Azacitidine maintenance therapy for blastic plasmacytoid dendritic cell neoplasm allograft: A case report

BACKGROUND Blastic plasmacytoid dendritic cell neoplasm(BPDCN)is a rare,highly invasive malignant neoplasm.There is no universally accepted standard of care because of its rarity and the dearth of prospective research.It is still challenging for some patients to achieve persistent clinical remission or cure,despite the success of allogeneic hematopoietic stem cell transplantation(allo-HSCT),indicating that there is still a significant recurrence rate.We report a case of prevention of BPDCN allograft recurrence by azacitidine maintenance therapy and review the relevant literature.CASE SUMMARY We report a 41-year-old man with BPDCN who was admitted to hospital due to skin sclerosis for>5 mo’*** was diagnosed by combined clinical assessment and laboratory examinations.Following diagnosis,the patients underwent induction consolidation chemotherapy to achieve the first complete remission,followed by bridging allo-HSCT.Post-transplantation,azacitidine(75 mg/m2 for 7 d)was administered as maintenance therapy,with repeat administration every 4–6 wk and appropriate extension of the chemotherapy cycle.After 10 cycles,the patient has been disease free for 26 mo after transplantation.Regular assessments of bone marrow morphology,minimal residual disease,full donor chimerism,Epstein–Barr virus,and cytomegalovirus all yielded normal results with no abnormalities detected.CONCLUSION Azacitidine may be a safe and effective maintenance treatment for BPDCN following transplantation because there were no overt adverse events during the course of treatment.

Diabetic nephropathy(DN)is a severe complication of diabetes,characterized by changes in kidney structure and function.The natural product rosmarinic acid(RA)has demonstrated therapeutic effects,including anti-inflammation and anti-oxidative-stress,in renal damage or dysfunction.In this study,we characterized the heterogeneity of the cellular response in kidneys to DN-induced injury and RA treatment at single cell levels.Our results demonstrated that RA significantly alleviated renal tubular epithelial injury,particularly in the proximal tubular S1 segment and on glomerular epithelial cells known as podocytes,while attenuating the inflammatory response of macrophages,oxidative stress,and cytotox-icity of natural killer cells.These findings provide a comprehensive understanding of the mechanisms by which RA alleviates kidney damage,oxidative stress,and inflammation,offering valuable guidance for the clinical application of RA in the treatment of DN.

Advances in clinical applications of bioceramics in the new regenerative medicine era

In this editorial,we comment on the hard and soft tissue applications of different ceramic-based scaffolds prepared by different mechanisms such as 3D printing,sol-gel,and electrospinning.The new concept of regenerative medicine relies on biomaterials that can trigger in situ tissue regeneration and stem cell recruitment at the defect site.A large percentage of these biomaterials is ceramic-based as they provide the essential requirements of biomaterial principles such as tailored multisize porosity,antibacterial properties,and angiogenic properties.All these previously mentioned properties put bioceramics on top of the hierarchy of biomaterials utilized to stimulate tissue regeneration in soft and hard tissue wounds.Multiple clinical applications registered the use of these materials in triggering soft tissue regeneration in healthy and diabetic patients such as bioactive glass nanofibers.The results were promising and opened new frontiers for utilizing these materials on a larger scale.The same results were mentioned when using different forms and formulas of bioceramics in hard defect regeneration.Some bioceramics were used in combination with other polymers and biological scaffolds to improve their regenerative and mechanical properties.All this progress will enable a larger scale of patients to receive such services with ease and decrease the financial burden on the government.

Evaluation of hybrid closed-loop insulin delivery system in type 1 diabetes in real-world clinical practice:One-year observational study

BACKGROUND In 2016,the Food and Drug administration approved the first hybrid closed-loop(HCL)insulin delivery system for adults with type 1 diabetes(T1D).There is limited information on the impact of using HCL systems on patient-reported outcomes(PROs)in patients with T1D in real-world clinical practice.In this independent study,we evaluated glycemic parameters and PROs over one year of continuous use of Medtronic’s 670G HCL in real-world clinical practice.AIM To assess the effects of hybrid closed loop system on glycemic control and quality of life in adults with T1D.METHODS We evaluated 71 patients with T1D(mean age:45.5±12.1 years;59%females;body weight:83.8±18.7 kg,body mass index:28.7±5.6 kg/m2,A1C:7.6%±0.8%)who were treated with HCL at Joslin Clinic from 2017 to 2019.We measured A1C and percent of glucose time-in-range(%TIR)at baseline and 12 months.We measured percent time in auto mode(%TiAM)for the last two weeks preceding the final visit and assessed PROs through several validated quality-of-life surveys related to general health and diabetes management.RESULTS At 12 mo,A1C decreased by 0.3%±0.1%(P=0.001)and%TIR increased by 8.1%±2.5%(P=0.002).The average%TiAM was only 64.3%±32.8%and was not associated with A1C,%TIR or PROs.PROs,provided at baseline and at the end of the study,showed that the physical functioning submodule of 36Item Short-Form Health Survey increased significantly by 22.9%(P<0.001).Hypoglycemia fear survey/worry scale decreased significantly by 24.9%(P<0.000);Problem Areas In Diabetes reduced significantly by-17.2%(P=0.002).The emotional burden submodules of dietary diversity score reduced significantly by-44.7%(P=0.001).Furthermore,analysis of Clarke questionnaire showed no increase in awareness of hypoglycemic episodes.WHO-5 showed no improvements in subject’s wellbeing among participants after starting the 670G HCL system.Finally,analysis of Pittsburgh Sleep Quality Index showed no difference in sleep quality,sleep latency,or duration of sleep from baseline to 12 mo.CONCLUSION The us

