Background The presence of mental health conditions is pervasive in patients who experienced acute myocardial infarction(AMI),significantly disrupting their *** timely and easily accessible psychological interventions using virtual reality-based cognitive-behavioural therapy(VR-CBT)could potentially improve both acute and long-term symptoms affecting their mental *** We aim to examine the effectiveness of VR-CBT on anxiety symptoms in patients with AMI who were admitted to the intensive care unit(ICU)during the acute stage of their *** In this single-blind randomised clinical trial,participants with anxiety symptoms who were admitted to the ICU due to AMI were continuously recruited from December 2022 to February *** who were Han Chinese aged 18-75 years were randomly assigned(1:1)via block randomisation to either the VR-CBT group to receive VR-CBT in addition to standard mental health support,or the control group to receive standard mental health support ***-CBT consisted of four modules and was delivered at the bedside over a 1-week *** were done at baseline,immediately after treatment and at 3-month *** intention-to-treat analysis began in June *** primary outcome measure was the changes in anxiety symptoms as assessed by the Hamilton Anxiety Rating Scale(HAM-A).Results Among 148 randomised participants,70 were assigned to the VR-CBT group and 78 to the control *** 1-week VR-CBT intervention plus standard mental health support significantly reduced the anxiety symptoms compared with standard mental health support alone in terms of HAM-A scores at both post intervention(Cohen’s d=−1.27(95%confidence interval(CI):−1.64 to−0.90,p<0.001)and 3-month follow-up(Cohen’s d=−0.37(95%CI:−0.72 to−0.01,p=0.024).Of the 70 participants who received VR-CBT,62(88.6%)completed the entire *** was the main reported adverse event(n=5).Conclusions Our results indicate that VR-CB

BACKGROUND There is scant literature on hepatocellular carcinoma(HCC)in patients with Budd-Chiari syndrome(BCS).AIM To assess the magnitude,clinical characteristics,feasibility,and outcomes of treatment in *** A total of 904 BCS patients from New Delhi,India and 1140 from Mumbai,India were *** prevalence and incidence of HCC were determined,and among patients with BCS-HCC,the viability and outcomes of interventional therapy were *** In the New Delhi cohort of 35 BCS-HCC patients,18 had HCC at index presentation(prevalence 1.99%),and 17 developed HCC over a follow-up of 4601 person-years,[incidence 0.36(0.22-0.57)per 100 person-years].BCS-HCC patients were older when compared to patients with BCS alone(P=0.001)and had a higher proportion of inferior vena cava block,cirrhosis,and long-segment vascular *** median alpha-fetoprotein level was higher in patients with BCS-HCC at first presentation than those who developed HCC at follow-up(13029 ng/mL vs 500 ng/mL,P=0.01).Of the 35 BCS-HCC,26(74.3%)underwent radiological interventions for BCS,and 22(62.8%)patients underwent treatment for HCC[transarterial chemoembolization in 18(81.8%),oral tyrosine kinase inhibitor in 3(13.6%),and transarterial radioembolization in 1(4.5%)].The median survival among patients who underwent interventions for HCC compared with those who did not was 3.5 years vs 3.1 mo(P=0.0001).In contrast to the New Delhi cohort,the Mumbai cohort of BCS-HCC patients were predominantly males,presented with a more advanced HCC[Barcelona Clinic Liver Cancer C and D],and 2 patients underwent liver *** HCC is not uncommon in patients with *** interventions and liver transplantation are feasible in select primary BCS-HCC patients and may improve outcomes.

Neutral and metallic *** and semiconducting surface layer in acceptor doped CeO_(2)

