Pro-Oxidant Antioxidant Balance in Inflammatory Bowel Disease

Background: Inflammatory bowel disease (IBD) is associated with increased morbidity and incident of colon cancer;however its etiology is still unclear. Currently, one of the most probable pathogenesis patterns is increased permeability of bowel membrane and it seems that oxidative stress plays an important role in this pathway. This study was done to assess the pro-oxidant antioxidant balance (PAB) in these patients. Materials: This was a cross sectional study of 2 groups including 50 patients with diagnosed IBD and 50 healthy controls. Patients were selected purposively from those referring to adult gastroenterology clinic in 2013. SPSS (ver11.5) has been. A P value < 0.05 was regarded as statistically significant. Results: Mean PAB in patients and controls group was 119.98 ± 38.98 HK unite and 52.67 ± 22.80 HK unite, respectively, (P value < 0.001). PAB mean in ulcerative colitis patients was 120.60 ± 33.90 HK unite and in CD patients was 118.20 ± 45.99 HK unite, and showed no significant difference (P value = 0.85). PAB and MDA could detect healthy subjects from IBD patients with sensitivity more that 90% and specificity more than 84%. Conclusion: PAB shifted to pro-oxidants in IBD patients;moreover this shift was unrelated to disease type. These tests could use as screening test.

Assessment of mechanism, type and severity of injury in multiple trauma patients： A cross sectional study of a trauma center in Iran

Purpose： To accurately assess the mechanism, type and severity of injury in Iranian multiple trauma patients of a trauma center. Methods： Patients with multiple traumas referring to the emergency department of Hasheminejad university hospital in mashhad, lran, entered this cross sectional study from March 2013 to December 2013. All the patients with injury severity score （ISS）〉 9 were included in this study. Data analysis was performed by SPSS software （Version 11.5） and P values less than 0.05 were considered as significant differences. Results： Among the 6306 hospitalized trauma patients during this period, 148 had ISS〉9. The male fe- male ratio was 80%. The mean age of the patients was （33.5 m 19.3） years. And 71% of the patients were younger than 44 years old. There were 19 （13%） deaths from which 68.5% were older than 44 years old. The mean transfer time from the injury scene to hospital was （55 e 26） minutes. The most frequent mechanisms of injury were motorcycle crashes and falling from height, which together included 66.2% of all the injuries. A total of 84% of hospital deaths occurred after the first 24 h of hospitalization. Head and neck were the most common body injured areas with a prevalence of 111 cases （75%）. Conclusion： Motorcycle crashes have high frequency in lran. Since most victims are young males, injury prevention strategies should be considered to reduce the burden of injuries.

Background: Coccydynia is a painful condition with tenderness and ache in sacrococcygeal area which may radiate to the buttocks and lower back. Coccydynia is a multifactorial disorder and the most common cause of it is trauma. The initial treatment of the disease is conservative methods. One of the most common conservative treatments of chronic coccydynia is the local corticosteroid injection, which performed usually by palpation-guided method. This study was conducted to compare the effectiveness of local corticosteroid injection using palpation and needling method with palpation method alone. Patients and Methods: In this prospective study, 50 patients with the diagnosis of coccydynia were enrolled between 2010 and 2017. All patients had chronic coccydynia which did not respond to conservative therapy for at least three months. Participants were divided into two groups and each group consisted of 25 individuals. Patients who underwent local corticosteroid injection using palpation guide technique alone were assigned as group A and others who underwent palpation guiding injection with needling technique were considered as group B. Resternotomy Following Sternal Bone Cement Implantation: A Great Challenge for Cardiac Surgeons

