A Comparative Study on Efficacy of Fludrocortisones versus Glucocorticoids and Vasodilators in the Treatment of Idiopathic Sensorineural Cochlear Hearing Loss

Introduction and Objectives: The idiopathic sensorineural cochlear hearing loss is one of the most frequent human sensory deficits and there is no specific drug therapy for it. The possible hearing recovery is related with the reestablishment of normal ionic homeostasis of the endolymph controlled by the mineralocorticoid as could be demonstrate experimentally. The purpose of this clinical trial was to confirm the efficacy of mineralocorticoids to the recovery of hearing level in patients suffering idiopathic sensorineural hearing loss (SNHL) against the glucocorticoids and vasodilator drugs. Material and Methods: The research lasted three months and involved 90 patients allocated into four different groups: Placebo group, consisted of 20 patients (10 men and 10 women);the group consisting of 22 patients treated with glucocorticoid therapy (12 men and 10 female);the group treated with mineralocorticoid therapy encompassed 26 patients (13 males and 13 females) and the group of vasodilators formed by 22 patients (12 men and 10 women). The level of hearing loss was estimated by the tests Liminal Tone Audiometry (LTA) and Auditory Brainstem Response (ABR). Results: The main features in this research were overall better response in improving the hearing level with the mineralocorticoid therapy. This improvement in hearing levels was greater in women than in men, and a higher response was found in the left ear regardless of patient’s gender. Conclusions: The hearing gain was significantly superior in the mineralocorticoids group followed by the glucocorticoids group whereas the response to vasodilators was lesser and with no statistical significance.

Extramural vascular invasion and response to neoadjuvant chemoradiotherapy in rectal cancer: influence of the CpG island methylator phenotype

AIM To identify whether Cp G island methylator phenotype(CIMP) is predictive of response to neoadjuvant chemoradiotherapy(NACRT) and outcomes in rectal *** Patients undergoing NACRT and surgical resection for rectal cancer in a tertiary referral centre between 2002-2011 were identified. Pre-treatment tumour biopsies were analysed for CIMP status(high, intermediate or low) using methylation specific PCR. KRAS and BRAF status were also determined using pyrosequencing analysis. Clinical information was extracted from case records and cancer services databases. Response to radiotherapy was measured by tumour regression scores determined uponhistological examination of the resected specimen. The relationship between these molecular features, response to NACRT and oncological outcomes were *** There were 160 patients analysed with a median followup time of 46.4 mo. Twenty-one(13%) patients demonstrated high levels of CIMP methylation(CIMP-H) and this was significantly associated with increased risk of extramural vascular invasion(EMVI) compared with CIMP-L [8/21(38%) vs 15/99(15%), P = 0.028]. CIMP status was not related to tumour regression after radiotherapy or survival, however EMVI was significantly associated with adverse survival(P < 0.001). Intermediate CIMP status was significantly associated with KRAS mutation(P = 0.01). There were 14(9%) patients with a pathological complete response(pC R) compared to 116(73%) patients having no or minimal regression after neoadjuvant chemoradiotherapy. Those patients with pC R had median survival of 106 mo compared to 65.8 mo with minimal regression, although this was not statistically significant(P = 0.26). Binary logistic regression analysis of the relationship between EMVI and other prognostic features revealed, EMVI positivity was associated with poor overall survival, advanced "T" stage and CIMP-H but not nodal status, age, sex, KRAS mutation status and presence of local or systemic ***

Predicting a novel pathogenicity island in Helicobacter pylori by genomic barcoding

AIM:To apply a new,integrated technique for visualizing bacterial genomes to identify novel pathogenicity islands in Helicobacter pylori(***).METHODS:A genomic barcode imaging method(converting frequency matrices to grey-scale levels)was designed to visually distinguish origin-specific genomic regions in *** complete genome sequences of the six *** strains published in the National Center for Biotechnological Information prokaryotic genome database were scanned,and compared to the genome barcodes of Escherichia coli(***)O157:H7 strain EDL933 and a random nucleotide *** following criteria were applied to identify potential pathogenicity islands(PAIs):(1)barcode distance distinct from that of the general background;(2)length greater than 10000 continuous base pairs;and(3)containing genes with known virulence-related functions(as determined by PfamScan and Blast2GO).RESULTS:Comparison of the barcode images generated for the 26695,HPAG1,J99,Shi470,G27 and P12 *** genomes with those for the *** and random sequence controls revealed that *** genomes contained fewer anomalous *** the *** continuous anomalous regions(longer than 20 kbp in each strain's genome),two fit the criteria for identifying candidate *** bioinformatic-based functional analyses revealed that one of the two anomalous regions was the known pathogenicity island cag PAI,this finding also served as proof-of-principle for the utility of the genomic barcoding approach for identifying PAIs,and characterized the other as a novel PAI,which was designated as ***,the cag-PAI and tfs3-PAI harbored genes encoding type IV secretion system proteins and were predicted to have potential for functional ***:Genomic barcode imaging represents an effective bioinformatic-based approach for scanning bacterial genomes,such as ***,to identify candidate PAIs.

Embryonal sarcoma of the liver with chondroid differentiation

A 31-year-old female complained of upper abdominal and back pain. Laboratory tests showed elevated levels of aspartate aminotransferase, alanine aminotransferase and α-fetoprotein. Computed tomography revealed that the tumor, measuring 14.5 cm × 10.4 cm, occupied the anterior and medial segments of the liver and consisted of multicystic and solid lesions. The preoperative diagnosis was a hepatic cystadenocarcinoma. The operation was performed urgently because of tumor rupture. Histo- pathologically, spindle and asteroid cells were found to have proliferated diffusely. There were no neoplastic epithelial tumor cells. Tumor cells had periodic acid- Schiff-positive hyalin globules. At the periphery, trapped normal bile duct cells were observed. The final diagnosis was embryonal sarcoma of the liver (ESL). Interestingly, irregular islands of chondrosarcoma-like lesions were found in the tumor and the tumor-associated vascular endothelium showed immunoreactivity for KIT. Two months after the operation, the tumor recurred. At 6 mo follow-up, the patient is alive with the disease and undergoing chemotherapy. This is the first report of ESL with chondroid differentiation.
