BACKGROUND Nonalcoholic fatty liver disease(NAFLD)is one of the most common chronic liver diseases in children and adolescents.NAFLD ranges in severity from isolated hepatic steatosis to nonalcoholic steatohepatitis(NASH),wherein hepatocellular inflammation and/or fibrosis coexist with steatosis.Circulating microRNA(miRNA)levels have been suggested to be altered in NAFLD,but the extent to which miRNA are related to NAFLD features remains unknown.This analysis tested the hypothesis that plasma miRNAs are significantly associated with histological features of NAFLD in adolescents.AIM To investigate the relationship between plasma miRNA expression and NAFLD features among adolescents with NAFLD.METHODS This study included 81 adolescents diagnosed with NAFLD and 54 adolescents without NAFLD from the Teen-Longitudinal Assessment of Bariatric Surgery study.Intra-operative core liver biopsies were collected from participants and used to characterize histological features of NAFLD.Plasma samples were collected during surgery for miRNA profiling.A total of 843 plasma miRNAs were profiled using the HTG EdgeSeq platform.We examined associations of plasma miRNAs and NAFLD features using logistic regression after adjusting for age,sex,race,and other key covariates.Ingenuity Pathways Analysis was used to identify biological functions of miRNAs that were associated with multiple histological features of NAFLD.RESULTS We identified 16 upregulated plasma miRNAs,including miR-193a-5p and miR-193b-5p,and 22 downregulated plasma miRNAs,including miR-1282 and miR-6734-5p,in adolescents with NAFLD.Moreover,52,16,15,and 9 plasma miRNAs were associated with NASH,fibrosis,ballooning degeneration,and lobular inflammation,respectively.Collectively,16 miRNAs were associated with two or more histological features of NAFLD.Among those miRNAs,miR-411-5p was downregulated in NASH,ballooning,and fibrosis,while miR-122-5p,miR-1343-5p,miR-193a-5p,miR-193b-5p,and miR-7845-5p were consistently and positively associated with all histological features

Modeling the impact of changing sexual behaviors with opposite-sex partners and STI testing among women and men ages 15-44 on STI diagnosis rates in the United States 2012-2019

Objective:To estimate the potential contributions of reported changes in frequency of penile-vaginal sex(PVS),condom use and STI screening to changes in gonorrhea and chlamydial diagnoses from 2012 to 2019.Methods:An agent-based model of the heterosexual population in the U.S.simulated the STI epidemics.Baseline was calibrated to 2012 diagnosis rates,testing,condom use,and frequency of PVS.Counterfactuals used behaviors from the 2017-2019 NSFG,and we evaluated changes in diagnosis and incidence rates in 2019.Results:Higher testing rates increased gonorrhea and chlamydia diagnosis by 14%and 13%,respectively,but did not reduce incidence.Declining frequency of PVS reduced the diagnosis rate for gonorrhea and chlamydia 6%and 3%respectively while reducing incidence by 10%and 9%respectively.Declining condom use had negligible impact on diagnosis and incidence.Conclusion:Understanding how changing behavior drives STI incidence is essential to addressing the growing epidemics.Changes in testing and frequency of PVS likely contributed to some,but not all,of the changes in diagnoses.More research is needed to understand the context within which changing sexual behavior and testing are occurring.

There was no global guidance or agreement regarding when a country has an adequate system to report on the service packages among human immunodeficiency virus(hiv)key populations.This article describes an approach to categorizing the system in a country for reporting the service package among hiv key populations.The approach consists of four dimensions,namely the epidemiological significance,comprehensiveness of the service packages,geographic coverage of services,and adequacy of the monitoring system.The proposed categorization approach utilizes available information and can inform the improvement of the service delivery and monitoring systems among hiv key populations.

Uptake of hepatitis C direct-acting antiviral treatment in China:a retrospective study from 2017 to 2021

Background Direct-acting antivirals(DAAs)for hepatitis C treatment in China became available since 2017.This study expects to generate evidence to inform decision-making in a nationwide scale-up of DAA treatment in China.Methods We described the number of standard DAA treatment at both national and provincial levels in China from 2017 to 2021 based on the China Hospital Pharmacy Audit(CHPA)data.We performed interrupted time series analysis to estimate the level and trend changes of the monthly number of standard DAA treatment at national level.We also adopted the latent class trajectory model(LCTM)to form clusters of the provincial-level administrative divisions(PLADs)with similar levels and trends of number of treatment,and to explore the potential enablers of the scale-up of DAA treatment at provincial level.Results The number of 3-month standard DAA treatment at national level increased from 104 in the last two quarters of 2017 to 49,592 in the year of 2021.The estimated DAA treatment rates in China were 1.9%and 0.7%in 2020 and 2021,which is far below the global target of 80%.The national price negotiation at the end of 2019 resulted in DAA inclusion by the national health insurance in January 2020.In that month,the number of treatment increased 3668 person-times(Phepatitis service delivery with prevention and control programme of hepatitis C within the existing services demonstrated earlier and faster scale-up of treatment.Conclusions Central negotiations to reduce prices of DAAs resulted in inclusion of DAA treatment under the universal health insurance,which are critical elements that support scaling up access to hepatitis C treatment in China.However,the current treatment rates are still far below the global target.Targeting the PLADs lagged behind through raising public awareness,strengtheni

