Deep brain stimulation for autism spectrum disorder

Deep brain stimulation(DBS)is a medical treatment that aims to obtain therapeutic effects by applying chronic electrical impulses in specific brain structures and neurological circuits.Over the years,DBS has been studied for the treatment of many psychiatric disorders.Scientific research on the use of DBS in people with autism has focused this interest mainly on treatment-resistant obsessive-compulsive disorder,drug-resistant epilepsy,self-injurious behaviors(SIB),and aggressive behaviors toward the self.Autism spectrum disorder(ASD)includes a group of developmental disabilities characterized by patterns of delay and deviance in the development of social,communicative,and cognitive skills and the presence of repetitive and stereotyped behaviors as well as restricted interests.People with autism often have numerous medical and psychiatric comorbidities that worsen the quality of life of patients and their caregivers.Obsessive-compulsive symptoms can be found in up to 81.3%of people with autism.They are often severe,refractory to treatment,and particularly difficult to treat.SIB has a high prevalence in severely retarded individuals and is often associated with autism.Drug treatment of both autism and SIB presents a therapeutic challenge.To describe the current state of the art regarding the efficacy of DBS in people with ASD,a literature search was conducted for relevant studies using the PubMed database.Thirteen studies have been considered in this paper.Up to date,DBS has been used for the stimulation of the nucleus accumbens,globus pallidus internus,anterior limb of the internal capsule,ventral anterior limb of the internal capsule,basolateral amygdala,ventral capsule and ventral striatum,medial forebrain bundle,and posterior hypothalamus.In the total sample of 16 patients,4 were adolescents,and 12 were adults.All patients had symptoms resistant to multiple drug therapy.Many patients taken into consideration by the studies showed clinical improvements as evidenced by the scores of the psychopathological scales

Application of wearable devices for monitoring cardiometabolic dysfunction under the exposome paradigm

Environmental factors,including chemical/physical pollutants,as well as lifestyle and psychological factors,contribute greatly to the pathways leading to cardiometabolic diseases with a heavy disease burden and economic loss.The concept of exposomes provides a novel paradigm for combining all exposure characteristics to evaluate disease risk.A solution-like exposome requires technological support to provide continuous data to monitor vital signs and detect abnormal fluctuations.Wearable devices allow people to conveniently monitor signals during their daily routines.These new technologies empower users to more actively prevent and manage cardiometabolic disease by reviewing risk factors of the disease,especially lifestyle factors,such as sleeping time,screen time,and mental health condition.Devices with multiple sensors can monitor electrocardiography data,oxygen saturation,intraocular pressure,respiratory rate,and heart rate to enhance the exposome study and provide precise suggestions for disease prevention and management.

Psychosis as an indicator of recurrent non-Hodgkin's lymphoma:a rare presentation

Psychotic manifestations of brain tumours are rare but described in the literature mostly along with other neurological deficits. Memory loss, difficulty in attention and concentration, depression, anxiety, and mood symptoms are commonly described in brain tumours. A schizophrenia-like picture without a deficit in motor or sensory function may land the clinician into a diagnostic dilemma. Primary central nervous system lymphoma(PCNSL) is a highly malignant disease with high mortality and needs immediate attention. Our case which had a unique recurrence in the postoperative period with psychotic symptoms can be an eye-opener to be more vigilant about underlying clinical extension.

目的调查约旦冠心病患者心理韧性现状及相关因素。方法采用横断面调查研究,便利选取在约旦安曼市2所公立医院心内科门诊就诊的134例患者,并采用心理韧性量表(The Connor-Davidson Resilience Scale,CD-RISC)对其进行面对面调查。采用Mann-Whitney U检验、Kruskal Wallis检验对数据进行分析。结果冠心病患者心理韧性得分为68.00(60.75,74.00),属于中等水平。没有心脏病手术史的患者心理韧性总得分[69.50(63.25,75.00)比65.50(58.00,72.00)]和“个人能力、高标准、坚韧维度”得分[22.00(18.50,26.00)比21.00(15.75,23.00)]均高于有心脏病手术史患者(P<0.05)。在职的患者“积极接受改变并确保关系维度”得分高于已经退休的患者[15.00(14.00,16.00)比14.00(12.00,15.00),P<0.05]。其他不同人口学资料患者的心理韧性及维度得分比较,差异均无统计学意义。结论应将冠心病患者的心理韧性水平及其相关因素纳入综合护理计划,以提高患者的生活质量,加强有效的应对策略,改善身心健康,并进一步预防并发症的发生。

