Transarterial chemoembolization(TACE)is widely applied for the treatment of hepatocellular *** TACE is often required in clinical practice because a satisfactory tumor response may not be achieved with a single ***,repeated TACE procedures can impair liver function and increase treatment-related adverse events,all of which prompted the introduction of the concept of“TACE failure/refractoriness”.Mainly based on evidence from two retrospective studies conducted in Japan,sorafenib is recommended as the first choice for subsequent treatment after TACE failure/*** studies have investigated the outcomes of other subsequent treatments,including locoregional,other molecular targeted,anti-programmed death-1/anti-programed death ligand-1 therapies,and combination therapies after TACE failure/*** this review,we summarize the up-to-date information about the outcomes of several subsequent treatment modalities after TACE failure/refractoriness.

New model of PIRADS and adjusted prostatespecific antigen density of peripheral zone improves the detection rate of initial prostate biopsy:a diagnostic study

This study explored a new model of Prostate Imaging Reporting and Data System(PIRADS)and adjusted prostate-specific antigen density of peripheral zone(aPSADPZ)for predicting the occurrence of prostate cancer(PCa)and clinically significant prostate cancer(csPCa).The demographic and clinical characteristics of 853 patients were ***-specific antigen(PSA),PSA density(PSAD),PSAD of peripheral zone(PSADPZ),aPSADPZ,and peripheral zone volume ratio(PZ-ratio)were calculated and subjected to receiver operating characteristic(ROC)curve *** calibration and discrimination abilities of new nomograms were verified with the calibration curve and area under the ROC curve(AUC).The clinical benefits of these models were evaluated by decision curve analysis and clinical impact *** AUCs of PSA,PSAD,PSADPZ,aPSADPZ,and PZ-ratio were 0.669,0.762,0.659,0.812,and 0.748 for PCa diagnosis,while 0.713,0.788,0.694,0.828,and 0.735 for csPCa diagnosis,*** nomograms displayed higher net benefit and better overall calibration than the scenarios for predicting the occurrence of PCa or *** new model significantly improved the diagnostic accuracy of PCa(0.945 vs 0.830,P<0.01)and csPCa(0.937 vs 0.845,P<0.01)compared with the base *** addition,the number of patients with PCa and csPCa predicted by the new model was in good agreement with the actual number of patients with PCa and csPCa in high-risk *** study demonstrates that aPSADPZ has a higher predictive accuracy for PCa diagnosis than the conventional *** aPSADPZ with PIRADS can improve PCa diagnosis and avoid unnecessary biopsies.

Transarterial embolization for massive gastrointestinal hemorrhage following abdominal surgery

AIM:To evaluate the clinical results of angiography and embolization for massive gastrointestinal hemorrhage after abdominal ***:This retrospective study included 26 patients with postoperative hemorrhage after abdominal surgery. All patients underwent emergency transarterial angiography,and 21 patients underwent emergency embolization. We retrospectively analyzed the angiographic features and the clinical outcomes of transcatheter arterial ***:Angiography showed that a discrete bleeding focus was detected in 21(81%) of 26 *** angiographic findings included extravasations of contrast medium(n = 9),pseudoaneurysms(n =9),and fusiform aneurysms(n = 3). Transarterial embolization was technically successful in 21(95%) of 22patients. Clinical success was achieved in 18(82%) of 22 patients. No postembolization complications were observed. Three patients died of ***:The positive rate of angiographic findings in 26 patients with postoperative gastrointestinal hemorrhage was 81%. Transcatheter arterial embolization seems to be an effective and safe method in the management of postoperative gastrointestinal hemorrhage.

