Effects of aromatherapy with Rosa damascena on nulliparous women's pain and anxiety of labor during first stage of labor

Background： Reducing labor pain and anxiety is one of the most important goals of maternity care. Objective： This study aimed to assess the effects of aromatherapy with Rosa damascena on pain and anxiety in the first stage of labor among nulliparous women. Design, setting, participants and interventions： This was a randomized clinical trial of 110 nulliparous women. The eligible participants were randomly assigned to two groups of aromatherapy and control in an Iranian maternity hospital. The participants received 0.08 mL of Rosa damascena essence in the aro- matherapy group and 0.08 mL of normal saline in the control group, every 30 min. Pain was measured 3 times, once each at three stages of cervical dilation （4-5, 6-7, and 8-10 cm）. Anxiety was measured twice, once each at two stages of cervical dilation （4-7 and 8-10 cm）. The tools for data collection were the Spielberger anxiety questionnaire, numerical pain rating scale, demographic and obstetric questionnaire, and an observational checklist. Data analyses included the t-test, Mann-Whitney U test and Chi-square test. Main outcome measures： Severity of labor pain and severity of anxiety were used as primary outcome measures. Labor and delivery characteristics （including number of contractions, duration of contractions in second stage, Bishop score, augmentation by oxytocin, Apgar score, and mode of delivery）, demographic characteristics, and fertility information were used as secondary outcome measures. Results： Pain severity in the group receiving aromatherapy with R. damoscena was significantly lower than in the control group after treatment at each pain assessment （cervical dilation of 4-5, 6-7, and 8-10 cm; P 〈 0.05）. Anxiety levels were also significantly lower in the treatment group than in the control group after treatment at each time of measurement （cervical dilation of 4-7 and 8-10 cm; P 〈 0.05）. Relationship between spiritual well-being and Health-Related Quality of Life and some related factors in patients with AIDS/HIV

Objective:The study of effective factors on the quality of life(QoL),as an important criterion,in the outcome of new therapeutic and preventive strategies in human immunodeficiency virus(HIV)has been considered by researchers.To study the relationship between spiritual well-being and Health-Related Quality of Life(HRQoL)and some related factors in patients with Acquired Immunodeficiency Syndrome([AIDS]/HIV).Methods:In this cross-sectional study,224 people were selected as the study population(112 were male and 112 were female)from patients with AIDS/HIV at the Behavioral Counseling Center of Shiraz with available sampling method.The research tool was Palutzian&Ellison well-being scale and QoL Questionnaire(SF-36).Data were analyzed using Pearson correlation test.Results:There was a significant direct relationship between physical function dimensions(P=0.003).There was a significant relationship between general well-being,mental well-being,and total spiritual well-being(P<0.001).There was a statistically significant relationship between the dimensions of QoL and spiritual well-being with some age section of the patients,and their occupation and education(P<0.001).Conclusions:Spirituality has an impact on the QoL dimensions.Therefore,HIV positive people’s quality of life could be improved by the possibility of the spiritual well-being promotion as a method of adaptation with the disease.

Use of ginger versus stretching exercises for the treatment of primary dysmenorrhea: a randomized controlled trial

BACKGROUND： Dysmenorrhea is a common gynecologic problem. In some cases, non-medical treatments are considered to be more effective, with fewer side effects. Ginger and exercise are alternative treatments for dysmenorrhea, but in the present study they were not combined. OBJECTIVE： In this study, the effects of ginger and exercise on primary dysmenorrhea were compared. DESIGN, SETTING, PARTICIPANTS AND INTERVENTIONS： This randomized controlled tdal was performed in Mazandaran University of Medical Sciences, Iran. Two groups of female students were recruited by simple random allocation. In each group, 61 students with moderate to severe primary dysmenorrhea with regular menstrual cycles and without a history of regular exercise were assessed. The ginger group received 250 mg ginger capsules from the onset of menstruation. In the exercise group, belly and pelvic stretching exercises were performed for 10 min, 3 times per week. MAIN OUTCOME MEASURES： Intensity of pain was assessed according to a visual analogue scale after the first and the second month. RESULTS： Exercise was significantly more effective than ginger for pain relief （31.57± 16.03 vs 38.19 ± 20.47, P = 0.02）, severity of dysmenorrhea （63.9% vs 44.3% mild dysmenorrhea, P = 0.02） and decrease in menstrual duration （6.08 ± 1.22 vs 6.67± 1.24, P = 0.006）, in the second cycle. CONCLUSION： Stretching exercises, as a safe and low-cost treatment, are more effective than ginger for pain relief in primary dysmenorrhea.

