Objective To determine the morphological features of 18 pathologically proven intrahepatic peripheral cholangiocarcinoma (IHPCC) cases in computerized tomography (CT) image. Methods All 18 patients had CT, using Picker I.Q.T/C and taking pre contrast continuous 10 mm sections throughout the liver and post contrast continuous 10 mm sections throughout the focuses. Results The disease was characterized in CT image by the following: all focuses were found in the periphery of the liver and shown as a lobulated or fused hypodense space occupying mass; there were one or more divergent or confluent, circular or irregular cystic areas with much lower density, in the majority. All focuses could be enhanced slightly and most revealed a dim edge. Dilated bile ducts around the focus were found frequently; the dilated bile ducts especially seemed to encircle the focus (33.3%, 6/18). This phenomenon were referred to as “encysted sign of dilated bile ducts”. Conclusions CT scanning should be one of the most important investigative methods for IHPCC due to the disease characteristics identified in CT image, especially “encysted sign of dilated bile ducts” which possesses specificity in diagnosing the disease.

Closed loop bowel obstruction is a specific type of mechanical obstruction with a high risk of strangulation and bowel infarction, especially in the small bowel. It is associated with a high mortality rate. Hence, it is important for emergency physicians to identify the presence of strangulation, while making the diagnosis of closed loop small bowel obstruction. We reported three patients with strangulated closed loop small bowel obstruction associated with severe abdominal pain, who had been treated at the emergency department. Urgent computerized tomography was performed in the patients. Two patients were discharged with stable conditions, and one patient died after hemodialysis. Urgent computerized tomography of the abdomen serves as an important diagnostic tool in view of its ability to detect the site, level and cause of obstruction along with the distinctive CT appearance of closed loop small bowel obstruction and signs of ischemia. Early definitive diagnosis will guide subsequent management and improve outcomes.

Primary pure squamous cell carcinomas(SCC) of the duodenum are very uncommon. To the best of our knowledge, only a few cases of SCC of the duodenum exist in the reported literature. Here, we report a case of SCC of the duodenum in northern China. A 45-year-old Chinese Han male patient presented with abdominal pain and weight loss. CT, endoscopic examinations, X-rays, and immunohistochemical markers were used to confirm this rare diagnosis of SCC. We performed a pancreaticoduodenectomy with a curative intention. However, histological examination revealed SCC of the duodenum. Postoperative chemotherapy was started after surgery. To the best of our knowledge, pancreaticoduodenectomy is the preferred form of treatment for carcinoma of the duodenum. This is supplemented with chemotherapy, which can further prolong survival.

Prostate cancer has the third highest cancer incidence in Palestinian men with incidence rate about 4.5/100,000, and an increase in new cases by 50% between 2000 and 2015. Prostate cancer death rates have been obviously decreased in many countries due to treatment, precise screening. The current study aims to review and evaluate the new diagnosed prostate cancer in the Gaza Strip, Palestine. As a cross-sectional, quantitative and analytical approach based on structured review, the current study was conducted at Al-Shifa and European hospitals (main oncology departments in governmental hospitals). Interviews with newly diagnosed cancer prostate patients were carried out during the period of March and June, 2014;a total of 41 newly diagnosed with primary cancer prostate. Furthermore, demographic, health status, medical investigations and tests are reported from oncologists and hospital archives over the last five years (2010-2014). The current study showed that there were 41 new prostate cancer cases in three months with incidence rate 5.1 per 100,000. The highest rate of incidence was among the patients above age of 70+, 70 - 74 (29.3%). The results showed that 58.1% of the patients were smokers, and 22.6% used to smoke, while 16.1% responded they don’t smoke at all. The most common type of treatment used by participants was chemotherapy therapy followed by hormonal therapy and prostatectomy. The least common methods used for treatment were radiation therapy and combination of hormonal, radiation, and chemotherapy as only 5 participants used each type of these treatment options. The prevalence of prostate cancer was higher among men who lived in Gaza Governorate compared to other governorates. Limited use of radiation therapy was due to the unavailability of this treatment in Gaza Strip.

Effectiveness and Therapeutic Impact of CT-Guided Percutaneous Drainage for Deep Neck Abscesses

Objective: The purpose of this study is to evaluate the effectiveness and safety of CT-guided percutaneous drainage (CPD) in the management of deep neck abscesses. Factors associated with successful treatment in patients with DNA will be identified. Methods: We retrospectively studied 29 patients who presented to the department of otolaryngology with deep neck abscesses between April 2011 and April 2015. These 29 patients were managed with CPD after antibiotic therapy or needle aspiration failed. Data on patient demographics, location of infection, existing comorbidity, duration of hospitalization, treatment received, and complications were reviewed. Results: The average age of 29 patients, including 18 men and 11 women, was 56 years old. Abscess was found in parapharyngeal space (n = 16), submandibular space (n = 7), retropharyngeal space (n = 5) and pretracheal space (n = 1). The maximum transverse diameter of abscess ranged from 4.8 cm to 8.0 cm (mean 6.03 cm). Positive cultures were found in 24 cases and the most common pathogen found was Streptococcus viridans. Average hospital stay was 6.7 days. Deep neck abscesses were completely removed without residual in all patients. No one had complications and no one died during and after CPD. Conclusion: CPD is a safe and highly effective procedure for treating patients with deep neck abscesses who do not respond to antibiotics therapy. This technique can also provide reliable evidence on pathogens responsible for deep neck abscesses and help otolaryngologists choose effective treatment to achieve better clinical success rate. We recommend that most deep neck abscesses should be managed initially by CPD before resorting to open surgery.
