Objective hyperbaric oxygen treatment(HBOT)has demonstrated efficacy in improving hearing levels of patients with idiopathic sudden sensorineural hearing loss(ISSHL);however,the underlying mechanisms are not well understood.HBOT alleviates the inflammatory response,which is mediated by Toll-like receptor(TLR)4 and nuclear factor(NF)-κB.In this study we investigated whether HBOT attenuates inflammation in ISHHL patients via alteration of TLR4 and NF-κB expression.Methods ISHHL patients(n=120)and healthy control subjects(n=20)were enrolled in this study.Patients were randomly divided into medicine group treated with medicine only(n=60)and HBO group receiving both HBOT and medicine(n=60).Audiometric testing was performed pre-and posttreatment.TLR4,NF-кB,and TNF-αexpression in peripheral blood of ISSHL patients and healthy control subjects was assessed by ELISA before and after treatment.Results TLR4,NF-κB,and TNF-αlevels were upregulated in ISSHL patients relative to healthy control subjects;the levels were decreased following treatment and were lower in the HBO group than that in the medicine group post-treatment(P<0.05 and P<0.01).Conclusion HBOT alleviates hearing loss in ISSHL patients by suppressing the inflammatory response induced by TLR4 and NF-κB signaling.

Objective:We launched the fourth national hyperbaric oxygen(HBO)medicine week publicity campaign in China to increase patients’and medical professionals’understanding of HBO medicine and expand the influence of HBO.Methods:The theme of 2019 was“oxygen protects life,healthy China”.The main components of the campaign were free medical consultation,free HBO experience,and telemedicine lectures.At least one component was required for each participating hospital.Results:(1)In total,195 hospitals in 26 provinces,autonomous regions,and municipalities directly under the central government participated in the campaign.Shandong province of China had the highest number of hospitals in a single area(48 hospitals);(2)The free medical consultation was organized by the HBO department.In some hospitals,other medical professionals were also invited.Among all hospitals,up to 13 different HBO-related departments participated;(3)The free HBO experiences were held in some hospitals,and the number of experiences ranged from one to three per person;(4)Three telemedicine lectures involving a total of 8 live interactions were held in 55 different hospitals.Lecture videos were provided to play locally in hospitals without the setting necessary for live interactions.Conclusion:Our campaigns created a brand effect beginning in 2016.In the herein-described 2019 study,we considered the previous three campaigns and explored different methods of publicizing HBO medicine in China,including telemedicine.

This guidance is provided for reference to hyperbaric oxygen staffs during the epidemic.

Objective To investigate the current status of management of artificial airway cuff pressure in patients receiving mechanical ventilation in hyperbaric oxygen chamber in China.Methods From November to December in 2021，a self-developed electronic questionnaire was employed to investigate medical staff in the department of hyperbaric oxygen. The survey was conducted among medical staff from 15 Chinese hospitals that provided mechanical ventilation in hyperbaric oxygen chamber. This study focused on four aspects：the general data of the department and medical staff，the actual status of cuff pressure control，the satisfaction levels of medical staff regarding cuff pressure control methods，and their willingness to change the methods. Results Grade-A tertiary hospitals accounted for 86.7% among the surveyed hospitals. The treatment times of mechanical ventilation in hyperbaric oxygen chamber was（25.9 ± 39.5）cases per year. The average working years of service in hyperbaric oxygen department of the medical staff were（14.9 ± 7.49）years. All the surveyed departments had the requirements of cuff pressure control and had regulatory regime related to cuff pressure. The departments carrying out regular training and assessments on cuff pressure monitoring accounted for 53.3%. The departments that had specialized equipment for cuff pressure monitoring accounted for 46.7%. The medical staff estimating the cuff pressure by finger touch accounted for 63.4%. The cuff pressure inside chamber being maintained within the range of 25-30 cmH2O accounted for 19.5%，while the pressure being maintained within other ranges accounted for 17.1%. The satisfaction levels with different cuff pressure control methods ranked from high to low as follows：automatic cuff pressure controller，cuff pressure gauge，syringe inflation and deflation method，and replacement with equal volume saline. Acute carbon monoxide poisoning in Shandong, China: an observational study