Background: Median sternotomy and resternotomy is the standard technique for coronary artery bypass grafting (CABG), valvular heart disease, and congenital heart disease. Despite ad-vances in many areas of cardiac surgical procedures, there is a lack of innovation in sternal closure techniques. Several studies have examined sternal closure techniques including wiring, interlocking, plate and screw, and bone cementation. However, none of them achieved widespread acceptability. On one hand, serious post-operative complications are associated with the use of wiring and plating techniques in high-risk patients. The aim of this study is showing challenges and difficulties with resternotomy in patient with a history of previous cardiac surgery and usage of biologic bone cements. Case Report: The case was a 56-year-old woman with a history of previous sternotomy for mitral and aortic valve replacement (mechanical sj. No. 29 and mechanical sj. No. 21, respectively) using biologic bone cement (Kryptonite TM, Doctors Research Group Inc.) for her osteopenic sternum. Four years after the mitral valve replacement (MVR), she was referred to emergency department with a thrombosis at the mitral valve. She under-went emergent cardiac surgery with a very difficult resternotomy under femoral cannulation support. Conclusion: Resternotomy in patients with previous sternotomy with Kryptonite bone cements or calcium phosphate cements (CPC) is safe and can be done similar to other cardiac reoperations. It seems that reoperation in this patients does not increase the risk of bleeding, morbidity, and mortality.

BACKGROUND: In Iran, few studies have evaluated emergency medicine.as a career option. In the present study, we aimed to find out how Iranian emergency-medicine.specialists view their specialty as a ***: Following a qualitative study, a Likert-scale questionnaire was developed. Iranian emergency physician specialists who had at least two years' job experience were contacted via email. A uniform link to a Web-based survey and a cover letter that explained the survey were sent to the recipients. We used the Kruskal-Wallis test and post hoc analysis to determine the differences between demographic ***: A total of 109 eligible responses were received, a response rate of 72.63%. Of the responders, 57.8% were 30–40 years of age, 86.2% were male, 86.2% were single, 84.4% were faculty members and 90.8% had fewer than 10 years' job experience. The main problems occurring during the career of Iranian emergency physicians were: insuffi cient income, inadequate recognition of the specialty by the community, inadequate union support, insecurity in the emergency wards, overcrowding, job stresses and night shifts. Despite insufficiency of income, Iranian emergency physicians(EPs) did not care about the financial benefits of patient care. Academic activity had positive effects on the perspectives of Iranian emergency physicians regarding their ***: Iranian emergency physicians and leaders in emergency medicine.should struggle to improve the present situation, aiming at an ideal state.

Synbiotic for Prevention of SARS-Cov2 Infection in High Risk hospital Staffs: A Randomized Controlled Trial

Introduction: COVID-19 pandemic caused by the novel coronavirus SARS-CoV-2 increasingly involves people worldwide. Probiotics can improve immune system functions via different mechanisms. We proposed that Synbiotic Lactocare? may also reduce SARS-Cov2 infection in high-risk medical staff working in COVID-19 hospital wards. Method: In a randomized, controlled trial, 60 hospital staff without any history of clinical or laboratory evidence of SARS-Cov2 infection were received either once-daily oral synbiotic capsule (Lactocare?) that contains 1 billion CFU/Cap of L. (Lactobacillus) casei, L. rhamnosus, Streptococcus thermophilus, Bifidobacterium breve, L. acidophilus, Bifidobacterium infantis, L. bulgaricus, and Fructooligosacharide (Zist Takhmir, Tehran, Iran) or placebo with the same appearance for 30 days. They were followed for two months. Result: During the two-month period of this study, SARS-COV-2 RT-PCR test results were positive in three participants (9.67%) in placebo group compared to zero positive tests in synbiotic group. The differences were not statistically significant (p = 0.238). During the study, two persons (7 %) of placebo group had respiratory complaints such as cough, rhinorrhea and/or dyspnea, compared with one in synbiotic group (p = 0.492). Conclusion: This study showed that overall frequency of SARS-COV2 infection in participants receiving synbiotic and those receiving placebo did not differ significantly. Impact of intravenous acetaminophen therapy on the necessity of cervical spine imaging in patients with cervical spine trauma
