Objective:To review follow-up imaging after equivocal bone scans in men with castration resistant prostate cancer(CRPC)and examine the characteristics of equivocal bone scans that are associated with positive follow-up ***:We identified 639 men from five Veterans Affairs Hospitals with a technetium-99m bone scan after CRPC diagnosis,of whom 99(15%)had equivocal *** with equivocal scans were segregated into“high-risk”and“low-risk”subcategories based upon wording in the bone scan *** follow-up imaging(bone scans,computed tomography[CT],magnetic resonance imaging[MRI],and X-rays)in the 3 months after the equivocal scan were *** were compared between patients with a positive *** follow-up imaging after an equivocal bone ***:Of 99 men with an equivocal bone scan,43(43%)received at least one follow-up imaging test,including 32/82(39%)with low-risk scans and 11/17(65%)with high-risk scans(p=0.052).Of follow-up tests,67%were negative,14%were equivocal,and 19%were *** those who underwent follow-up imaging,3/32(9%)low-risk men had metastases vs.5/11(45%)high-risk men(p=0.015).Conclusion:While 19%of all men who received follow-up imaging had positive follow-up imaging,only 9%of those with a low-risk equivocal bone scan had metastases versus 45%of those with *** preliminary findings,if confirmed in larger studies,suggest follow-up imaging tests for low-risk equivocal scans can be delayed while high-risk equivocal scans should receive follow-up imaging.

Evidence on the lifetime risk for atherosclerotic cardiovascular disease （ASCVD） is insufficient; yet, estimating an individual＇s lifetime risk allows for a comprehensive assessment of ASCVD burden. We developed and validated lifetime risk prediction equations for ASCVD using four large and ongoing prospective cohorts of Chinese, the China-PAR project （Prediction for ASCVD Risk in China）. Sexspecific equations were developed using two cohorts （as the derivation cohort） of 21,320 participants. Two other independent cohorts with 14,123 and 70,838 participants were used for their external valida- tion, respectively. We evaluated both calibration and discrimination measures for model performance. Furthermore, we estimated ASCVD-ffee years lost or excess absolute risk attributable to high 10-year risk （≥10.0%） and]or high lifetime risk （≥32.8%）. After 12.3 years＇ follow-up of the derivation cohort, 1048 ASCVD events and 1304 non-ASCVD deaths were identified. Our sex-specific equations had good internal validation, with discriminant C statistics of 0.776 （95% confidence interval [CI]： 0.757-0.794） and 0.801 （95% CI： 0.778-0.825）, and calibration Z2 of 9.2 （P = 0.418） and 5.6 （P = 0.777） for men and women, respectively. Good external validation was also demonstrated with predicted rates closely matched to the observed ones. Compared with men having both low 10-year and low lifetime risk, men would develop ASCVD 3.0, 4.6 and 8.6 years earlier if they had high 10-year risk alone, high lifetime risk alone, or both high 10-year and high lifetime risk at the index age of 35 years, respectively. We developed well- performed lifetime risk prediction equations that will help to identify those with the greatest potential to avert ASCVD burden after implementation of innovative clinical and public health interventions in China.

