Association between COX-2-1195G>A polymorphism and gastrointestinal cancer risk: A meta-analysis

AIM To perform a meta-analysis to investigate the association between cyclooxygenase-2(COX-2)-1195G>A gene polymorphism and gastrointestinal cancers. METHODS Publications related to the COX-2-1195G>A gene polymorphism and gastrointestinal cancers published before July 2016 were retrieved from Pub Med, EMBASE, Web of Science, China Biological Medicine Database, China National Knowledge Infrastructure, and CQVIP Database. Meta-analysis was performed using Stata11.0 software. The strength of the association was evaluated by calculating the combined odds ratios(ORs) and the corresponding 95%CIs. The retrieved publications were excluded or included one by one for sensitivity analysis. In addition, the funnel plot, Begg's rank correlation test, and Egger's linear regression method were applied to analyse whether the included publications had publication bias. RESULTS A total of 24 publications related to the COX-2-1195G>A gene polymorphism were included, including 28 studies involving 11043 cases and 18008 controls. The meta-analysis results showed that the COX-2-1195G>A gene polymorphism significantly correlated with an increased risk of gastrointestinal cancers, particularly gastric cancer(A vs G: OR = 1.35; AA/AG vs GG: OR = 1.54; AA vs GG/AG: OR = 1.43; AA vs GG: OR = 1.80; AG vs GG: OR = 1.35). Compared to the Caucasian population in America and Europe, the COX-2-1195G>A gene polymorphism in the Asian population(A vs G: OR = 1.30; AA/AG vs GG: OR= 1.50; AA vs GG/AG: OR = 1.35; AA vs GG: OR = 1.71; AG vs GG: OR = 1.37) significantly increased gastrointestinal cancer risk. The sensitivity analysis(P A gene polymorphism might be a potential risk factor for gastrointestinal cancers. Further validation by a large homogeneous study is warranted.

Novel method for esophagojejunal anastomosis after laparoscopic total gastrectomy:Semi-end-to-end anastomosis

AIM: To test a new safe and simple technique for circular-stapled esophagojejunostomy in laparoscopic total gastrectomy(LATG). METHODS: We selected 26 patients with gastric cancer who underwent LATG and Roux-en-Y gastrointestinal reconstruction with semi-end-to-end esophagojejunal anastomosis. RESULTS: LATG with semi-end-to-end esophagojejunal anastomosis was successfully performed in all 26 patients. The average operation time was 257 ± 36 min, with an average anastomosis time of 51 ± 17 min and an average intraoperative blood loss of 88 ± 46 mL. The average postoperative hospital stay was 8 ± 3 d. There were no complications and no mortality in this series.CONCLUSION: The application of semi-end-to-end esophagojejunal anastomosis after LATG is a safe and feasible procedure, which can be easily performed and has a short operation time in terms of anastomosis.

Synchronous Adenocarcinomas of the Jejunum and Sigmoid Colon—A Rare Case

Primary adenocarcinomas of the small bowel are rare. Because of their rarity, non-specific symptoms and diagnostic difficulty, small bowel tumors are often diagnosed and treated late in their course. The diagnostic difficulty is increased when these tumors arise in association with primary synchronous tumors of the colon. We present a case of a 17-year-old female who was evaluated for anemia and bleeding per rectum. Colonoscopy revealed multiple colonic polyps. Intraoperatively, she was surprisingly found to be having a synchronous jejunal growth apart from multiple colonic polyoid lesions. Total colectomy was performed along with resection of jejunal growth. Histopathology of colonic specimen revealed multiple polypoid projections with one polyp showing adenocarcinoma and rest severe dysplasia. Histopathology of jejunal growth revealed poorly-differentiated adenocarcinoma. Very rarely, cases have been reported in literature, where the primary adenocarcinoma of the jejunum developed synchronously with colonic adenocarcinoma, and both treated surgically on the same occasion.

