Effects of internal iliac artery embolization on systemic inflammatory response syndrome in dogs with simulatedpelvic-fracture combined with massive bleeding

Background: Pelvic fracture combined with massive bleeding(PFCMB) is a complex issue in clinical practice. Currently, the use of angiography and embolization for the treatment of PFCMB obtains good results. The aim of this study is to observe the effects of early internal iliac artery embolization on the systemic inflammatory response syndrome(SIRS) in dogs with simulated-pelvic-fracture combined with massive ***: Twenty adult dogs were randomly divided into an embolization group(EG) and a control group(CG). For the two groups, heart rate, respiratory rate and body temperature and other physiological variables were measured, and IL-6, TNF-α and arterial blood gas levels were monitored. These variables were assayed every 30 min until death in the CG, while dogs in the EG underwent arterial angiography after 60 min of modeling. The internal iliac artery was embolized on the injured ***: The average time to SIRS in the CG was 3.56 h, occurring at a rate of 90%(9/10) within 24 h, with a mortality rate of 50%(5/10); the average time to SIRS for the EG was 5.33 h, occurring at a rate of 30%(3/10) within 24 h, with a mortality rate of 10%(1/10). When SIRS occurred in the EG, the mean plasma IL-6 level was 52.66±7.38pg/ml and the TNF-ps, tα level was 11.45±2.72ng/ml, showing a significant difference with those of the CG(P<0.05). In the two grouhe respiratory rate and leukocyte levels were higher at each monitored time after modeling than those before modeling; the mean arterial pressure, levels of hemoglobin and oxygen partial pressure were significantly lower at each time point after modeling than those before modeling except for the mean arterial pressure at 0h in EG; the platelet levels at 4 and 8h were higher than those before modeling; and the differences were statistically significant(P<0.05). Factors Associated with Coronary Artery Disease in Young Population(Age≤40):Analysis with 217 Cases

Objective To investigate the relevant factors of coronary artery disease （CAD） in young people under 40 years of age. Methods The study population was 292 young patients accepting coronary angiography in Fuwai hospital from July to December 2006, including 272 men and 20 women, with the mean age being 36.7±3.7 years. The diagnosis of CAD was made in the cases presenting ~ 50% stenosis in coronary lumen in coronary angiography. Based on the diagnosis, 217 patients （204 men, 13 women） were assigned to CAD group, and 75 （68 men, 7 women） to non-CAD group. Clinical data and metabolic characteristics of the patients were collected and analyzed using t-test, Z2 test, and multinomial logistic regression with SPSS 8.0 software. Results Most study subjects were current smokers （209/292, 71.6%）, and more than half had body mass index （BMI）〉24 kg/m2 （230/292, 78.8%） and usually took high-fat diet （162/292, 55.5%）. The proportion of heavy smokers （smoking history ≥10 years and t〉20 cigarettes per clay） were significantly higher in the CAD group than in the non-CAD group [20.7% （45/217） vs. 9.3% （7/75）, P=0.015）]. Heavy smoking [odds ratio （OR）, 1.89; 95% confidence interval （CI）, 1.74-2.05], hypertension （OR, 1.56; 95% CI, 1.48-1.65）, alcohol （OR, 1.37; 95% CI, 1.30-1.46）, type 2 diabetes mellitus （OR, 1.37; 95% CI, 1.25-1.50）, high-fat diet （OR, 1.35; 95% CI, 1.28-1.43）, and BMI〉24 kg/m2 （OR, 1.09; 95% CI, 1.03-1.17） were factors related to CAD in the young patients （all P〈0.05）. Assessing inflammation in Chinese subjects with subtypes of heart failure: an observational study of the Chinese PLA hospital Heart Failure Registry