Background: Carbon monoxide (CO) poisoning remains a major cause of accidental injuries and multiple studies have indicated that CO is also associated with significantly severe or long-term toxicity to the central nervous system. Given that CO poisoning causes serious morbidity and mortality, a better understanding of epidemiological features and clinical characteristics of acute CO poisoning in China is crucial.Methods: We collected the clinical data of acute CO poisoning in patients between November 2019 and April 2020 across Shandong province, China and analyzed its characteristics focusing on the weekly amount and the severity of the confirmed cases.Results: A total number of 21,088 acute CO poisoning cases were diagnosed. The overall incidence of acute CO poisoning was approximately 0.021%. On severity rankings, 63% of confirmed cases (n=13,378) were mild, 27%(n=5635) were moderate, and 10% (n=2075) were severe. Interestingly, the coastal cities had more confirmed cases than the inland/suburban areas in Shandong. Meanwhile, the number of confirmed cases was negatively correlated with the local mean daily temperature (P=0.0167).Conclusions: Mild acute CO poisoning cases accounted for the majority of all confirmed cases during the winter of 2019. In Shandong province, which is located in east China, residents of the coastal cities are more susceptible to CO poisoning than residents of inland cities.

Investigation of Myocardial Stunning after Cardiopulmonary Resuscitation in Pigs

Objective To investigate cardiac function and myocardial perfusion during 48 h after cardiopulmonary resuscitation （CPR）, further to test myocardial stunning and seek indicators for long‐term survival after CPR. Methods After 4 min of untreated ventricular fibrillation, fifteen anesthetized pigs were studied at baseline and 2 h, 4 h, 24 h, and 48 h after restoration of spontaneous circulation （ROSC）. Hemodynamic data, echocardiography and gated‐single photon emission computed tomography myocardial perfusion images were carried out. Results Mean arterial pressure （MAP）, coronary perfusion pressure （CPP） and cardiac troponin I （CTNI） showed significant differences between eventual survival animals and non‐survival animals at 4 h after ROSC （109.2±10.7 mmHg vs. 94.8±12.3 mmHg, P=0.048; 100.8±6.9 mmHg vs. 84.4±12.6 mmHg, P=0.011; 1.60±0.13 ug/L vs. 1.75±0.10 ug/L, P=0.046）. Mitral valve early‐to‐late diastolic peak velocity ratio, mitral valve deceleration time recovered 24 h; ejection faction and the summed rest score recovered 48 h after ROSC. Conclusion Cardiac systolic and early active relaxation dysfunctions were reversible within survival animals; cardiac stunning might be potentially adaptive and protective after CPR. The recovery of MAP, CPP, and CTNI could be the indicators for long‐term survival after CPR.

Neuroprotective Effect of Shenfu Injection (参附注射液) Following Cardiac Arrest in Pig Correlates with Improved Mitochondrial Function and Cerebral Glucose Uptake

Objective：To test whether Shenfu Injection（参附注射液,SFI）might attenuate the impact of cerebral energy dysfunction after resuscitation in a pig model of cardiac arrest（CA）.Methods：Thirty-four Wuzhishan miniature inbred pigs were randomly divided into three groups：the SFI group（n=12）,the saline group（SA group,n=12）,and the sham-operated group（sham group,n=10）.Following successful return of spontaneous circulation（ROSC）from 8-min untreated ventricular fibrillation,animals received a continuous infusion of either SFI（0.2 mL/min）or saline for 6 h.Cerebral performance category score was evaluated at 24and 48 h after ROSC,followed by positron emission tomography and computed tomography scans of cerebral glucose uptake.Surviving pigs were euthanized 48 h after ROSC,and the brains were removed for detecting mitochondrial function.Results：Compared with the SA group,SFI treatment produced a better neurologic outcome48 h after ROSC（P〈0.05）.However,there was no significant difference of survival rate between the SA and SFI groups（83.3%vs.81.8%,P〉0.05）.After ROSC,the SA group showed a decrease in the maximum standardized uptake value of different regions in the brain tissue,where SFI treatment can ameliorate these decreases（P〈0.01or P〈0.05）.Improved mitochondrial respiratory properties and higher mitochondrial membrane potential were also found following SFI treatment compared with the SA group at 48 h after ROSC（P〈0.05 or P〈0.01）.Conclusion：SFI treatment after resuscitation has significant neuroprotective effects against disruption of cerebral energy metabolism from CA by improving glucose uptake and by normalizing mitochondrial function.