Efficacy of anlotinib combined with radioiodine to treat scalp metastasis of papillary thyroid cancer:A case report and review of literature

BACKGROUND Papillary thyroid cancer(PTC)is one of the well-differentiated thyroid tumors.Cutaneous metastasis from differentiated thyroid cancers occurs in<1%of primary thyroid carcinomas but produces the worst survival prognosis.The multi-targeting tyrosine kinase inhibitor anlotinib has been approved to treat refractory advanced non-small-cell lung cancer as well as advanced soft-tissue and clear cell sarcomas in China.CASE SUMMARY In a patient with scalp metastasis caused by PTC,thyroid and skull metastasis tumor sizes were significantly reduced after a trial of neoadjuvant anlotinib therapy for 3 cycles.Anlotinib maintenance medication after thyroidectomy further reduced the metastatic skull tumor size thereby preventing the requirement for craniotomy.CONCLUSION The outcome of the present trial confirmed the potential of anlotinib therapy to treat scalp metastasis induced by PTC and point the way for the treatment of similar diseases in the future.

Expression of Twist Gene in Primary Liver Cancer

In order to investigate the possibility of overexpression of Twist in primary liver cancer （PLC）, the Twist expression was detected by using immunohistochemical analysis and RT-PCR assay in 45 patients with PLC. Control tissues were obtained from 9 patients with liver hemangioma. It was found that in 36 （80.0%） out of 45 PLC patients, cancerous regions showed positive cytoplasm and nucleus staining for Twist with a diffuse pattern. In noncancerous adjacent areas and control liver tissues, the expression of Twist was 57.8% and 22.2% respectively. The results of RT-PCR assay revealed that the expression of Twist was stronger in the cancerous tissues than that in the noncancerous adjacent tissues. It was suggested that the expression of Twist was up-regulated in PLC, which play an important role in the progression of PLC.

Background:The purpose of this study was to determine the learning curve for robot-assisted gastrectomy using the Cumulative Summation(CUSUM) technique.Methods:Two series of 50 and 55 consecutive robotic gastrectomy were performed between March 2010 and January 2014.Patient demographics,surgical performance,and short-term clinical outcomes were examined from a retrospective database for analysis.Data of operation time were abstracted for the learning curve analysis using the CUSUM method.Results:Similar processes occurred in the two surgeons from our institution.Surgeon A’s learning curve of robotic gastrectomy was best modeled as a third-order polynomial,with equation CUSUMOT in minutes equal to 0.0495 case number- 4.217 case number + 91.206 case number-100.11(R = 0.8731).Surgeon B’s learning curve was also best modeled as a third-order polynomial,with equation CUSUMOT in minutes equal to 0.0314 case number- 2.4106 case number + 33.682 case number + 315.28(R = 0.8816).They both included three unique phases:an initial phase for the first 12-14 cases of operation,a well-developed phase for the following17-20 cases,and a mastery phase after the accumulation of additional experience.Conclusion:The CUSUM method is a useful tool for objective evaluation of practical skills for surgeons during the learning phase of robotic surgery training.The robotic gastrectomy is found to have a short learning curve for experienced laparoscopic surgeons and the popularity of this new technology won’t reduce because of its difficulty to learn.

背景保留十二指肠的胰头切除术是替代胰十二指肠切除术治疗良性肿瘤、囊性肿瘤及神经内分泌瘤的可行之选。方法检索Pubmed、Embase及Cochrane数据库中保留十二指肠胰头切除术的研究结果。根据手术指征、围手术期并发症及微创应用等纳入1994年1月至2021年3月的38项队列研究共702例患者的临床资料。结果共702例患者分别因良性肿瘤、癌前病变、囊性肿瘤、胰腺神经内分泌肿瘤、十二指肠乳头周围腺瘤等行保留十二指肠的胰头切除术。702例患者中,214例(30.5%)行保留十二指肠的部分胰头切除术,488例(69.5%)行保留十二指肠的全胰头切除术,后者中微创手术占17.3%。两种术式的术后早期严重并发症发生率(术后B及C级胰瘘、胆瘘、腹腔出血及腹腔脓肿)均2 cm的有或无功能的良性内分泌肿瘤外科治疗的首选术式。

