The clinical characteristics and risk factors for necrotizing soft tissue infection in children

Importance:Necrotizing soft tissue infection(NSTI)is a serious infectious ***,the early clinical manifestations and indicators of NSTI in children are still ***:The purpose of this study was to analyze the clinical characteristics and risk factors of NSTI in pediatric ***:A total of 127 children with skin and soft tissue infection(SSTI)were treated at our hospital and divided into two groups:the NSTI group and the non-NSTI group,based on their discharge diagnosis from January 2011 to December ***,we collected and analyzed the clinical characteristics and risk factors of all patients,including sex and age,disease inducement,admission temperature,local skin manifestations,infection site,the presence of sepsis,bacterial culture,and laboratory ***:In our study,there was a statistical difference in the age distribution and disease inducement between NSTI and non-NSTI *** occurrence of local skin manifestations(blisters/bullae and ecchymosis)and the presence of sepsis significantly increased in the NSTI group compared to the non-NSTI ***,only the platelet count on laboratory tests was statistically different between the NSTI and non-NSTI ***,the logistic regression analysis suggested that local skin manifestations such as blisters/bullae,and ecchymosis,as well as the presence of sepsis,were identified as risk factors for ***:Children with SSTI and skin manifestations such as blisters/bullae,ecchymosis,and the presence of sepsis are at a higher risk of developing *** symptoms serve as useful indicators for early detection of NSTI.

Mesenchymal stem cells(MSCs),the most well-studied cell type in the field of stem cell therapy,have multi-lineage differentiation and self-renewal ***-based thera-pies have been used to treat diverse diseases because of their ability to potently repair tissue and locally restore *** increasing body of evidence demonstrates that paracrine func-tion is central to the effects of MsC-based *** factors,cytokines,chemokines,extracellular matrix components,and extracellular vehicles all contribute to the beneficial ef-fects of MSCs on tissue regeneration and *** paracrine substances secreted by MSCs change depending on the tissue microenvironment and biological *** this review,we discuss the bioactive substances secreted by MsCs depending on the microenvironment and biological behavior and their regulatory mechanisms,which explain their potential to treat human diseases,to provide new ideas for further research and clinical cell-free therapy.

To the Editor:Adipose-derived stem cells(ASCs)are promising multipotent stem cells for regeneration of skeletal muscle(SM)tissues.[1]However,the low myogenic differentiation ability limits the application.[2]Therefore,it is necessary to summarize the current differentiation strategy to improve the differentiation *** methods(biochemical induction)and non-traditional strategies(co-culture,biomechanical approaches,and biomaterial utilization)are commonly used for the differentiation of ASCs into myogenic *** both strategies can effectively induce differentiation of myogenic cells,cellular unhealthy or immature myogenic differentiation status is still the greatest challenge for ***,the combination of non-traditional methods and traditional methods has the potential to significantly enhance myogenesis of ASCs with remarkable efficacy,however,evidence is currrently *** review discusses the traditional and non-traditional strategies,as well as several combination motheds of above strategies,for inducing ASCs differentiation into myogenic cells[Figure 1].

Piezoelectric film promotes skin wound healing with enhanced collagen deposition and vessels regeneration via upregulation of PI3K/AKT

Skin wounds are common in accidental injuries,and the intricacies of wound repair are closely linked to endogenous electric *** stimulation plays a pivotal role in the restorative processes of skin injuries,encompassing collagen deposition,angiogenesis,inflammation,and *** electrical stimulation therapy replicates and enhances the effects of endogenous wound electric fields by applying an external electric field to the wound site,thereby promoting skin wound *** this study,we developed a self-powered repetitive mechanical impacts-electrical stimulation(RMI-ES)system utilizing a BaTiO_(3)/polydimethylsiloxane(PDMS)piezoelectric composite *** to conventional electrical stimulation devices,the fabricated piezoelectric composite film efficiently harvests energy from the pressure applied by the stimulation device and the tensile force occurring during natural rat *** results demonstrated that piezoelectric stimulation generated by the composite membrane expedited the cell cycle,promoting fibroblast ***,piezoelectric stimulation induced favorable changes in fibroblast gene expression,including increased expression of transforming growth factor-β1(TGF-β1),connective tissue growth factor(CTGF),collagen 1,collagen 3,vascular endothelial growth factor(VEGF),and alpha-smooth muscle actin(α-SMA),while reducing interleukin-6(IL-6)*** analysis revealed that piezoelectric stimulation may induce fibroblast migration,proliferation,and collagen expression by influencing PI3K/AKT serine/threonine kinase(AKT)*** confirmation through the addition of the PI3K inhibitor LY294002 validated that piezoelectric stimulation can regulate the repair process after skin injury through the ***,in vivo results demonstrated that the electric field at the wound site effectively promoted wound healing,reduced inflammation,and stimulated collagen deposition and ne

Construction of millimeter-scale vascularized engineered myocardial tissue using a mixed gel

