External validation of the Moroccan Arabic version of the European Organization for Research and Treatment of Cancer colorectal(CR29)module:Monocentric study

Biomimetic coatings constituted of gelatin and nanocrystalline apatite were deposited on titanium substrates from a slightly supersaturated calcium phosphate (CaP) solution enriched with different amounts of gelatin. Although the biopolymer inhibits the crystallization of the inorganic phase, as shown by the reduction of the mean dimensions of the spherical aggregates and of the degree of crystallinity of the apatitic phase on increasing gelatin concentration, the deposition of a uniform layer of nanocrystalline apatite takes place in a few hours. Gelatin incorporation into the precipitate increases on increasing its concentration in solution, up to about 20 wt%. Osteoblast-like MG63 cells cultured on the coatings display good proliferation and increased values of the differentiation parameters with respect to the control. The presence of gelatin improves significantly the biological response to the biomimetic coatings, as shown by the higher values of proliferation, collagen type I and osteocalcin production, as well as alkaline phosphatase activity stimulation.

Pancreatic cancer is one of the deadliest cancers with a very poor prognosis. Recently, there has been a significant increase in research directed towards identifying potential biomarkers that can be used to diagnose and provide prognostic information for pancreatic cancer. These markers can be used clinically to optimize and personalize therapy for individual patients. In this review, we focused on 3 biomarkers involved in the DNA damage response pathway and the necroptosis pathway: Chromodomainhelicase-DNA binding protein 5, chromodomain-helicaseDNA binding protein 7, and mixed lineage kinase domain-like protein. The aim of this article is to review present literature provided for these biomarkers and current studies in which their effectiveness as prognostic biomarkers are analyzed in order to determine their future use as biomarkers in clinical medicine. Based on the data presented, these biomarkers warrant further investigation,and should be validated in future studies.

Hepatitis C virus(HCV)infection is considered a systemic disease because of involvement of other organs and tissues concomitantly with liver *** the extrahepatic manifestations,neuropsychiatric disorders have been reported in up to 50%of chronic HCV infected *** the central and peripheral nervous system may be involved with a wide variety of clinical *** HCV-associated neurological conditions include cerebrovascular events,encephalopathy,myelitis,encephalomyelitis,and cognitive impairment,whereas"brain fog",depression,anxiety,and fatigue are at the top of the list of psychiatric ***,HCV infection is known to cause both motor and sensory peripheral neuropathy in the context of mixed cryoglobulinemia,and has also been recently recognized as an independent risk factor for *** extrahepatic manifestations are independent of severity of the underlying chronic liver disease and hepatic *** brain is a suitable site for HCV replication,where the virus may directly exert neurotoxicity;other mechanisms proposed to explain the pathogenesis of neuropsychiatric disorders in chronic HCV infection include derangement of metabolic pathways of infected cells,alterations in neurotransmitter circuits,autoimmune disorders,and cerebral or systemic inflammation.A pathogenic role for HCV is also suggested by improvement of neurological and psychiatric symptoms in patients achieving a sustained virologic response following interferon treatment;however,further ad hoc trials are needed to fully assess the impact of HCV infection and specific antiviral treatments on associated neuropsychiatric disorders.

高毒力碳青霉烯耐药肺炎克雷伯菌(Hv-CRKP)已成为全球公共卫生领域的重大挑战.该研究报告了2017年至2022年浙江一家三甲医院Hv-CRKP分离株的定植和传播情况.对来自72名患者的90个不同的CRKP菌株进行测序分析,发现院内感染Hv-CRKP的流行主要依赖于ST11-K64克隆传播.对11个代表性分离株进行了全基因组测序,得到了31个完整的质粒基因序列,其中包括12个携带KPC-2耐药基因的质粒和10个携带rmpA毒力基因的质粒.除了二元质粒外,还发现了两种可介导rmpA和KPC-2共同传播的融合质粒.研究人员通过捕获其祖先质粒,提出了该融合质粒的形成机制.此外,发现了五种rmpA启动子变异体(P9T到P13T),其活力与地理分布存在差异.通过CRISPR/Cas9基因编辑技术,证实活跃的“P11T-rmpA”调控子是“高风险”ST11-K64/CRKP克隆群的生物标志物.该研究拓展了对ST11-K64/Hv-CRKP流行性克隆的播散和临床演进的认知.

To the Editor:Cystic tumors of the pancreas are rare,accounting for 1%-5%of exocrine pancreatic *** cystadenoma(SCA)was firstly described by Compagno and Oertel in 1978 as a benign glycogenrich neoplasm[1].In 1996 Perez-Ordonez et al.[2]reported the first case of a solid variant of SCA;since then,23 cases of this extremely uncommon tumor variant were reported.

背景：接受非心脏手术的老年患者出现红细胞压积异常和心血管并发症的危险较高。尽管患者术前几乎都要进行红细胞压积筛查，但是有关术前贫血及红细胞增多症的不良影响的证据较为有限。目的：于实施非心脏大手术的退伍老兵中评估术前贫血和红细胞增多症的发生率及其对术后30天预后的影响。设计：利用全国退伍军人事务委员会（VA）于术质量改进计划数据库进行回顾性队列研究。根据术前红细胞压积水平将患者分为三类：贫血（红细胞压积〈39．0％）、正常（红细胞压积39．0％～53．9％）和红细胞增多症（红细胞压积≥54％）。对红细胞压积异常与术后30天心血管事件发生率及死亡率的关系进行评估。地点及患者：纳入1997至2004年于全美132个退伍军人医疗中心接受非心脏大手术的310311例患者（年龄≥65岁）。主要观测指标：主要观测指标为术后30天死亡率，次要观测指标包括术后30天死亡率或心脏事件（心脏骤停或Q波心肌梗死）发生率。结果：与红细胞压积正常者相比，红细胞压积发生正向或负向偏离者术后30天死亡率和心脏事件发生率均有所增加。研究发现，红细胞压积水平较正常范围每增减1个百分点，术后30天死亡率就可增加1．6％（95％可信区间，1．1％-2．2％）。进一步分析显示，当红细胞压积降至39％以下或超过51％时，校正后30天死亡风险和心脏病患病风险就开始增加。结论：接受非心脏大手术的老年患者（绝大多数为男性退伍军人）术前轻度贫血及红细胞增多症均可增加术后发生死亡和心脏事件的危险。今后的研究应明确上述发现可否见于其他人群，明确术前纠正贫血和红细胞增多症能否降低术后死亡风险。

Purposes: Intralobar pulmonary sequestration is a rare congenital anomaly that causes nonspecific respiratory symptoms. This report presented our surgical experience for pediatric cases of intralobar pulmonary sequestration in our institute. Methods: Between 2002 and 2014, 30 pediatric patients with intralobar pulmonary sequestration underwent lobectomy. The clinical characteristics of patients were retrospectively analyzed using medical records and imaging data. Results: The median age at operation was 23 months. The sequestered lung was situated in the right lower lung in 15 cases and in the left lower lung in 15 cases. Eight cases were prenatally diagnosed by fetal ultrasound and magnetic resonance imaging. One aberrant artery was seen in 26 cases and two aberrant arteries in 4 cases. The drainage vein from the sequestered lung was: 1) the pulmonary vein in 27 cases;2) the inferior vena cava in 1 case;and 3) the azygos vein and the pulmonary vein in 2 cases. All patients underwent lower lobectomy of the affected side without postoperative early and long-term complications. Conclusion: Due to the increased rate of prenatal diagnoses, the age at operation has decreased. With respect to safety, it is vital to ascertain the number of aberrant arteries and the venous drainage route before lobectomy.
