Promoting recovery from severe mental illness: Implications from research on metacognition and metacognitive reflection and insight therapy

Research indicates that individuals with schizophrenia recover. Recovery, however means different things to different individuals and regardless of what kind of experiences define recovery, the individual diagnosed with the serious mental illness must feel ownership of their recovery. This raises the issue of how mental health services should systematically promote recovery. This paper explores the practical implications for research on metacognition in schizophrenia for this issue. First, we present the integrated model of metacognition, which defines metacognition as the spectrum of activities which allow individual to have available to themselves an integrated sense of self and others as they appraise and respond to the unique challenges they face. Second, we present research suggesting that many with schizophrenia experience deficits in metacognition and that those deficits compromise individuals' abilities to manage their lives and mental health challenges. Third, we discuss a form of psychotherapy inspired by this research, Metacognitive Reflection and Insight Therapy which assists individuals to recapture the ability to form integrated ideas about themselves and others and so direct their own recovery. The need for recovery oriented interventions to focus on process and on patient's purposes, assess metacognition and consider the intersubjective contexts in which this occurres is discussed.

Posttraumatic stress symptoms among Chinese university students during the COVID-19 pandemic:Prevalence,correlates,and mental health help-seeking

BACKGROUND Revisiting the epidemiology of posttraumatic stress symptoms(PTSSs)among university students during the coronavirus disease 2019(COVID-19)pandemic as well as understanding the mental health help-seeking behavior of individuals with PTSSs has critical implications for public mental health strategies in future medical *** To investigate the prevalence and correlates of PTSSs among university students during the first wave of the COVID-19 pandemic in China and to examine mental health help-seeking behaviors among these *** A total of 2507 Chinese university students were recruited via snowball *** students completed the Seven-item Screening Scale for Post-traumatic Stress Disorder during the first wave of the COVID-19 pandemic in ***-graphic characteristics,pandemic-related characteristics,and mental health help-seeking behaviors of students with PTSSs were also *** The prevalence of PTSSs among the participants was 28.0%.Seven significant correlates of PTSSs were identified(odds ratio=1.23-3.65,P≤0.024):Female sex,being 19 years old or older,living with others or alone,a low level of family economic status,fair or poor interpersonal relationships,severe or very severe local pandemic,and having family members diagnosed with ***,only 3.28%of the students with PTSSs reported seeking help from mental health *** the 23 students who sought help from mental health specialists,13 opted for online or telephone-based psychological *** Our data suggest that there was a high risk of PTSSs among university students and a high level of unmet mental health needs during the COVID-19 *** delivery of mental health services online or via telephone is a promising approach to address these unmet needs.

Dear Editor, Diseases of the genitourinary system were the fifth most common disease of inpatients in Chinese city hospitals in 2011 （6.0%）.1 Andrology in China has grown rapidly within the last decade, however, little is known about the scientific publications by Chinese authors in andrology. We, therefore, sought to reveal the contributions of Chinese authors in three major regions of China-the Mainland （ML）, Taiwan （TW）, and Hong Kong （HK）-to the research in the fields of andrology.

Influence of intranasal oxytocin on fear consolidation in healthy humans

Background Although recent data in healthy humans suggestthat treatment with intranasal oxytocin(0T)may facilitate extinction recall,to date,little is known about the effects of 0T on memory *** To examine the effect of intranasal administration of OT compared with placebo on memory consolidation blockade of a de novo fear memory in a classical 2-day fear conditioning *** There were no significant differences between the 0T and the placebo groups on the first two extinction trials(mean(SD)=0.01(0.39)vs 0.15(0.31),t=-1.092,p=0.28).Similarly,during early extinction,analysis of variance for repeated measures failed to show significant main effects of extinction trials:trials(F(4,112)=1.58,p=0.18),drug(F(1,112)=0.13,p=0.72)or drug x trials interaction(F(4,112)=0.76,p=0.56).Conclusion Our results suggest that 0T administered in a double-blind fashion immediately after fear conditioning does not significantly reduce consolidation of fear learning as measured by a differential skin conductance response tested at the beginning of extinction.