Background Inflammation is an important element of the pathophysiological process of heart failure (HF) and is correlated with subtypes of HF. The association between multiple biomarkers of inflammation and HF subtypes in Chinese subjects remains unclear. This study aimed to compare the differences in inflammation biomarkers among Chinese patients with different subtypes of HF who have been identified to date. Methods We included 413 consecutive patients with HF,including 262 with preserved ejection fraction (HFpEF),55 with middle-ranged ejection fraction (HFmrEF) and 96 with reduced ejection fraction (HFrEF). Ten inflammation biomarkers were analyzed and compared according to the HF subtypes. One hundred contemporary non-HF subjects were also recruited as the control group. Moreover,the correlations between the inflammatory biomarkers and left ventricular ejection fraction of the HF subtypes were assessed. Results The mean age of the HF patients was 65.0 ± 12.0 years,65.8% were male. Distinct subtypes of HF demonstrated different inflammation biomarker panels. IL-6,PTX-3,ANGPTL-4 and TNF-α were correlated with HFrEF;IL-1β and PTX-3 were correlated with HFmrEF;and IL-1β and IL-6 were correlated with HFpEF. The multivariable logistic regression showed that IL-1β[relative ratio (RR)= 1.08,95% CI: 1.02–1.15,P = 0.010],IL-6 (RR = 1.03,95% CI: 1.01–1.06,P = 0.016),PTX-3 (RR = 1.31,95% CI: 1.11–1.55,P = 0.001),and ANGPTL-4 (RR = 1.05,95% CI: 1.02–1.07,P < 0.001) were independently associated with HF,while IL-6 (RR = 1.03,95% CI: 1.01–1.04,P = 0.019),PTX-3 (RR = 1.23,95% CI: 1.06–1.43,P = 0.007),and ANGPTL-4 (RR = 1.03,95% CI: 1.01–1.06,P = 0.005) were independently associated with the HF subtype. Conclusions Diverse inflammation biomarkers have multifaceted presentations according to the subtype of HF,which may illustrate the diverse mechanisms of inflammation in Chinese HF patients. Inflammation-based different association between anatomical severity of coronary artery disease and lung cancer

BACKGROUND Coronary artery disease(CAD)is associated with *** role of inflammation in the association of CAD with cancer remains *** study investigated whether inflammation could impact the relationship between CAD and lung *** The study involved 96 newly diagnosed lung cancer patients without receiving anti-cancer therapy and 288 matched non-cancer *** the patients underwent coronary angiography and were free from previous percutaneous coronary inter-vention or coronary artery bypass *** score(SXscore)were used to assess severity of *** SXscore(SXhigh)grade was defined as SXscore>16(highest quartile).Neutrophil-to-lymphocyte ratio(NLR)served as an inflammatory ***-high grade referred to NLR>2.221(median).RESULTS Among 384 study patients,380 patients(98.96%)had NLR value(median:2.221,interquartile range:1.637-3.040).Com-pared to non-cancer patients,lung cancer patients had higher rate of SXhigh among total study patients(P=0.014)and among pat-ients with NLR-high(P=0.006),but had not significantly higher rate of SXhigh among patients with NLR-low(P=0.839).Multiv-ariate logistic regression analysis showed that SXhigh was associated with lung cancer[odds ratio(OR)=1.834,95%CI:1.063-3.162,P=0.029].Subgroup analysis showed that SXhigh was associated with lung cancer among patients with NLR-high(OR=2.801,95%CI:1.355-5.794,P=0.005),however,the association between SXhigh and lung cancer was not significant among pati-ents with NLR-low(OR=0.897,95%CI:0.346-2.232,P=0.823).CONCLUSIONS Inflammation could lead different association between anatomical severity of CAD and lung *** of CAD was significantly associated with increased risk of lung cancer among patients with high inflammation rather than am-ong patients with low inflammation.

BACKGROUND Gastrointestinal stromal tumors(GISTs) are the most common mesenchymal tumor type in the gastrointestinal system. Presently, various classification systems to prognosticate GISTs have been *** To evaluate the application value of four different risk stratification systems for *** Patients who were diagnosed with GISTs and underwent surgical resection at four hospitals from 1998 to 2015 were identified from a database. Risk of recurrence was stratified by the modified National Institute of Health(NIH)criteria, the Armed Forces Institute of Pathology(AFIP) criteria, the Memorial Sloan Kettering Cancer Center(MSKCC) prognostic nomogram, and the contour maps. Receiver operating characteristic(ROC) curves were established to compare the four abovementioned risk stratification systems based on the area under the curve(AUC).RESULTS A total of 1303 patients were included in the study. The mean age of the patients was 55.77 ± 13.70 yr; 52.3% of the patients were male. The mean follow-up period was 64.91 ± 35.79 mo. Approximately 67.0% the tumors were located in the stomach, and 59.5% were smaller than 5 cm; 67.3% of the patients had a mitotic count ≤ 5/50 high-power fields(HPFs). Thirty-four tumors ruptured before and during surgery. Univariate analysis demonstrated that tumor size > 5 cm(P 5/50 HPFs(P < 0.05), non-gastric location(P < 0.05), and tumor rupture(P < 0.05) were significantly associated with increased recurrence rates. According to the ROC curve, the AFIP criteria showed the largest AUC(0.754).CONCLUSION According to our data, the AFIP criteria were associated with a larger AUC than the NIH modified criteria, the MSKCC nomogram, and the contour maps, which might indicate that the AFIP criteria have better accuracy to support therapeutic decision-making for patients with GISTs.