Engineering myocardium has shown great clinal potential for repairing permanent myocardial ***,the lack of perfusing blood vessels and difficulties in preparing a thick-engineered myocardium result in its limited clinical *** prepared a mixed gel containing fibrin(5 mg/ml)and collagen I(0.2 mg/ml)and verified that human umbilical vein endothelial cells(HUVECs)and human-induced pluripotent stem cell-derived cardiomyocytes(hiPSC-CMs)could fom microvascular lumens and myocardial cell clusters by harnessing the low-hardness and hyperelastic characteristics of ***-CMs and HUVECs in the mixed gel formed self-organized cell clusters,which were then cultured in different media using a three-phase *** successfully constructed vascularized engineered myocardial tissue had a spherical structure and final diameter of *** tissue exhibited autonomous beats that occured at a frequency similar to a normal human heart *** internal microvascular lumen could be maintained for 6 weeks and showed good results during preliminary surface re-vascularization in vitro and vascular remodeling in *** summary,we propose a simple method for constructing vascularized engineered myocardial tissue,through phased cultivation that does not rely on high-end manufacturing equipment and cutting-edge preparation *** constructed tissue has potential value for clinical use after preliminary evaluation.

Adaptability of bipedicled scalp advancement flap for coverage of huge scalp defects after tumor resection

Background:Several reconstructive techniques have been used to reconstruct scalp *** researchers have studied the utilization of bipedicled scalp advancement flaps for scalp defect *** this study,we will assess the adaptability of a bipedicled scalp advancement flap for coverage of huge scalp defects after tumor ***:This study was performed at the plastic surgery department,Fayoum University Hospital,on 20 patients with huge scalp tumors treated by adequate excision and reconstructed using a bipedicled scalp advancement flap from December 2021 to July ***:The study was conducted on 20 patients:16(80%)were men and 4(20%)were women presenting with large scalp *** mean age was 57.4 years(33-68 years).The sites of the tumors were 10(50%)occipital,6(30%)parietal,and 4(20%)*** major complications occurred postoperatively,and the bipedicled flaps survived well in all cases,with no ***:Huge scalp defects after tumor resection can be safely reconstructed with a large versatile bipedicled advancement scalp flap,either transversely or longitudinally oriented,with favorable functional results and accepted aesthetic results,apart from the alopecic area over the grafted donor site,which can be treated by tissue expansion if desired later.

Evaluation of the versatility of perforators-enhanced rhomboid flap technique for ischial sores reconstruction

Background: Prolonged pressure on the skin known as pressure sores, can cause frequent injuries to the skin and underlying tissues. However, several prevention approaches, including non-surgical and surgical management,are available. In flap surgery for pressure sores, a variety of flap types may be used;each offers certain benefits and some incidences of complications and recurrence. This study evaluated the versatility of the perforatorenhanced rhomboid flap technique for ischial sore ***: This prospective study was conducted in the plastic surgery department of Fayoum University *** perforator-enhanced rhomboid flap technique was employed in 20 patients with ischial pressure sores who failed to respond to conservative treatment. A proper history was obtained and a preoperative arterial duplex was performed to determine the site of the prominent perforators for their preservation at the base of the *** follow-ups and evaluations were ***: The mean age of the study group was 43.7 ± 13.1, ranging from 19 to 65 years. Moreover, 80% of the participants were males while 20% were female. The main etiology of the sores was paraplegia in 45% of *** mean surface area of sores was 24.8 ± 6.3 cm2. In 75% of cases, the color match of the flap was rated as excellent. Similarly, 70% of the patients rated flap thickness as excellent. In 55% of the cases, the scar appearance was deemed excellent. Overall satisfaction was reported as excellent by 40% of the patients, while only 5% reported poor satisfaction. Lateral circumflex femoral artery perforator flap for the reconstruction of head soft tissue defects: Cross-region venous anastomosis

Background: Owing to its unique characteristics, the lateral circumflex femoral artery perforator (LCFAP) flap is often preferred for repairing head wounds with exposed skulls. However, given the vascular distribution in the head, particularly the veins, can lead to postoperative complications such as venous congestion of the flap. The rates of vascular exploration and necrosis in these flaps are significantly higher than in other body regions. Therefore, it is crucial to identify a safe and effective method for venous anastomosis of free flaps in the head region. Methods: This retrospective case series study included 10 patients with large head soft tissue defects treated at the burn and plastic surgery department of the Affiliated Hospital of Zunyi Medical University from January 2020 to December 2022. The head defects were reconstructed using LCFAP flaps, with flap veins anastomosed to the external jugular vein in the neck, either directly or via a bridging technique. Results: Among the 10 adult patients with massive head wound defects, 7 (70%) were men. The patients’ mean age was 53.0 years (48–59 years). The wound defects were caused by trauma in 6 (60%) patients and by tumors in 4 (40%) patients. Postoperatively, no significant complications occurred, and all LCFAP flap survived without necrosis. Conclusion: The descending branch of the LCFAP flap effectively repairs massive head wound defects. The venous anastomosis method for this flap is associated with a low incidence of venous complications and a high patency rate, making it a clinically valuable reference. © 2024 China Medical Cosmetology Press Co.
