The prevalence,associated factors,and impact of urinary incontinence in pregnant and postpartum women in Nanjing,China:A cross-sectional study

Objective:Urinary incontinence(UI)is highly prevalent in antenatal and postnatal women while the prevalence of UI varied largely from 3.84%to 38.65%.This study was to assess the prevalence of UI,the associated factors,and the impact of UI on daily life in pregnant and postpartum women in Nanjing,China.Methods:The prevalence of UI and the impact of UI on life were assessed by the validated Chinese version of International Consultation on Incontinence Questionnaire-urinary incontinence-short form and the validated Chinese version of urinary incontinence quality of life.The associated factors were estimated by using logistic regression analysis.Results:UI affected 37.80%of pregnant women and 16.41%of postpartum women of the study population.Among the pregnant participants,the prevalence rates of stress UI,urge UI,and mixed UI were 25.77%,4.47%,and 7.10%,respectively.Among the postpartum women,the prevalence rates of stress UI,urge UI,and mixed UI were 11.15%,1.92%,and 2.69%,respectively.In both pregnant women and postpartum women,vaginal delivery had significantly increased the odds of reporting UI(p=0.007,p=0.003,respectively).The impact of UI on daily life was significantly greater in postpartum women compared to pregnant women especially in social embarrassment(p=0.000).Conclusion:The prevalence rates of UI were high in pregnant women in Nanjing,China.Vaginal delivery significantly increased odds of reporting UI.UI has a great impact on pregnant and postpartum women’s life,especially in social embarrassment.

Lavender aromatherapy on anxiety and depression in patients with Acute Coronary Syndrome: a single-blind randomized clinical trial

Objective:In cardiovascular disease,a patient’s anxiety and depression can increase cardiac rehabilitation duration and recovery.Lavender aromatherapy as a non-pharmacological intervention effective in other contexts may be an efficient intervention to alleviating anxiety and depression in patients with Acute Coronary Syndrome(ACS).Methods:In this study,110 ACS patients were randomly assigned to two intervention and control groups.Inhalation of the lavender fragrance was prescribed for the intervention group and the drop of aromatic almond for the control group for 3 days.Results:The first-day anxiety and depression were significantly different in the two groups at 1 h and 9 h after the intervention.The’morning’s difference before the intervention was not significant,but it was substantial 1 h after the intervention.On the third morning of the intervention,this difference was confirmed.Conclusions:This study confirmed the effectiveness of lavender aromatherapy in reducing anxiety and depression in ACS patients.This’study’s results enable intensive care nurses to use aromatherapy with lavender oil as a non-pharmacological and cost-effective intervention to reduce their psychological tensions and increase patient satisfaction during hospitalization in the cardiac care units(CCU).

Evidence-based practice profiles of nurses: a transcultural study

Objective: To compare(i) the evidence-based practice(EBP) profiles and(ii) socio-demographic characteristics of nurses in 4 selected low-middle income countries(LMICs).Methods: A cross-sectional survey was conducted among 677 nurses from 4 hospitals in 4 countries(Afghanistan, Ghana, Iran, and Uganda).Results: The findings revealed that there are significant differences between the 3 EBP subscales(that is, practice/use, attitude, and knowledge/skill) across the 4 countries(P < 0.05). No significant EBP differences were identified across age groups and gender in the subscales of practice, knowledge, and attitude. Practice and knowledge subscales were significantly associated with working schedules(that is, par t-time and full-time)(P < 0.05). Analyses also revealed significant differences between attitude and knowledge subscales and working experience(P < 0.05).Conclusions: Nurses in the 4 countries have moderate practice, attitude, and knowledge/skil s of EBP. Nurses in LMICs require continuous professional development programs and suppor t to enhance their practice and knowledge regarding EBP in clinical settings.

目的鉴于院外护理服务的重要性,家庭护理的出现,以及护理实践中伦理框架的需求,本研究旨在探讨护士对家庭护理中伦理问题的看法和体验。方法采用面对面个人访谈收集资料。2020年,通过目的性抽样方法选取在伊朗4个城市的家庭护理中心中工作的20名护士进行访谈,了解他们在家庭护理过程中关于伦理问题的看法和体验。基于内容分析法,应用Granehei和Lundman方法对资料进行分析。结果本研究共提取编码416个。基于相似性对编码进行合并,提取出7个主题和16个子主题。主题包括对职业身份的感知、尊重患者的自主权、尊重隐私、建立人际交往、维护相互安全、遵守公平性原则和文化-宗教能力。子主题包括责任、专业发展及专业间互动、在家庭护理中保持专业性、提供整体性和艺术性的护理、尊重患者隐私、尊重护士隐私、信息保密、尊重服务对象的选择、诚实告知、富有同情心的互动、平衡权力地位、维护患者安全、维护护士安全、公平性的建立、尊重家庭宗教信仰,以及注重文化敏感性。结论研究参与者表示由于在家庭护理中涉及患者隐私,与在医院环境中的护理实践相比,家庭护理中对于护士职业身份的认知、隐私、家庭互动的管理、相互安全和文化-宗教能力等伦理问题显得尤为重要。

