OBJECTIVE: To assess the efficacy and safety of Sancai powder in patients with type 2 diabetes mellitus(T2DM) inadequately controlled with single oral metformin in a randomized controlled trial(RCT).METHODS: A total of 132 patients with T2 DM were enrolled in the study, who only took metformin(500-1000 mg/day) for at least three months and with inadequate glycemic control(7.0% ≤ hemoglobin A1 c ≤ 9.0%) in the past three *** patients stopped taking metformin with lifestyle interventions for three weeks, and 105 patients qualified for the program. They were randomly divided into the Sancai powder group and the metformin group(1500 mg/day). The follow-up period was for 12 weeks. Comparisons of several variables were ***: No significant differences were found between the two groups in hemoglobin A1c(Hb A1c),fasting plasma glucose(FPG) and 2 h post-meal glucose(2h PG), although they had decreased significantly(P < 0.01). Homeostasis model assessment of beta cell function index was significantly improved in Sancai powder group(P < 0.01), and there were significant differences in the changes of homeostasis model assessment of insulin resistance and insulin sensitivity index in the two groups(P < 0.05). Sancai powder significantly reduced triglyceride level(P < 0.05), although there was no significant difference in the body weight and body mass index in the two ***: In this 12-week study, Sancai powder could significantly reduce hemoglobin A1 c,FPG and 2h PG levels, improved beta-cell function and insulin resistance of the T2 DM inadequately controlled with metformin.

Use of Xinfeng capsule to treat abarticular pathologic changes in patients with rheumatoid arthritis

OBJECTIVE: To observe the influence of Xinfengcapsule(XFC) on abarticular pathologic changes(APCs) and other indices of patients with rheumatoid arthritis(RA) and explore the mechanism of action of XFC in improving such ***: Three-hundred RA patients were divided randomly into a treatment group(n=150) and control group(n=150). Combination of ranibizumab with photodynamic therapy vs ranibizumab monotherapy in the treatment of age-related macular degeneration: a systematic review and meta-analysis of randomized controlled trials

Electroacupuncture and moxibustion promote regeneration of injured sciatic nerve through Schwann cell proliferation and nerve growth factor secretion

Using electroacupuncture and moxibustion to treat peripheral nerve injury is highly efficient with low side effects. However, the electroacupuncture-and moxibustion-based mechanisms underlying nerve repair are still unclear. Here, in vivo and in vitro experiments uncovered one mechanism through which electroacupuncture and moxibustion affect regeneration after peripheral nerve injury. We first established rat models of sciatic nerve injury using neurotomy. Rats were treated with electroacupuncture or moxibustion at acupoints Huantiao （GB30） and Zusanli （ST36）. Each treatment lasted 15 minutes, and treatments were given six times a week for 4 consecutive weeks. Behavioral testing was used to determine the sciatic functional index. We used electrophysiological detection to measure sciatic nerve conduction velocity and performed hematoxylin-eosin staining to determine any changes in the gastrocnemius muscle. We used immunohistochemistry to observe changes in the expression of S100—a specific marker for Schwann cells—and an enzyme-linked immunosorbent assay to detect serum level of nerve growth factor. Results showed that compared with the model-only group, sciatic functional index, recovery rate of conduction velocity, diameter recovery of the gastrocnemius muscle fibers, number of S100-immunoreactive cells,and level of nerve growth factor were greater in the electroacupuncture and moxibustion groups. The efficacy did not differ between treatment groups. The serum from treated rats was collected and used to stimulate Schwann cells cultured in vitro. Results showed that the viability of Schwann cells was much higher in the treatment groups than in the model group at 3 and 5 days after treatment. These findings indicate that electroacupuncture and moxibustion promoted nerve regeneration and functional recovery; its mechanism might be associated with the enhancement of Schwann cell proliferation and upregulation of nerve growth factor.

Efficacy and safety of blood pricking and cupping for treating nonspecific low back pain: a systematic review and Meta-analysis

OBJECTIVE: To systematically evaluate the efficacy and safety of blood pricking and cupping in the treatment of nonspecific low back pain(NLBP).METHODS:The China National Knowledge Infrastructure Database(CNKI), Wanfang Med Online Database(Wanfang), China Science and Technology Journal Database(VIP), Pub Med, Web of Science,EMBase, and Cochrane Library databases were searched from inception to October 31, 2021, for randomized controlled trials(RCTs) of the treatment of NLBP by blood pricking and cupping. The quality of each included study was evaluated according to the Cochrane Systematic Assessor’s Manual evaluation criteria, while the Meta-analysis was performed using RevMan ***: A total of 13 thirteen RCTs included a total of 1088 subjects. The visual analogue scale scores indicated that blood pricking and cupping was superior to the other treatments at relieving NLBP pain [mean difference(MD) =-1.43;95% confidence interval(CI):-2.31 to 0.54;Z = 3.15;P = 0.002). The Oswestry Disability Index score of blood pricking and cupping was superior to that of the control group in terms of improving NLBP dysfunction(MD =-6.25;95%CI:-8.37 to-4.31);Z = 5.77;P < 0.000 01). 7 RCTs mentioned no adverse reactions, while one study reported mild syncope [7%(n = 3), all in the 17-30 years group] that was gradually relieved after rest. CONCLUSIONS: Blood pricking and cupping therapy can safely and effectively reduce pain and improve functional impairment in patients with NLBP. However, this study included limited high-grade studies with a small overall sample size;therefore, further high-quality large-sample RCTs are needed to improve clinical evidence.

