卫生经济学评价是对备选行动方案的成本和结果的比较分析。2013年发表的《卫生经济学评价报告标准共识》(CHEERS)提出了卫生经济学评价的特点,确保结果正确阐释,以支持决策制订。2013版CHEERS旨在指导研究报告撰写者准确报告卫生干预和对照措施、背景、过程、结果等评价细节,使审稿人理解文章内容,帮助读者使用研究结果。本版共识将取代2013版共识,其更加适用于各类卫生经济学评价,结合学科发展和方法更新的需要,更注重患者、公众等相关利益方的参与。本共识适用多种个体或人群健康干预措施(简单或复杂)在不同政策场景的应用分析,包括医疗服务、公共卫生、教育、社会服务等。本文概要介绍了CHEERS 2022共识中包含的28个检查项及每一个检查项的相关建议。CHEERS 2022共识主要用于指导研究人员撰写拟投递同行评议学术期刊的卫生经济学评价文章,亦可用于期刊编辑和审稿专家对待发表文章的评价。熟悉了解本共识要求,还有助于研究人员规划评价研究。随着决策透明度要求提高,本共识可支持卫生技术评估机构建立评价报告标准。

Proton pump inhibitors and colorectal cancer:A systematic review

BACKGROUND The use of proton pump inhibitors(PPI)is common worldwide,with reports suggesting that they may be *** studies have found that PPI may affect colorectal cancer(CRC)*** To summarize current knowledge on the relationship between PPI and CRC from basic research,epidemiological and clinical *** This systematic review was based on the patients,interventions,comparisons,outcome models and performed according to PRISMA ***,EMBASE,Scopus,and Web of Science databases were searched from inception until May 17,*** initial search returned 2591 articles,of which,28 studies met the inclusion criteria for this *** studies were categorized as basic research studies(n=12),epidemiological studies(n=11),and CRC treatment studies(n=5).The quality of the included studies was assessed using the Newcastle-ottawa Scale or Cochrane Risk of Bias 2.0 tool depending on the study *** Data from basic research indicates that PPI do not stimulate CRC development via the trophic effect of gastrin but instead may paradoxically inhibit *** studies also suggest that PPI may have properties beneficial for CRC *** appear to have anti-tumor properties(omeprazole,pantoprazole),and are potential T lymphokine-activated killer cell-originated protein kinase inhibitors(pantoprazole,ilaprazole),and chemosensitizing agents(pantoprazole).However,these mechanisms have not been confirmed in human *** epidemiological studies suggest that there is no causal association between PPI use and increased CRC *** studies show that concomitant PPI and capecitabine use may reduce the efficacy of chemotherapy resulting in poorer oncological outcomes,while also suggesting that pantoprazole may have a chemosensitizing effect with the fluorouracil,leucovorin,oxaliplatin(FOLFOX)*** An unexpected inhibitory effect of PPI on CRC carcinogenesis by way of several potential mechanisms is *** review

Objective:To describe and compare the epidemiological features of scorpionism during six years(2005–2010)in two regions of Morocco:Marrakesh-Tensift-Al Haouz and ***:Clinical and epidemiological data were obtained from medical records of the Moroccan Poison Control Center during 2005–*** data comprised demographics,sting characteristics and clinical severity *** maps were produced for envenomation and death incidence with the distribution of all scorpion species present on the studied ***:A total of 75313 scorpion sting cases were *** incidence of scorpion stings was 244 cases/100000 population/year and was significantly higher at Marrakesh-Tensift-Al Haouz than *** general lethality rate was on an average of 0.28%with a higher rate in Marrakesh-Tensift-Al Haouz compared to *** was a different distribution of cases between genders in the two studied *** respect to age groups,adults(more than 15 years)were affected most compared to *** analyzed according to the incidence in each province,the highest envenomation incidence was observed in *** lethality,the highest lethality incidence was observed in the *** on this study,we could distinguish three zones:low scorpion stings occurrence without death,high incidence with low lethality and high scorpion stings rate with high ***:Our data clearly demonstrate the correlation between scorpion stings incidence and the percentage of rural population in the different ***,the lethality incidence could be linked to the scorpion species of the studied area.

