The authors report a case of bilateral pulmonary embolism (PE) with intermediate risk at the University Hospital center of Treichville (CHUT). This is a postpartum PE in a 37-year-old obese, multiparous woman with postpartum hemorrhage from uterine rupture after vaginal delivery initiated by injectable oxytocin. This postpartum haemorrhage was managed by massive transfusion and hysterectomy. The initiation of thromboprophylaxis was delayed in view of its coagulation record, the first 3 days. Later, the patient presented respiratory distress for which the completion of a pulmonary angioscanner made it possible to make the diagnosis of PE whose clinical evolution under heparinotherapy was favorable.

Background Calcitonin gene-related peptide （CGRP） is the predominant neurotransmitter in capsaicin-sensitive sensory nerves. Participation of CGRP in hypertension is one of the most extensively studied topics in the field. There is growing evidence to the effect that CGRP is associated with essential hypertension （EH）. The aims of this study were to pinpoint whether single nucleotide polymorphisms （SNPs） in the genes coding for CALCA were associated with EH susceptibility in a Hunan Han population. Methods A total of 293 subjects with EH and 208 controls were enrolled in the study. Genomic DNA was extracted from peripheral blood leucocytes by a phenol-chloroform method. The CALCA T-692C was genotyped using a restriction fragment length polymorphism method. Results A statistically significant difference in CALCA T-692C genotypic distribution was observed between cases and controls （P=0.001）. Moreover, the frequencies of the C allele were 14.85% in the EH group and 7.45% in the control group, prevalence of C alleles in EH subjects and controls was significantly incomparable （P 〈0.001）. Furthermore, the results of Logistic regression analysis showed that the carriers of C allele （TC＋CC genotypes） were associated with increased EH risk （OR=2.093, 95% CI： 1.317-3.326, P 〈0.01）. Conclusions CALCA genetic polymorphism is associated with EH susceptibility. Carriers of at least one C allele at the polymorphic site CALCA T-692C showed increased risk for EH.

Background It is unclear whether a history of paroxysmal atrial fibrillation （PAF） would impact the effect of catheter ablation on persistent atrial fibrillation （AF）. This study aimed to compare the effect of catheter ablation on persistent AF with and without a history of PAF. Methods One hundred and eighty-three patients underwent catheter ablation of persistent AF lasting for 〉1 month and were reviewed. Patients were divided into two groups according to whether they had a history of PAF or not. Group I consisted of persistent AF patients with a history of PAF, and group II consisted of persistent AF patients without such a history. All patients received catheter ablation focused on pulmonary vein isolation and were observed for arrhythmia recurrences, which were defined as documented episodes of AF or atrial tachycardia after a blanking period of 3 months. Results One hundred and three patients （60.9%） in group I and sixty-six patients （39.1%） in group II were successfully followed and included in analysis. There were no significant differences in clinical and echocardiographic characteristics between both groups except for a younger age and more male patients in group II. After （15.5±10.7） months of follow-up, 59 （57.3%） patients in group I and 49 （74.2%） patients in group II maintained sinus rhythm free of anti-arrhythmia drugs （P=0.025）. Multivariate analyses found left atrial anteroposterior diameter （P=0.006） and persistent AF with a history of PAF （OR 1.792, 95% CI 1.019-3.152; P=0.043） as the only independent statistical predictors of arrhythmia recurrences. Conclusion The arrhythmia recurrence rate of catheter ablation based on pulmonary vein isolation in persistent AF with a history of PAF was hiQher than those without a history of PAF

Background Myocardial ischemia-reperfusion injury is characterized by the inability of tissue and organ function to recover after the perfusion blood flow is restored followed by myocardial ischemia. Previous studies have shown that many Chinese herbal compounds can reduce myocardial ischemia-reperfusion injury,but the efficacy of Wuling Powder in treating myocardial ischemia-reperfusion injury has not been reported. The present study aimed to investigate the mechanism of Wuling Powder in treating myocardial ischemia-reperfusion injury by network pharmacology and molecular docking. Methods The active constituents of Wuling Powder were obtained by the Traditional Chinese Medicine Systems Pharmacology Database and Analysis Platform(TCMSP), and the related targets of active constituents of Wuling Powder were screened by the Swiss Target Prediction database. Disease targets of myocardial ischemia-reperfusion injury were obtained by Gene Cards, Dis Ge NET and OMIM gene databases. The active compound-target network of Wuling Powder was constructed using Cytoscape software. The drug prediction targets were mapped to the disease target set, and the intersection targets were obtained. The Gene Ontology(GO) function and Kyoto Encyclopedia of Genes anf Genomes(KEGG) pathway enrichment analysis were performed on the intersection targets obtained by using String database. Auto Dock Vina was used for molecular docking of core targets and the top 3 key active ingredients with degree values. Results Through screening, 56 active ingredients and 116 targets of Wuling Powder were obtained. There were 27 common targets of Wuling Powder and myocardial ischemia-reperfusion injury in Myocardial ischemia-reperfusion injury(MIRI). Protein-Protein Interaction(PPI) network analysis in this study showed that CASP3, PTGS2, CAT, JUN, ESR1, CASP8, CASP9 and RELA may be the key targets of Wuling Powder to exert the protective effect of MIRI. Variation and significance of microtubules in rat volume overload cardiac hypertrophy

