Neo-adjuvant radiotherapy in rectal cancer

Impact of comorbidity in elderly prostate cancer patients treated with brachytherapy

Objective： To analyze the correlations among comorbidity and overall survival （OS）, biochemical progression-free survival （b-PFS） and toxicity in elderly patients with localized prostate cancer treated with 125I brachytherapy. Methods： Elderly men, aged ≥65 years, with low-intermediate risk prostate cancer, were treated with permanent 125I brachytherapy as monotherapy. Comorbidity data were obtained from medical reports using age-adjusted Charlson comorbidity index （a-CCI）. The patients were categorized into two age groups （〈75 and 〉75 years old）, and two comorbidity score groups （a-CCI ≤3 and 〉3）. Toxicity was scored with radiation Therapy oncology Group （RTOG） scale. Results： From June 2003 to October 2009, a total of 92 elderly patients underwent prostate brachytherapy, including 57 men （62%） with low-risk prostate cancer, and 35 men （38%） with intermediate-risk prostate cancer. The median age of patients was 75 years （range, 65-87 years）. Forty-seven patients （51%） bad a-CCI _〈3 and 45 patients （49%） a-CCI 〉3. With a median follow-up period of 56 months （range, 24-103 months）, the 5-year actuarial OS and b-PFS were 91.3% and 92.4% respectively, without statistical significance between two Charlson score groups. Toxicity was mild. None of the patients experienced gastrointestinal （GI） toxicity, and only 4 patiens （4%） experienced late genitourinary （GU） grade-3 （G3） toxicity. No correlation between acute GU and GI toxicity and eomorbidity was showed （P=0.50 and P=0.70, respectively）. Conclusions： Our data suggest that elderly men with low-intermediate risk prostate cancer and comorbidity can be considered for a radical treatment as 12Sl low-dose rate brachytherapy.

Significance of a novel sucrose permeability test using serum in the diagnosis of early gastric cancer

AIM： To investigate the usefulness of sucrose permeability test using serum in the diagnosis of gastric diseases, with special reference to early gastric cancer （EGC）. METHODS： A total of 63 subjects, including 11 patients with gastric ulcer, 20 patients with gastric cancer （13, early; 7, advanced） and 32 healthy controls, were studied. Blood and urine samples were collected repeatedly for 5 h before and after the sucrose loading. Sucrose levels were measured by a newly developed RESULTS： Serum sucrose levels started to increase 15 min after loading, and peaked at 60 min in the gastric disease groups. The levels for gastric ulcer, EGC and advanced gastric cancer （AGC） at 60 min were significantly higher than that in the healthy controls （26.9±2.4, 34.4±5.0, and 71.8±15.6 vs 7.9±0.7 mol/L, respectively, P〈0.01）. The cut-off level set at 15.4 mol/L （60 min） offered the best distinction between EGC patients and healthy controls; and the sensitivity and specificity were 92.3% and 93.8%, respectively, while those of the urine method were 76.9% and 93.8%, respectively. CONCLUSIONS： The gastric permeability test using serum is reliable for the detection of EGC, and this test can provide results much earlier than the conventional urine method. This test may offer a useful alternative to more invasive tests for EGC.

Management of extramedullary plasmacytoma: Role of radiotherapy and prognostic factor analysis in 55 patients

Objective： To investigate potential prognostic factors affecting patient outcomes and to evaluate the optimal methods and effects of radiotherapy（RT） in the management of extramedullary plasmacytoma（EMP）.Methods： Data from 55 patients with EMP between November 1999 and August 2015 were collected. The median age was 51（range, 22–77） years. The median tumor size was 3.5（range, 1.0–15.0） cm. The median applied dose was 50.0（range, 30.0–70.0） Gy. Thirty-nine patients（70.9%） presented with disease in the head or neck region. Twelve patients received RT alone, 9 received surgery（S） alone, 3 received chemotherapy（CT） alone, and 3 patients did not receive any treatment. Combination therapies were applied in 28 ***：The median follow-up duration was 56 *** 5-year local recurrence-free survival（LRFS）,multiple myeloma-free survival（MMFS）,progression-free survival（PFS）and overall survival（OS）rates were 79.8%,78.6%,65.2% and 76.0%,*** analysis revealed that RT was a favourable factor for all examined ***,head and neck EMPs were associated with superior LRFS,MMFS and *** size〈4 cm was associated with superior MMFS,PFS and OS;serum M protein negativity was associated with superior MMFS and PFS;age≥50 years and local recurrence were associated with poor *** dose≥45 Gy group exhibited superior 5-year LRFS,MMFS and PFS rates（94.7%,94.4%,90.0%,respectively）,while the corresponding values for the dose〈45 Gy group were 62.5%（P=0.008）,53.3%（P=0.036）and 41.7%（P〈0.001）.Conclusions：Involved-site RT of at least 45 Gy should be considered for ***,patients with head and neck EMP,tumor size〈4 cm,age〈50 years and serum M protein negativity had better outcomes.