Background and Aims:To validate prognostic performance of the China liver cancer(CNLC)staging system as well as to compare these parameters with those of the Barcelona Clinic Liver Cancer(BCLC)staging system for Chinese hepatocellular carcinoma(HCC)treated with transarterial chemoembolization(TACE).Methods:This multicenter retrospective study included 1,124 patients with HCC between January 2012 and December 2020 from six Chinese *** on overall survival(OS),the prognostic performance outcomes for the CNLC and BCLC staging systems were compared by model discrimination[C statistic and Akaike information criterion(AIC)],monotonicity of the gradient(linear trend chi-square test),homogeneity(likelihood ratio chisquare test),and calibration(calibration plots).A prospective cohort of 44 patients receiving TACE-based therapy included between January 2021 and December 2022 was used to prospectively validate the ***:Median OS was 19.1(18.2–20.0)months,with significant differences in OS between stages defined by the CNLC and BCLC observed(p<0.001).The CNLC performed better than the BCLC regarding model discrimination(C-index:0.661 vs.0.644;AIC:10,583.28 vs.10,583.72),model monotonicity of the gradient(linear trend chi-square test:66.107 vs.57.418;p<0.001),model homogeneity(159.2 vs.158.7;p<0.001).Both staging systems had good model *** results were observed in the prospective ***:Combining model discrimination,gradient monotonicity,homogeneity,and calibration,the CNLC performed better than the BCLC for Chinese HCC patients receiving TACE.

Lower gastrointestinal bleeding:Role of 64-row computed tomographic angiography in diagnosis and therapeutic planning

AIM: To determine the value of computed tomographic angiography(CTA) for diagnosis and therapeutic planning in lower gastrointestinal(GI) ***: Sixty-three consecutive patients with acute lower GI bleeding underwent CTA before endovascular or surgical treatment. CTA was used to determine whether the lower GI bleeding was suitable for endovascular treatment, surgical resection, or conservative treatment in each patient. Treatment planning with CTA was compared with actual treatment decisions or endovascular or surgical treatment that had been carried out in each patient based on CTA ***: 64-row CTA detected active extravasation of contrast material in 57 patients and six patients had no demonstrable active bleeding, resulting in an accuracy of 90.5% in the detection of acute GI bleeding(57 of 63). In three of the six patients with no demonstrable active bleeding, active lower GI bleeding recurred within one week after CTA, and angiography revealed acute bleeding. The overall location-based accuracy, sensitivity, specificity, positive predictive value(PPV) and negative predictive value(NPV) for the detection of GI bleeding by 64-row CTA were 98.8%(249 of 252), 95.0%(57 of 60), 100%(192 of 192), 100%(57 of 57), and 98.5%(192 of 195), respectively. Treatment planning was correctly established on the basis of 64-row CTA with an accuracy, sensitivity, specificity, PPV and NPV of 98.4%(248 of 252), 93.3%(56 of 60), 100%(192 of 192), 100%(56 of 56), and 97.5%(192 of 196), respectively, in a location-based evaluation. CONCLUSION: 64-row CTA is safe and effective in making decisions regarding treatment, without performing digital subtraction angiography or surgery, in the majority of patients with lower GI bleeding.

This study aims to conduct a cost-effectiveness analysis of three different anesthesia strategies,namely chatting while under local anesthesia(Chat-LA),total intravenous anesthesia(TIVA),and general anesthesia with laryngeal mask airway(GA-LMA),employed in transperineal magnetic resonance imaging(MRI)/ultrasound(US)fusion prostate biopsy(TP-MUF-PB).A retrospective study was conducted involving 1202 patients who underwent TP-MUF-PB from June 2016 to April 2023 at The first affiliated hospital of soochow university(Suzhou,China).Clinical data and outcomes,including total costs,complications,and quality-adjusted life years(QALYs),were *** sensitivity and subgroup analyses were also ***-LA was found to be the most cost-effective option,outperforming both TIVA and ***,subgroup analyses revealed that in younger patients(under 65 years old)and those with smaller prostate volumes(<40 ml),TIVA emerged as a more cost-effective *** Chat-LA may generally be the most cost-effective and safer anesthesia method for TP-MUF-PB,personalization of anesthesia strategies is crucial,considering specific patient demographics such as age and prostate volume.
