Background: Obesity is a fundamental factor in metabolic disorders such as hyperlipidemia, insulin resistance, fatty liver, and atherosclerosis. However, effective preventive measures are still lacking. This study aimed to investigate different surgical protocols for removing partial adipose tissue before the onset of obesity and determine whether, and by which protocol, preliminary adipose removal could exert potent preventive effects against diet-induced metabolic ***: Male low-density lipoprotein receptor (LDL-R) knockout (KO) mice were randomly divided into four groups and subjected to epididymal fat removal (Epi-FR) surgery, subcutaneous fat removal (suQ-FR) surgery, both subcutaneous and epididymal fat removal (Epi + suQ-FR) surgery, or sham-operation. After 1 week of recovery, all mice were given a high-fat diet (HFD) for 10 weeks to induce metabolic ***: In the Epi-FR group and the sham-operated group, the mean numbers of the residual subcutaneous fat were 28.59 mg/g and 18.56 mg/g, respectively. The expression of relative genes such asPparg, Cebpa, Dgat2, Fabp4 and Cd36 in the residual subcutaneous fat increased 2.62, 3.90, 3.11, 2.06, 1.78 times in the Epi-FR group compared with that in the sham-operated group. Whereas in the other fat-removal groups, the residual fat depots had no significant change in either size or gene expression, as compared with those of the sham-operated group. Plasma lipid and glucose levels and insulin sensitivity, as detected by the glucose tolerance test, were not significantly alleviated in the three fat removal groups. Liver mass or lipid content was not attenuated in any of the three fat removal groups. Background: The aim of this study is to evaluate the short-term results of surgical correction of Tetralogy of Fallot in adult patients. Methods: Between 1999 and 2007, surgical correction was performed in 16 adult patients (mean age: 34.6 years;range: 18 to 62). Five previous procedures had been performed in 4 patients. Thirteen patients were in NYHA functional classification III or IV (81%), and 12 were cyanotic. Echocardiography was performed in all patients and 14 of them were catheterized. The mean gradient in the right ventricular outflow tract was 82 mmHg. Four patients had bad anatomy, consisting in coronary fistulas (2) and small pulmonary artery branches (2). Results: Infundibular resection was performed in all patients, ventricular septal defect closure in 15, pulmonary valve commissurotomy in 8, right ventricular outflow tract patch enlargement in 9 and transannular patch placement in 4. Three patients died during the postoperative hospital stay, 2 of them with sepsis. All of them had bad anatomy and severe preoperative condition. No residual defects were found. The other 13 patients had a good recovery. Eleven had less than 24 hours of mechanical ventilation. Early hospital discharge was possible in all survivors. Conclusion: Surgical correction of Tetralogy of Fallot in adulthood is an infrequent procedure. The short-term results depend mostly on the anatomic substrate and the preoperative clinical condition of the patient.

Background and objective： Rivaroxaban is a new oral anticoagulant for stroke prevention in patients with non-valvular atrial fibrillation （NVAF）, which has less drug-food interaction than warfarin. We conducted this pro- spective randomized study to evaluate the metabolic benefits as well as the safety and efficacy with rivaroxaban versus warfarin in patients with NVAF following radiofrequency catheter ablation （RFCA）. Methods： From April to July 2014, 60 patients with NVAF undergoing RFCA were prospectively enrolled in our study. Following RFCA, all patients were randomly assigned to receive rivaroxaban （Group R, n=30） or warfarin （Group W, n=30）. Metabolic indices including serum total protein, albumin, globulin, and high-density lipoprotein （HDL） as well as bleeding, stroke, and systemic thromboembolism events were evaluated and compared during follow-up after 15, 30, 60, and 90 d of RFCA procedure. Results： Serum total protein, albumin, globulin, and HDL levels were all significantly elevated at each follow-up stage in Group R when compared to the baseline （P〈0.05 respectively）. In Group W, the metabolic indices decreased at first and then had an increasing trend. There were no deaths or thromboembolic complications in each group. The prevalence of total bleeding complications was similar between Group R and Group W （11/30, 36.7% vs. 10/30, 33.3%, P=0.70）. Conclusions： Patients with NVAF receiving rivaroxaban after RFCA procedures appear to benefit from a metabolic perspective compared with warfarin, providing practical clinical reference for the choice of the anticoagulant. Rivaroxaban seems to be as safe and effective in preventing thromboembolic events as warfarin for these patients.

Given the unsatisfactory hypertension control rates and high rates of non-adherence to antihypertensive medications worldwide,device therapy which can safely provide durable blood pressure-lowering effects can fulfill the unmet need.A series of second-generation randomized sham-controlled renal denervation(RDN)trials have demonstrated the efficacy and safety of RDN in a wide range of hypertensive *** four representative consensus documents on RDN(from the Chinese Taiwan Hypertension Society and Taiwan Society of cardiology[THS/TSOC 2019],Asia Renal Denervation Consortium 2019,European Society of Hypertension[ESH 2021],and Society for Cardiovascular Angiography&Intervention and National Kidney Foundation[SCAI/NKF 2021])consistently recommend RDN as an alternative or complementary treatment strategy for patients with uncontrolled *** addition,both documents from Asia further recommend that RDN can be considered as an initial treatment strategy for drug-naïve hypertensive *** is still inconsistency regarding whether ambulatory blood pressure monitoring should be used routinely both before and after RDN,and whether patients with a secondary cause of hypertension could be treated with RDN if their blood pressure remains uncontrolled after definitive treatment(treatment-resistant secondary hypertension).The THS/TSOC consensus provides acronyms to summarize key aspects of patient selection(RDNi2)and pre-RDN assessments(RAS).The ESH and SCAI/NKF documents recommend establishing structured pathways for clinical practice and issues regarding *** documents identify knowledge gaps in RDN,from identifying predictors of super-responders to demonstrating effects on cardiovascular *** gaps should be urgently filled to facilitate the wider application of this device therapy for patients with hypertension.

Cardiac involvement of sarcoid lesions is diagnosed by myocardial biopsy which is frequently false-negative,and patients with cardiac sarcoidosis(CS) who have impaired left ventricular(LV) systolic function are sometimes diagnosed with dilated cardiomyopathy(DCM).Late gadolinium enhancement(LE) in magnetic resonance imaging is now a critical finding in diagnosing CS,and the novel Japanese guideline considers myocardial LE to be a major criterion of *** article describes the value of LE in patients with CS who have impaired LV systolic function,particularly the diagnostic and clinical significance of LE distribution in comparison with *** existed at all LV segments and myocardial layers in patients with CS,whereas it was localized predominantly in the midwall of basal to mid septum in those with ***(nodular),circumferential,and subepicardial and subendocardial LE distribution were highly specific in patients with CS,whereas the prevalence of striated midwall LE were high both in patients with CS and with *** sarcoidosis patients with LE have higher incidences of heart failure symptoms,ventricular tachyarrhythmia and sudden cardiac death,the analyses of extent and distribution of LE are crucial in early diagnosis and therapeutic approach for patients with CS.