Many hypotheses exist regarding the mechanism underlying delayed encephalopathy after acute carbon monoxide poisoning(DEACMP),including the inflammation and immune-mediated damage hypothesis and the cellular apoptosis and direct neuronal toxicity hypothesis;however,no existing hypothesis provides a satisfactory explanation for the complex clinical processes observed in DEACMP.Leucine-rich repeat and immunoglobulin-like domain-containing protein-1(LINGO-1)activates the Ras homolog gene family member A(Rho A)/Rho-associated coiled-coil containing protein kinase 2(ROCK2)signaling pathway,which negatively regulates oligodendrocyte myelination,axonal growth,and neuronal survival,causing myelin damage and participating in the pathophysiological processes associated with many central nervous system diseases.However,whether LINGO-1 is involved in DEACMP remains unclear.A DEACMP model was established in rats by allowing them to inhale 1000 ppm carbon monoxide gas for 40 minutes,followed by 3000 ppm carbon monoxide gas for an additional 20 minutes.The results showed that compared with control rats,DEACMP rats showed significantly increased water maze latency and increased protein and m RNA expression levels of LINGO-1,Rho A,and ROCK2 in the brain.Compared with normal rats,significant increases in injured neurons in the hippocampus and myelin sheath damage in the lateral geniculate body were observed in DEACMP rats.From days 1 to 21 after DEACMP,the intraperitoneal injection of retinoic acid(10 mg/kg),which can inhibit LINGO-1 expression,was able to improve the above changes observed in the DEACMP model.Therefore,the overexpression of LINGO-1 appeared to increase following carbon monoxide poisoning,activating the Rho A/ROCK2 signaling pathway,which may be an important pathophysiological mechanism underlying DEACMP.This study was reviewed and approved by the Medical Ethics Committee of Xiangya Hospital of Central South Hospital(approval No.201612684)on December 26,2016.

Background:Microribose nucleic acids(miRNAs)are implicated in the progression of lung adenocarcinoma.MicroRNA-345-5p(miR-345-5p)is a recently identified anti-oncogene in some human cancers,but its functional role and possible molecular mechanism in lung adenocarcinoma remain unknown.This study aimed to identify the biological function and underlying mechanism of miR-345-5p in lung adenocarcinoma cells.Methods:In this study,lung adenocarcinoma tissues and adjacent tissues were collected in the First Affiliated Hospital of Anhui Medical University between April 2016 and February 2017.The expression of miR-345-5p and ras homolog family member A(RhoA)in lung adenocarcinoma tissues and human lung adenocarcinoma cell lines(A549,H1650,PC-9,and H441)was detected by reverse transcription quantitative polymerase chain reaction analysis.Functional assays including colony formation,flow cytometry analysis,wound healing,and transwell assays were performed to assess the proliferation,apoptosis,migration,and invasion of lung adenocarcinoma cells.In addition,RNA pulldown and luciferase reporter assays were conducted to evaluate the relationship between miR-345-5p and RhoA.Difference between the two groups was analyzed with Student’st test,while that among multiple groups was analyzed with one-way analysis of variance.Results:MiR-345-5p expression displayed lower level in lung adenocarcinoma tissues(0.241±0.095vs.1.000±0.233,t=19.247,P<0.001)and cell lines(F=56.992,P<0.001)than control tissues and cells.Functional experiments demonstrated that upregulation of miR-345-5p inhibited the malignant phenotypes of lung adenocarcinoma cells via suppressing cell proliferation,migration,invasion,and facilitating cell apoptosis.Additionally,RhoA was verified to be the downstream target of miR-345-5p.Expression of RhoA was downregulated by overexpression of miR-345-5p in PC-9(0.321±0.047vs.1.000±0.127,t=8.536,P<0.001)and H1650(0.398±0.054vs.1.000±0.156,t=4.429,P=0.011)cells.Rescue assays revealed that overexpression of RhoA rescued the suppr