Resting state brain default network in patients with motor aphasia resulting from cerebral infarction

To explore the brain default mode network(DMN)in patients with motor aphasia resulting from cerebral infarction,we used resting state functional magnetic resonance imaging(fMRI)to investigate the possible neural *** patients with motor aphasia resulting from cerebral infarction and ten matched controls were selected in this *** subjects were examined using resting state *** chose the posterior cingulate cortex as the region of interest and then used functional connectivity analysis to calculate the DMN functional connectivity and analyze differences in the functional connectivity between the two *** with normal controls,aphasia patient group showed a significantly decreased functional connectivity in bilateral medial frontal gyrus,superior frontal gyrus,middle frontal gyrus,middle temporal gyrus,precuneus and *** aphasia patient group showed increased functional connectivity mainly in bilateral medial frontal gyrus,middle frontal gyrus,inferior frontal gyrus,precentral gyrus,*** DMN in cerebral infarction motor aphasia patients showed significantly decreased functional connectivity in the resting *** DMN most likely plays an important role in motor aphasia resulting from cerebral ***,functional connectivity in the brain regions surrounding the left and right Broca’s areas was significantly enhanced due to compensatory *** may be helpful for the recovery of language function in cerebral infarction patients with motor aphasia.

A Bibliometric Analysis of Research on the Behavior Therapy in China and Its Trend

To get formed of the status of research and application of the domestic behavior therapy and its development trend, the time distribution and the subject distribution were bibliometricallly analyzed of the literature on behavior therapy from 1981 to 2000 in the CBMdisc. Our results showed that the number of literature of behavior therapy has been increasing in exponential manner over the past 20 years; the behavior modification, the biofeedback and the cognitive therapy are extensively used in China. In clinical practice, the behavior modification and the biofeedback have been applied in all departments of medical institutions, especially for treating the cardiovascular and the neurological conditions. The cognitive therapy has been employed mainly for the treatment of mental disorders (or dysphrenia), the aversive therapy mainly for material withdrawal, and the systematic desensitization for phobia. There was no report found on the clinical use of meditation. It is concluded that the study and application in behavior therapy in China is currently developing very fast.

Epidemiology of functional gastrointestinal disorders in children and adolescents: A systematic review

To assess the prevalence of functional gastrointestinal disorders (FGIDs) in children and ***, EMBASE, and Scopus databases were searched for original articles from inception to September 2016. The literature search was made in accordance with the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) recommendations. For inclusion, each study had to report epidemiological data on FGIDs in children between 4 and 18 years old and contain standardized outcome based on Rome II, III or IV criteria. The overall quality of included epidemiological studies was evaluated in accordance with Loney’s proposal for prevalence studies of health literature. Two reviewers assessed each study for data inclusion and extraction. Discrepancies were reconciled through discussion with *** total of 659 articles were identified from the databases and 16 through manual search. A total of 43 articles fulfilled the eligibility criteria for full-text reading, with 26 remaining to be included in the final analysis. All studies were written in English and published between 2005 and 2016. Eight (30.8%) articles were performed in North America, five (19.2%) in Latin America, five (19.2%) in Europe, seven (27%) in Asia, and one (3.8%) in Africa. Sample size varied between 114 and 99416 subjects, totaling 132600 individuals. Fourteen (53.9%) studies recruited their target samples from schools, 11 (42.3%) from healthcare settings and the remaining one (3.8%) from online panel community. The overall FGID prevalence rates for student samples ranged from 9.9% to 29% to as high as 87% in clinical samples. Cyclic vomiting, irritable bowel syndrome and functional constipation were the most researched conditions, with a prevalence ranging from 0.2% to 6.2%, 0% to 45.1% and 0.5% to 86.9%, respectively.
