Although gastric cancer(GC)is one of the leading causes of cancer-related death,major therapeutic advances have not been made,and patients with GC still face poor *** prognosis of GC also remains poor because the molecular mechanisms of GC progression are incompletely ***(miRNAs)are noncoding RNAs that are associated with gastric *** investigating the regulation of gene expression by miRNAs have made considerable progress in recent years,and abnormalities in miRNA expression have been shown to be associated with the occurrence and progression of *** contribute to gastric carcinogenesis by altering the expression of oncogenes and tumor suppressors,affecting cell proliferation,apoptosis,motility,and ***,a number of miRNAs have been shown to be associated with tumor type,tumor stage,and patient survival and therefore may be developed as novel diagnostic or prognostic *** this review,we discuss the involvement of miRNAs in GC and the mechanisms through which they regulate gene expression and biological ***,we review recent research on the involvement of miRNAs in GC prognosis,their potential use in chemotherapy,and their effects on Helicobacter pylori infections in GC.A greater understanding of the roles of miRNAs in gastric carcinogenesis could provide insights into the mechanisms of tumor development and could help to identify novel therapeutic targets.

AiM To determine changes in the morphology and function of pancreatic cancer cells after irreversible electroporation(iRE) treatment, and to explore the clinical significance of iRE treatment for pancreatic cancer providing an experimental basis for the clinical application of iRE treatment. METHODS iRE was carried out in an athymic nude mouse model of pancreatic carcinoma generated with human pancreatic cancer cells 1. in therapy groups, iRE electrodes were inserted with 90 pulses per second at 800 V/cm applied to ablate the targeted tumor tissues. Histological assessment of the affected tissue was performed by hematoxylin and eosin staining(HE). Quantification of cell proliferation and apoptosis was performed by evaluating Ki67 and caspase-3 levels, respectively. Flow cytometry was used to assess cell apoptosis. Ultrasound imaging was carried out to evaluate iRE treatment results. Pathological correlation studies showed iRE is effective for the targeted ablation of pancreatic tumors in an orthotopic mouse *** iRE was efficacious in removing tumors in the orthotopic mouse model. The iRE-ablated zone displays characteristics of nude mouse models at different time-points as assessed by hematoxylin and eosin staining. immunohistochemical analysis of samples from the pancreatic cancer models showed significantly enhanced caspase-3 cleavage and Ki67. Flow cytometry data corroborated the above findings that apoptosis in tumor cells was observed immediately on the first postoperative day, and with time the middle and late stages of apoptosis were observed. For ultrasound imaging studies, the iRE ablation zone became a hyperechoic area due to increasing inflammatory and immunologic cellular contents. CONCLUSiON iRE is a promising new approach for pancreatic cancer, with many potential advantages over conventional ablation techniques.

Clinical utility of the platelet-lymphocyte ratio as a predictor of postoperative complications after radical gastrectomy for clinical T2-4 gastric cancer

AiMTo identify simple and sensitive markers for postoperative complications after gastrectomy, the predictive values were compared among candidate preoperative ***-hundred and twelve patients with previously untreated clinical T2-4 gastric cancer who underwent a D2 standard gastrectomy (distal gastrectomy or total gastrectomy) were included in the analysis. Correlations between 21 parameters that can be determined by preoperative routine blood tests and clinically relevant postoperative complications (grade ii or higher according to the Clavien-Dindo classification) were evaluated. The optimal cutoff values and clinical significance of the selected markers were further evaluated by subgroup analyses according to age, body mass index, operative procedure and clinical disease ***-six patients (21.1%) experienced grade ii or higher postoperative complications. The platelet-lymphocyte ratio (PLR, total lymphocyte count/platelet count × 100) exhibited the highest area under the curve value (0.639) for predicting postoperative complications among the 21 parameters, and the optimal cutoff value was determined to be 0.71 (sensitivity = 70%, specificity = 56%). in the univariate analysis, the odds ratio of a low PLR for the occurrence of postoperative complications was 2.94 (95%Ci: 1.66-5.35, P ivariate binomial logistic analysis involving other potential risk factors identified a low PLR as an independent risk factor for postoperative complications (OR = 3.32, 95%Ci: 1.82-6.25, P in subgroups classified according to age, body mass index, operative procedure and clinical disease stage, the low PLR group exhibited an increased incidence of postoperative *** preoperative PLR is a simple and useful predictor of complications after curative gastrectomy in patients with clinical T2-4 gastric cancer.

