Factors Facilitating Adherence to Recommended Lifestyle-Modification amongst Hypertensive Clients at Malamulo Hospital, Thyolo, Malawi

Background: Adherence to recommended lifestyle modification regarding hypertension control remains a serious problem in Malawi, leading to poor blood pressure control. Objective: This study examined factors that facilitate adherence to recommended lifestyle modification among hypertensive clients’ at Malamulo hospital in Thyolo Malawi. Methods: A descriptive cross-sectional quantitative study was conducted with 108 clients who were recruited through a systematic sampling method. Data were collected through face-to-face interviews using a pre-tested structured questionnaire. Using statistical Packages for Social Sciences (SPSS) version 22, Chi-Square (x2) test was applied to test each predictor variable for statistical significance. The significant predictors were entered into the logistic regression model to determine the independent predictors of recommended lifestyle modifications amongst clients. Odds ratio (OR) at 95% Confidence Interval (CI) was used to declare the independent effect of each variable on the outcome variable. Results: 108 respondents were enrolled in the study, the majority of who were women (60.2%). Findings demonstrated that clients had adequate knowledge of lifestyle modification with an overall knowledge rate of 92% though the overall adherence rate was poor at 53.8%. Having family support (OR = 4.587, 95% CI: 1.625, 12.947, p = 0.004) and support groups (OR = 4.043, 95% CI: 1.496, 10.926, p = 0.006) are more likely to facilitate adherence to recommended lifestyle modification unlike situations of non-involvement of family and lack of support groups. Conclusion: Clients demonstrated adequate knowledge of recommended lifestyle modifications with poor adherence to them. The presence of support groups and a strong family support system are important factors for fostering adherence to the recommended lifestyle modifications regarding hypertension control.

Optimum care for patients with a terminal illness is dependent on understanding their values,beliefs,and preferences on end-of-life issues and providing service that meets their preferences.As the only way to explore patients’living wills,advance care planning(ACP)is an important means to honor patient autonomy and respect their human rights and dignity.ACP has been recognized as a key indicator for quality palliative care and endorsed into national health systems by many high-income countries;however,it is little known in China's Mainland.China is now facing a growing aging and terminally ill population with many unmet needs for end-of-life care;therefore,it is imperative to implement ACP in China.In this review,we clarify the relevant concept and suitable population for ACP,describe the current situation of implementation in high-income countries,and provide better suggestions for the future development of ACP in the mainland of China.

Nutrition Uptake among Pregnant Women: A Case of Solwezi District, North Western Province

Background: Zambia has a population of 17.4 million people of which 48% are unable to meet their minimum calorie requirement and 35% of the children in Zambia are stunted (ZDHS, 2018). In the 2018 ZDHS report, 31% of women aged 15 - 49 had anaemia. Most cases of anaemia in pregnancy are due to micronutrient deficiency and maternal malnutrition. The 2020 Zambia Global Nutrition Report shows an increase in the prevalence of anaemia among women of reproductive age (45%) and low birth weight infants (13.0%) which suggested a need to explore determinants of pregnant women’s nutrition uptake in order to improve the nutritional status of pregnant women particularly those in Solwezi district of the north-western province of Zambia. Methods: An analytical cross-sectional study was conducted on antenatal mothers aged 15 - 49 accessing routine antenatal care from four selected health facilities in Solwezi district from July 2021 to February 2021. Cluster sampling method was used to select the 4 health facilities and the 98 antenatal mothers were selected using systematic sampling method. Data was collected using a pretested researcher-assisted semi-structured questionnaire and analyzed using the SPSS version 26. Chi-square test was used to determine associations between the independent and independent variables. The level of significance was set at 0.05 and the confidence interval was set at 95%. Multiple logistic regression analysis was done to predict associations among variables. Results: The findings revealed that a large proportion of respondents (71.4 %) had high knowledge regarding nutrition during pregnancy compared to (19.4%) and (9.2%) who expressed medium and low knowledge levels respectively. Attitude towards nutrition in pregnancy was positive in the majority, 82 (83.7%) of the respondents, and over half, (55%) of the respondents reported good nutrition uptake during pregnancy. An Investigation on the Utilization of Intra Uterine Contraceptive Device among Women in Kabwe, Central Province

