Objective:This study seeks to explore efficient and multiple-item detection modes in new-style hiv labs,as well as access the accuracy and reliability of CD4 cell count detected by point of care(POC)to analyze POC work feasibility in district or county labs.Methods:POC devices adopted in grassroots-level labs and flow cytometers adopted in prefecture-level labs were used to analyze the same group of blood samples.The individual results were collected and compared for parametric tests in correlation and consistency.Results:The Pearson correlation coefficients(r)between results detected by FACSPresto and those by FACSCalibur,FACSVia,FACSCantoII,and EPICSXL were 0.922,0.938,0.914,and 0.823,respectively;the average deviations were−25.64,24.68,3.05,and 70.97 cells/μL,respectively;the Pearson correlation coefficient(r)between results by Pima and FACSCalibur,FACSVia,FACSCantoII,and EPICSXL were 0.900,0.950,0.954,and 0.876,respectively;and the average deviations were−73.99,−40.78,−29.32,and−22.75 cells/μL,respectively.Discussion:Strong positive correlations and good consistency were observed between the CD4 count tested by POC and flow cytometers.These findings provide theoretical support for new-style hiv labs and one-stop services,which can provide shorter testing duration and simpler testing processes,so that the most comprehensive testing results can be obtained in the shortest amount of time.

Background:Low-fee female sex workers(FSWs)are at high risk of acquiring and spreading human immunodeficiency virus(hiv)/sexually transmitted diseases(stds)in China.There is an urgent need to develop comprehensive intervention measures targeted towards low-fee FSWs to reduce hiv/std infections.Thus,this study aimed to reduce hiv/std infections among low-fee FSW through a matched-pair,comiDunity-based randomized intervention trial carried out in 12 cities in three provinces in China.Methods:Four cities from Guangxi Zhuang Autonomous Region,four from Yunnan Province,and four from Hunan Province were paired and participants received either the intervention package(including condom promotion,hiv and syphilis testing,reimbursement for syphilis treatment costs,and free anti-retroviral therapy or the current standard of care.Venue-based,convenience sampling was used to recruit FSWs.A face-to-face interview and hiv and syphilis blood testing was conducted at baseline and follow-up intervals of 24 months.Generalized linear mixed models(GLMM)were used to evaluate the effect of the intervention package on reducing hiv/std infection in the FSWs.Results:A total of 1024 eligible FSWs were enrolled in the baseline survey and 843 in the follow-up.GLMM results showed that syphilis infeaion was reduced by 49%in the intervention group compared to the current standard of care group(P=0.0378,OR=0.51;95%CI:0.27-0.96).FSWs aged 35 years or older were 2.38 times more likely to get syphilis infection compared to those younger than 35 years old(P<0.0001,OR=2.38,95%CI:1.55-3.65).The risk of syphilis infection among more educated FSWs was 0.43 times less than those with lower levels of education(P<0.05,OR=0.43,95%CI:0.63-0.93).Conclusions:This study demonstrates that comprehensive interventions can lead to significant declines in syphilis infection amongst low-tier FSWs.Integrating both behavioral and biomedical intervention measures should be considered when developing programs for low-fee FSWs.Trial registration:CHiCTR-TRC-12002655.

This paper reviews the current epidemics of human immunodeficiency virus(hiv)infection in China,particularly the globally available prevention strategies developed and implemented.This review focuses on hiv prevention measures in general,such as education,testing,and counseling and in specific responses to transmission modes,such as blood safety,harm reduction for people who inject drugs,and condom promotion to reduce sexual transmission.We also assess newly developed prevention measures,such as prevention treatment,pre-exposure prophylaxis,post-exposure prophylaxis,male circumcision,and promising potential future preventions,including microbicides and vaccines.Based on this assessment,we provide recommendations for their implementation in China.We conclude that there is no magic bullet for hiv prevention,particularly sexual transmission of the disease,but only a combination of these prevention strategies can control the hiv epidemic.

Background： CD4 count is used to determine antiretroviral therapy （ART） eligibility. In China, flow cytometers are mostly located in urban areas with limited access by patients residing in remote areas. In an attempt to address this issue, we conducted a study to validate the performance ofAlere PIMA point-of-care CD4 analyzer. Methods： Venous and finger-prick blood specimens were collected from hiv-positive participants from two voluntary counseling and testing sites in Yunnan Province. Both venous and finger-prick blood specimens were tested with the PIMA analyzer. Venous blood specimens tested with the Becton Dickinson FACSCalibur were used as a reference. Results： Venous specimens from 396 and finger-prick specimens from 387 persons were available for analysis. CD4 counts by PIMA correlated well with those from FACSCalibur with an R2 of 0.91 for venous blood and 0.81 for finger-prick blood. Compared to FACSCalibur, the PIMA analyzer yielded lower counts with a mean bias of- 47.0 cells/μl （limit of agreement, [LOA]： -204-110 cells/μl） for venous blood and -71.0 cells/μl （LOA： -295-153 cells/μl） for finger-prick blood. For a CD4 threshold of 350 cells/μl, the positive predictive value （PPV） of PIMA was 84.2% and 75.7% and the negative predictive value （NPV） was 97.6% and 95.8% for venous and finger-prick blood, respectively. For an ART threshold of 500 cells/μl, the corresponding PPV was 90.3% and 84.0% and NPV was 94.3% and 93.4%, respectively. Conclusions： CD4 counting using venous blood with PIMA analyzers is a feasible alternative to a large flow cytometer to determine ART eligibility.