AiM： To focus on the role of CD40 and CD40L in their pathogenesis. METHODS： We analyzed by immunohistochemistry the CD40 and CD40L expression in the pouch mucosa of 28 patients who had undergone RPC for UC, in the terminal ileum of 6 patients with UC and 11 healthy subjects. We also examined by flow cytometry the expression of CEH0 by B lymphoo/tes and monocytes in the peripheral blood of 20 pouch patients, 15 UC patients and 11 healthy controls. RESULTS： ileal pouch mucosa leukocytes presented a significantly higher expression of CD40 and CD40L as compared to controls. This alteration correlated with pouchitis, but was also present in the healthy pouch and in the terminal ileum of UC patients. CD40 expression of peripheral B lymphocytes was significantly higher in patients with UC and pouch, respect to controls. increased CD40 levels in blood B cells of pouch patients correlated with the presence of spondyloarthropathy, but not with pouchitis, or inflammatory indices. CONCLUSiON： High CD40 expression in the ileal pouch mucosa could be implied in the pathogenesis of pouchitis following proctocolectomy for UC, whereas its increased levels on peripheral blood B lymphocytes are associated with the presence of extraintestinal manifestations.

We congratulate *** and colleagues for their recently published study entitled“Precision oncology in surgery:Patient selection for operable pancreatic cancer”in Annals of surgery(1).in this unique study,the authors tried to define preoperative clinical and molecular characteristics in order to improve patient selection for operable pancreatic *** ductal adenocarcinoma(PDAC)remains the most lethal type of human cancer due to its high chemoresistance and invasiveness(2).in resected PDAC,the median overall survival has increased from 22.1 to 35 months during the past 10 years,largely owing to improvements in adjuvant therapies(3,4).On the other hand,the high recurrence rate even in patients who underwent curative resection,and chemoresistance in the recurrent sites to current systemic chemotherapies(FOLFiRiNOX and nab-paclitaxel plus gemcitabine),are major issues in the treatment for unselected PDAC patient *** resection offers the only chance of cure,but surgery can be associated with significant morbidity and decreased activity of daily life(ADL),especially in the Whipple procedure(pancreaticoduodenectomy).Recently,the importance of preoperative chemotherapy even in operable PDAC has been increasingly recognized(5).Although molecular markers are being used more frequently to select patients for systemic targeted agents,only imaging modalities are used to stage patients and to assess their suitability for operative *** on primary surgery or a neoadjuvant approach are made without evaluation of biological measures of tumor aggressiveness,or of the risk of occult metastatic disease.

AiM： To examine the effects of anti-high mobility group box 1 （HiGB1） neutralizing antibody in experimental severe acute pancreatitis （SAP）. METHODS： SAP was induced by creating closed duodenal loop inC3H/HeN mice. SAP was induced immediately after intrapedtoneal injection of anti-HMGB1 neutralizing antibody （200 pg）. Sevedty of pancreatitis, organ injury （liver, kidney and lung）, and bacterial translocation to pancreas was examined 12 h after induction of SAP. RESULTS： Anti-HHGB1 neutralizing antibody significantly improved the elevation of the serum amylase level and the histological alterations of pancreas and lung in SAR Anti-HHGB1 antibody also significantly ameliorated the elevations of serum alanine aminotransferase and creatinine in SAR However, anti-HHGB1 antibody worsened the bacterial translocation to pancreas. CONCLUSiON： Blockade of HHGB1 attenuated the development of SAP and associated organ dysfunction, suggesting that HHGB1 may act as a key mediator for inflammatory response and organ injury in SAR
