Quantitative Analysis of Photodynamic Therapy Effects in Rat Mammary Tumor Vascular Density Using Image-Pro plus Software

Photodynamic therapy (PDT) is a treatment modality that has advanced rapidly in recent years. It causes tissue and vascular damage with the interaction of a photosensitizing agent (PS), light of a proper wavelength, and molecular oxygen. Evaluation of vessel damage usually relies on histopathology evaluation. Results are often qualitative or at best semi-quantitative based on a subjective system. The aim of this study was to evaluate, using CD31 immunohistochemistry and image analysis software, the vascular damage after PDT in a well-established rodent model of chemically induced mammary tumor. Fourteen Sprague-Dawley rats received a single dose of 7,12-dimethylbenz(a)anthraxcene (80 mg/kg by gavage), treatment efficacy was evaluated by comparing the vascular density of tumors after treatment with Photogem®?as a PS, intraperitoneally, followed by interstitial fiber optic lighting, from a diode laser, at 200 mW/cm and light dose of 100 J/cm directed against his tumor (7 animals), with a control group (6 animals, no PDT). The animals were euthanized 30 hours after the lighting and mammary tumors were removed and samples from each lesion were formalin-fixed. Immunostained blood vessels were quantified by Image Pro-Plus version 7.0. The control group had an average of 3368.6 ± 4027.1 pixels per picture and the treated group had an average of 779 ± 1242.6 pixels per area (P 0.01), indicating that PDT caused a significant decrease in vascular density of mammary tumors. The combined immunohistochemistry using CD31, with selection of representative areas by a trained pathology, followed by quantification of staining using Image Pro-Plus version 7.0 system was a practical and robust methodology for vessel damage evaluation, which probably could be used to assess other antiangiogenic treatments.

Background: Continuous spinal anesthesia (CSA) is an underused technique in modern anesthesia practice. Compared with other techniques of neuraxial anesthesia, CSA allows incremental dosing of an intrathecal local anesthetic for an indefinite duration, whereas traditional single-shot spinal anesthesia (SSA) usually involves larger doses, a finite, unpredictable duration, and greater potential for detrimental hemodynamic effects including hypotension, and epidural anesthesia via a catheter may produce lesser motor block and suboptimal anesthesia in sacral nerve root distributions. It is indicated in elderly patients undergoing lower limbs and sub umbilical surgery. Aim: This work aims to highlight the advantages of CSA on hemodynamic stability and as an alternative to heavy anesthetic procedures in already fragile patients. Case presentation: Our cases were two elderly patients, both of them with past stories of cardiac diseases. Both of them were undergoing amputation indicated for wet gangrene on lower limbs. They had unstable hemodynamics states due to inflammatory state. They were all rated ASA 3. CSA was performed with low doses of local anesthetics and maintenance by reinjections of mixture with the same doses. The interventions took place without major incidents and all patients survived. Conclusion: CSA is an underused technique in modern anesthesia. However, there is renewed interest due to the quality of the blocs and the hemodynamics stability. We report a case series of 2 elderly patients with past stories of cardiac diseases undergoing amputation for dry gangrene that had been operated under CSA.

Multiple Myeloma: Modification and Adaptation of Plasma Cell Morphology Algorithm for the Prognosis of Congolese Patient

Objectives: To determine the multiple myeloma (MM) prognostic formulas applicable in Congolese with many aberrant and/or immature plasma cells according to GOASGUEN’s plasma cell morphology algorithm (PMA). Methods: The prospective, observational and descriptive study was carried out at the Ngaliema Clinic in Kinshasa from July 2016 to December 2019. The variables studied were the plasma cell types and the prognosis groups according to GOASGUEN and ZANDECKI. The nucleolus, the chromatin and the N/C ratio of 2 for each plasma cell encountered in multiple myeloma are examined in order by asking three successive questions as follows: 1) Is the nucleolus present? If yes, we denote 1;if not, note 0;2) Is the chromatin thin? If yes, we score 1;if not we score 0;3) The N/C ratio is greater than 6, if so, 1 is noted;if not, we note 0. We obtain a number with 3 successive digits preceded by the letter P for each plasma cell. Results: The overall rate of immature plasma cells and aberrant plasma cells in MM among Congolese was high at 26.6%. Their integration in the prognostic formulas showed for group A" = 22 patients, group B" = 5 patients and group C" = 32 patients. Examination of plasma cell types in the deceased showed that 14/17 had a P001 plasma cell count ≥ 15%. Conclusion: The high rate of aberrant and/or immature plasma cells, the P001 plasma cell type at a rate ≥ 15%, found mostly in group C" with poor prognosis, are responsible for the aggressive nature of MM in Congolese Blacks.

