier test). Childhood-onset systemic lupus erythematosus （cSLE） is a severe multisystem autoimmune disease. Renal involvement occurs in the majority of cSLE patients and is often fatal. Renal biopsy is an important investigation in the management of lupus nephritis. Treatment of renal lupus consists of an induction phase and main-tenance phase. Treatment of childhood lupus nephritis using steroids is associated with poor outcome and excess side-effects. The addition of cyclophosphamide to the treatment schedule has improved disease control. In view of treatment failure using these drugs and a tendency for non-adherence, many newer agents such as immune-modulators and monoclonal antibodies are being tried in patients with cSLE. Trials of these novel agents in the pediatric population are still lacking making a consensus in the management protocol of pediatric lupus nephritis diffcult.

创面愈合是需要多种细胞及细胞因子参与的复杂过程。在创面愈合过程中,任何一个环节出现异常,都可能导致创面发展成为慢性难愈合创面。研究证实多种干细胞可促进创面愈合,但部分干细胞因存在提取困难、易凋亡、伦理及法律等问题,临床应用受限。而口腔黏膜干细胞可规避上述问题,这类干细胞具有胚胎干细胞特性、免疫调节能力,而且同质性强,在口腔创面无瘢痕愈合过程中发挥重要作用,成为促进创面愈合及减少瘢痕形成的研究热点。该文综述口腔黏膜干细胞促进创面愈合的机制研究、临床应用前景及目前存在的问题。

Objective:To analyze the risk factors of hyperuricemia occurrence in Ig A nephropathy patients with renal function compensatory phase in western part of Guangdong ***:The clinical and pathological data of 328 patients with Ig A nephropathy were selected to investigate the risk factors of ***:The incident of hyperuricemia in Ig A nephropathy was 38.4%.Univariate regression analysis suggested that the risk factors of hyperuricemia in patients with Ig A nephropathy were related to age,serum creatinine,blood urea nitrogen,serum triglyceride,serum total cholesterol,glomerulus sclerosis score,glomerular injury integral,tubular-interstitial injury score,renal small vessel injury score and Lee’s *** regression analysis showed that the independent risk factors for hyperuricemia in patients with Ig A nephropathy were serum triglyceride and renal small vessel injury ***:Among the multiple risk factors of hyperuricemia in Ig A nephropathy with renal function compensatory phase in western part of Guangdong province,the independent risk factors were serum triglyceride and renal small vessel injury *** obtaining therapy strategies with regional characteristics,also provide a reference for other Ig A nephropathy patients with renal function compensatory phase.
