The recurrence of atrial fibrillation(AF)in patients after successful radiofrequency catheter ablation(RFCA)appears to be an unresolved clinical issue and needs to be clearly *** are many factors associated with AF recurrence,such as duration of AF,male sex,concomitant heart failure,hemodynamic parameters,chronic obstructive pulmonary disease,hypertension,obstructive sleep apnea,hyperthyroidism,smoking and ***,the inflammatory changes are strongly associated with electrical and structural cardiac remodeling,cardiac damage,myocardial fibrotic changes,microvascular dysfunction and altered reparative *** this context,biomarkers reflecting the different stages of AF pathogenesis deserve thorough *** authors of the retrospective study revealed that one-year recurrence rate of non-valvular AF in the high systemic immune inflammation(Sii)index group was significantly increased compared to that of the low Sii index group and provided additional predictive value to the ***,the authors suggest that this biomarker may help physicians to optimize the selection of AF patients and to develop a personalized treatment *** conclusion,the Sii index may serve as a valuable indicator of recurrent AF in patients after RFCA and may be a biomarker with plausible predictive value for poor clinical outcomes.

BACKGROUND Fluoropyrimidines are metabolized in the liver by the enzyme dihydropyrimidine dehydrogenase(DPD),encoded by the DPYD *** 7%of the European population is a carrier of DPYD gene polymorphisms associated with reduced DPD enzyme *** To assess the prevalence of DPYD polymorphisms and their impact on fluoropyrimidine tolerability in Italian patients with gastrointestinal *** A total of 300 consecutive patients with a diagnosis of gastrointestinal malignancy and treated with a fluoropyrimidine-based regimen were included in the analysis and divided into two cohorts:(1)149 patients who started fluoropyrimidines after DPYD testing;and(2)151 patients treated without DPYD *** the patients in cohort A,15%tested only the DPYD2A polymorphism,19%tested four polymorphisms(DPYD2A,HapB3,c.2846A>T,and DPYD13),and 66%tested five polymorphisms including *** Overall,14.8%of patients were found to be carriers of a DPYD variant,the most common being DPYD6(12.1%).Patients in cohort A reported≥G3 toxicities(P=0.00098),particularly fewer nonhematological toxicities(P=0.0028)compared with cohort B,whereas there was no statistically significant difference between the two cohorts in hematological toxicities(P=0.6944).Significantly fewer chemotherapy dose reductions(P=0.00002)were observed in cohort A compared to cohort B,whereas there was no statistically significant differences in chemotherapy *** Although this study had a limited sample size,it provides additional information on the prevalence of DPYD polymorphisms in the Italian population and highlights the role of pharmacogenetic testing to prevent severe toxicity.

BACKGROUND Colorectal cancer(CRC)ranks high among the most common types of malignant *** primary cause of cancer-related mortality is metastasis,with lung metastases accounting for 32.9%of all cases of metastatic CRC(MCRC).However,cases of MCRC in the lungs,which present concurrently with primary peripheral lung adenocarcinoma,are exceptionally *** SUMMARY This report describes the case of a 52-year-old female patient who,following a colonoscopy,was diagnosed with moderately differentiated adenocarcinoma based on rectal mucosal biopsy findings.A preoperative chest computed tomography scan revealed a ground-glass nodule in the right lung and a small nodule(approximately 0.6 cm in diameter)in the extramural basal segment of the left lower lobe,which suggested multiple lung metastases from rectal *** treatment and follow-up led to a diagnosis of rectal cancer with left lung metastasis and peripheral adenocarcinoma of the lower lobe of the right *** This case report describes the therapeutic journey of a patient with lung metastasis from rectal cancer in addition to primary peripheral adenocarcinoma,thus underscoring the critical roles of multidisciplinary collaboration,personalized treatment strategies,and comprehensive patient rehabilitation guidance.

Maxillary bone loss impairs essential functions (chewing, swallowing, speech) and gives patients a very unaesthetic appearance due to the removal of facial support tissues, leading to serious psychological consequences. Treatment is multidisciplinary and requires a resective surgery if the cause is tumor-related, or an additive surgery if the cause is traumatic. This article aims to show the role of making a prosthesis to restore function (chewing, swallowing, speech) and aesthetics following maxillary bone loss. We will eighter present a clinical case involving a right maxillary tumor that was surgically resected followed by radiotherapy, and subsequently rehabilitated with a maxillofacial prosthesis in the consultation and dental treatment center of the university hospital center of Casablanca.