Targets and molecular mechanisms of triptolide in cancer therapy

Triptolide（TPL/TL） is a natural drug with novel anticancer effects. Preclinical studies indicated that TPL inhibits cell proliferation, induces cell apoptosis, inhibits tumor metastasis and enhances the effect of other therapeutic methods in various cancer cell lines. Multiple molecules and signaling pathways, such as caspases, heat-shock proteins, NF-κB, and deoxyribonucleic acid（DNA） repair-associated factors, are associated with the anti-cancer effect. TPL also improves chemoradiosensitivity in cancer therapy. Phase I trials indicate the potential clinical value of TPL use. However, further trials with larger sample sizes are needed to confirm these results.

Prostate cancer affects over 200000 men annually in the United States *** role of conventionally fractionated external beam radiation therapy (RT) is well established as a treatment option for eligible prostate cancer patients; however,the use of stereotactic body radiotherapy (SBRT) in this setting is less well *** the past decade,there have been a number of studies investigating the feasibility of SBRT as a potential treatment option for prostate cancer *** has been well studied in other disease sites,and the shortened treatment course would allow for greater convenience for *** may also be implications for toxicity as well as disease *** this review we present a number of prospective and retrospective trials of SBRT in the treatment of prostate *** focus on factors such as biochemical progression-free survival,prostate specific antigen (PSA) response,and toxicity in order to compare SBRT to established treatment *** also discuss future steps that the clinical community can take to further explore this new treatment *** conclude that initial studies examining the use of SBRT in the treatment of prostate cancer have demonstrated impressive rates of biochemical recurrencefree survival and PSA response,while maintaining a relatively favorable acute toxicity profile,though long-term follow-up is needed.

INHIBITION AND radiation SENSITIZING EFFECT OF INDOLEACETIC ACID COMBINED WITH HORSERADISH PEROXIDASE ON HELA CELLS

Objective To observe the inhibition and radiation-sensitizing effect of Indoleacetic acid (IAA) combined with horseradish peroxidase(HRP)on Hela cells. Methods Hela cells were cultured in vitro and classified into control group, drug group incubated with different doses of IAA(30, 60, 90μmol·L -1) plus 1.2μg·mL -1 HRP, radiation group (6MV-X, 4Gy ) and group of radiation plus IAA plus HRP(same dose as above). All the above were treated for 24-96 *** growth inhibition, radiation-sensitizing effect were observed with methyl thiazolyl tetrazolium (MTT) photocolorimetric assay and trypan blue dye assay. The-effect on cell proliferation cycle was determined by flow cytometry. Results The antiproliferation activities showed a significant time-effect and dose-effect relationship to some extent after Hela cells were treated with IAA combined with HRP. The group of radiation plus 60μmol·L -1 IAA plus 1.2μg·mL -1 HRP and radiation plus 90μmol·L -1 IAA plus 1.2μg·mL -1 HRP showed an obvious radiation sensitizing effect. After treatment with 90μmol·L -1 IAA plus 1.2 μg·mL -1 HRP for 72 hours, the determination of cell cycle showed that the percentages of the cells on stages G 2-M and S were all higher than those of the control group. For the group of radiation plus IAA combined with HRP, the percentages of the cells on stages G 2-M were higher than those of the radiation group. Conclusion The above findings suggest that IAA combined with HRP has an inhibitive and killing effect on Hela cells. The effect was stronger during the cell cycles of G 2-M and S. It also has a radiation sensitizing effect. Its mechanism might be that Hela cells were blocked on stages G 2-M, and it presents a collaborative killing effect during miototic time.