目的:探讨阿尔茨海默病(AD)和遗忘型轻度认知功能障碍(aMCI)在默认脑网络(DMN)、突显网络(SN)和执行控制网络(ECN)这三个脑网络中的半球间脑功能连接的差异性。创新点:利用体素镜像同伦功能连接(VMHC)来观察AD和aMCI在多个脑网络基础上的半球间功能连接特点。方法:该研究纳入了浙江省人民医院就诊的30例AD患者、14例aMCI患者和18例老年健康对照者,均给予静息态功能磁共振扫描,利用VMHC进行数据分析,联合简易智力状态检查量表(MMSE)和蒙特利尔认知评估量表(MOCA)进行相关分析。结论:(1)位于三个脑网络的异常半球功能连接主要存在于AD组,可以作为AD诊断的一个敏感性指标;(2)VMHC值可以作为预测AD进展包括aMCI发展为AD的一个敏感性指标。

A Cross-sectional Survey of Disability Attributed to mental Disorders and Service Use in China

Background： mental disorders are strongly associated with disabilities. National survey on disability could provide a reliable basis for policymaking in care and rehabilitation of disabled persons. This study aimed to describe the disability prevalence rates attributed to mental disorders, their distribution by sociodemographic factors, and utilizations of service. Methods： This study is a secondary data analysis of the Second National Sample Survey on Disability in 2006. The disability and severity were assessed using the World health Organization Disability Assessment Schedule 2.0. mental disorders were diagnosed according to the International Statistical Classification of Diseases and Related health Problems 10th Revision Classification of mental and Behavioral Disorders. Using descriptive and analytic epiderniological methods, prevalence rates of disability attributed to mental disorders and service use were calculated. Results： Data of 2,526,145 respondents were analyzed. The disability prevalence rate attributed to mental disorders in China was 6.3%0, accounting for 9.9% of all disabled people. Regarding disability prevalence attributed to mental disorders, it showed that gender, residential area, marital status, education level, and economic area were related to the prevalence distributions. The proportions of mild disability were highest in the disabled people with onset age of 18-64 years, while the proportion of extremely severe disability was highest in the disabled people with onset age of 65 years and above. Only 58.6% of disabled people attributed to mental disorders used some of the services. Conclusions： There are statistical differences of disability prevalence attributed to mental disorders by people and region in China. Service use in disabled people with mental disorders is insufficient.

Background:Although clozapine is an effective option for treatment-resistant schizophrenia(TRS),there are still 1/3 to 1/2 of TRS patients who do not respond to clozapine.The main purpose of this randomized,double-blind,placebocontrolled trial was to explore the amisulpride augmentation efficacy on the psychopathological symptoms and cognitive function of clozapine-resistant treatment-refractory schizophrenia(CTRS)patients.Methods:A total of 80 patients were recruited and randomly assigned to receive initial clozapine plus amisulpride(amisulpride group)or clozapine plus placebo(placebo group).Positive and Negative Syndrome Scale(PANSS),Scale for the Assessment of Negative Symptoms(SANS),Clinical Global Impression(CGI)scale scores,Repeatable Battery for the Assessment of Neuropsychological Status(RBANS),Treatment Emergent Symptom Scale(TESS),laboratory measurements,and electrocardiograms(ECG)were performed at baseline,week 6,and week 12.Results:Compared with the placebo group,amisulpride group had a lower PANSS total score,positive subscore,and general psychopathology subscore at week 6 and week 12(PBonferroni<0.01).Furthermore,compared with the placebo group,the amisulpride group showed an improved RBANS language score at week 12(PBonferroni<0.001).Amisulpride group had a higher treatment response rate(P=0.04),lower scores of CGI severity and CGI efficacy at week 6 and week 12 than placebo group(PBonferroni<0.05).There were no differences between the groups in body mass index(BMI),corrected QT(QTc)intervals,and laboratory measurements.This study demonstrates that amisulpride augmentation therapy can safely improve the psychiatric symptoms and cognitive performance of CTRS patients.