目的鉴于院外护理服务的重要性,家庭护理的出现,以及护理实践中伦理框架的需求,本研究旨在探讨护士对家庭护理中伦理问题的看法和体验。方法采用面对面个人访谈收集资料。2020年,通过目的性抽样方法选取在伊朗4个城市的家庭护理中心中工作的20名护士进行访谈,了解他们在家庭护理过程中关于伦理问题的看法和体验。基于内容分析法,应用Granehei和Lundman方法对资料进行分析。结果本研究共提取编码416个。基于相似性对编码进行合并,提取出7个主题和16个子主题。主题包括对职业身份的感知、尊重患者的自主权、尊重隐私、建立人际交往、维护相互安全、遵守公平性原则和文化-宗教能力。子主题包括责任、专业发展及专业间互动、在家庭护理中保持专业性、提供整体性和艺术性的护理、尊重患者隐私、尊重护士隐私、信息保密、尊重服务对象的选择、诚实告知、富有同情心的互动、平衡权力地位、维护患者安全、维护护士安全、公平性的建立、尊重家庭宗教信仰,以及注重文化敏感性。结论研究参与者表示由于在家庭护理中涉及患者隐私,与在医院环境中的护理实践相比,家庭护理中对于护士职业身份的认知、隐私、家庭互动的管理、相互安全和文化-宗教能力等伦理问题显得尤为重要。

The focus of men’s health is within the context of preconception health and primary care,which involves a new method of engaging men’s attention to their preconception and general health.The aim of this review research is to offer health care practitioners a complete framework for men’s preconception care.From 2000 to October 2021,researchers examined electronic databases such as Web of Science,PubMed,Scopus,ScienceDirect,ProQuest,Cochrane,SAGE,Springer,UpToDate,and Google Scholar for published papers on men’s preconception care.Two independent authors assessed the quality of the included studies using techniques suitable for this type of investigation.Only 14 of the 835 studies that were found in the original search matched the inclusion criteria.Individual responsibility across the life span(evaluation of reproductive life plan,acquiring information about contraception)and risk assessment(age,medical and surgical history,medications,family history and genetic risks,social history,sexual health assessment,physical examination,laboratory testing,mental health,and intimate partner violence)are the two main topics of preconception care in men according to a review of studies.Our findings clearly demonstrated the critical components of a comprehensive preconception care approach for men.Therefore,preconception care for men remains a developing concept.Further research is required to develop standards for evaluating males during the preconception period.

Evidence-based practice profiles of nurses: a transcultural study

Objective: To compare(i) the evidence-based practice(EBP) profiles and(ii) socio-demographic characteristics of nurses in 4 selected low-middle income countries(LMICs).Methods: A cross-sectional survey was conducted among 677 nurses from 4 hospitals in 4 countries(Afghanistan, Ghana, Iran, and Uganda).Results: The findings revealed that there are significant differences between the 3 EBP subscales(that is, practice/use, attitude, and knowledge/skill) across the 4 countries(P < 0.05). No significant EBP differences were identified across age groups and gender in the subscales of practice, knowledge, and attitude. Practice and knowledge subscales were significantly associated with working schedules(that is, par t-time and full-time)(P < 0.05). Analyses also revealed significant differences between attitude and knowledge subscales and working experience(P < 0.05).Conclusions: Nurses in the 4 countries have moderate practice, attitude, and knowledge/skil s of EBP. Nurses in LMICs require continuous professional development programs and suppor t to enhance their practice and knowledge regarding EBP in clinical settings.

