Comparative Evaluation of Implant Placement with Conventional and Digital Surgical Guide Techniques: Two Case Reports

Background: Implant placement using a conventional surgical guide and digital surgical guide techniques is well documented in the literature. The most frequently reported disadvantages of conventional surgical guide placement are lack of accuracy in implant placement when compared to three-dimensional assessment in digital technique. Other factors listed are longer time duration and the need for impression techniques. In this case report, the authors present a comparison between the two techniques and the time taken between both cases one done conventionally and another case by digital technique. Case Presentation: For the digital surgical guide, a 44-year-old, male reported with the chief complaint of missing teeth needing replacement was considered. For the conventional technique, a female patient aged fifty-seven who had gone through various dentists with an existing bridge was considered. This patient wanted a good outcome at a reasonable cost. In both cases, molars were missing and needed replacement. The steps for digital flow for a surgical guide and step-by-step conventional methods are both highlighted in this article. Conclusion: Hence the digital technique saved time and was accurate when compared to the conventional in our experience.

Exploring the Association between Oral Microbiome and Mild Cognitive Impairment: A Narrative Review

Objective: Some studies have investigated the association between oral microbiome and mild cognitive impairment (MCI). However, there needs to be more narrative reviews synthesizing this evidence. This study aimed to bridge this gap in the current knowledge. Methods: A comprehensive search was conducted on PubMed (MEDLINE) to identify studies examining the association between the oral microbiome and MCI. Search parameters and inclusion criteria were clearly defined, encompassing terms related to the oral microbiome, MCI, and their association. Two authors independently selected relevant studies and performed data extraction. Result: Four studies were included. Two cohort studies and two case-control reported an association between the oral microbiome and MCI. Conclusion: Based on the evidence synthesized from the included studies, the review suggests an association between MCI and the oral microbiome. Specifically, all included studies identified significant differences in the abundance of specific microbial species between individuals with MCI and those with normal cognitive function, underscoring the potential role of these species in neuroinflammatory diseases.

Bearing capacity of foundations on rock slopes intersected by non-persistent discontinuity

Rocks encountered in foundations of heavy structures are invariably intersected by discontinuities(joints).In the past,several studies have been performed by researchers to incorporate the effect of fully persistent joints in the assessment of the load-carrying capacity of ***,in the field,the joints are non-persistent,and an assumption of full persistency will underestimate the ***,Shaunik&Singh have studied the influence of non-persistency,number of joint segments and discontinuity orientation on the strength behaviour of rock specimens(Shaunik and Singh,2019).Bell’s approach can be used to obtain the bearing capacity of shallow foundations placed in jointed *** the present study,results of the experimental work(Shaunik and Singh,2019)conducted by Shaunik&Singh have been used to suggest expressions by extending Bell’s approach for computing bearing capacity of the foundation placed near the crown of a rock *** to use design charts are also presented for field ***,a real-life problem from Indian Garhwal Himalayas is considered,and the approach suggested in this study is utilised to obtain the bearing capacity of a bridge foundation as a function of uniaxial compressive strength(UCS)of intact rock,joint friction,spacing and orientation of joint,nonpersistency and number of joint segments.

The prosthodontic treatment for maxillofacial defects

The maxillofacial defects include the maxillofacial (mandibular) defects and the facial defects, which both conduce to the severe physiological and mental obstructions。The restoration of maxillofacial (mandibular) defects will resume the physiological functions, and that of facial defects pays more attention for the personal feature。The maxillofacial defects should be restored by a series therapy: firstly a manufactured resin palatal plate should be fabricated before the operation; then the plate would be fixed in the palatal defect area during the operation; 7-10 days later, the resin plate would be changed as a temporary obturator; two months later the permanent obturator should be ready for the patient, which could be satisfactorily restored by the magnet attachment, bar-clip attachment, telescope crown and other dental attachments。For the bilateral total maxillofacial defect, we initiated the "ring framework+implant" method, which is the best solution for this defect type and could reconstruct the mastication and deglutition functions。It is called "Chinese way" by foreign prosthodontists。For the mandibular defects, firstly many means, such as bone transplant, may be used to resume the low bone continuity。Then the "implant q-attachment" method could reconstruct the mandibular physiological functions。

Fracture strength of custom-fabricated Celay all-ceramic post and core restored endodontically treated teeth