Psychometric properties of the Persian version of the European heart failure self-care behavior scale

Objective:To investigate the psychometric properties of the self-care behavior scale in heart failure(HF)patients.The development of self-care behaviors is one of the goals of non-pharmacological care management programs.Methods:After translating the European Heart Failure Self-care Behavior scale(EHFScBS),validity was assessed through explanatory factor analysis;additionally,reliability was assessed through Cronbach alpha and test-retest.The samples comprised of 120 HF patients admitted to hospitals in Tehran during 2017-2018.Results:The mean age of the samples was 67.3±14.04 years.Fifty-four patients(45%)were female.All items of the scale had an acceptable factor loading.Cronbach alpha coefficient was 0.72.The intra-cluster correlation coefficient(ICC)was 0.69.Conclusions:Nurses and clinicians could benefit from the Persian version of HF self-care scale since it would enable them to rapidly assess patients’self-care behaviors and to evaluate the effectiveness of the HF management.

A causal model of job stress among Thai nurse-midwives

Objective: To test a causal model of job stress among nurse-midwives working in labor and delivery units in Thailand.Methods: Random and convenience sampling was used to recruit 282 nurse-midwives with at least 6 months of work experience from 16 regional ter tiary hospitals in Thailand. Data were collected from May to December 2020. Research instruments with good internal consistency reliability ranged from 0.83 to 0.91 including the Job Stress Scale and the Thai version of the Job Content Questionnaire(TJCQ). Descriptive statistics and a structural equation model were used for data analysis.Results: Job demands were the strongest predictor of job stress. At the theoretical level, high job control plays a crucial role in directly reducing job stress. However, the present research provides contrary evidence to the theoretical predictions. When nurse-midwives perceive high job control, they perceive pressure to meet the expectations of their supervisors and colleagues. Therefore, high job control can contribute to job stress. Likewise, job suppor t had an indirect effect on job stress among nurse-midwives through job control. The modified model fitted the empirical data(χ^(2) = 57.76, df = 22, CMIN/df = 2.62, goodness of fit(GFI) =0.96, adjusted goodness of fit(AGFI) =0.91, comparative fit index(CFI) = 0.95, and root mean square error of approximation(RMSEA) = 0.07). The effects of job demands, job control, and job suppor t on job stress among Thai nurse-midwives can explain 67% of the model’s total variance for job stress.Conclusions: Nurse-midwives who encounter high job demands and less control over their work control suffer from job stress. Job support does not directly affect nurse-midwives’ job stress but influences it through perceived job control. Strategies to decrease job stress among Thai nurse-midwives should focus on how to balance job demands, and enhance job control, and job suppor t.

Stature and Body Weight Estimations from Footprint Dimensions among Asante and Fante Females in Ghana

Background:Footprints recovered from crime scenes can assist in establishing the identity(stature,body weight,and sex)of a person.Due to variations(genetic and environmental factors)in the morphology of the foot,several authors have derived populationspecific regression equations for stature and weight estimations.Aims and Objectives:Tribal differences in the footprint dimensions of Ghanaians have not been established.Hence,this study was done to determine if they were tribal differences in footprint dimensions.Materials and Methods:The footprints of Ghanaian females,aged 19-35 years,and belonging to two tribal groups(72 Asante and 73 Fante)were recovered using an ink pad method.Height,body weight,and footprint measurements were obtained following standard procedures.Results:Although Fante females had longer foot lengths and shorter foot breadths than Asante females,the difference was significant(P<0.001)only in the left big toe-pad length(eta squared=0.496).Height correlated better with footprint dimensions of Fante(r=0.246-0.809)than Asante(r=0.214-0.660)females.Body weight correlated weakly with footprint dimensions.Unlike in Asante females,foot length measurements did not correlate significantly with body weight among Fante females.Conclusion:The use of mixed population in deriving stature,weight and sex equations is cautioned especially when there is increased genetic diversity.The findings of this study have important applications in forensic anthropometric investigations.

