NUCLEOLAR ORGANIZER REGIONS IN CUTANEOUS T CELL LYMPHOMAS

Paraffin sections from 50 mycosis fungoides (22 MFI-Premycotic stage 24 MF Ⅰ infiltrative stage and 4 MF Ⅲ - tumor stage) 2 nonepidermotropic cutaneous T cell lymphoma (NECTCL) and 9 CPL were investigated. In each case 200 cells randomly selected were examined using a × 100 oil immersion lens. The mean number standard deviation and standard error of the mean of AgNOR counts were as follows: MFⅠ 1.17±0.09 SEM = 0.01 MⅡ 1.17±0.01 SEM = 0.01 MF Ⅲ. 3.55±0.87 SEM = 0.43 NECTCL 4.5±0.28 SEM -0.199 CPL 1.17±0.1 SEM ± 0.03. The results revealed a highly significant difference between CTCL (MFⅢ+NECTCL) and CPL (t = 4.75 P<0.001) tumor stage (MF Ⅲ) and pretumor stage (MFI MF Ⅱ) of mycosis fungoides (t = 4.75 P<0.001). Thus. AgNOR count is valuable in differential diagnosis.

CHARACTERISTICS OF MYCOSIS FUNGOIDES CELLS AND THEIR DIAGNOSTIC SIGNIFICANCE

This paper reports the morphology of mycosis fungoides (MF) cells observed in 112 specimens of skin lesions of various developmental stages and 3 lymph nodes from 85 cases with MF. After reviewing the tissue imprints, the findings of electron micro-scope and immunological assays, and the results of clinical follow-up, it seems most likely that MF cells are derived from peripheral helper T cells during the process of transformation and their changes of morphology and size are in the form of a spectrum. The presence of MF cells in MF is a decisive factor for its diagnosis.

Myospherulosis is a rare disorder that may develop in various organs including the skin. It is characterized by a typical histological presentation resulting from the interaction between erythrocytes and lipids. We present a case of cutaneous myospherulosis associated with membranous lipodystrophy. The lesions were supervening on severe steroid-induced skin atrophy upon which ointments had been applied.

Onychomycosis is a frequent infection. Contagion rarely depends on direct contamination, but rather on environmental propagule dispersion including the intervention of fomites. The potential release of fungal cells in the environment from the affected nails has not been thoroughly studied. Observations made by histomycology suggest that arthroconidia and chlamydoconidia issued from invading fungi can be involved. In addition, the sporodochium, which is an exophytic fungal ball, may be attached underneath the distal free edge of some onychomycoses. It might represent an important source of fungal propagules in patients with poor nail trimming.

Preterm premature rupture of membranes (PPROM) likely results from weakening of the constituent connective tissue. It is uncertain if the alterations are limited to the fetal membranes or are also present in other sites such as the skin. The aim of the study was to compare the dermal structure of women suffering from recurrent PPROM with the dermis of both nonpregnant women and women with uneven tful gestation. Skin biopsies were taken from 42 women who recently under went P PROM, 33 women with uneventful gestation and 33 nonpregnant women. Histochemistr y, immunohistochemistry, and electron microscopy were performed on these specime ns and analyzed blindly. Morphologic changes were found in the dermis of most (3 3 of 42 at optical microscopy and 40 of 42 at ultrastructure) of the women suffe ring from PPROM. They were reminiscent of those found in some types of Ehlers-D anlos Syndrome (EDS). However, there was no clinical evidence suggesting a recog nized form of EDS. We conclude that some cases of PPROM are a systemic connectiv e tissue disorder representing a yet unidentified type of EDS.

Two cases of diffuse dermal angiomatosis are reported in middle-aged women. This rare disease of unknown origin is characterized by increased dermal angiomatosis and ulceration. The clinical and histologic presentations of the presently reported lesions were typical for this disorder. Endothelial cells exhibited a normal immunophenotype. The perivascular basement membranes showed a distribution of collagen αchains typical for blood vessels, but not for lymphatics. Immunohistochemistry revealed other undescribed features. At the site of the clinical lesions, linear and granular deposits of immunoglobulins A and M, and complement were found around the vessels and at the dermal-epidermal junction. The same deposits were also found restricted to the dermal-epidermal junction in the peripheral clinically intact skin. No serological signs of auto-immune disorder were detected in one patient. A monoclonal gammopathy was disclosed in the other patient. A pattern of immunoreactant deposits similar to that disclosed in the two patients was not found in the control specimens, and has not been described so far in other types of vascular hyperplasia and neoplasia. A pathogenic role of these deposits is unsettled and should be further explored.