Background: Since the appearance of severe acute respiratory coronavirus 2 (SARS-CoV-2) and the coronavirus disease 2019 (COVID-19) pandemic, a growing body of evidence has suggested that weather factors, particularly temperature and humidity, influence transmission. This relationship might differ for the recently emerged B.1.617.2 (delta) variant of SARS-CoV-2. Here we use data from an outbreak in Sydney, Australia that commenced in winter and time-series analysis to investigate the association between reported cases and temperature and relative humidity.Methods: Between 16 June and 10 September 2021, the peak of the outbreak, there were 31,662 locally-acquired cases reported in five local health districts of Sydney, Australia. The associations between daily 9:00 am and 3:00 pm temperature (°C), relative humidity (%) and their difference, and a time series of reported daily cases were assessed using univariable and multivariable generalized additive models and a 14-day exponential moving average. Akaike information criterion (AIC) and the likelihood ratio statistic were used to compare different models and determine the best fitting model. Background:Emerging evidence indicates nitrite inhalants have become increasingly prevalent among men who have sex with men(MSM).The present study aimed to describe the prevalence and correlates of nitrite inhalants use and its association with risky sexual behaviors and human immunodeficiency virus(hiv)/syphilis infection among MSM in Chongqing,a city in China where MSM were burdened with the highest pooled hiv prevalence in the country.Methods:This cross-sectional study was conducted in Chongqing between March 2019 and February 2020.Information of demographics,drug use,sexual behaviors and hiv testing was collected through an anonymous survey.Blood samples were drawn from each participant for the diagnoses of hiv and syphilis.Logistic regression analysis was performed to evaluate factors correlated with nitrite inhalants use and its relationship with risky sexual behaviors and hiv/syphilis infection.Results:Of the 1151 eligible participants,18.9%(218)reported use of at least one type of recreational drugs in the past 6 months,and nitrite inhalants were the most commonly used substance(17.7,95%confidence interval[CI]:15.6-20.2%).The proportions of participants reported engaging in group sex and practicing condomless internal ejaculation during anal sex in the past six months were 5.8%(95%CI:4.4-7.2%)and 41.7%(95%CI:38.7-44.7%),respectively.The general prevalence of hiv and syphilis infection among the enrolled MSM were 16.8%(95%CI:14.7-19.0%)and 12.6%(95%CI:10.7-14.4%),respectively.Factors positively associated with nitrite inhalants use included:age<25(adjusted odds ratio[aOR]=2.08,95%CI:1.10-3.94),monthly individual income≥CNY 3000(Chinese Yuan)(aOR=1.95,95%CI:1.18-3.22),preferring receptive anal intercourse(aOR=2.27,95%CI:1.34-3.84)and versatile anal intercourse(aOR=2.60,95%CI:1.64-4.13),age at first anal intercourse<18(aOR=1.79,95%CI:1.21-2.67),engaging in group sex in the past six months(aOR=9.34,95%CI:4.95-17.63),having multiple male sex partners in the past 6 months(aOR=2.32,95%CI:1.50-3.58),practicing CI

Background： Successful antiretroviral therapy （ART） has been demonstrated to be effective in reducing the infectivity of human immunodeficiency virus （hiv）. We conducted a study to predict the potential effect of ART on the spread of hiv in Chaoyang District, Beijing, China, using the Asian Epidemic Model （AEM）. Methods： The AEM baseline workbook was used to determine the current infection status and to project the future spread of hiv under current conditions. We changed the input on the ART coverage from 2014 to 2025 and also modified the treatment eligibility in the AEM intervention workbook, in order to allow for analysis of the projected downstream impact of ART. Results： By gradually increasing the ART coverage rate from 29.7% （rate of 2013） to 40.0%, 50.0%, 60.0%, 70.0%, 80.0%, and 90.0% （at CD4＋ ≤350 cells/μl）, and by changing the dates of coverage from 2014 to 2020, the number of new infections showed a cumulative decline of 0.60%, 1.59%, 2.94%, 5.33%, 9.32%, and 14.98%, respectively. After 2020, the projected rates of infection rebounded slightly, so with the exception of the years with very high coverage （90.0%）, new infections continued to decrease. When we changed the initial threshold of therapy to CD4＋ cell counts ≤500 cells/μl, new infections decreased 6.00%, 11.64%, 15.92%, 21.11%, 26.92%, 33.05%, and 38.75%, respectively, under varying ART coverages. Conclusion： Our study demonstrates that the early initiation of ART for people living with hiv/acquired immune deficiency syndrome （AIDS） has a positive effect in slowing the spread of hiv.

Drug use （DU）, particularly injecting drug use （IDU） has been the main route of transmission and spread of Human Immunodeficiency Virus （hiv）/Acquired Immune Deficiency Syndrome （AIDSJ among injecting drug users （IDUs）[1]. Previous studies have proven that needles or cottons sharing during drug injection were major risk factors for hiv/AIDS transmission at the personal level[z4]. Being a social behavioral issue, hiv/AIDS related risk factors should be far beyond the personal level. Therefore, studies on hiv/AIDS related risk factors should focus not only on the individual factors, but also on the association between hiv/AIDS cases and macroscopic-factors, such as economic status, transportation, health care services, etc[1]. The impact of the macroscopic-factors on hiv/AIDS status might be either positive or negative, which are potentially reflected in promoting, delaying or detecting hiv/AIDS epidemics.