Quality indicators in healthcare refer to measurable and quantifiable parameters used to assess and monitor the performance,effectiveness,and safety of healthcare services.These indicators provide a systematic way to evaluate the quality of care offered,and thereby to identify areas for improvement and to ensure that patient care meets established standards and best practices.Respiratory therapists play a vital role in areas of clinical administration such as infection control practices and quality improvement initiatives.Quality indicators serve as essential metrics for respiratory therapy departments to assess and enhance the overall quality of care.By systematically tracking and analyzing indicators related to infection control,treatment effectiveness,and adherence to protocols,respiratory care practitioners can identify areas to improve and implement evidence-based changes.This article reviewed how to identify,implement,and monitor quality indicators specific to the respiratory therapy departments to set benchmarks and enhance patient outcomes.

Exosome therapy holds great promise as a novel approach to improve acute skin wound healing.This review provides a comprehensive overview of the current understanding of exosome biology and its potential applications in acute skin wound healing and beyond.Exosomes,small extracellular vesicles secreted by various stem cells,have emerged as potent mediators of intercellular communication and tissue repair.One advantage of exosome therapy is its ability to avoid potential risks associated with stem cell therapy,such as immune rejection or stem cells differentiating into unwanted cell types.However,further research is necessary to optimize exosome therapy,not only in the areas of exosome isolation,characterization,and engineering,but also in determining the optimal dose,timing,administration,and frequency of exosome therapy.Thus,optimization of exosome therapy is critical for the development of more effective and safer exosome-based therapies for acute skin wound healing and other diseases induced by cancer,ischemia,or inflammation.This review provides valuable insights into the potential of exosome therapy and highlights the need for further research to optimize exosome therapy for clinical use.

Mindfulness Facets and Psychological Well-Being among Meditators:Serenity as a Mediating Process

Guided by the theoretical processes of mindfulness and psychological well-being,this study examined serenity as a mediator between mindfulness facets and psychological well-being,as indexed by depressive symptoms and life satisfaction.Participants were 133 mindfulness practitioners who took part in a 3-day transnational meditation event in Hong Kong.Upon informed consent,participants completed a self-report questionnaire.The findings from structural equation modeling showed that serenity mediated the relation between two facets of mindfulness,including describing and nonreacting to inner experience,and life satisfaction.Serenity also mediated the relation between the mindfulness facet of describing and depressive symptoms.Direct associations were indicated between two mindfulness facets,including observing and nonjudging of inner experience,and depressive symptoms.Taken together,the findings revealed mindfulness facets as major correlates of serenity and psychological outcomes among Chinese meditation practitioners.To foster psychological well-being,researchers,educators,and practitioners should pay attention to the role of serenity,describing,and nonreacting to inner experience in mental health.

Synergistic Effects of Bay of Bengal Tropical Cyclones and Tibetan Plateau Vortices on Water Vapor Transport over the Tibetan Plateau in Early Summer

The Bay of Bengal(BoB)tropical cyclones(TCs)and the Tibetan Plateau vortices(TPVs)are two crucial weather systems influencing the Tibetan Plateau(TP).Their synergistic effects can lead to widespread heavy precipitation events on the TP.In this study,we employ the Hybrid Single-Particle Lagrangian Integrated Trajectory(HYSPLIT)model to track the trajectory of water vapor transport during three large-scale precipitation events on the TP under the combined influence of BoB TCs and TPVs.The results indicate that low-level water vapor from the BoB under the influence of BoB TCs was cyclonically entangled into the cyclonic circulation,lifted and transported northward by southwesterly flow to the southeastern part of the TP,which contributes to the moistening of the entire troposphere there.Additionally,convergence of the cyclonic circulation of the TPVs on the northern TP further transports water vapor collected in the southeastern TP northward,conducive to the maintenance and development of precipitation systems,thus inducing widespread heavy precipitation events over the TP.

Spatio-temporal variation of water salinity in mangroves revealed by continuous monitoring and its relationship to floristic diversity

Salinity is among the most critical factors limiting the growth and species distribution of coastal plants.Water salinity in estuarine ecosystems varies temporally and spatially,but the variation patterns across different time scales and salinity fluctuation have rarely been quantified.The effects of salinity on floristic diversity in mangroves are not fully understood due to the temporal and spatial heterogeneity of salinity.In this study,we monitored water salinity at an interval of 10-min over one year in three mangrove catchment areas representing the outer part,middle part,and inner part respectively of Dongzhai Bay,Hainan,China.The number of mangrove community types and dominant mangrove species of the three catchment areas were also investigated.We found that the diurnal variation and dry-season intra-month variation in water salinity were driven by tidal cycles.The seasonal variation in water salinity was mainly driven by rainfall with higher salinity occurring in the dry season and lower salinity occurring in the wet season.Spatially,water salinity was highest at the outer part,intermediate at the middle part,and lowest at the inner part of the bay.The intra-month and annual fluctuations of water salinity were highest at the middle part and lowest at the outer part of the bay.The number of mangrove community types and dominant species were lowest at the outer part,intermediate at the middle part,and highest at the inner part of the bay.These results suggest that the temporal variation of water salinity in mangroves is driven by different factors at different time scales and therefore it is necessary to measure water salinity at different time scales to get a complete picture of the saline environment that mangroves experience.Spatially,lower salinity levels benefit mangrove species richness within a bay landscape,however,further research is needed to distinguish the effects of salinity fluctuation and salinity level in affecting mangrove species richness.
