Effect of jailed balloon technique on side branch protection in percutaneous coronary intervention for bifurcation lesions

Background Data comparing the jailed balloon technique （JBT） and conventional jailed wire technique （JWT） during percutaneous coronary intervention （PCI） for bifurcation lesions is scanty. Methods From January 2013 to June 2013, 192 consecutive patients undergoing elective PCI for de novo Medina 1,1,1 bifur- cation lesions were enrolled in this study. JBT and JWT were applied to protect the side branch （SB） at the discretion of the operator. Procedural outcomes were compared between the JBT and the JWT group. Results Seventy-four patients were treated with JBT and 118 with JWT. Proportion of patients achieving TIMI 3 flow in main branch （MB） was similar in both groups while in SB it was much lower in JWT group compared with JBT group （74.6% vs. 93.2%, P = 0.001）. In addition, patients in JWT group showed higher incidence of periprocedural myocardial infarction （MI） compared with patients in JBT group （11.9% vs. 2.7%, P = 0.008）. Similar tendency was observed in SB occlusion （18.6% vs. 5.4%, P = 0.009）. No device-related complications were observed in both groups. Conclusion JBT is an effective and safe measure to protect the SB during PCI for bifurcation lesions.

Background: The QT interval shortens in response to sympathetic stimulation. Head-up tilt-table (HUT) testing is a straightforward way to achieve brisk sympathetic stimulation. There is not enough information about the response of the QT interval to HUT, particularly, in patients with orthostatic hypotension (OH). Objective: Analyse the response of the RR, QT and QTc intervals in patients with OH and reflex syncope (NM) during HUT and find differences between groups. Methods: We reviewed the electrocardiograms and compare the RR and QT/QTc intervals during 1) baseline;2) HUT plus hyperventilation;3) positive test. Results: We studied 137 patients, 62 control group (no syncope and negative HUT). On average, the RR HUT interval was shorter than the resting RR by −171 ± 110.4 ms in controls;−228.6 ± 119.4 ms (NM) and −194 ± (OH) (P Conclusion: Significant differences between the reflex group and the OH during a positive test, the QTc decreased in the NM group, but in the OH population increased. This observation has not been described. We hypothesize that QTc prolongation could reflect autonomic nervous system downregulation and could explain to a degree, the increased mortality in this group.

Viral myocarditis(VM) is an inflammatory disease of the myocardium associated with heart failure, which is caused by common viral infections. A majority of the infections are initiated by coxsackievirus B3(CVB3). Micro RNAs(mi RNAs)have a major role in various biological processes, including gene expression, cell growth, proliferation, and apoptosis, as well as viral infection and antiviral immune responses. Although, mi RNAs have been found to regulate viral infections,their role in CVB3 infection remains poorly understood. In the previous study, mi RNA microarray results showed that mi R-324-3 p expression levels were significantly increased when cells and mice were infected with CVB3. It was also found that miR-324-3p downregulated TRIM27 and decreased CVB3 replication in vitro and in vivo. In vitro, analysis of downstream signaling of TRIM27 revealed that, miR-324-3p inhibited CVB3 infection, and reduced cytopathic effect and viral plaque formation by reducing the expression of TRIM27. In vivo, miR-324-3p decreased the expression of TRIM27,reduced cardiac viral replication and load, thereby strongly attenuating cardiac injury and inflammation. Taken together,this study suggests that miR-324-3p targets TRIM27 to inhibit CVB3 replication and viral load, thereby reducing the cardiac injury associated with VM.

Background Sinus venosus atrial septal defect（SVASD） is a rare atrial septal defect. The right-sided pulmonary veins may drain into the right atrium directly or via vena cava. Two-dimensional transthoracic echocardiography（2D-TTE） is certainly the initial choice of evaluation of SVASD but is unable to show its spatial relation to the surrounding structures and the configuration of the veins accurately. The purpose of this study was to evaluate the diagnostic accuracy of real-time three-dimensional transesophageal echocardiography（RT3D-TEE） for detecting SVASD and partial anomalous pulmonary venous connection（PAPVC）. Methods2D-TTE and RT3D-TEE were performed in 25 patients with the SVASD before surgical repair. Records of these patients were compared with surgical assessments. Results Twenty-three of the 25 patients with SVASD were detected by 2D-TTE. All the 23 patents were associated with PAPVC. Two of the 25 patients with SVASD were misdiagnosed as the secundum atrial septal defect by 2D-TTE. SVASD was demonstrated in all the 25 cases by RT3D-TEE, and 25 patents were associated with PAPVC. Conclusions RT3D-TEE is highly accurate, scientific method for displaying the location, size, shape of the defect and its spatial relation to the surrounding structures, and provides additional value to the surgeon and physicians for better understanding of spatial intracardiac morphology and making more accurate diagnosis and treatment.