Usefulness of coronary epicardial collaterals for the retrograde approach treatment of coronary chronic total occlusions

Background The retrograde approach allows to significantly increase the success rate of coronary chronic total occlusion（CTO） percutaneous intervention（PCI）. Epicardial collateral channels（CCs） remain an essential channel for retro-recanalization. But paucity of data in this area limits the use of epicardial CCs for retrograde canalization. The present study was to explore the feasibility and significance of coronary epicardial collaterals for the retrograde approach treatment of coronary CTO. Methods We retrospectively analyzed 347 CTO cases which the retrograde approach was attempted after reviewing the coronary angiograms of 1290 patients. And there were 89 retrograde approach PCI cases applying epicardial CCs. We collected the clinical and angiography data of those patents for analyzing the safety and usefulness of this technology. Results The study consisted of347 CTO PCI. Mean age was 60.8±10.4 and 60.3±10.2 years in epicardial CCs and septal CCs groups respectively. The total retrograde success rate was 79.8%. The success rate of retrograde approach applying epicardial CCs was not lower（82.0% vs. 79.1%, P=0.646） than that in cases applying septal CCs. There were 8 epicardial CCs injury cases（9.0%） and 3 cases resulted in cardiac tamponade and required emergency pericardiocentesis. Coils were used for all of them with success. No patients were dead in retrograde approach applying epicardial *** Coronary epicardial collaterals can be used as an access for the retrograde approach in the percutaneous treatment of CTO, which is relative feasible, and has a high success rate. Epicardial CCs injury can be safety as long as it is treated timely.

The effect and safety of chronic total occlusion etrograded recanalization by epicardial collateral channel

Background Data on retrograde recanalization of epicardial collateral channels in patients with chronic total occlusion(CTO) undergoing percutaneous coronary intervention(PCI) are limited. Fearing about the complications of perforation, few CTO patients have chosen retrograde recanalization of epicardial collateral channels. Methods A retrospective analysis was conducted in this study. We followed up eligible patients who underwent continuous CTO via epicardial CCS from August 2011 to October 2018 in our center, and analyzed the detailed clinical baseline and angiographic data, surgical success rate and hospital complications to determine the efficacy and safety. The Cox model was used to compare major adverse cardiac events(MACE) and related co-variables between CTO patients with epicardial CCS perforation and non-perforated PCI patients. Results The study enrolled 191 patients with epicardial CCs PCI of CTO and the long term follow-up time was 24.03±19.18 *** CCs PCI of CTO was achieved in 161(84.8 %) patients. Procedural collateral perforation occurred in23(12.0%) patients. MACEs occurred in 18 patients(9.4%) at 1-year follow-up. Kaplan-Meier curves showed no significant difference in MACEs between perforating recanalization of CCs PCI of CTO and non-perforating PCI of CTO.( adjusted hazard ratios(HR): 0.467; 95% confidence intervals(CI): 0.13 to 1.69, P=0.245). The multivariate Cox regression analysis identified the Werner score=0/1(HR: 5.80; 95% CI: 1.89 to 17.8; P=0.002) as significant independent predictors of perforation of CCs PCI of CTO. Conclusions The major complication of the procedure is collateral perforation. Recanalization of CTOs through epicardial collateral channel is safe and effective. It may be a available choice for recanalization of complex CTO.[S Chin J Cardiol 2019;20(1):37-43]

Remote organ injury,which is a common secondary complication of sterile tissue damage,is a major cause of poor prognosis and is difficult to ***,we report the critical role of tissue-resident macrophages in lung injury after trauma or stroke through the inflammatory *** found that depleting tissue-resident macrophages rather than disrupting the recruitment of monocyte-derived macrophages attenuated lung injury after trauma or *** findings revealed that the release of circulating alarmins from sites of distant sterile tissue damage triggered an inflammatory response in lung-resident macrophages by binding to receptor for advanced glycation end products(RAGE)on the membrane,which activated epidermal growth factor receptor(EGFR).Mechanistically,ligand-activated RAGE triggered EGFR activation through an interaction,leading to Rab5-mediated RAGE internalization and EGFR phosphorylation,which subsequently recruited and activated P38;this,in turn,promoted RAGE translation and trafficking to the plasma membrane to increase the cellular response to RAGE ligands,consequently exacerbating *** study also showed that the loss of RAGE or EGFR expression by adoptive transfer of macrophages,blocking the function of RAGE with a neutralizing antibody,or pharmacological inhibition of EGFR activation in macrophages could protect against trauma-or stroke-induced remote lung ***,our study revealed that targeting the RAGE-EGFR signaling pathway in tissue-resident macrophages is a potential therapeutic approach for treating secondary complications of sterile damage.