The histologic diagnosis of desmoplastic melanoma can be challenging in circumstances in which biopsy specimens are small, or there are unsuspected clinical settings, re-excision scars, and unusual sites. This is particularly true when the overlying junctional component is absent or the spindle cells lack melanin pigment. In these instances, the importance of immunohistochemistry cannot be overemphasized. S-100 protein is the primary immunohistochemical stain used for this purpose, with a sensitivity approaching 90%. HMB-45, although a more specific marker for melanocytes, is less sensitive and often negative in these cases. In addition, NGFR, a marker of Schwannian differentiation, has been shown to be a useful confirmatory stain for desmoplastic melanoma, with staining intensity comparable with, or better than that of S-100 protein. We report 2 cases of desmoplastic melanomas that stained only focally and weakly with S-100 protein, but showed diffuse and intense staining with NGFR. In both cases, S-100 staining could have been interpreted as non-confirmatory, thus misguiding the diagnosis. We suggest that NGFR can be a useful complementary marker to S-100 in those desmoplasticmelanomas in which staining for S100 protein is only focal or weak.

Background: There are few reports of the histological changes in rosacea,and little attempt has been made to correlate such changes with clinical findings. In the present study,we describe in detail the histopathological features of rosacea in a large number of patients and simultaneously investigate the aetiopathogenesis of the disease based on the comparative assessment of epidemiological,clinical and histological findings. Methods: The study included 73 patients with rosacea. A skin biopsy with a 4-mm punch was performed in each case. All biopsy specimens included subcutaneous tissue. In 10 randomly selected patients,facial biopsy specimens were obtained from both involved and uninvolved (nonlesional) skin. Demodexmite presencewas estimated semi-quantitatively under light microscopy. Patients with self-reported gastro-intestinal symptoms were submitted to upper gastro-intestinal endoscopy,and a rapid urease test was performed. Serological antibodies,IgG and IgA,against Helicobacter pylori were also detected. Results: The patients had a broad clinical spectrum of lesions. No specific histological features associated with either erythematous-telangiectatic or papulopustular clinical forms were noticed. Histological examination showed that there is no histological pattern unique to rosacea. Three different types of granulomaswere observed: small palisaded ones around altered collagen and other granulomas of elastolytic and non-specific epithelioid type,all coexisting in 5 cases. The deep dermis and subcutis were frequently involved. Comparative study in 10 rosacea patients between lesional and nonlesional skin biopsies revealed almost the same histological changes to the latter biopsies,to a lesser degree though. Conclusion: Rosacea seems to be a reaction pattern to which a variety of pathogenetic routes may lead.

The patient was a 77-year-old male former smoker, with history of several basal cell carcinomas (BCCs) in sun-protected areas around the waistline, who presented with another small ulceration on the anterior right upper scrotum near the base of the penis. BCC was suspected clinically and the lesion was treated with cryosurgery. The tumor recurred, became raised, and began to bleed. On examination, itwas found to be a 3 cm ×4 cm, raised, light and dark gray variegated lesion with fissures, crusted bloody scale, and *** were no other penile, testicular, or scrotal lesions or masses. There was no regional lymphadenopathy. An excisional biopsy was performed. It showed nodular BCC surrounded by a cellular proliferation of round histiocytic cells with convoluted, lobulated and reniform nuclei and abundant cytoplasm (Fig. 1). The histiocytic proliferationwas associatedwith numerous eosinophils. Histological features and immunohistochemical phenotype, including expression of CD1 a and S-100 antigens by histiocytic cells, were all consistent with Langerhans cell histiocytosis. As expected, BCC expressed keratin. The patient had no history of exposure to ionizing radiation, chemotherapy, immunosuppressive medications, prior lymphoma or other malignancy. However, he spent 4 years on a ship loading coal into the furnace of a steam engine, during which he slept in adjacent quarters that were covered with coal dust. Additionally, he had a severalyear history of occupational skin exposure to machine oil, oil refinery waste, sulfur waste, hydraulic fluid, and asbestos. He also reported a history of nude sunbathing His current medical problems include pleural asbestosis, poorly controlled type II diabetes, and congestive heart failure. His past dermatologic history includes intertrigo, onychomycosis, and an unknown skin lesion removed from the face 40 years ago.