The effects of Xuezhikang on vascular remodeling and aortic fibers in spontaneously hypertensive rats

Background Xuezhikang is a traditional Chinese medicine for the therapy of patients with cardiovascular diseases. However, the effect of Xuezhikang on vascular remodeling and aortic fibers in spontaneously hypertensive rats remains unclear. Methods Thirty spontaneously hypertensive rats（SHRs） aged 8 weeks were randomized into three groups： SHRs control group（SHRs group, n = 10）, group treated with low dose Xuezhikang（XZK-L,20 mg/kg/d, n = 10） and group treated with high dose Xuezhikang（XZK-H, 200 mg/kg/d, n = 10）. The normal group was comprised of ten Wistar-Kyoto（WKY） rats of the same age. While under pentobarbital anesthesia, in vivo aortic stiffness was determined by a pulse wave velocity（PWV） technique and measured locally in the descending thoracic aorta by Doppler ultrasound echocardiography. Fixed tissues were sectioned and stained with hematoxylin and eosin, masson trichrome, aldehyde-fuchsin and Gordon-Sweets stain. Results The PWV, thicknesses of the aortic wall, wall-to-lumen（W/L） ratio, elastic fibers thicknesses and collagen volume fraction were measured. XZK-L and XZK-H groups were observed to be significantly greater than those in the normal group（P 〈 0.01）. The levels of PWV, W/L ratio and collagen fibers were observed to be significantly reduced in the XZK-H group when compared with the control group（P 〈 0.05）. There was no significance in reticular fiber distributing between the XZK-treated group and SHR group. The reticular fibers in SHR rats were obviously more than those of WKY rats. Conclusion The hypertensive vascular remodeling is associated with changes of elastic fibers and collagen and reticular fibers. Xuezhikang improves hypertensive vascular remodeling by repairing the elastic fibers and inhibiting the levels of collagen.

Background and purpose Rapid genotyping is useful for guiding early antiplatelet therapy in patients with high-risk nondisabling ischaemic cerebrovascular events(HR-NICE).Conventional genetic testing methods used in CYP2C19 genotype-guided antiplatelet therapy for patients with HR-NICE did not satisfy the needs of the Clopidogrel in High-Risk Patients with Acute Nondisabling Cerebrovascular Events(CHANCE)-2 ***,we developed the rapid-genotyping GMEX(point-of care)system to meet the needs of the CHANCE-2 *** Healthy individuals and patients with history of cardiovascular diseases(n=408)were enrolled from six centres of the CHANCE-2 *** compared the laboratory-based genomic test results with Sanger sequencing test results for accuracy ***,we demonstrated the accuracy,timeliness and clinical operability of the GMEX system compared with laboratory-based technology(YZY Kit)to verify whether the GMEX system satisfies the needs of the CHANCE-2 *** Genotypes reported by the GMEX system showed 100%agreement with those determined by using the YZY Kit and Sanger sequencing for all three CYP2C19 alleles(*2,*3 and*17)*** average result’s turnaround times for the GMEX and YZY Kit methods were 85.0(IQR:85.0-86.0)and 1630.0(IQR:354.0-7594.0)min(p<0.001),*** Our data suggest that the GMEX system is a reliable and feasible point-of care system for rapid CYP2C19 genotyping for the CHANCE-2 trial or related clinical and research applications.

Background: Serum level of cholesterol is one of the most vital risk factors for cardiovascular diseases (CVD). Statins are highly effective drugs for reducing serum cholesterol;hence, preventing coronary heart disease (CHD). Rosuvastatin (Crestor) is one of the most potent and widely prescribed statins. Even though generic statins have been approved based on their bioequivalence with brand-name drugs, there remains considerable concern as regards their effectiveness and safety. Most clinicians and patients welcome the generic drug decreased costs;however, it is indispensable for them that effectiveness and safety are not compromised. Thus, the rationale intended for this study is to compare brand rosuvastatin and generic rosuvastatin as regard their economic impact using a cost-minimization analysis. Methods: This cost-minimization model estimates potential impact of rosuvastatin brand versus generic on the healthcare resource utilization for one-year frame from the payer perspective. The model conforms to real practice of management of hyperlipidemia in Egypt and was validated by experts. Results: The drug costs in the rosuvastatin brand group were 3,155,250 EGP while in the generic group were 2,299,030 EGP. The costs of CVD events in the rosuvastatin brand group were 5,863,558 EGP, while in the generic group were 6,810,180 EGP. The total costs in the rosuvastatin brand group were 9,018,808 EGP, while in the generic group were 9,109,210 EGP with a difference of −100,047 EGP. Conclusions: In conclusion, the real cost of generic treatment is more than that of the brand statin when taking into consideration the cardiovascular events.

Photoacoustic imaging(PAI)has been used to characterize the spatial and quantitative features of lipid-rich atherosclerotic plaques with high sensitivity and *** this Letter,we first validate that the ultra-low temperature and formaldehyde treatment have no effect on photoacoustic characteristics of the artery *** experiments between the PAI and histological results demonstrate that the ultra-low temperature or formaldehyde treatment has few effects on the PAI of the lipid-rich atherosclerotic plaques;the lipid relative concentration and the lipid percentage by PAI hold high correlation with histology.
