Purpose: Pain assessment is a key component of good pain management in hospitalized infants.This study aimed to translate and adapt a version of pain measurement in infants,the Premature Infant Pain Profile Revised (PIPP-R) into Indonesian.Method: The adaptation process of the measuring instrument used a modified Brislin method which included forward translation,back translation 1,group discussion 1,back translation 2,group discussion 2,and pilot testing on neonatal nurses: feasibility test,inter-rater reliability using intraclass correlation (ICC),and internal consistency using Cronbach's α coefficient.Results: The PIPP-R version in English has been translated into Indonesian.In general,nurses assessed this measuring instrument as feasible.The inter-rater reliability showed a high agreement (ICC =0.968,P=0.001) and this measuring instrument had good internal consistency (Cronbach's α=0.856).Conclusion: The Indonesian version of PIPP-R is easy to use and shows good psychometric properties.The use of this measuring instrument will help nurses and researchers obtain accurate infant pain intensity measurement values.

Purpose:This study aimed to determine the effectiveness of Relaxation Breathing Exercise(RBE)on fatigue in patients with gynaecological cancer undergoing chemotherapy.Methods:We used a quasi-experiment with pre-and post-test design.Forty-two patients were consecutively sampled,21 of whom were assigned to the group receiving RBE four times a day and 21 to the group receiving RBE twice a day.Fatigue scores were measured every day for 7 days for both groups using the Piper Fatigue Scale.Data obtained were analysed using repeated-ANOVA and independent ttest with a significant level of a<0.01.Results:We found a significant decrease(P<0.01)in the mean fatigue scores of both groups(four times RBE in a day group=3.29±0.59 and twice RBE in a day group=4.19±0.61)after the completion of the intervention.However,the four times RBE a day group showed larger decrease on fatigue scores than the twice RBE a day group did(Mean Difference=0.91;99%CI=0.41 to 1.41;P=0.001).Conclusion:Conducting RBE four times a day effectively alleviated fatigue better than RBE twice a day on gynaecological cancer patients undergoing chemotherapy.

目的本系统性评价旨在探究产后尿潴留的危险因素,以期为预防产后尿潴留提供依据。方法﹐根据系统综述和荟萃分析优先报告的条目(The Preferred Reporting Tiems for Systematic Reviews and Meta-Analyses,PRISMA),对CINAHL,NHC Evidence,PubMed,PsycINFO,Trip,ScienceDirect,Web of Science,Cochrane Library,maternity and Infant Care,AMED 11个外文数据库进行文献检索,检索文献发表时间限定为2009年1月1日至2019年1月1日,仅纳入英文文献。通过关键评估技能计划工具(The Critical AppraisalSkills Programme tools,CASP)或横断面研究评价工具(The Appraisal tool for Cross-Sectional Studies,AXIS)评价纳入文章的质量。两名作者独立进行文章选择、质量评价和数据提取。提取数据用Review Managerversion 5.0.2软件进行统计分析。结果共纳入9篇文章,提取5个危险因素:包括会阴侧切术(OR=2.99,95%CI=1.31~6.79,P=0.009),硬膜外镇痛(OR=2.48,95%CI=1.09~5.68,P=0.03),初产(OR=2.17,95%CI=1.06~4.46,P=0.03),阴道助产(OR=4.01,95%CI=1.97~8.18,P<0.001),第二产程过长(MD=15.24,95%CI=11.20^-19.28,P<0.001)。尚不能确定新生儿出生体重大于3800 g是否是产后尿潴留的危险因素(MD=64.41,95%CI=-12.59~141.41,P=0.10)。结论结果表明会阴侧切术、硬膜外镇痛、阴道助产、第二产程过长和初产是产后尿潴留的独立危险因素。在临床工作中,医务工作者应该要更加关注有这些危险因素的产妇,从而预防产后尿潴留的发生。