Approach to Helicobacter pylori infection in geriatric population

The prevalence of Helicobacter pylori(H. pylori) infection and its complications increase with age. The majority of infected individuals remain asymptomatic throughout the life but 10%-20% develops peptic ulcer disease and 1% gastric malignancies. The incidence of ulcers and their complications are more common in the older population resulting in higher hospitalization and mortality rates. The increased use of medications causing gastric mucosal damage and the decreased secretion of protective prostaglandins in elderly are major factors increasing gastric mucosal sensitivity to the destructive effects of H. pylori. Due to higher prevalence of gastrointestinal(GI) malignancies,upper GI endoscopy is mostly preferred in elderly for the diagnosis of infection. Therefore,"endoscopy and treat" strategy may be more appropriate instead of "test and treat" strategy for dyspeptic patients in older age. Urea breath test and stool antigen test can be used for control of eradication,except for special cases requiring follow-up with endoscopy. The indications for treatment and suggested eradication regimens are similar with other age groups; however,the eradication failure may be a more significant problem due to high antibiotic resistance and low compliance rate in elderly. Multidrug usage and drug interactions should always be consid-ered before starting the treatment. This paper reviews briefly the epidemiology,diagnosis,disease manifesta-tions,and treatment options of H. pylori in the geriatric population.

Preparedness of community-based organisations in biohazard: reliability and validity of an assessment tool

The purpose of this study was to develop a tool for community-based health organisations(CBHOs)to evaluate the preparedness in biohazards concerning epidemics or *** searched concepts on partnerships of CBHOs with health systems in guidelines of the Centers for Disease Control and Prevention and ***,we validated the researcher-made tool by face validity,content validity,exploratory factor analysis(EFA),confirmatory factor analysis(CFA)and criterion *** were collected by sending the tool to 620 CBHOs serving under supervision of Iran’s ministry of *** of health professionals and stakeholders in CBHOs were used to assess face and content *** loads in EFA were based on three-factor structure that verified by *** used SPSS V.18 and Mplus7 software for statistical *** 105 health-based CBHOs *** conducting face validity and calculating content validity ratio and content validity index,we reached 54 items in the field of planning,training and *** conducted construct validity using 105 *** items exchanged between the fields according to factor loads in EFA,and CFA verified the model fit as Comparative Fit Index,Tucker-Lewis index and root mean square error of approximation were 0.921,0918 and 0.052,*** Cronbach’s of the whole tool was *** correlation coefficient confirmed criterion validity as coefficient was ***,training and infrastructure fields are the most important aspects of preparedness in health-based *** the new assessment tool in future studies will show the weaknesses and capabilities of health-based CBHOs in biohazard and clear necessary intervention actions for health authorities.

目的通过循证和专家讨论研究法建立第一版针对脑卒中的一套综合ICF核心分类模板和与之相应的简明ICF核心分类模板。方法来自于不同国家和地区的国际专家从初步研究中搜集证据,结合这些证据进行正式的决策并最终达成共识。初步研究包括Delphi研究方法、对ICF分类的系统评价以及实证数据的收集。结果来自12个不同国家的39位专家出席会议并达成共识。专家初步研究决定在ICF分类的第二、第三和第四级水平上针对脑卒中确定一套448个类目的ICF核心分类摸板,其中193项类目属于身体功能,26项属于身体结构,165项属于活动和参与,64项属于环境因素。综合性ICF核心分类模板包括在ICF分类第二级水平上的130个类目,其中41项属于"身体功能",5项属于"身体结构",51项属于"活动和参与",33项属于"环境因素"。简明型ICF核心分类模板包括在ICF分类第二级水平上的18个类目,其中6项属于身体功能,2项属于身体结构,7项属于活动参与,以及3项属于环境因素。结论专家组通过整合研究过程中的证据,以及专家们基于ICF框架和分类的观点达成正式的共识,并确定针对"脑卒中"的ICF核心分类模板,同时界定综合型ICF核心分类模板和简要型ICF核心分类模板。