The monomolecular surface layer of acceptor doped CeO_(2) may become neutral and metallic or charged and *** is revealed in the theoretical analysis of the oxygen pressure dependence of the surface defects concentration in acceptor doped ceria with two different dopant types and operated under different oxygen *** published experimental data for highly reduced Sm0.2Ce0.8O1.9-x(SDC)containing a fixed valence dopant Sm3+are very different from those published for Pr0.1Ce0.9O_(2)-x(PCO) with the variable valence dopant Pr4+/Pr3+being reduced under milder *** theoretical analysis of these experimental results fits very well the experimental results of SDC and *** leads to the following predictions:the highly reduced surface of SDC is metallic and neutral,the metallic surface electron density of state is gs=0.9×10^(38)J-1·m^(-2)(1.4×1015eV^(-1)·cm^(-2)),the electron effective mass is meff,s=3.3me,and the phase diagram of the reduced surface has theα(fcc)structure as in the *** PCO a double layer is predicted to be formed between the surface and the bulk with the surface being negatively charged and *** surface of PCO maintains high Pr^(3+) defect concentration as well as relative high oxygen vacancy concentration at oxygen pressures higher than in the *** reasons for the difference between a metallic and semiconducting surface layer of acceptor doped CeO_(2) are reviewed,as well as the key theoretical considerations applied in coping with this *** that we make use of the experimental data and theoretical analysis available for acceptor doped ceria.

A numerical investigation into the effect of altering compression ratio, injection timing, and injection duration on the performance of a diesel engine fuelled with diesel-biodiesel-butanol blend

Using renewable fuels for diesel engines can reduce both air pollution and dependence on fossil fuels.A computer simulation was constructed to predict the performance,combustion characteristics,and NOx emissions of a diesel engine fuelled with diesel-biodiesel-butanol *** simulation was validated by comparing the modelling results against experimental data and a good agreement between the results was *** fuels used for the validation were diesel(B0),biodiesel(B100),diesel-biodiesel blend(B50),and two diesel-biodiesel-butanol blends with 45%diesel-45%biodiesel-10%butanol(Bu10)and 40%diesel-40%biodiesel-20%butanol(Bu20)by *** results showed that the addition of butanol reduced NOx emissions but deteriorated the engine *** aim of the current work was the numerical optimization of the different parameters to enhance the engine performance while using butanol to decrease NOx *** engine compression ratio(CR)varied from 14 to 24,in increments of *** injection timing(IT)was reduced from 30°before top dead centre(bTDC)to 5°bTDC in increments of 5°.Also,the fuel injection duration(IDur)was extended from 20°to 50°in increments of 10°.Results showed that the increase in the CR improved engine performance for the two investigated fuels,Bu10 and *** maximum engine brake power(BP),thermal efficiency(BTE),and minimum brake-specific fuel consumption(BSFC)of 1.46 kW,32.3%,and 0.273 kg/kWh respectively,were obtained when the Bu10 fuel was injected under the optimum conditions of 24 CR,15°bTDC IT,and 40°*** these optimum conditions,the BP,BTE,and BSFC improved by 3%-3.5%for Bu10 and Bu20 fuel blends compared with the base engine conditions of a CR of 22,30°IDur,and 10°bTDC *** heat release rate during the premixed phase increased when the IT was advanced,while the mixing-controlled combustion phase was enhanced when the IT was *** emissions increased with increasing CR,while both an increase in IDur at con

Integrase strand transfer inhibitors(INSTIs)have emerged as the first‐line choice for treating human immunodeficiency virus(HIV)infection due to their superior efficacy and ***,the impact of INSTIs on the development of neuropsychiatric conditions in people living with HIV(PLWH)is not fully understood due to limited *** this study,we conducted a cross‐sectional examination of PLWH receiving antiretroviral therapy,with a specific focus on HIV‐positive men who have sex with men(MSM)on INSTI‐based regimens(n=61)and efavirenz(EFV)‐based regimens(n=28).Participants underwent comprehensive neuropsychiatric evaluations and multimodal magnetic resonance imaging(MRI)scans,including T1‐weighted images and resting‐state functional *** to the EFV group,the INSTI group exhibited primarily reduced gray matter volume(GMV)in the right superior parietal gyrus,higher regional homogeneity(ReHo)in the left postcentral gyrus,lower ReHo in the right orbital part of the inferior frontal gyrus,and increased voxel‐wise functional connectivity for the seed region in the left inferior temporal gyrus with clusters in the right ***,the analysis revealed a main effect of antiretroviral drugs on GMV changes,but no main effect of neuropsychiatric disorders or their *** repeated analysis of participants who did not switch regimens confirmed the GMV changes in the INSTI group,validating the initial *** study demonstrated gray matter atrophy and functional brain changes in PLWH on INSTI‐based regimens compared to those on EFV‐based *** neuroimaging results provide valuable insights into the characteristics of brain network modifications in PLWH receiving INSTI‐based regimens。