目的旨在调查埃及护士专业共享治理及职业动机的现状,并探讨其相关性。方法采用横断面调查法,于2022年5—8月选取埃及亚历山大大学附属医院内科、外科和ICU工作的724名护士为研究对象。采用专业护理治理指数(Index of Professional nursing Governance,IPNG)和职业动机量表进行调查。结果本组护士IPNG总分为(109.18±22.76)分(量表得分范围50~250分,50个条目),表明护士感知专业治理水平较低。其中获取信息维度的均分最高,为(2.81±0.76)分;其次是设定目标和解决冲突的能力维度,为(2.75±0.53)分;参与委员会组织维度均分最低为(1.65±0.37)分。职业动机的总分为(69.82±9.70)分(量表得分范围25~105分),表明护士职业动机处于中等水平,其中职业洞察力维度均分最高为(3.56±0.34)分,职业适应力维度均分最低为(3.07±0.49)分。男性、年龄在30岁以下、本科学历、在ICU工作的护士IPNG和职业动机量表总分较高(P<0.001)。相关分析结果显示,护士专业共享治理得分与职业动机得分呈正相关(r=0.239,P=0.003)。结论研究结果可以为护理管理者提供理论依据,重新规划管理模式,并制订相应策略,从而为护士提供更好的职业规划。

Objective:To examine the effect of upright and ambulant positions versus lying down during the active first stage of labor on birth outcomes for nulliparous women.Methods:This is a randomized controlled clinical trial conducted at the delivery department of Damanhour Educational Institute,El Behira Governorate,Egypt.The study sample involved 150 parturient women equally divided into intervention and control groups using randomization block technique.The researchers used four tools for data collection:Demographic data interview schedule,World Health Organization Partograph,Apgar’s score,to evaluate neonatal outcomes,and visual analogue pain intensity scale.For the study group,the parturient women were encouraged to assume one of the upright positions or ambulating around the bed so as to maintain the pelvis in vertical plane as far as possible for 20-25 min for every 1 h.The control group received routine hospital care,which includes lying down in bed.IBM SPSS 23.0 was used to analyze the data.Results:Significant differences(P0.05)were observed between both groups in term of emergency cesarean birth rates,oxytocin use,and neonatal outcomes.Conclusions:This study proves that upright and ambulant positions significantly enhance uterine contractility,cervical dilatation,and fetal head descent and reduce the first stage duration.

Objective:Despite getting sufficient sleep being one of the most important things that we can do to keep our body and mind healthy,sleep deprivation has become a major public health concern.This study aimed to determine the prevalence of sleep deprivation and its associated factors among students of the Institute of Health in Jimma University.Methods:An institution-based cross-sectional study was conducted from April 10 to April 24 in 2019.Data were collected from 365 randomly selected participants and analyzed using Statistical Package for the Social Sciences(SPSS;version 23).Multivariate logistic regression analysis was used to identify the factors associated with sleep deprivation.The level of significance was declared at a P-value of<0.05.Results:A total of 365 participants participated,with a response rate of 97.6%.The study findings showed that 60.8%of students had sleep deprivation and 68.2%had sleep latency.Health problems(adjusted odds ratio[AOR]=1.91[95%confidence interval{CI}=1.01–3.58]),alcohol consumption[AOR=0.42(95%CI=0.2–0.89)],cigarette smoking[AOR=0.31(95%CI=0.09–0.93)],khat chewing[AOR=0.47(95%CI=0.12–0.82)],use of an electronic device before bedtime[AOR=5.26(95%CI=1.78–15.52)],and cumulative grade point average(CGPA)[AOR=0.48(95%CI=0.27–0.83)]were significantly associated with sleep deprivation.Conclusions:Sleep deprivation was common in the study area.This is mainly due to health problems,substance utilization,long-time use of an electronic device,and low CGPA.Therefore,a strategy and system should be established to limit substance use,enhance proper use of electronic devices,and create awareness on the impact of nonoptimal sleep.