本文是《PLoS medicine》关于卫生系统知证政策指导三篇系列文章的第一篇,阐述了应如何将证据转化为指导,以形成卫生系统政策,并促进临床和公共卫生干预的实施。其要点如下:①薄弱的卫生系统阻碍了有效干预的实施,加强此系统的政策需要基于当前可得的最佳证据;②卫生系统证据最好以政策制定过程参考的指导的形式呈现,但目前卫生系统指导制定质量较差;③将问题、干预及实施相关的研究转化为影响卫生系统组织方式的决策和政策是卫生系统指导制定面临的一个挑战;④制定及时且适用于广大卫生系统利益相关者的指导,并确定评价卫生系统指导质量的方法是另一个挑战;⑤改编现有方法(如用于临床指南的那些方法)制定有意义的建议来应对卫生系统、政策系统及背景的复杂性,尚需进一步研究。

To review studies on hypertension in Nigeria over the past five decades in terms of prevalence,awareness and treatment and *** our search on Pubmed,African Journals Online and the World Health Organization Global cardiovascular infobase,1060 related references were identified out of which 43 were found to be relevant for this *** overall prevalence of hypertension in Nigeria ranges from 8%-46.4% depending on the study target population,type of measurement and cut-off value used for defining *** prevalence is similar in men and women(7.9%-50.2% vs 3.5%-68.8%,respectively) and in the urban(8.1%-42.0%) and rural setting(13.5%-46.4%).The pooled prevalence increased from 8.6% from the only study during the period from 1970-1979 to 22.5%(2000-2011).Awareness,treatment and control of hypertension were generally low with attendant high burden of hypertension related *** order to improve outcomes of cardiovascular disease in Africans,public health education to improve awareness of hypertension is *** epidemiological studies on hypertension are required to adequately understand and characterize the impact of hypertension in society.

Long term recurrence,pain and patient satisfaction after ventral hernia mesh repair

AIM: To compare long term outcomes of laparoscopic and open ventral hernia mesh repair with respect to recurrence, pain and ***: We conducted a single-centre follow-up study of 194 consecutive patients after laparoscopic and open ventral hernia mesh repair between March 2000 and June 2010. Of these, 27 patients(13.9%) died and 12(6.2%) failed to attend their follow-up appointment. One hundred and fifty-three(78.9%) patients attended for follow-up and two patients(1.0%) were interviewed by telephone. Of those who attended the follow-up appointment, 82(52.9%) patients had received laparoscopic ventral hernia mesh repair(LVHR) while 73(47.1%) patients had undergone open ventral hernia mesh repair(OVHR), including 11 conversions. The follow-up study included analyses of medical records, clinical interviews, examination of hernia recurrence and assessment of pain using a 100 mm visual analogue scale(VAS) ruler anchored by word descriptors. Overall patient satisfaction was also determined. Patients with signs of recurrence were examined by magnetic resonance imaging or computed tomography ***: Median time from hernia mesh repair to follow-up was 48 and 52 mo after LVHR and OVHR respectively. Overall recurrence rates were 17.1% after LVHR and 23.3% after OVHR. Recurrence after LVHR was associated with higher body mass index. Smoking was associated with recurrence after OVHR. Chronic pain(VAS > 30 mm) was reported by 23.5% in the laparoscopic cohort and by 27.8% in the open surgery cohort. Recurrence and late complications were predictors of chronic pain after LVHR. Smoking was associated with chronic pain after OVHR. Sixty point five percent were satisfied with the outcome after LVHR and 49.3% after OVHR. Predictors for satisfaction were absence of chronic pain and recurrence.
