BACKGROUND Diarrhoea is a frequent symptom in children with cancer, and occurs due to a composite effect of underlying disease and immunosuppression consequent to therapy, malnutrition, and non-infective aetiologies such as mucositis. In a large proportion of cases, the aetiology of diarrhoea remains unknown but is often attributed to multiple pathogens including *** To identify and describe the pathogens causing diarrhoea in Bangladeshi children with *** Two cross-sectional pilot studies were conducted involving paediatric oncology patients with diarrhoea. Stool samples were collected from children who were hospitalised with or without being treated with chemotherapy during the study period, and had diarrhoea at any stage during their admission. In the first study,stool samples were tested by conventional microbiological methods and by polymerase chain reaction for parasites, and by immunoassays for Clostridium difficile. In the second study, conventional microbiology was conducted for bacteria and parasites including an enzyme-linked immunosorbent assay for Cryptosporidium antigen, and in a subset, immunoassays for Clostridium *** In the first study Giardia lamblia was detected in 68.5% of samples, Entamoeba histolytica in 13%, Cryptosporidium in 5.6%, non-toxigenic C. difficile in 22.4%, and other bacteria in 5.2%. In the second study, E. histolytica was detected in 10% of samples, Cryptosporidium in 4.3%, G. lamblia in 1.4%, C. difficile in 5.1%, and other bacteria in 5.7% of *** These pilot data suggest that parasites are important aetiologies of diarrhoea in Bangladeshi children with malignancy. While molecular diagnostic tools detect an array of stool pathogens with greater sensitivity, conventional diagnostic methods are also useful.

Objective:Intermittent hypoxia(IH),as the most important pathophysiologic characteristic of obstructive sleep apnea(OSA) and a recognized risk factor for cardiovascular disorders,may lead to proliferation of smooth muscle cells(SMCs) and then pulmonary arterial remodeling through the RhoA/ROCK signaling *** this preliminary study,an IH animal model was developed,and the muscularization of small pulmonary arteries and RhoA/ROCK levels in these rats exposed to IH were ***:Model rats were exposed to IH[cycles of hypoxia(30s of 5%O) and normoxia(90s of 21%O)]or intermittent normoxia[cycles of normoxia(30s of 21%O) and normoxia(90s of 21%O)]for 4 weeks,9 AM to 5 PM in every *** of asmooth muscle actin(a-SMA) and proliferating cell nuclear antigen(PCNA) were used to measure muscularization and status of cell proliferation in small pulmonary ***-time polymerase chain reaction(RT-PCR) and Western blot analyses were used to assess levels of RhoA and ROCK ***:A significantly increased ratio of a-SMA positive muscularized vessels and higher proliferative rate were observed in IH group compared with Control group(P < 0.05).Levels of RhoA/ROCK mRNA and protein in IH group were significantly higher than those in Control group(P < 0.05).Conclusion:Our preliminary data implicated that IH may induce proliferation of SMCs and then pulmonary arterial remodeling through the RhoA/ROCK signaling pathway,which may involve the development of PH.

Aim: To investigate the expression levels of receptor activator of NF-κB ligand (RANKL) and oste-oprotegerin (OPG) in human periodontal ligament fibroblasts when stimulated with heat in infective conditions. Methods: Periodontal ligament fibroblasts were subjected to various temperature increases for 5 min with or without 10 ng/mL lipopolysaccharide (LPS) and then maintained at 37℃ for 6 h. After that, the expression levels of RANKL and OPG were investigated using real-time RT-PCR and ELISA. As a positive or negative control, the cells were cultured at 37℃ with or without 10 ng/mL LPS. Data were analyzed using one-way ANOVA at a significant level of p = 0.05. Results: The mRNA expression levels of RANKL and OPG were both down-regulated when the cells were heated in infective conditions. The release of sRANKL was increased at low temperatures in such infection;while at high temperatures heat treatment down-regulated the release of sRANKL induced by LPS. The relative RANKL/OPG expression ratios were increased at low temperatures in infective conditions. Conclusions: The interactions between heat and LPS would affect the balance between RANKL and OPG in periodontal ligament fibroblasts when they were heated in infective conditions. Such infection may be the reason why bone resorption occurs in the local area after warm vertical compaction.