Introduction: Intrauterine contraceptive device (IUCD) is an effective long acting reversible contraceptive but its use is low. This study examined factors contributing to the utilization of intrauterine contraceptive devices (IUCDs) among women in Kabwe Central Province. Methodology: This was an analytical cross sectional study. Primary data was collected from 150 respondents in using simple random sampling method. Data was analysed using the statistical package of social sciences (SPSS) version 22. Chi-square test was used to test associations among the dependent and independent variables. Logistic regression analysis was conducted. The confidence interval was set at 95% and significant level was at 0.05%. Findings: The study found that 44% of the respondents had high level of knowledge about IUCD. About 28% of the respondents, said that they used IUCD because it maintains menstrual bleeding, 26% said that they used because IUCD it does not cause infections, and 19% said that they used IUCD because it does not migrate to other body parts. Majority (81%) of the respondents agreed with a statement that age contributes to underutilization of IUCD. About 77% of the respondents agreed with a statement that marital status contributes to underutilization of IUCD and 75% agreed with a statement that, education level contributes to underutilization of IUCD and was significant. Furthermore, 91% of the respondents agreed with a statement that lack of knowledge about IUCD contributes to underutilization of IUCD and 74% agreed with a statement that religion contributes to underutilization of IUCD. Age (p = 0.003), marital status (p = 0.002), education level (p = 0.003), and employment status (p = 0.02), were found to have a significant relationship with the utilization of IUCD. About 36% of the respondents said that knowledge or education affects the utilization of IUCD at a large extent. Predicting risk of postpartum hemorrhage using machine learning approach:A systematic review

Background Postpartum hemorrhage(PPH)could be avoided by identifying high-risk women.The objective of this systematic review is to determine PPH predictors using machine learning(ML)approaches.Method This strategy included searching for studies from inception through November 2022 through the database included:Cochrane Central Register,PubMed,MEDLINE,EMBASE,ProQuest,Scopus,WOS,IEEE Xplore,and the Google Scholar database.The search methodology employed the PICO framework(population,intervention,control,and outcomes).In this study,“P”represents PPH populations,“I”represents the ML approach as intervention,“C”represents the traditional statistical analysis approach as control,and“O”represents prediction and diagnosis outcomes.The quality assessment of each included study was performed using the PROBAST methodology.Results The initial search strategy resulted in 2048 citations,which were subsequently refined by removing duplicates and irrelevant studies.Ultimately,four studies were deemed eligible for inclusion in the review.Among these studies,three were classified as having a low risk of bias,while one was considered to have a low to moderate risk of bias.A total of 549 unique variables were identified as candidate predictors from the included studies.Nine distinct models were chosen as ML algorithms from the four studies.Each of the four studies employed different metrics,such as the area under the curve,false positive rate,false negative rate,and sensitivity,to report the accuracy of their models.The ML models exhibited varying accuracies,with the area under the curve(AUC)ranging from 0.706 to 0.979.Several weighted predictors were identified as significant factors in PPH risk prediction.These included pre-pregnancy maternal weight,maternal weight at the time of admission,fetal macrosomia,gestational age,level of hematocrit at the time of admission,shock index,frequency of contractions during labor,white blood cell count,pregnancy-induced hypertension,the weight of the newborn,duration of the second stage of labo

An umbrella review of Lianhua Qingwen combined with Western medicine for the treatment of coronavirus disease 2019

Objective:Lianhua Qingwen combined with Western medicine(LHQW+WM)has been proposed as a viable treatment for coronavirus disease 2019(COVID-19).Interestingly,umbrella reviews of systematic reviews(SRs),which provide the most comprehensive evidence,are the best evidence in evidence-based medicine.Therefore,an umbrella review of SRs that summarizes and evaluates the efficacy of LHQW+WM for COVID-19 is urgently required.Methods:Overall,6 databases were used to conduct a comprehensive literature search from inception to January 22,2022.The corrected covered area(CCA)was used to analyze the overlapping between SRs.Meta-analysis was conducted when that of the included SRs was inappropriate.A MeaSurement Tool to Assess Systematic Reviews(AMSTAR-2)was also employed to assess the quality of the included SRs.Results:In total,12 SRs were identified,which included 12 unique primary studies.The included SRs ranged in quality from moderate to critically low and had an extremely high CCA(36.4%).Compared to conventional treatment,LHQW+WM showed efficacy concerning fatigue recovery[risk ratio(RR)=1.69,95%confidence interval(CI):1.04-2.73,n=2,I2=0%],cough recovery(RR=1.65,95%CI:1.09-2.51,n=3,I2=39.1%),and overall effective rates(RR=1.17,95%CI:1.07-1.28,n=3,I2=17.5%).Conclusion:LHQW+WM may improve the clinical symptoms of patients with COVID-19;however,the results should be interpreted cautiously because of the rigorous processes in the included SRs.