Objective To investigate the function of microtubules in volume overload cardiac hypertrophy of rat. Methods The structure of microtubules was observed using an immunofluorescent microscope, while the pixel intensity and distribution of microtubule imaging was estimated from laser scanning confocal images of left ventricular cardiocytes immuno-labeled with an antibody to β-tubulin. Results The pixels of the microtubule image taken just after volume overload were not evenly distributed. At 6 hours after overload, the pixel intensity of the microtubule image was decreased to less than 150 (arbitrary units), which was the same as the pixel intensity and distribution of the colchicine depolymerized microtubule image. The changes were partially recovered to 200 (arbitrary units) after 4 more days. The pixel intensity of the control microtubule image was 250 (arbitrary units) and had an even distribution. The structuring of the microtubules was more disordered as volume overload hypertrophy developed. Conclusions There are disorders in the signal transduction pathways governing the hypertrophic response of cardiomyocytes in the hypertrophic myocardium and microtubule is one of the members of the signal transduction pathways governing the hypertrophic response of cardiomyocytes in the hypertrophic myocardium. The disordered microtubule array may be targeted during heart failure treatment.

Cardiac rehabilitation helps improve the prognosis and quality of life for patients with heart disease. To show its interest in the African context, in the management of heart disease, a prospective study was carried out. Its objective was to evaluate the effects of physical rehabilitation on cardiac function. Methods: Patients were evaluated at the beginning and at the end of cardiovascular rehabilitation by cardiovascular, biological, and echocardiographic parameters including the size of the heart chambers, the kinetics of the walls, and the systolic function of the ventricles. Results: The study involved 12 patients, 67% of whom were men. After cardiac rehabilitation, the mean hemodynamic constants had not been significantly modified. However, a significant decrease in Total-Cholesterol, LDL-Cholesterol and triglyceride levels was noted. Mean fasting blood sugar decreased from 1.25 ± 0.48 g/L to 0.92 ± 0.18 g/L and glycated hemoglobin from 7.72% ± 0.01% to 6.45% ± 0.008%. The echocardiographic parameters studied showed an improvement in the dilation of the heart chambers in 8.33% of the patients, the normalization of the ejection fraction of the left ventricle in 16% patients, the improvement of the kinetic disorders in 16% of patients and recovery of right ventricular systolic function in all patients. Conclusion: Cardiac rehabilitation as a secondary preventive measure for cardiovascular disease has contributed significantly to improving the clinico-biological parameters of the disease.

Background and Aim: Cardiovascular risk (CVR) factors, namely hypertension (HT), diabetes mellitus and obesity, are a public health problem in sub-Saharan Africa because of health transition. The additional effect of the social gradient within the railway companies in Congo-Brazzaville on high CVR is not yet established. The aim of this study was to determine the extent of hypertension, diabetes mellitus, overweight, and obesity and to identify the contributing factors of fatness and hypertension. Methods: This was a cross-sectional study conducted in April 2013. A simple random sample of 255 out of all 2550 workers from the railway companies of Congo was examined for epidemiological, clinical and biological variables. Results: Out of the study sample, 231 (90.6%), 79 (31%), and 52 (20.4%) were men, rural dwellers, and senior executives, respectively. The mean age was 45 ± 13 years (range 19 to 63 years). The rates of overweight, hypertension, obesity, and diabetes mellitus were 40.3% (n = 103), 29.4% (n = 75), 7.5% (n = 19), and 3.5% (n = 9), respectively. In univariate analysis, female sex (OR 2.7, 95% CI 1.13 - 6.45, p = 0.01), senior executive (OR 2.4;95% CI: 1.3 - 4.5;p = 0.003) and physical inactivity (OR 2.5;95% CI: 1.5 - 4.2;p clusions: Fatness is emerging and it is the most contributing factor of hypertension among workers at the Congolese railway companies.