自新型冠状病毒肺炎(coronavirus disease 2019, COVID-19)疫情发生以来,中国大多数的病例集中在武汉市。虽然早期病例数和死亡人数迅速增加,但通过采取多种防控措施,疫情得以快速遏制。纵观全球,疫情已蔓延至全球六大洲的187个国家,确诊病例数已超过300万,这一数字仍在快速增长。在此特殊背景下,有必要对我国疫情防控措施开展科学的、定量的评估,为全球疫情防控提供决策依据。为此,本研究评估了临床诊断标准实施和全城症状排查对武汉市疫情控制的贡献。考虑COVID-19的传播机理、隔离措施等,建立SEIR+Q传播动力学模型。基于武汉市截至2020年2月14日官方公布的每日确诊病例数和未确诊的临床诊断病例数进行建模,并预测2月14日以后的疫情态势。基于实际疫情数据,与模型预测趋势相比较,评价防控措施效果。结果显示,若维持2月14日以前防控措施不变,那么预测将于3月25日和4月29日,每日新增病例数分别降至100例和10例以下,将于5月31日首次现零。而事实上,截至3月6日,武汉市每日新增病例数降至100例以下,截至3月11日降至10例以下,3月18日首次实现零增长,较之模型预测结果分别提前了19 d、49 d和74 d。截至3月30日,实际累计病例数为50 006例,比模型预测值减少19 951例。有效再生数[effective reproductive number, R(t)]分析显示,2月6-10日的第一次全城症状排查后,R(t)显现出下降趋势,2月12-14日的临床诊断标准实施和2月17-19日的第二次全城症状排查后,R(t)显现出较大的降幅,与实际情况较为一致。综上所述,武汉市临床诊断标准的实施和全城症状排查等综合防控措施成效显著,可为世界各国的疫情防控决策提供科学依据。

In-hospital outcomes of transapical versus surgical aortic valve replacement: from the U.S. national inpatient sample

As a rare autosomal-recessive metabolic disorder, cystinosis is caused by defective transport of cystine across the lysosomal membrane. Once the diagnosis of cystinosis is confirmed, specific treatment with cysteamine, an aminothiol, should be applied to the patient as soon as possible in order to preserve renal function and improve growth in affected *** is the only specific drug approved by the Food and Drug Administration for cystinosis. Although it cannot reverse the existing kidney damage, it was reported that 1 7 patients treated with cysteamine for 7 years recovered almost normal kidney function and, more importantly, they grew *** report suggested that cysteamine treatment could postpone end-stage kidney damage for 10-20 years. However, the immediate challenge lies in the access to the necessary medication in China for the management ofcystinosis. Here, we report the case of two sibling boys with cystinosis in order to raise awareness to the international community about the urgent need for worldwide policy and action for such rare diseases.

Quasi-Binomial Regression Model for the Analysis of Data with Extra-Binomial Variation

Objectives: Developing inference procedures on the quasi-binomial distribution and the regression model. Methods: Score testing and the method of maximum likelihood for regression parameters estimation. Data: Several examples are included, based on published data. Results: A quasi-binomial model is used to model binary response data which exhibit extra-binomial variation. A partial score test on the binomial hypothesis versus the quasi-binomial alternative is developed and illustrated on three data sets. The extended logit transformation on the binomial parameter is introduced and the large sample dispersion matrix of the estimated parameters is derived. The Nonlinear Mixed Procedure (NLMIXED) in SAS is shown to be very appropriate for the estimation of nonlinear regression.

Hepatocellular carcinoma in non-cirrhotic liver: A comprehensive review

Hepatocellular carcinoma(HCC) is the most common type of primary liver cancer, which in turns accounts for the sixth most common cancer *** being the 6 th most common cancer it is the second leading cause of cancer related deaths. HCC typically arises in the background of cirrhosis, however,about 20% of cases can develop in a non-cirrhotic liver. This particular subgroup of HCC generally presents at an advanced stage as surveillance is not performed in a non-cirrhotic liver. HCC in non-cirrhotic patients is clinically silent in its early stages because of lack of symptoms and surveillance imaging; and higher hepatic reserve in this population. Interestingly, F3 fibrosis in non-alcoholic fatty liver disease, hepatitis B virus and hepatitis C virus infections are associated with high risk of developing HCC. Even though considerable progress has been made in the management of this entity, there is a dire need for implementation of surveillance strategies in the patient population at risk, to decrease the disease burden at presentation and improve the prognosis of these patients. This comprehensive review details the epidemiology, risk factors, clinical features,diagnosis and management of HCC in non-cirrhotic patients and provides future directions for research.

Body Weight Misperception and Weight Disorders among Chinese Children and Adolescents:A Latent Class Analysis
