BACKGROUND Severe acute pancreatitis(SAP)is a familiar critical disease in the intensive care unit(ICU)*** staff are important spiritual pillars during the treatment of patients,and in addition to routine nursing,more attention needs be paid to the patient’s psychological *** To investigate the effects of psychological intervention in ICU patients with *** One hundred ICU patients with SAP were hospitalized in the authors’hospital between 2020 and 2023 were selected,and divided into observation and control groups per the hospitalization *** control and observation groups received routine nursing and psychological interventions,*** groups are being compared,using the Self-rating Anxiety Scale(SAS),Self-Determination Scale(SDS),Acute Physiology and Chronic Health Evaluation(APACHE)Ⅱ,and 36-item Short Form Health Survey(SF-36)scores;nursing satisfaction of patients;ICU care duration;length of stay;hospitalization expenses;and the incidence of *** After nursing,the SDS,SAS,and APACHEⅡ scores in the experimental group were significantly lower than in the control group(Pnursing satisfaction of patients in the experimental group was 94.5%,considerably higher than that of 75.6% in the control group(P<0.05).The ICU care duration,length of stay,and hospitalization expenses in the observation group were significantly lower than those in the control group,and the incidence of complications was lower(P<0.05).CONCLUSION For patients with SAP,the implementation of standardized psychological intervention measures can effectively alleviate adverse psychological conditions.

目的从医护人员的视角评价上海市已故肿瘤患者的善终质量现状,为提高安宁疗护服务水平提供科学依据。方法采用横断面调查法,于2023年4月-7月,通过方便抽样邀请上海市2所三级医院、2所二级医院以及4所社区卫生服务中心的261名有照顾临终肿瘤患者经历的医护人员参与调查。采用一般资料问卷及基于健康照护视角的优逝量表(The Good Death Scale for Patients in China, GDS-PCN)对其进行调查。GDS-PCN包括专业照护、家人陪伴、有备无憾、平静离世、保持尊严、维持控制、躯体安适7个维度,共36个条目。每名参与调查的医护人员使用问卷评价其最近照顾的1例已故肿瘤患者的死亡情况,共评价261例已故肿瘤患者的离世情况。结果 GDS-PCN量表总分为(144.11±17.86)分,其中专业照护维度平均分最高为(4.21±0.58)分,有备无憾维度和维持控制维度得分较低,分别为(3.75±0.70)分和(3.86±0.70)分。不同医疗机构等级、病房类型、住院时间、与医护人员之间沟通(病情、治疗方案和死亡相关话题)情况、治疗方案决策类型的患者量表得分的比较,差异有统计学意义(P<0.05)。曾在社区卫生服务中心住院的患者的得分高于三级医院的患者(P<0.05);安宁疗护病房的患者得分高于非安宁疗护病房的患者(P<0.05);住院15天以上患者的得分高于住院少于3天的患者(P<0.05);与医护人员沟通部分或全部病情的患者得分高于从未沟通过的患者(P<0.05);与医护人员讨论部分、大部分或更多的治疗计划的患者得分分别高于从未讨论过治疗计划的患者(P<0.05)和仅有限沟通过治疗计划的患者(P<0.05)。与医护人员部分谈论过死亡相关话题的患者的得分高于从未谈论过的患者(P<0.05);而与家庭成员共同参与治疗决策的患者的得分高于仅家庭成员参与决策的患者(P<0.05)。结论上海市肿瘤患者的善终情况总体处于中高等水平。提高安宁疗护资源利用、加强医护人员与患者沟通临终事宜是提高患者善终水平的有效途径。未来需开展更多的研究,从多角度探究中国患者的善终质量。

Factors Associated with Neonatal Sepsis: A Case Study at Chilenje General Hospital in the Neonatal Unit and Paediatric Wards

Background: Neonatal sepsis is known to result in 26% of mortalities in children below the age of five worldwide, countries in Sub-Saharan African recording the highest deaths. Although the deaths of neonates have reduced world over up to 3.6 million counts, Chilenje General Hospital continues to register significant numbers of neonatal sepsis. This study aimed at investigating the factors associated with neonatal sepsis at Chilenje General Hospital in Lusaka, Zambia. Methods: An institution based cross-sectional study design was conducted at Chilenje General Hospital. Data were collected by the use of a researcher designed questionnaire and analysed using SPSS version 25. Frequencies were used for descriptive analysis while bivariate analysis was used to establish association among risk factors and outcome variables. Variables with significant association at 5% level were further subjected to multivariate analysis at α ≤ 0.05. Results: The study showed that out of 156 neonates, 40.4% (63/156) developed neonatal sepsis. Maternal factors that projected the incidence of sepsis amongst neonates were distance to nearest health facility [AOR: 6.3 (95% CI: 1.8 - 21.3), p = 0.003], occupation [AOR: 5.8 (95% CI: 1.2 - 27.6), p = 0.026], number of antenatal visits [AOR: 6.3 (95% CI: 1.9 - 21.6), p = 0.003], number of vaginal examinations [AOR: 10.8 (95% CI: 2.8 - 42), p = 0.001], and pregnancy induced hypertension [AOR: 5.4 (95% CI: 1.4 - 20), p = 0.013]. Neonatal risk factor which projected the incidence of sepsis was Neonate’s age [AOR: 18.8 (95% CI: 4.9 - 72.5), p = 0.000]. Conclusions: The chance of developing neonatal sepsis was strongly correlated with both mother and child variables, according to the study. In order to lower the chance of the neonate acquiring sepsis, encouraging maternal antenatal care use would assist to detect risk factors during prenatal, perinatal and postnatal care and apply the proper therapies.

Effect of rapid rehabilitation nursing on improving clinical outcomes in postoperative patients with colorectal cancer

BACKGROUND Surgical resection is the cornerstone treatment for colorectal *** rehabilitation care predicated on evidence-based medical theory aims to improve postoperative nursing care,subsequently reducing the physical and mental traumatic stress response and helping patients who undergo surgery recover *** To assess the effect of rapid rehabilitation care on clinical outcomes,including overall postoperative complications,anastomotic leaks,wound infections,and intestinal obstruction in patients with colorectal *** We searched the PubMed,Web of Science,Embase,Elsevier Science Direct,and Springer Link databases from January 1,2010,to January 1,2024,to screen eligible studies on rapid rehabilitation care among patients who underwent colorectal cancer *** were screened based on the inclusion and exclusion *** 5.4 software was used for statistical analysis of the *** Twelve studies were enrolled,which included 2420 *** results showed that rapid rehabilitation care decreased the incidence of overall postoperative complications(OR:0.44,95%CI:0.26–0.74,P=0.002),anastomotic leaks(OR:0.68,95%CI:0.41–1.12,P=0.13),wound infections(OR:0.45,95%CI:0.29–0.72,P=0.0007),and intestinal obstruction(OR:0.54,95%CI:0.34–0.86,P=0.01)compared to conventional *** trials and studies are needed to confirm these *** Rapid rehabilitation care decreased the occurrence of postoperative complications,anastomotic leaks,wound infections,and intestinal obstruction compared to conventional care in patients who underwent colorectal ***,promoting the application of rapid rehabilitation care in clinical practice cannot be overemphasized.