Objective:This study aimed to determine the effect of the relaxation breathing exercise(RBE)on fatigue and cortisol levels among Indonesian women with gynecological cancer undergoing chemotherapy.Methods:This pilot study consecutively recruited 44 gynecological cancer patients to receive RBE(22)or usual care(22).Cortisol level was measured before and after completion of the intervention(day 8).Fatigue was measured using the Piper Fatigue Scale(PFS).Anxiety and depression were assessed using the Hospital Anxiety and Depression Scale(HADS)at the baseline days of the study for days 1,4,and 8.The obtained data were analyzed using Fisher’s exact test,the independent t-test,and the Mann-Whitney U test.Results:There were significant increases in cortisol levels within the groups,either the intervention or control groups,respectively(P-value=0.0003 and 0.001).Despite there being no statistical significance between the intervention and control groups,there were noticeable differences in the cortisol levels,indicating the extreme increase in cortisol levels in the control group(Median[IQR1-IQR3]:2.30[0.99-9.09];Min-Max:0.43-23.38)compared with the intervention group(Median[IQR1-IQR3]=2.97[1.26-5.18];Min-Max=0.39-6.91).Conclusions:RBE helps prevent a significant increase in cortisol levels that can alleviate fatigue for women with gynecological cancer.Further research was recommended to compare several intervention modalities for fatigue and cancer-related symptom management based on cortisol level changes.

Objective： This article aimed to evaluate the effects of an educational intervention programme on improving peer educator （PE） knowledge in issues around growth and development and its risk behavior problems of adolescents in Indonesia. Methods： The study was conducted in 31 of PEs to evaluate their knowledge in adolescents. The PEs received health education and were assessed by individual work during the structured three-week programme. Data were collected before and after the training programme as pre- and post-tests. Results： The PE demonstrated significant improvements in their knowledge after attending the three-week structured training programme. The post-test scores had significant effects on the dimensions of PE knowledge scores. The PE become knowledgeable to maintain and monitor adolescents health issues around growth and development and its risk behavior problems of adolescents. Conclusions： The PE become knowledgeable to respond to the adolescent problems and readiness to become PE during puberty.

Objective: The aim of this article is to reflect on the role of theater nurses in a multidisciplinary team, understand the factors that have influenced theater nurses' practice, and improve the authors' clinical practice ultimately.Methods: The author used Smyth's model to guide the process of reflection on the practice issue. Critical reflection, critical emancipatory theory, reflexivity, and critical social theory were used to help the author analyze the factors that have affected theater nurses' practice in the organization.Results: There are gaps between the espoused and enacted theories. A theater nurse's practice is determined by multiple factors, such as political, structural, social, historical, cultural issues, and so on. The hierarchy of the health context could hinder possible changes in theater nurses' practice. To better understand our practice and implement transformation, we should shape a supportive environment,bear in mind the practice motto of "patient-centered" care, and improve our knowledge and reflection skills.Conclusions: Reflection plays a significant role in the advancing of practice among theater nurses and needs to be combined with clinical practice. To provide the best service of care to perioperative patients, a theater nurse should have an insightful understanding of the factors that have influenced her/his behaviors historically, socially, and culturally. By improving their critical reflection skills,practitioners could gain knowledge from experience.

Development of an assessment and intervention protocol for postpartum hemorrhage in the mainland of China: an evidence-based method and Delphi consult

Objective: Postpartum hemorrhage(PPH) is a leading cause of maternal death. Although guidelines have been updated, those with detailed protocols are limited for nursing practice. This study aims at establishing an early assessment and intervention protocol as a toolkit for PPH for midwives and obstetrical nurses.Methods: Employing the evidence-based method, a systematic Internet search of guidelines was conducted and appraisal of literatures was conducted with AGREE system and Oxman-Guyatt Overview Quality Assessment Questionnaire(OQAQ), according to which a protocol draft was therefore developed. Then, a two-round modified Delphi method was utilized to reach a consensus of the protocol built on best practices. Selection criteria for each intervention measure included consensus level with a threshold of 70%, mean of importance(M) >3.5, and coefficient of variation(CV) <0.25. Reliability of experts' opinion was calculated by positive coefficient and authoritative coefficient. Items without consistency were enlisted in the second-round consult. When all items met the selection criteria, the protocol would be finally formulated.Results: A 122-measure protocol was established, including prevention, assessment, and intervention of PPH. With a panel of 14 experts participated in the consult, the positivity coefficient was 0.93 and 1.00 for two rounds, respectively, and the authority coefficient was 0.88. After a two-round consult and revision of the draft, the final program was formulated, containing 5 first-level indexes and 14 second-level indexes with a total item of 120.Conclusions: The PPH protocol, based on high-quality evidences, was formulated with a two-round Delphi method, which can provide insight for midwives and obstetrical nurses to effectively deal with PPH.