目的:探讨达芬奇机器人远端胃癌根治术的安全性、可行性,评价其临床疗效。方法:对2010年3月至2013年7月远端胃癌行机器人辅助胃癌根治术113例患者临床及术后随访资料及同期在我科行腹腔镜远端胃癌根治术279例患者进行回顾性对照分析研究,比较两组患者手术指标,术后并发症,术前及术后第1,3,7天外周血中性粒细胞淋巴细胞比值（粒淋比）及术后生存率等临床疗效指标。结果:机器人胃癌手术在淋巴结清扫数量,术中出血量等方面优于腹腔镜手术组（P＜0.05）,术后并发症发生率、粒淋比（NLR）等与腹腔镜组比较差异无统计学意义,随访结果机器人手术组术后1、2、3年生存率分别为91.7%、77.4%和72.9%,腹腔镜手术组术后1、2、3年生存率分别为91.2%、76.2%和70.4%,两组比较差异无统计学意义。结论:机器人远端胃癌根治术安全、有效,且具有创伤小、术中出血少、淋巴结清扫更为彻底、术后恢复快等优点,值得进一步推广应用。

Background:The current indications for using a robotic technique in gastric cancer remain unclear,though robotic surgery has been reported as a feasible alternative to the conventional laparoscopic or open surgery for treating early gastric cancer.The aim of this study was to assess the safety and feasibility of robot-assisted gastrectomy for gastric cancer.Methods:A retrospective database of patients who underwent robotic gastrectomy for gastric cancer between March 2010 and March 2014 was examined.231 patients underwent gastrectomy for gastric cancer were found,among which 218 patients underwent robotic radical gastrectomy for gastric cancer were chosen for further analyses.Patient demographics,surgical performance and short-term clinical outcomes were examined.Results:158 distal,54 total,and 6 proximal robotic radical gastrectomies were completed.There were eight cases converted to open surgery,one for massive hemorrhages and seven for serious adhesion due to the neoplasm invasion to adjacent tissues.Standard lymph node dissection and digestive tract reconstruction were successfully carried out.Mean operative time was 238.0+30.4min and blood loss was 105.1 + 61.3 ml.Mean number of nodes retrieved was 34.3 + 10.7 and all resection margins were negative.The time of first flatus and days to eating liquid diet,and length of postoperative hospital stay were 2.6 ± 1.2,3.0+1.0,6.1 +1.8 respectively.Conclusions:Robot-assisted gatrectomy is a feasible and safe operative procedure with clear operation field,thorough lymph nodes dissection and quick recovery.More studies with long follow up time are needed to assess the indications and oncological effectiveness of this new technology in the treatment of gastric cancer.

Effects of simulated carbon dioxide and helium peumoperitoneum on proliferation and apoptosis of gastric cancer cells

AIM:To investigate the effects of carbon dioxide (CO2) and helium insufflation administered at different pressures on the growth and apoptosis of cultured human gastric cancer cells. METHODS:The gastric cancer cells MKN-45 were exposed to a CO2 and helium environment maintained at different pressures (0, 5, 10 and 15 mmHg). The cells were exposed to simulated pneumoperitoneum environment for 4 h, and pH of the culture media was measured after it was moved to normal conditions for 0, 2, 4, 6 and 8 h. Proliferation viability of MKN-45 was examined by 3-4,5Dimethylthiazol-2-yl,5-diphenyltetrazolium bromide or triazolyl blue (MTT) assay after it was moved to normal conditions. Apoptotic ratio was measured by Annexin V-FITC/PI double labelled staining. RESULTS:The pH of media was acid and recovered to normal after 4 h in the CO2 group while it was basic in the helium group. There was no difference between CO2 groups (under 10 mmHg ) and control group (P > 0.05) in the proliferative viability of the cells. The cultured cells exposed to 15 mmHg CO2 environment grew more slowly than control group from 4 to 7 d (P 0.05). The proliferative viability in helium group was not obviously different from the control group (P > 0.05). The apoptotic ratio of the cultured cells was markedly higher than that of the control group (P 0.05). CONCLUSION:There is no obvious effect in the proliferation and apoptosis of MKN-45 cells under 10 mmHg CO2 insufflation pressure and helium in any pressure. Fifteen mmHg CO2 insufflation pressure can inhibit the proliferation of the cells and improve apoptosis.