Background The increased use of all-ceramic crown provides a rationale for tooth-colored core. Due to superior mechanical properties, Vita Celay infiltration ceramic developed for crown and bridge works presents the potential for fabricating all-ceramic posts and cores in one piece. This study was conducted to compare the fracture strength of endodontically treated teeth which were thereafter given different types of posts and cores and crowns restoration, respectively. The evaluated post-and-core systems are： custom-fabricated Celay all-ceramic post-core, custom cast metal post-core, and prefabricated stainless steel post （Parapost） with and without 2.0 mm dentine ferrule. Methods Sixty freshly extracted human maxillary central incisors were endodontically treated and randomly divided into five groups with 12 samples each. Group A： Celay ceramic post-cores restored teeth with 2.0 mm dentine ferrule. Group B： Celay ceramic post-cores restored teeth with no dentine ferrule. Group C： cast metal post-cores restored teeth with 2.0 mm dentine ferrule. Group D： cast metal post-cores restored teeth with no dentine ferrule. Group E： prefabricated post and composite cores restored teeth with 2.0 mm dentine ferrule. All teeth were restored with Celay ceramic crowns. Each specimen was subjected to a load at a 45-degree angle to the long axis on MTS 810 material testing machine until failure, at crosshead speed of 0.02 cm/minute. Analysis of variance followed by the Newman-Keuls pairwise multiple comparison tests were used to compare the results of the groups tested. Results There was a statistically significant difference among the five groups （P〈0.01）. Celay ceramic post-cores restored teeth with 2.0 mm dentine ferrule [（758.35± 119.26） N] and cast metal post-cores restored teeth with 2.0 mm dentine ferrule [（756.63± 166.22） N] had a significantly greater mean fracture strength than the other three groups in which no significant difference was observed. Clinical Study on the Relationship between Fixation of Prosthesis and Health of Surrounding Tissues

[Objectives] To explore the effect of mouth outside bonding screw fixation and inside bonding fixation in implanting the prosthesis( crown or bridge) on the health of surrounding tissues,to provide a certain reference for the selection of fixation methods for clinically implanting the prosthesis. [Methods]160 cases of patients needing implanting the prosthesis due to dental arch deficiency were selected. They had excellent alveolar bone. They did not smoke,had no diabetes,and their bone tissue healing was at the early stage. They were randomly divided into A and B groups,80 cases in each group. Group A: the mouth inside and abutment of prosthesis was bonded to a single abutment crown or a single abutment bridge; the bonder on the surface of prosthesis was completely removed and then moved into the implant,and fixed with central retaining screw,this group was called " mouth outside bonding group". Group B: the abutment was located in the implant and fixed using central retaining screw,then the prosthesis( crown or bridge) was bonded to the abutment,the bonder on the surface of prosthesis was removed,and this group was called mouth " mouth inside bonding group". The health condition was compared between two fixing methods for the surrounding tissues of the implant in 24-36 months after restoration. [Results] Group A: the surrounding tissues of the implants for 126 teeth of 80 patients were healthy without occurrence of peripheral inflammation; Group B: in the implants of 112 teeth of 80 patients,7 teeth suffered the peripheral inflammation. Dental implant examination found that there was residue of bonder in the bonding of prosthesis and abutment,3 teeth exfoliated,and 4 teeth were restored after treatment. Through the X2 test,there was significant difference between Group A and Group B( P < 0. 05). [Conclusions] Implant-supported denture often adopts the mouth inside bonding. Objective:To analyze the clinical efficacy of Mixing natural teeth and implant supported denture sleeve,and provide reference for clinical ***:46 patients with Molarless in hospital from December 2010-December 2014 were selected,patients were randomly divided into observer group and control group,two groups of patients were designed and planted natural teeth fixed bridge and natural teeth and implants mixed support sleeve denture,regular follow patients and the surrounding alveolar bone height changes,5-year cumulative retention rate,clinical efficiency and other ***:During clinical observation,patients with no obvious symptoms,the use of feel good,no loose superstructure *** groups of patients were 2 cases of patients with implant loosening occurs,the amount of bone resorption annual observation group(0.22±0.10)mm less than the control group(0.24±0.08)mm,but no significant difference(P>0.05),clinical observation group efficiency(100%)was significantly higher(72.3%)(P<0.05),the observation group study implants 5-year cumulative retention rate was 94.4%.Conclusion:Mixing natural teeth and implant-supported telescopic denture success rate of 100%was observed during the clinical results were satisfactory,with use value.
