hepatocellular carcinoma(HCC)is the third leading cause of cancer-related deaths worldwide(1).It often arises from the backgrounds of damaged liver due to viral hepatitis,alcoholic,or non-alcoholic liver *** the proportion of patients with HCC of non-hepatitis B and C origins has increased in recent years,hepatitis B and C viruses still cause more than half of all carcinogenesis in patients with HCC(2).Patients with HCC often develop portal hypertension(PHT)from cirrhosis due to the deterioration of the background liver,which proves to be the greatest obstacle to various types of treatments for HCC.

Background:Despite advances in the diagnosis of patients with hepatocellular carcinoma(HCC),70%-80%of patients are diagnosed with advanced stage *** vein tumor thrombus(PVTT)is among the most ominous signs of advanced stage disease and has been associated with poor survival if *** sources:A systematic search of MEDLINE(PubMed),Embase,Cochrane Library and Database for Systematic Reviews(CDSR),Google Scholar,and National Institute for Health and Clinical Excellence(NICE)databases until December 2022 was conducted using free text and MeSH terms:hepatocellular carcinoma,portal vein tumor thrombus,portal vein thrombosis,vascular invasion,liver and/or hepatic resection,liver transplantation,and systematic ***:Centers of surgical excellence have reported promising results related to the individualized surgical management of portal thrombus versus arterial chemoembolization or systemic *** elements to the individualized surgical management of HCC and portal thrombus include precise classification of the portal vein tumor thrombus,accurate identification of the subgroups of patients who may benefit from resection,as well as meticulous surgical *** review addressed five specific areas:(a)formation of PVTT;(b)classifications of PVTT;(c)controversies related to clinical guidelines;(d)surgical treatments versus non-surgical approaches;and(e)characterization of surgical techniques correlated with classifications of ***:Current evidence from Chinese and Japanese high-volume centers demonstrated that patients with HCC and associated PVTT can be managed with surgical resection with acceptable results.

We read with great interest the paper from Jena(Germany)recently published in hepatobiliary surgery and Nutrition by Deeb et al.,which caught our attention,given its clinical *** fact,post-hepatectomy liver failure(PHLF)remains nowadays a significant clinical challenge,occasionally leading to death or potentially requiring,in highly selected cases,rescue liver transplantation(1).Within quaternary care centers,the incidence of PHLF fluctuates from 5%to 20%,depending on the criteria employed,the underlying liver parenchyma quality and the type of hepatectomy(2).Hence,liver regeneration(LR)holds paramount importance in hepatic surgery as it directly impacts post-operative outcomes(3):it is a strictly orchestrated phenomenon that entails the activation of hepatocytes and hepatic progenitor cells,as well as the regulation of cell-cycle genes and growth *** contributing to PHLF include pre-operative,intra-operative and post-operative elements,leading to careful patient selection(4).
