3D Non-Fluoroscopic Cryoablation of Right-Sided Accessory Pathways in Children:Monocentric Study and Literature Review

Background:Cryoablation of accessory pathways(APs)is effective and very safe in children,as previously reported by our *** aim of this retrospective study was to evaluate the current efficacy of 3D non-fluoroscopic cryoablation of right sided APs in children,comparing results obtained with the Ensite VelocityTM and the more recent Ensite PrecisionTM 3D mapping *** and Results:From January 2016 to December 2019,102 pediatric patients[mean age 12.5±2.8,62 males(61%of total cohort)]with right APs underwent 3D non-fluoroscopic transcatheter cryoablation at our ***(14.7%)patients had previously undergone catheter *** procedural success rate was 95.1%(n=97).No significant differences were detected in acute success rates achieved with Ensite Velocity^(TM)or Ensite PrecisionTM systems nor between manifest(94%)and concealed APs(100%).No permanent complications *** follow-up(428±286 days,median 396 days[interquartile range 179-713]),19 patients(19.6%)had *** were more frequent for parahissian/anterior APs compared to midseptal/posterior and lateral APs(p=0.043).Recurrences were not related to the Ensite system used.A redo ablation procedure was attempted in 13 cases,11 cryoablation and 2 radiofrequency ablations:the former was successful in 10 cases out of 11(90.9%).Conclusion:3D cryoablation of right-sided APs is associated with a very high acute success rate with limited use of fluoroscopy,resulting in great benefit to the *** rates are not high and patients can be retreated with cryo-energy with higher success rates.

Factors associated with stroke direct cost in francophone West Africa, Benin example

Background: In sub-Saharan Africa, stroke constitutes a public health issue. Few studies were conducted to assess the cost involved in its treatment. Objective: To determine the factors involved in direct cost of stroke in Cotonou-Benin. Method: It consists in a transversal and prospective research of economic type with analytical and descriptive aim. It was conducted from 20thFebruary 2011 to 30thSeptember 2011. The research dealt with 122 stroke patients. With regard to the economic approach, bottom-up was the data collection technique which was adopted. Cost was estimated not only based on the patient himself/herself but considering societal aspect. Cost estimation period was hospitalization period. Data analysis was conducted via software such as Epi info and SPSS. Results: Overall expenses in terms of direct cost varied from $144.9 to $9393.9;average expenses were $1030.1 ± $101.7. Patients aged 50 and above had higher stroke hospitalization cost ($1277.4) than those aged below 50 ($857.4) p = 0.001;male patients made more expenses than females (FCFA 1157.5 against $831.8) p = 0.01;direct cost of stroke was increased in proportion to neurological deficit (score NIHSS) p = 0.043. This cost was higher in cases of hemorrhagic stroke than ischemic stroke (FCFA $1375 against $1098) p = 0.002. Stroke direct cost was also increased in proportion to severance of disability level of patients. Stroke type (hemorrhagic) and RANKIN score were firmly correlated to stroke hospitalization cost. Conclusion: Stroke is very expensive for patientsin Benin and they constitute a burden for both patients and their family. There is a great need to increase awareness regarding risk factor control in order to reduce the cost involved in treating this malady.

An investigation on the wind profiles and gravity wave dynamics in MLT region based on the meteor radars from the Meridian Project

The meteor radar can detect the zenith angle,azimuth,radial velocity,and altitude of meteor trails so that one can invert the wind profiles in the mesosphere and low thermosphere(MLT)region,based on the Interferometric and Doppler *** this paper,the horizontal wind field,gravity wave(GW)disturbance variance,and GW fluxes are analyzed through the meteor radar observation from 2012−2022,at Mohe(53.5°N,122.4°E)and Zuoling(30.5°N,114.6°E)stations of the(Chinese)Meridian *** Lomb−Scargle periodogram method has been utilized to analyze the periodic variations for time series with observational data *** results show that the zonal winds at both stations are eastward dominated,while the meridional winds are southward *** variance of GW disturbances in the zonal and meridional directions increases gradually with height,and there is a strong pattern of annual *** zonal momentum flux of GW changes little with height,showing weak annual *** meridional GW flux varies gradually from northward to southward with height,and the annual periodicity is *** both stations,the maximum values of zonal and meridional wind occur close to the peak heights of GW flux,with opposite *** observational evidence is consistent with the filtering *** horizontal wind velocity,GW flux,and disturbance variance of the GW at Mohe are overall smaller than those at Zuoling,indicating weaker activities in the MLT at *** power spectral density(PSD)calculated by the Lomb−Scargle periodogram shows that there are 12-month period and 6-month period in horizontal wind field,GW disturbance variance and GW flux at both stations,and especially there is also a 4-month cycle in the disturbance *** PSD of the 12-month and 6-month cycles exhibits maximum values below 88 km and above 94 km.