Persistent high-risk human papillomavirus (HR-HPV) infection is associated with anogenital and head & neck squamous epithelial (HNSCC) tumors, which altogether cause about 550,000 new cases every year. Several evidences suggest that the microbiota could have a role on the inflammatory, epithelial mesenchymal transition and tumorigenesis processes promoted by HR-HPV infection, yet the mechanisms involved remain to be clarified. In this review we report the state of the art on this topic and on the most promising in vitro developed models for studying the host-pathogen interactions. Using MEDLINE, several terms were searched and combined to select the most pertinent papers. The investigation was limited to the international indexed articles published in PubMed in the last 10 years. This review reports the latest knowledge in the field of the microbial-associated anogenital tumors and HNSCC. In addition, we also discuss the in vitro epithelial culture systems that reproduce the pathophysiological features of the tumoral microenvironment and the in vivo response to microbial agents, thus representing a useful tool for analyzing at cellular and molecular levels the role played by infective agents in tumorigenesis.

Clinical Characteristics and Follow-Up Data in a Series of Twenty infective Endocarditis Complicated of Rheumatic Manifestations

The infective endocarditis is a septicemia complicated of many systemic manifestations. Rheumatic manifestations can be revelatory, leading sometimes to a lateness diagnosis. The aim of this study is to determine the epidemiological, clinical, bacteriological profile and the follow up in patients affected of an infective endocarditis complicated of rheumatic manifestations. It’s a retrospective study concerning the period from January 1990 to December 2015. The analysis had implicated epidemiological, clinical, biological data, in association to immunological, bacteriological and radiological examinations. All the patients fulfilled the revised Duke’s criteria for the infective endocarditis. Articular and osseous radiographs, blood culture, transthoracic and/or transoesophageal echocardiography were performed in all patients. Twenty patients had rheumatic manifestations. The mean age was 37 years. There were arthralgia in 15 cases, myalgia in 5 cases and arthritis in 8 cases: 6 cases of mono-arthritis and 2 cases of oligo-arthritis. All patients had an inflammatory biological syndrome. The blood culture was positive in all the cases. Echocardiography revealed vegetations in all the patients: mitral in 10 cases, aortic in 4 cases, mitral and aortic in 4 cases, aortic and tricuspid in 1 case, tricuspid in 1 case. The radiological examination found 2 cases of spondylodiscitis and a case of saco-iliitis. The articular outcome was favorable in 17 cases. Three patients had died. The infective endocarditis should be considered in case of febrile arthritis. An early diagnosis and adapted treatment are guarantors of better prognosis.

Introduction: In emerging countries where studies are rarer, the epidemiological profile of infective endocarditis resembles that of developed countries at the beginning of the antibiotic era. There is little data available in Mali on this subject, motivating this study, the aim of which was to study the etiological factors of infective endocarditis in children aged between 1 month and 15 years hospitalised in the paediatric department of the CHU Gabriel Touré. Patients and Methods: We conducted a cross-sectional and descriptive study, which took place from August 2018 to August 2019 (13 months), in children aged between 1 month and 15 years hospitalised in the paediatric department of the Gabriel Touré University Hospital with an ultrasound- confirmed cardiac disease and a temperature above 38°C. Results: The hospitalization rate was 0.4%. The average age was 11 months, the sex ratio was 1.35. Respiratory distress (63.6%) and fever (30.3%) were the most common reasons for referral. Congenital heart disease (87.9%) and dyspnoea on feeding (24.2%) were the most common medical histories. IVC was the most frequent congenital heart disease (36.4%). The mean temperature was 39.1°C. Blood cultures were taken in 24 patients (72.7%) and were positive in 27.3%. Staphylococcus aureus was found in 55.6% of cases. Trans-thoracic ultrasound found vegetations in 03 cases. According to the modified DUKE criteria, the diagnosis of endocarditis was retained in 27.30%. The evolution was marked by a high case fatality rate (45.45%). Conclusion: Endocarditis is an uncommon disease in children with a pulmonary portal of entry and Staphylococcus aureus as the main germ. Its mortality remains high.