Perception of Nursing and midwifery Trainees towards Clinical Environment at Cape Coast in the Central Region of Ghana

Purpose: The purpose of this study was to investigate the perception of students’ nurses in the nurses training colleges towards clinical environment. Methodology: A cross-sectional survey approach was used. Nursing and midwifery trainees at Cape Coast were the respondents. Simple random sampling was used to select 605 respondents for the study. Descriptive and inferential statistics were employed to analyze the data. Results: The majority of the respondents were female, Christians, singled and attended Cape Coast Nursing and midwifery Training College (CCNMTC). The staff were helpful, friendly and considerate. There were repetition of student’s tasks in every shift but new and different ways of teaching were used. The students had positive perception of clinical learning environment. There were statistically significant differences in perception towards clinical learning environment among students based on academic class (levels). Conclusion: It was recommended that the staff nurses should assign students with varied task depending on their level.

An Assessment of Predisposing Factors of Atherogenic Dyslipidemia in an Urban Pediatric Population in Cameroon

Background: Dyslipidemia in childhood contributes to an increased risk of cardiovascular diseases later in life. This study sets out to determine the prevalence of dyslipidemia and describe the associations between selected variables and dyslipidemia in Cameroon urban children. Methods: This cross-sectional hospital-based study included 415 children (188 boys and 227 girls) between the ages of 5 and 16 years. Dyslipidemia was defined as an abnormal value of one or more of the following lipids: Total cholesterol (TC), triglycerides (TG), low-density lipoprotein cholesterol (LDL-C) and high-density lipoprotein cholesterol (HDL-C). Percentage body fat (%BF) was estimated using bio-electric impedance analysis. A structured questionnaire was used by parents to report physical activity, screen time and eating habits of the children. Relationships between predictors and dyslipidemia were assessed using multiple binary logistic regression analyses. Results: The prevalence of dyslipidemia was 46.0%. Dyslipidemic children had significantly higher %BF, TG and LDL-C and lower HDL-C than the normal children (p p = 0.004), short stature (OR 2.8, 95% CI 1.1 - 6.8, p = 0.041), physical activity p Conclusion: This study confirms that obesity, short stature, physical activity < 60 minutes/day and never/occasional consumption of fruits/vegetables were associated with dyslipidemia. Also, a high proportion of children had one or more lipid disorders. This is concerning and indicates the importance of assessing dyslipidemia in pre-school children in future studies.

The Contribution of Linear Growth Components to Hypertension in Secondary School Adolescents in an Urban Setting in Cameroon

Background: Components of height have been found to be positively associated with blood pressure (BP) both in developed and developing nations. However, amongst Cameroon secondary school adolescents, the relationship between heights, SH and SH/H with BP has rarely been studied. The purpose of this study was to determine the proportion of secondary school adolescents with elevated BP and high BP and to evaluate the relationship between the different components of linear growth with BP. Methods: An institution-based cross-sectional study involving 602 adolescents (399 girls and 203 boys, mean age 14.9 ± 2.3 years) attending some public and private secondary schools in the Bamenda municipality of the North West Region of Cameroon. Anthropometric and BP measurements were carried out following standard procedures. Pearson correlation and linear regression were used to determine the relationship between the various components of height (height, SH, SH/H) with BP amongst the children. Results: The overall prevalence of elevated BP and hypertension amongst the study participants was 21.9% and 15.6% respectively (with 8.3% and 7.3% of the hypertensive children in Stage I and Stage II respectively). However, there were no significant gender differences in the prevalence of elevated BP and high BP (p = 0.497). Predictors of death in hospitalized elderly patients with COVID-19 in Mashhad, Iran, in 2021: A historical cohort study

Objective:The present study aimed to identify predictive factors for mortality among elderly individuals infected with COVID-19.Methods and Materials:This historical cohort study was conducted from July to December 2021 in the specialized departments for COVID-19 patients at one of the hospitals in Mashhad,Iran.Data were collected from the medical records of 404 elderly patients.Sampling was conducted using the convenience sampling method.Data were gathered through a demographic and clinical checklist developed by the researcher.Univariate and multivariate Cox regression were used to analyze the data.Results:The mortality rate among elderly individuals was 25%(n=101).Multiple regression analysis revealed significant associations between mortality and age(hazard ratio[HR]=0.58,95%confidence interval[CI]:0.38,0.88;P=0.011),level of consciousness(HR=0.31,95%CI:0.19,0.50;P<0.001),and SpO2(HR=0.58,95%CI:0.37,0.92;P=0.022).The probability of survival after the 19th day of hospitalization was 50%.Conclusions:Determining predictors of death allows for early identification of elderly individuals at risk and enables the health-care team to provide more effective care,ultimately saving the lives of elderly individuals by allocating appropriate facilities and equipment.