Dynamical entanglement for Fermi coupled stretching and bending modes

The dynamical entanglement for Fermi coupled C-H stretch and bend vibrations in molecule chd3 is studied in terms of two negativities and the reduced yon Neumann entropy, where initial states are taken to be direct products of photon-added coherent states on each mode. It is demonstrated that the negativity defined by the sum of negative eigenvalues of the partial transpose of density matrices is positively correlated with the von Neumann entropy. The entanglement difference between photon-added coherent states and usual coherent states is discussed as well.

Lower clearance of sodium tanshinone IIA sulfonate in coronary heart disease patients and the effect of total bilirubin: a population pharmacokinetics analysis

This study developed a population pharmacokinetic model for sodium tanshinone IIA sulfonate(STS) in healthy volunteers and coronary heart disease(chd) patients in order to identify significant covariates for the pharmacokinetics of STS. Blood samples were obtained by intense sampling approach from 10 healthy volunteers and sparse sampling from 25 chd patients, and a population pharmacokinetic analysis was performed by nonlinear mixed-effect modeling. The final model was evaluated by bootstrap and visual predictive check. A total of 230 plasma concentrations were included, 137 from healthy volunteers and 93 from chd patients. It was a two-compartment model with first-order elimination. The typical value of the apparent clearance(CL) of STS in chd patients with total bilirubin(TBIL) level of 10 μmol×L^(–1) was 48.7 L×h^(–1) with inter individual variability of 27.4%, whereas that in healthy volunteers with the same TBIL level was 63.1 L×h^(–1). Residual variability was described by a proportional error model and estimated at 5.2%. The CL of STS in chd patients was lower than that in healthy volunteers and decreased when TBIL levels increased. The bootstrap and visual predictive check confirmed the stability and validity of the final model. These results suggested that STS dosage adjustment might be considered based on TBIL levels in chd patients.

Combining the complex variable reproducing kernel particle method and the finite element method for solving transient heat conduction problems

In this paper, the complex variable reproducing kernel particle （CVRKP） method and the finite element （FE） method are combined as the CVRKP-FE method to solve transient heat conduction problems. The CVRKP-FE method not only conveniently imposes the essential boundary conditions, but also exploits the advantages of the individual methods while avoiding their disadvantages, then the computational efficiency is higher. A hybrid approximation function is applied to combine the CVRKP method with the FE method, and the traditional difference method for two-point boundary value problems is selected as the time discretization scheme. The corresponding formulations of the CVRKP-FE method are presented in detail. Several selected numerical examples of the transient heat conduction problems are presented to illustrate the performance of the CVRKP-FE method.

Effect of Rare Earth Composite Modification on Microstructure and Properties of a New Cast Hot-Work Die Steel

The effects of RE modification on structure and the properties of a new cast hot work die (chd) steel were investigated. The grains of the chd steel are refined by RE modification. With the increase of RE addition, both grain size and inclusion amount are reduced. Appropriate amount of RE results in decrease in inclusion amount and formation of spheroidal inclusions uniformly-distributed in steel, so that the morphology and distribution of inclusions are improved. RE composite modification favors the formation of bainite, austenite and fine lath martensite with dense dislocation. When the residual RE content reaches 0.02%, no obvious changes in strength and hardness are found, while fracture toughness and threshold of fatigue crack growth are increased. The impact toughness, elongation and reduction of cross sectional area are increased by a factor of two, and thermal fatigue resistance is also improved.
