Surgical outcome after complete repair of tetralogy of Fallot with absent pulmonary valve: comparison between bovine jugular vein-valved conduit and monocusp-valve patch

Background The prognosis of tetralogy of Fallot with absent pulmonary valve (TOF/APV) without operation is poor. We evaluated the surgical outcome of TOF/APV in a single center. Methods Twenty-two TOF/APV patients underwent complete surgical correction in our hospital. Right ventricular outflow tract reconstruction was performed using bovine jugular vein (BJV)-valved conduit implantation (n=10), homograft-valved conduit implantation (n=2), or monocusp-valve patch (n=10). Health-related quality of life (QOL) was evaluated during follow-up. Results The overall survival at 5 and 10 years was 86.4±7.3% (confidence interval 69.4–97.2%). The survival rates were significantly different between patients with and without bronchial stenosis (40 and 100%, P=0.0003, log-rank test). The survival of patients aged>6 months was higher than those≤6 months (100 vs. 40%, P=0.0003, log-rank test). Patients with BJV-valved conduits had higher systolic gradients from the right ventricle to the pulmonary artery (RV–PA) compared to those with monocusp-valve patches. BJV-valved conduit implantation was a risk factor for post-operative pulmonary-valve stenosis. The QOL score for patients with BJV-valved conduits was lower than those with monocusp-valve patches (P<0.05). No reoperation was performed during follow-up. Conclusions Bronchial stenosis and lower age (≤6 months) were the main factors influencing post-operative survival. The use of a BJV-valved conduit was a main reason for RV–PA restenosis;thus, the use of a BJV-valved conduit may increase the need for repeat intervention and decrease the post-operative quality of life.

To the Editor： A 47-year-old male of chest discomfort for 2 weeks patient was admitted because He had no obvious incentive precordial discomtfort 2 weeks ago, accompanied by palpitation. He immediately went to the town hospital. Electrocardiogram （ECG） showed myocardial infarction. Coronary angiography showed coronary artery disease accompanied by a ventricular aneurysm. The patient was transferred to Beijing Anzhen Hospital. ECG showed sinus rhythm, heart axis deviation ＋ 111 °, anterior septal, anterior lateral, anterior myocardial infarction, and complete right bundle branch block. Chest X-ray showed no obvious abnormalities in heart and lung. Ecbocardiography showed abnormal motion of segmental ventricular wall; formation of a ventricular aneurysm in apex area of the heart, the diameter of the ventricular aneurysm was 2 cm, the diastolic function of left ventricular was reduced. Coronary angiography showed left anterior descending artery filling slowly and its intima was not smooth; the stenosis rate was 90%. The stenosis rate of the circumflex artery was 90% [Figure la and b]. He was diagnosed with acute anterior myocardial infarction, left ventricular aneurysm, and hypertension.

BACKGROUND Haemoglobin drop is common in acute coronary syndrome(ACS)patients and correlates with poor ***,the association between mild haemoglobin drop and adverse clinical outcome remains insufficiently *** study aimed to examine the association between in-hospital haemoglobin drop and risk for adverse clinical outcomes in ACS patients,especially those with mild *** Included patients from the BleeMACS(Bleeding complications in a Multicenter registry of patients discharged after an Acute Coronary Syndrome)registry were categorized into three groups by the presence and amount of in-hospital haemoglobin drop(non-drop,mild drop and severe drop).The cut-off point between mild drop and severe drop is≥3 g/*** Cox regression was used to assess the association between haemoglobin drop and major adverse cardiac endpoints(MACE).Patients taking potent P2Y_(12) inhibitors were selected for the additional *** score matching was used to avoid selective bias in the additional *** Of 6911 patients,4949 patients(71.6%)experienced in-hospital haemoglobin *** with non-drop group,patients with haemoglobin drop had higher risk of MACE[adjusted hazard ratio(HR)=1.36,95%CI:1.03–1.80 for mild drop group;adjusted HR=1.70,95%CI:1.07–2.68 for severe drop group].Patients in mild drop group were less likely to receive potent P2Y_(12) inhibitors at discharge(mild drop group *** drop group ***-drop group:10.9%vs.10.7%vs.23.8%).After propensity score matching adjustment among patients with potent P2Y_(12) inhibitors,patients in mild drop group were not associated with an increased risk of MACE than those in non-drop group(adjusted HR=1.52,95%CI:0.49–4.72).CONCLUSIONS In-hospital haemoglobin drop was common in ACS patients and associated with a higher risk for adverse *** prescription for potent P2Y_(12) inhibitors may be responsible for poor prognoses among patients with mild haemoglobin dro

Relationship between Two blood Stasis Syndromes and Inflammatory Factors in Patients with Acute Coronary Syndrome

Objective： To investigate the relationship between inflammatory factors and two Chinese medicine（CM） syndrome types of qi stagnation and blood stasis（QSBS） and qi deficiency and blood stasis（QDBS） in patients with acute coronary syndrome（ACS）. Methods： Sixty subjects with ACS, whose pathogenesis changes belongs to qi disturbance blood stasis syndrome, were divided into 2 groups： 30 in the QSBS group and 30 in the QDBS group. The comparative analysis on them was carried out through comparing general information, coronary angiography and inflammatory factors including intracellular adhesion molecule-1（ICAM-1）, chitinase-3-like protein 1（YKL-40） and lipoprotein-associated phospholipase A2（Lp-PLA2）. Results： Compared with the QSBS group, Lp-PLA2 and YKL-40 levels in the QDBS group showed no-significant difference（P〉0.05）; ICAM-1 was significantly higher in the QDBS group than in the QSBS group in the pathological processes of qi disturbance and blood stasis syndrome of ACS（P〈0.05）. Conclusion： Inflammatory factor ICAM-1 may be an objective basis for syndrome typing of QSBS and QDBS, which provides a research direction for standardization research of CM syndrome types.

BACKGROUND Aortic dissection during pregnancy is a rare but life-threatening event for mothers and fetuses. It often occurs in the third trimester of pregnancy and the postpartum period. Most patients have connective tissue diseases such as Marfan syndrome. Thus, the successful repair of a sporadic aortic dissection with maternal and fetal survival in the early second trimester is extremely rare. CASE SUMMARY A 28-year-old woman without Marfan syndrome presented with chest pain at the 16th gestational week. Aortic computed tomographic angiography confirmed an acute type A aortic dissection (TAAD) with aortic arch and descending aorta involvement. Preoperative fetal ultrasound confirmed that the fetus was stable in the uterus. The patient underwent total arch replacement with a frozen elephant trunk using moderate hypothermic circulatory arrest with the fetus in situ. The patient recovered uneventfully and continued to be pregnant after discharge. At the 38^th gestational week, she delivered a healthy female infant by cesarean section. After 2.5 years of follow-up, the patient is uneventful and the child’s development is normal. CONCLUSION A fetus in the second trimester may have a high possibility of survival and healthy growth after aortic arch surgery.