Effect of metoclopramide administration to mothers on neonatal bilirubin and maternal prolactin: a randomized, controlled, clinical trial

Background Jaundice is a common neonatal problem.This study was conducted to determine the effect of metoclopramide on neonatal bilirubin and maternal prolactin (primary outcomes) and milk volume (secondary outcome).Methods This triple-blind,randomized,controlled,clinical trial was conducted on 112 mothers.The participants were assigned to the intervention (metoclopramide) and control groups (placebo) using block randomization.Ten-mg metoclo-pramide and placebo tablets were taken by the participants three times a day.The intervention began in the first 2-10 hours after childbirth and continued until the fifth day.The mothers' prolactin level was measured on the first morning after the intervention and on the sixth day (1 day after the intervention was over).Neonatal total bilirubin was also measured before the intervention and on the sixth day.Results After the intervention,the two groups did not differ significantly in terms of the mean neonatal indirect bilirubin (P =0.565) and milk volume (P =0.261),but the mean serum prolactin was significantly higher in the metoclopramide group compared to the placebo group (adjusted mean difference 37;95% confidence interval 58.1-16.5;P =0.001).Conclusions Metoclopramide increased maternal serum prolactin but had no effects on neonatal jaundice.The insufficient numbers of studies on this subject mandate further research.

Exploring the type of social support available to aged male hypertensive clients in Ghana

Objective: To explores the social support available to aged hypertensive male clients in a municipality in Ghana.Methods: The inclusion criteria were as follows: age ≥60 years and are known hypertensive patients. Data were collected from 186 selected aged male hypertensive patients for 3 months. The questionnaire was self-developed and open-ended. The data were processed and analyzed using SPSS version 23. Some of the variables were subjected to statistical tests and ranks in the order of impor tance to respondents. The enrolled respondents were allowed to par ticipate in the study after their informed consent was obtained. It was evident that respondents do not obtain much support or help in taking care of their condition.Results: The result revealed that the repondents has no support to help take care of their condition as evident by the responses in the study. The scores on average mean for some variables are(AM = 2.25, SD = 0.381) more than the test value of 2.50. Some of these supports were on feeding support(M = 3.97, SD = 0.278, n = 186), health support(M = 2.87, SD = 0.167, n = 186), and cleaning support(M = 2.59, SD = 0.868, n = 186). Supports such as clothing, socialization, medication, washing, transportation, and financial support were lacking.Conclusions: Based on the finding that certain forms of support were lacking, the study concludes that steps undertaken by the government, community, religious bodies, and family toward improving these supports can be of immense help for aged male hypertensive patients living in the Ejura-Sekyedumase municipality.

Comparison of anterior segment parameters of the eye between type 2 diabetic with and without diabetic retinopathy and non-diabetic

AIM:To compare anterior segment parameters between two groups of type 2 diabetic with and without diabetic retinopathy(DR)and non-diabetic elderly subjects based on hemoglobin A1c(Hb A1c)levels and status of DR.METHODS:This study was conducted on 997 residents aged 60y or over in Tehran,Iran.Diabetic group had Hb A1c level≥6.4%with no other systemic problems.The non-diabetic participants had normal eye findings and no systemic diseases.K1,K2,mean K,Q-value,anterior,central,posterior,and total corneal densitometric findings,anterior chamber volume(ACV),anterior chamber depth(ACD),corneal volume(CV),and pachymetry were measured by Pentacam AXL.RESULTS:A total of 678 non-diabetic(39%male)and 319 diabetic(35%male)subjects with mean age of 66.31±5.23 and 67.22±4.96y were examined,respectively.No statistically significant difference was found in anterior segment parameters between non-diabetic and diabetic groups(all P>0.05).However,middle,posterior,and total corneal densitometric values were statistically different between two groups af ter controlling the ef fects of confounders(P=0.014,0.007,and 0.042,respectively).Corneal densitometric values in all layers,ACD,and ACV were different between diabetic subjects with and without DR(all P<0.05).In the diabetic group,only corneal densitometric values had a negative relationship with fasting blood sugar(P<0.001).ACD and ACV had a negative correlation with Hb A1c levels(all P<0.05,r=-0.129 and-0.146,respectively).However,the relationships were not observed after controlling the confounders(P=0.938,0.466,respectively).CONCLUSION:Considering the higher densitometric values of the cornea and lower ACD and ACV in diabetic subjects with DR,it is suggested that the examiners should perform comprehensive retinal examinations when faced with such conditions.