Distribution and Risk Factors of Disability Attributed to Personality Disorders： A National Cross-sectional Survey in China

Background： Personality disorders can lead to some disability. However, little is known about the disability prevalence and function impairments. This study aimed to describe the disability prevalence attributed to personality disorders, its distribution, impairments of daily activities and social functions, and risk factors in China. Methods： Using a descriptive and analytic epidemiological method, data from the Second China National Sample Survey on Disability in 2006 were analyzed. The disability prevalence attributed to personality disorders, its distribution in different people and regions, and risk factors were statistically calculated. Results： Respondents included 1,909,205 adults. The disability prevalence rate attributed to personality disorders in China was 5.9/100,000. The disability rate attributed to personality disorders of males was higher than that of females （P 0.012）, while the rate of the unemployed was higher than that of the employed （P 〈 0.001 ）. Furthermore, the rates of unmarried/divorced/widowed people and the illiterate population were higher than those of married and educated people （P 〈 0.001 ）. Regarding the severity of disability attributable to personality disorders, mild disability accounted for a majority or 60% of the respondents. The data showed that disability mainly impaired respondents＇ ability to engage in daily activities, get along with people, and participate in social situations. According to the case-control study, marriage, employment, and higher education were protective factors of disability. Conclusions： The prevalence of disability attributed to personality disorders is low in China and always leads to mild disability. The distribution of disability attributed to personality disorders also varies in the Chinese population.

Cognitive Function of Children and Adolescents with Attention Deficit Hyperactivity Disorder and Learning Difficulties： A Developmental Perspective

Background： The cognitive function of children with either attention deficit hyperactivity disorder （ADHD） or learning disabilities （LDs） is known to be impaired. However, little is known about the cognitive function of children with comorbid ADHD and LD. The present study aimed to explore the cognitive function of children and adolescents with ADH D and learning difficulties in comparison with children with ADHD and healthy controls in different age groups in a large Chinese sample. Methods： Totally, 1043 participants with ADHD and learning difficulties （the ADHD ＋ learning difficulties group）, 870 with pure ADHD （the pure ADHD group）, and 496 healthy controls were recruited. To investigate the difference in cognitive impairment using a developmental approach, all participants were divided into three age groups （6-8, 9-11, and 12-14 years old）. Measurements were the Chinese-Wechsler Intelligence Scale for Children, the Stroop Color-Word Test, the Trail-Making Test, and the Behavior Rating Inventory of Executive Function-Parents （BRIEF）. Multivariate analysis of variance was used. Results： The results showed that after controlling for the effect of ADHD symptoms, the A DHD ＋ learning difficulties group was still significantly worse than the pureADHD group, which was, in turn, worse than the control group on full intelligence quotient （98.66 ± 13.87 vs. 105.17 ± 14.36 vs. 112.93 ± 13.87, P 〈 0.001 ）. The same relationship was also evident for shift function （shifting time of the Trail-Making Test, 122.50 [62.00, 194.25] s vs. 122.00 [73.00, 201.50] s vs. 66.00 [45.00, 108.00] s, P 〈 0.001） and everyday life executive function （BRIEF total score, 145.71 ± 19.35 vs. 138.96± 18.00 vs. 122.71 ± 20.45, P 〈 0.001 ） after controlling for the effect of the severity of ADHD symptoms, intelligence quotient, age, and gender. As for the age groups, the differences among groups became nonsignificant in the 12-14 years old group for inhibition （meaning interference of the Stroop Color-Word Test, 18.00 [13.00, 25.00] s vs. 17.00