Effect of Zibushenjing Fang on skeletal development and brain tissue antioxidation in mice with kidney essence insufficiency: a mechanistic study

OBJECTIVE: To investigate the effects of Zibushenjing Fang (formula for tonifying the kidney essence) on skeletal development and brain tissue antioxidation in mice with kidney essence insufficiency. METHODS: Fifty male Kunming mice were randomly divided into five groups: normal group, model group, Jinkuishenqi Wan group, Zibushenjing Fang high dose group, and Zibushenjing Fang low dose group, with 10 mice in each group. The model of kidney essence insufficiency syndrome was established in all the mice except the normal group by using a cat to threaten the mice and by swimming until exhaustion daily which lasted about 21 days. Mice in the model group were administered 20 mL/ kg -1 · d -1 of normal saline intragastrically. The Jinkuishenqi Wan group was given 2.7 g/kg-1 · d-1 ofa solution of Jinkuishenqi Wan. The Zibushenjing Fang high dose group was given 20 g/kg -1 · d -1 and the Zibushenjing Fang low dose group was given 10 g/kg -1 · d -1 of a solution of Zibushenjing *** general condition of all the groups was observed, including the quantity of food and water intake, swimming time, length of femur, and weight of the femur and musculus quadriceps femoris. The total superoxide dismutase (T-SOD), glutathione peroxi- dase (GSH-Px), and methane dicarboxylic aldehyde (MDA) activities in the brain tissues were detected. RESULTS: Zibushenjing Fang could improve the manifestation of kidney essence insufficiency, increase the quantity of food and water intake, swimming time, femur length, and femur and musculus quadriceps femoris weight. It could also regulate the activities of T-SOD, GSH-Px, and MDA in brain tissue. CONCLUSION: Zibushenjing Fang may play an important role in treating kidney essence insufficiency syndrome by promoting body development and improving brain tissue antioxidation.

Effect of "Yang-warming and kidney essence-replenishing" herbal paste on cold-related asthma exacerbation

OBJECTIVE: To observe the effect of "Yang-warming and kidney essence-replenishing" herbal paste on cold-related asthma exacerbation. METHODS: One hundred and fifty one patients with moderate to severe persistent asthma were randomly divided into a treatment group (n=74) and a control group (n=69). Both groups were given basic treatment according to the Global Initiative for Asthma scheme. The treatment group was also treated with herbal paste in the winter. The frequency of asthma exacerbation, cold and cold-re-lated asthma exacerbation, scores of the asthma control test (ACT), and kidney-deficiency syndrome during the one-year follow-up in each group were recorded. RESULTS: Compared with the control group, the onset frequencies of catching cold, asthma exacerbation, cold-related asthma exacerbation, and kidney-deficiency syndrome score were significantly reduced. There was no significant difference in scores of ACT between the two groups. CONCLUSION: "Yang-warming and kidney essence-replenishing" herbal paste could reduce cold-related asthma exacerbation.

Tongguan Liqiao acupuncture therapy improves dysphagia after brainstem stroke

Tongguan Liqiao acupuncture therapy has been shown to effectively treat dysphagia after stroke-based pseudobulbar paralysis. We presumed that this therapy would be effective for dysphagia after bulbar paralysis in patients with brainstem infarction. Sixty-four patients with dysphagia following brainstem infarction were recruited and divided into a medulla oblongata infarction group（n = 22）, a midbrain and pons infarction group（n = 16）, and a multiple cerebral infarction group（n = 26） according to their magnetic resonance imaging results. All patients received Tongguan Liqiao acupuncture for 28 days. The main acupoints were Neiguan（PC6）, Renzhong（DU26）, Sanyinjiao（SP6）, Fengchi（GB20）, Wangu（GB12）, and Yifeng（SJ17）. Furthermore, the posterior pharyngeal wall was pricked. Before and after treatment, patient swallowing functions were evaluated with the Kubota Water Test, Fujishima Ichiro Rating Scale, and the Standard Swallowing Assessment. The Barthel Index was also used to evaluate their quality of life. Results showed that after 28 days of treatment, scores on the Kubota Water Test and Standard Swallowing Assessment had decreased, but scores on the Fujishima Ichiro Rating Scale and Barthel Index had increased in each group. The total efficacy rate was 92.2% after treatment, and was most obvious in patients with medulla oblongata infarction（95.9%）. These findings suggest that Tongguan Liqiao acupuncture therapy can repair the connection of upper motor neurons to the medulla oblongata motor nucleus, promote the recovery of brainstem infarction, and improve patient＇s swallowing ability and quality of life.