Prevalence of Glucose-6-Phosphate Dehydrogenase Deficiency among Steady-State Sickle Cell Disease Children at University of Ilorin Teaching Hospital, Ilorin

Background: Both glucose-6-phosphate dehydrogenase (G6PD) deficiency and sickle cell disease (SCD) are prevalent in malaria-endemic regions. Controversy however persists as to whether G6PD deficiency is commoner in SCD subjects compared with the general population. Co-existence of the enzyme-deficient state with a chronic haemolytic disorder, like SCD, could potentially predispose to fatal haemolytic episodes. There is however a dearth of paediatric studies on this subject. Aims and Objectives: To determine the prevalence and demographic determinants of G6PD deficiency in children with SCD. Methods: Red cell G6PD activity was determined in 115 steady-state sickle cell disease children aged 0.5 - 17 years, in steady state and equal number of age & gender-matched non-SCD controls using the quantitative method. A measured G6PD activity of U/gHb defined G6PD deficiency while values ≥6.97 U/gHb were regarded as normal. Data were analysed using SPSS version 20. Statistical analyses done include chi-square, student t-test and ANOVA. For all statistical analyses, p values less than 0.05 were considered significant. Results: There were 64 (55.7%) males and 51 (44.3%) females in each of the arms of the study. The mean age of the study population was 8.4 ± 4.7 years. Seven of the subjects were G6PD-deficient giving a prevalence of 6.1% which was not significantly different from the 7.0% obtained in the controls (p = 0.789). G6PD deficiency was more frequently encountered in the older age groups although the difference was not statistically significant (p = 0.438). Similarly, mean G6PD activity was highest in the under-5 age-group compared to the older age-groups analysed (p = 0.573). The condition was also commoner in the males than females although the difference did not attain statistical significance (p = 0.897). Conclusions: The prevalence of G6PD deficiency among SCD children was 6.1%. The condition is marginally commoner in males and older children.

A Study of Quasi-Monochromatic X-Ray Sources for Breast Cancer Early Detection

Objectives: Early detection and treatment provide the opportunity to decrease the mortality rate from breast cancer. Quasi-monochromatic technique can enhance low contrast lesion detection by eliminating beam-hardening artifacts. We hypothesized that this technique would be feasible and can be used to assist in breast cancer early detection. Methods: The performance of stationary Digital Breast Tomosynthesis with quasi-monochromatic X-ray sources was evaluated using both simulated and physical phantoms. Normalized spectra in the fraction of total photons vs. photon energy were generated. Results: As expected, the peaked energies from sources, from smallest to largest, are Mo/Mo, W/Ag-1000, and W/Ce-10, W/Ce-100. For contrast vs. noise standard deviation on the simulated CIRS phantom, W/Ce-100 and W/Ce-10 have similar performance on both low and high contrast objects. For low contrast object, W/Ce-100 is barely noticeably better than W/Ce-10, and they are better than both W/Ag-1000 and Mo/Mo. For high contrast objects W/Ce-10 is slightly better than W/Ce-100. The spectra of the implemented W/Ce-10 X-ray source were measured, which matched the simulation well. The contrast noise ratios of reconstructed objects in American College of Radiology mammographic phantom with and without using W/Ce-10 combination are 7.1 and 5.4, respectively. Conclusions: The combination of stationary digital breast tomosynthesis and quasi-monochromatic technique can compensate the loss of X-ray flux due to heavy K-edge filtering. This technique can enable the stationary DBT scanners to operate at acceptable scanning times with better low contrast lesion detectability. Advances in Knowledge: The stationary digital breast tomosynthesis can provide high quality images within short scanning time by using X-ray source array, which makes quasi-monochromatic technique feasible.

Tracheal stenosis is a serious, life-threatening disease with increasing incidence. Among these stenoses, the number of complicated tracheal lesions, where a resection and anastomosis is not successful or not applicable, is also increasing significantly. These acute situations often need to be managed by a combined surgical technique. Although stimulus frequency otoacoustic emissions (SFOAEs) have been used as a non-invasive measure of cochlear mechanics, clinical and experimental application of SFOAEs has been limited by difficulties in accurately deriving quantitative information from sound pressure measured in the ear canal. In this study, a novel signal processing method for multicomponent analysis (MCA) was used to measure the amplitude and delay of the SFOAE. This report shows the delay-frequency distribution of the SFOAE measured from the human ear. A low level acoustical suppressor near the probe tone significantly suppressed the SFOAE, strongly indicating that the SFOAE was generated at characteristic frequency locations. Information derived from this method may reveal more details of cochlear mechanics in the human ear.

Comparison of gliclazide vs linagliptin on hypoglycemia and cardiovascular events in type 2 diabetes mellitus: A systematic review

BACKGROUND Cardiovascular outcome trials have demonstrated cardiovascular safety of glimepiride(a sulfonylureas) against dipeptidyl peptidase-4 inhibitor ***(another newer sulfonylureas) has shown similar glycemic efficacy and 50% decreased risk of hypoglycemia compared to *** Considering the absence of cardiovascular outcome trials for gliclazide, we decided to conduct a systematic review of the literature to assess the cardiovascular(CV) safety by assessing the risk for major adverse CV events and hypoglycemia risk of gliclazide vs linagliptin in patients with type 2 diabetes(T2D).METHODS This systematic review followed the current Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines to analyze all the clinical studies published from 2008 that compared the two drugs in patients with T2D with no risk of CV disease(CVD). We included only evidence designated high quality by the Oxford Center for Evidence-based Medicine-Levels of *** Eight clinical studies were included in the narrative descriptive analysis(gliclazide: 5 and linagliptin: 3). The CV safety of gliclazide in the Action in Diabetes and Vascular Disease: Preterax and Diamicron Modified Release Controlled Evaluation trial and of linagliptin in the Cardiovascular and Renal Microvascular Outcome Study With Linagliptin(CARMELINA) and CARdiovascular Outcome study of LINAgliptin vs glimepiride in patients with T2D(CAROLINA)trials were excluded from the comparative analysis as these trials demonstrated CV and hypoglycemia benefits in patients at high risk of CVD. However, since these are landmark trials,they were discussed in brief to show the CV benefits and low hypoglycemia risk of gliclazide and linagliptin. We did not find any study comparing gliclazide with linagliptin.