China＇s rapidly evolving hiv/AIDS epidemic calls for a dramatic expansion of both prevention and treatment services. Official state media recently reported that for the first time, in 2008, hiv/AIDS became China＇s leading cause of death among infectious diseases. Estimates from the Ministry of Health indicate that around 700 000 people were living with hiv and 85 000 people had AIDS in 2007. Initially, hiv-1 infection was confined primarily to certain high-risk populations such as injection drug users （IDU） along drug-trafficking routes, and former plasma donors （FPD） in rural communities in east-central China. Now, however, hiv prevalence is increasing among female sex workers （FSW） and men who have sex with men （MSM）.

Therapeutic Itinerary of Patients Living with Viral hepatitis B and C in Guinea

Introduction: The therapeutic pathway affects the patient who changes care facilities. This change is linked to factors such as financial means, geographical accessibility of care facilities, quality of care and the socio-cultural perception of the patient. In countries with limited resources such as Guinea, the therapeutic itinerary remains multiple and hampered by obstacles. The objective of this study was to describe the therapeutic itinerary of patients living with hepatitis B and C at the Donka National Hospital. Material and methods: This was a retrospective, descriptive study lasting 43 months (24 January 2017 - 27 August 2020);it focused on patients living with hepatitis B or C or viral cirrhosis. We collected sociodemographic, clinical and treatment history variables. The data were analysed with SPSS software version 21.0. Results: Out of 5400 patients, the proportion of viral hepatitis B or C represented 393 patients (7.3%). The mean age of our patients was 35 ± 10 years, with extremes of 16 - 77 years. The sex ratio was 2.5. Three hundred and forty-two patients (87%) had recourse to modern medicine, mostly in private facilities (50.6%). One hundred and twenty-five patients (31.8%) had received previous treatment, half of whom (57.6%) had received inappropriate treatment. The delay in treatment was long in 45% of cases. The main reasons for delaying treatment were feeling unwell in 33.3%;not knowing to consult a specialist in 29.9% and feeling that they were being treated with useful drugs in 24.3%. Conclusion: The lack of training of general practitioners on the management of viral hepatitis and the scarcity of awareness campaigns on viral hepatitis contributed to the inadequacy of the management.

Asymptomatic neurosyphilis may need early intervention in people living with hiv

Syphilis is a sexually transmitted infection caused by the spirochete Treponema pallidum(T.pallidum)and is of global and historic significance.People living with hiv(PLWH)infection are frequently coinfected with syphilis,because of the overlapping modes of pathogen acquisition.Neurosyphilis can occur at any stage of syphilis infection,and PLWH are at a greater risk of developing neurosyphilis in early and late latent syphilis,including asymptomatic central nervous system(CNS)infection than those without hiv infection.Some patients with neurosyphilis present neurological manifestations such as meningitis,stroke,cranial nerve dysfunction,auditory or ophthalmic abnormalities,and acute changes in mental status.However,some neurosyphilis cases in PLWH lack signs or symptoms of CNS infection.Asymptomatic neurosyphilis is diagnosed in the absence of neurological signs or symptoms and can only be detected in the presence of cerebrospinal fluid(CSF)abnormalities indicative of CNS infection,including mononuclear pleocytosis,increased protein concentration,and/or a reactive CSF venereal disease research laboratory(VDRL)assay.This has raised concerns that a persistent reservoir of T.pallidum although asymptomatic in the CNS,may result in treatment failure and neurological sequelae in PLWH.Neurosyphilis might lead to cranial nerve palsies,syphilitic meningitis,general paresis,and tabes dorsalis even several years after infection.Thus,early intervention may be needed for asymptomatic neurosyphilis in PLWH.

Introduction: Cervical cancer is a public health concern and is mainly caused by Human papillomaviruses (HPV). In many parts of the world, studies are being carried out to understand the different genotypes to better tackle this issue. We conducted a study to determine the prevalence of HPV genotypes in women with chronic hepatitis B or C infection, co-infected or not with hiv, treated at the Hôpital Saint Camille in Ouagadougou (Burkina Faso). Methods: This study was conducted from April to July 2023, including 100 women in gastroenterology at Hôpital Saint Camille. A questionnaire on their socio-demographic and life style was administrated;and endocervical samples were collected using sterile swabs and then sent to Centre of Biomolecular Research Pietro Annigoni (CERBA). HPV molecular detection and genotyping were performed by PCR and hybridization using the HPV Direct Flow Chips kit. Data were analysis using chi square test or Fischer’s exact test with a significance threshold for p Results: The prevalence of HPV infection was 28% (28/100) on the sample of women tested. The most frequent genotypes were HPV 52 (8.33%), followed by HPV 18 and 68 (6.25% each) for high-risk HPVs, and HPV 6, 44/55 and 62/81 (8.33% each) for low-risk HPVs. Conclusion: This study, the first of its kind in Burkina Faso on this group of the population, reveals that the most frequent genotypes found in this study are not included in the vaccine available in Burkina Faso (Gardasil®4).
