Systemic immune-inflammation index (Sii) and Neutrophil-Lymphocyte Ratio (NLR) as Systemic inflammatory Predictors in the Diagnosis of Bullous Pemphigoid and Pemphigus Vulgaris

introduction: Autoimmune blistering skin disorders such as Bullous Pemphigoid and Pemphigus Vulgaris present diagnostic challenges. The Systemic immune-inflammation index (Sii) and Neutrophil-Lymphocyte Ratio (NLR), are inflammatory markers used to assess the body’s immune-inflammatory response. Objectives: The study aims to evaluate the significance of hematologic markers, specifically the Systemic immune-inflammation index (Sii) and Neutrophil-Lymphocyte Ratio (NLR), as diagnostic predictors of bullous pemphigoid (BP) and pemphigus vulgaris (PV). Methods: A retrospective study of 64 patients (36 with BP and 28 with PV). Patient clinical data: age, gender, complete blood count, autoimmune antibody levels (Dsg1, 3 and BP180, 230), igE and C-reactive protein, and history of hypertension, diabetes, brain infarction, and coronary heart disease. The data was analyzed using SPSS. Results: The study involved 36 (56.3%) diagnosed with bullous pemphigoid (BP) and 28 (43.75%) with pemphigus vulgaris (PV). The average age in BP was 71 ± 8 and 52 ± 13 in PV. Laboratory findings showed high levels of Dsg1, Dsg3, neutrophil count, and lymphocyte count in PV, while high levels of eosinophils with a significant increase in C-reactive protein (CRP) in BP. Blood biomarkers, including NLR, PLR, Sii, MPV, CRP, and igE, proved an overall of 84.4% in disease prediction. Dsg1, Dsg3, BP180, and BP230 showed an overall of 88.1%. No significant relationship was noted between NLR, Sii, and patients with comorbidities. Conclusion: The study highlights the diagnostic potential of Sii and NLR in addition to hematologic markers in BP and PV, emphasizing their role in early diagnosis and therapeutic interventions, requiring further validation in larger patient cohorts.

To the Editor:Actinic keratosis(AK)is a precancerous skin condition caused by chronic exposure to ultraviolet *** mainly occurs on sun-exposed areas like the head and face,leading to cosmetic concerns and an increased risk of progressing to primary squamous cell carcinoma(with a 0.60%risk at 1 year and 2.57%risk at 4 years).[1]Photodynamic therapy(PDT)is a targeted,non-invasive therapy that has been approved in Europe for the treatment of AK,squamous cell carcinoma in situ,and basal cell carcinomas.5-Aminolaevulinic acid(ALA)-based PDT,which utilizes the photosensitizer ALA,has shown high cure rates,low recurrence rates,and favorable cosmetic outcomes as arst-line treatment for AK.[1]However,due to variations in skin color,ethnicity,and lifestyle habits,the incidence of AK is lower among Asians compared to Caucasians.[1]There are limited clinical studies on PDT for AK in Asia—particularly lacking large-scale multicenter *** knowledge gap hinders the establishment of treatment standards specically tailored for Asian patients with AK undergoing ***,as the aging population increases in Asia,the incidence of AK demand for medical intervention among Asians is also rising *** conducted this multicenter study aiming to comprehensively investigate the efcacy and safety of ALA-PDT in Chinese patients with AK.

Neuroretinal dysfunction in patients affected by neurofibromatosis type 1

Peanut allergy is majorly related to severe food induced allergic *** food including cow's milk,hen's eggs,soy,wheat,peanuts,tree nuts(walnuts,hazelnuts,almonds,cashews,pecans and pistachios),fish and shellfish are responsible for more than 90%of food ***,we provide promising insights using a large-scale data-driven analysis,comparing the mechanistic feature and biological relevance of different ingredients presents in peanuts,tree nuts(walnuts,almonds,cashews,pecans and pistachios)and ***,we have analysed the chemical compositions of peanuts in different processed form raw,boiled and *** the data-driven approach we are able to generate new hypotheses to explain why nuclear receptors like the peroxisome proliferator-activated receptors(PPARs)and its isoform and their interaction with dietary lipids may have significant effect on allergic *** results obtained from this study will direct future experimeantal and clinical studies to understand the role of dietary lipids and PPARisoforms to exert pro-inflammatory or anti-inflammatory functions on cells of the innate immunity and influence antigen presentation to the cells of the adaptive immunity.

Systemic immune inflammatory index is Associated with Pustular Psoriasis: A Single Center Retrospective Study

introduction: Psoriasis is a chronic multi-systemic inflammatory skin disease that presents with erythema, thickness, and scaling of the skin. Genetic and environmental factors are associated with its etiology. Recently systemic immune inflammatory index, has been proposed as a biomarker for prognosis and severity prediction. Although it has been studied in psoriasis in general, no study exists for its association with the individual types of psoriasis. This study thus aimed to determine its association with clinical characteristics of psoriasis subtypes. Materials and Methods: Data were retrospectively retrieved from the hospital electronic medical database from January 2020 to August 2022. Only patients with CBC results were included. Clinical data retrieved were: Patients’ age, gender, type of psoriasis diagnosed, body mass index, duration of the disease, family history of psoriasis, history of smoking, diabetes, and hypertension records. Laboratory data retrieved were: Complete blood count (CBC), C-reactive protein, immunoglobulin E (igE), Total cholesterol, Triglycerides and Low-density lipoprotein cholesterol. Data were analyzed in SPSS and GraphPad prism. Results: The study enrolled 85 patients with psoriasis;56.47% males, and 43.53% females. 7.6% had psoriasis for less than 10 years, while 42.4% had the disease for more than 10 years. Psoriasis vulgaris was the most common diagnosis, 41.2%, followed by p. pustular, 30.6% and then p. erythroderma 28.2%. Mean age ± SD of the p. vulgaris, p. pustular and p. erythroderma patients were 47.3 ± 15.3;45.3 ± 14.6, and 57.1 ± 11.7 respectively. Sii was significantly higher in p. pustular than the rest, (p Conclusion: in summary, systemic immune inflammatory index (Sii) was significantly higher in psoriasis pustular than other subtypes of psoriasis, and had an association with hypertension in psoriasis pustular patients. Severe cutaneous adverse drug reactions： a review on epidemiology, etiology, clinical manifestation and pathogenesis

Purpose To review the current progress in epidemiology, etiology, clinical manifestation, and pathophysiology of severe cutaneous adverse drug reactions （SCADRs）. Data sources Data were acquired by using Blackwell-Synergy, PubMed, original articles published in the main Chinese journals and related medical textbooks materials. Study selection and data extraction Throughout the literature review 49 articles were selected. Results SCADRs cases are rare, however, the implication is life threatening with significant mortality rates. Epidemiology studies have shown various incidences from different regions, gender, age, race and concurrent illness. There are typical signs and symptoms for each type of SCADRs, but this is not always so. Drugs associated with inducing SCADRs are anticonvulsants, antibiotics, NSAiDs and antirheumatic drugs. in some countries, especially in Asia, traditional drugs are often the cause of SCADRs. Genetic polymorphisms and viral infections are predisposition factors of SCADRs. Patients with certain genetic alleles and underlying diseases are vulnerable to SCADRs. The exact pathogenesis of SCADRs is not well defined. Nonetheless, recent study showed that reactive metabolites and immunological processes have a significant role in SCADRs. Conclusions The different SCADRs reactions are attributed by different intrinsic factors, such as genetic polymorphisms, gender, age and race as well as extrinsic factors, such as underlying diseases. Different regions and culprit drugs also play a role in the various types of SCADRs.
