Background: Much is known about the worldwide rising incidence of skin cancers in the white populations. However,contradictory reports have been presented about a genderlinked risk for malignant melanoma (MM). Objective: To review the recent epidemiological information about primary and metastatic MM in south-east Belgium. Methods: Laboratory records of primary MM according to age and gender were compared per 3-year periods from 1988 to 2002. The numbers of MM per decade of patient age were corrected with regard to the number of citizens in the defi ned periods of age. This led to the calculation of the rate of estimated prevalence (REP). The same assessments were performed on 187 sentinel lymph node biopsies collected during the last 3-year period. Results: A total of 1,051 primary MM in women and 594 primary MM in men were diagnosed over the 15-year period. The femaleto-male ratio rose by 24%over time,with a mean value of 1.77. The most recent years showed a marked increase both in the numbers and REP of primary MM. This situationwas present in both genders,although it was most prominent in women of the reproductive age. MM metastases were found in 38/187 sen-tinel lymph nodes with a female-tomale ratio of 2.17. The age distribution for all sentinel lymph node biopsies copied that of primary MM,but the vast majority of the nodal metastatic MM was found in patients older than 50 years. The ratio between MMmetastases and primary MM reached 6.1%(12/198) inmen and 6.9%(26/378) in women. Conclusion: A recent increase in primary MM incidence was observed in our laboratory. The most likely cause is ascribed to more sensitive MM detection using the combination of clinical dermoscopy and laboratory immunohistochemistry. During the past 15 years,women were more affected than men by both primary and metastatic MM. Although patients with neurofibromatosis (NF) have an increased incidence of tumors, there is only one study that suggests that NF1 patients may have an incre ased risk for epithelial malignancies. We present a patient with known NF1 who h ad developed multiple epithelial tumors since early in his life. All but one of these tumors were morphologically most consistent with trichoepitheliomas. Loss of heterozygosity (LOH) for Patched 1 gene (PTCH) was demonstrated within two of the trichoepitheloma-like tumors and one tumor diagnosed as basal cell carcino ma, and the patient was show to have a PTCH gene deletion. To our knowledge, a p atient presenting with both NF1 and multiple trichoepitheliomas (MTE) has not pr eviously been reported. The dysregulation in cellular proliferation and signalin g induced by decreased NF1 along with the PTCH gene mutation may explain the pat tern of immunohistochemical staining within these tumors, and the rare associati on of NF1 with epithelial neoplasms.

Thepatientisa63- year-old woman who presented to her dermatologist for the removal of two nevi near her mouth. Histologic examination revealed melanocytic nevi showing maturation with dermal descent. Within the dermis, adjacent to these nevoid cells, were multiple large hair follicles that were surrounded by dense fibrosis. The fibrous sheaths contained thickened,dense collagen bundles and were well circumscribed, resembling perifollicular fibromas. Further discussion with the patient and the dermatologist revealed that the patient had “ plucked” hair from these nevi. We, therefore, believe that the perifollicular fibrosis that we observed is secondary to trauma. We present this case to remind all that posttraumatic events can simulate perifollicular fibromas and that the erroneous diagnosis of such could lead to the erroneous diagnosis of Birt- Hoggs Dubé syndrome.

Background: The histological diagnosis of malignant melanoma can be challenging. Immunohistochemical techniques may define a critical role in certain cases, specifically in establishing a primary diagnosis of melanoma. CD34 is a hemopoietic stem cell antigen expressed in bone marrow and endothelial cells, and may also be expressed in vascular and spindle cell tumors; it is generally negative in malignant melanoma. Case report: An 83-year-old white female presented with a 3-4 mm area on her right upper back, which had been present for several years. Histologic sections showed a polypoid distortion by sheets and nodules of transformed amelanotic melanocytes lying in intimate apposition to an attenuated epidermis without a concomitant radial growth phase. Tumor cells were extensively S-100 and CD34 positive and showed focal immunoreactivity with melan-A and HMB-45. Discussions: We present a case of malignant melanoma of nodular subtype, which strongly expressed CD34. The spectrum of abnormal phenotypes in malignant melanoma is reviewed, and a possible explanation for the presence of GD34 is discussed. This case demonstrates the potential of malignant melanoma to express CD34, defining an infrequently recognized aberrant phenotype. Whether or not expression of this marker is associated with a more aggressive clinical course remains to be determined.