Lavender aromatherapy on anxiety and depression in patients with Acute Coronary Syndrome: a single-blind randomized clinical trial

Objective:In cardiovascular disease,a patient’s anxiety and depression can increase cardiac rehabilitation duration and recovery.Lavender aromatherapy as a non-pharmacological intervention effective in other contexts may be an efficient intervention to alleviating anxiety and depression in patients with Acute Coronary Syndrome(ACS).Methods:In this study,110 ACS patients were randomly assigned to two intervention and control groups.Inhalation of the lavender fragrance was prescribed for the intervention group and the drop of aromatic almond for the control group for 3 days.Results:The first-day anxiety and depression were significantly different in the two groups at 1 h and 9 h after the intervention.The’morning’s difference before the intervention was not significant,but it was substantial 1 h after the intervention.On the third morning of the intervention,this difference was confirmed.Conclusions:This study confirmed the effectiveness of lavender aromatherapy in reducing anxiety and depression in ACS patients.This’study’s results enable intensive care nurses to use aromatherapy with lavender oil as a non-pharmacological and cost-effective intervention to reduce their psychological tensions and increase patient satisfaction during hospitalization in the cardiac care units(CCU).

Reducing Labor Anxiety with Auriculotherapy:A Randomized Clinical Trial Study

Objectives:To investigate the effect of auriculotherapy on reducing labor anxiety.Methods:In this randomized clinical trial,166 nulliparous pregnant women admitted to the Labor Ward of Izadi Hospital in Qom,Iran were included and assigned to two groups by randomized block design.In the intervention group(83 cases),auriculotherapy in"Shenmen,Thalamic,Zero,Brain,and Brain stem"was performed in dilatations of 4,6 and 8 cm,between the contractions for 30 s in both ears at interval 1 h.While the control group received routine care.The Spielberger Inventory were completed pre-and post-interventions.Results:The auriculotherapy was effective to reduce anxiety state in the intervention group compared to the control group(P=0.001).The score of state anxiety in the intervention group reduced for 7.0,8.0 and 11.0 U in 4,6,and 8 cm dilatations after the intervention.The score of trait anxiety in the intervention group reduced for 3.8,4.0 and 8.3 U in 4,6,and 8 cm dilatations after the intervention.There was no significant difference of delivery mode and newborns’weight and Apgar score between groups(P>0.05).No adverse postpartum outcome such as hemorrhage has been observed in both groups.Conclusion:Since most women are anxious during labor,it seems that auriculotherapy can be an easy and safe method to reduce labor anxiety.(Trial registration No.irct20121230011944 n4)

Psychological problems as the major cause of fatigue in clients undergoing hemodialysis: A qualitative study

Background:It is obvious that hemodialysis is a stressful process,leading to numerous social and psychological problems,and may cause fatigue and helplessness.Objective:This study aimed to explain the experiences of hemodialysis clients regarding the effect of psychological problems on fatigue.Methods:This article was extracted from a qualitative study conducted on clients undergoing hemodialysis in Khorramabad.To this end,a total of 15 participants were purposefully selected using the snowball sampling method and the required data was collected using individual,face to face,detailed and semi-structured interviews.There were 12 client interviewees and one doctor,one nurse and one hemodialysis client's spouse.Content analysis was used to analyze the data.Results:Continuous analysis of data and documents obtained from the interviews on the hemodialysis clients'experiences and comments regarding the main causes of fatigue revealed two main categories:1)Psychological effects with subcategories of psychological stress and exhaustion;2)Needs and constraints with subcategories of individual and social needs and constraints.Conclusion:According to the clients'speech and expression analysis,psychological impacts and disease-induced needs and constraints were the most important cause of fatigue and its continuation among these clients.

Exploring the barriers of utilizing theoretical knowledge in clinical settings: A qualitative study

Objectives: Although effective performance in clinical settings requires the integration between theory and practice,there is a gap between theoretical knowledge as taught in the classroom and what the students experience in clinical settings.This study aimed to elicit and explore the barriers of utilizing theoretical knowledge in clinical settings.Methods: A qualitative study was adopted with a conventional content analysis approach.Fifteen nursing and paramedic's students,faculty members and experienced nursing staff participated in the study.Data were collected by semi-structured individual interviews until data saturation and concurrently analyzed via MAXQDA 10.Results: Five main categories emerged as barriers of utilizing theoretical knowledge in the clinical settings i.***.non-standard practices in clinical settings;lack of trust in clinical competence;lack of perceived professional support;insufficiencies in teaching and learning process;and differences between doing things in simulated and real clinical situations.Conclusion: Transferring theory into practice in a structured manner requires professional support in the workplace,trust and the opportunity for direct experience,using valid and up-to-date knowledge by clinical staff and bridging the simulated situations with real life scenarios.