Chiral coordination molecular cages/capsules with discrete nanoconfined chiral cavities demonstrate significant potential applications across various fields. In this study, we utilized Tr?ger's base as the building block to design and synthesize two pairs of enantiopure ligands. These ligands were then self-assembled with Pd(ii) ions through chiral self-sorting coordination, resulting in the formation of two pairs of homochiral M2L4-type coordination molecular capsules. Notably, due to differences in the substitution positions on the Tr?ger's base, these two pairs of enantiomeric coordination molecular capsules exhibited distinct levels of cavity closures, cavity sizes, and host-guest recognition properties. This research offers valuable insights into the construction of novel chiral molecular capsules and the regulation of confined cavities.

Offshore Nile Delta in Egypt represents an enormous hydrocarbon province with recent projected gas and condensate discoveries of more than 50 trillion cubic feet “TCF”. Most of these occur in the post-salt hydrocarbon plays where biogenic gases are dominant. This study integrates organic geochemistry,seismic geomorphology and petrophysics in order to decipher the origin, and accumulation conditions of the wet gas/condensate blend in the Upper Miocene sub-salt Wakar Formation sandstones in Port Fouad Marine “PFM” Field, offshore Nile Delta. Hydrocarbon pay zones are scattered thin（ 1 w.t.%, hydrogen index“HI” > 200 mg HC/g TOC） and mature(Tmax> 430°C, vitrinite reflectance “VR” > 0.6 %R_o), source rocks are restricted to Upper Miocene Wakar and Oligo-Miocene Tineh formations. The latter contains more mature organofacies（up to 1.2 %R_o） and type ii/iiI kerogen, thereby demonstrating a good capability to generate wet gases. The studied gas is wet and has thermogenic origin with signs of secondary microbial alteration, whereas the condensate contains a mixture of marine and terrestrial input. Molecular biomarkers of the condensate, isotopic and molecular composition of the gas reveals a generation of condensate prior to gas expulsion from the source. The Wakar sandstones have a heterogeneous pore system where three reservoir rock types（RRTI, RRTii and RRTiiI）. RRTI rocks present the bulk composition of the Wakar pay zones. Spatial distribution of RRTI facies likely control the accumulation of the sub-salt hydrocarbons. Our results provide a new evidence on an active petroleum system in the sub-salt Paleogene successions in the offshore Nile Delta where concomitant generation of ga

For the first time we have shown here that the constituent from tectal extract (Te) which can support and promote the survival and growth of the retinal ganglion cell is a 30 kD protein. (i) Using MTT colorimetric microassay to measure the optical density for the survival and growth of the cultured retinal neuron, it was found that the optical densities for the experimental cultures with either Te, or its 10-30 kD fraction, or its ≥30 kD fraction were 2-4 times that of the control culture without Te (Pii) The retinal neurons cultured on Te Phast gels showed that large retinal neurons (>18 μm) grew only on the gel region containing the 30 kD protein. The retrograde prelabelled with horseradish peroxidase (HRP) for retinal ganglion cells indicated that these surviving neurons grown on Te Phast gel were HRP positive, i.e. retinal ganglion cells, (iii) The retinal explants cultured with 30 kD gels at a close distance of 1 mm apart revealed that many neurite outgrowths, up to 400 μm, extended from the retinal explants towards 30 kD gels, and that many individual cells and tissue masses migrated out from the explants and grew onto 30 kD gels. They were stained anti-neuron specific enolase and anti-Thy 1.1 positive, indicating they are retinal ganglion cells. Therefore, we concluded that the 30 kD protein is the neuronotrophic factor in the tectal extract specific for retinal ganglion cells.

Introduction: Obstructive Sleep Apnea-Hypopnea Syndrome (OSAHS) is a sleep-related breathing disorder characterized by repeated episodes of partial obstruction of the upper airways (hypopnea) and/or intermittent complete obstruction (apnea). Our aim is to study the role of adeno-tonsillectomy in the management of OSAHS in children. Methodology: This was a prospective descriptive study conducted in the ENT-Head and Neck Surgery department of Ignace Deen National Hospital over a six-month period. We included in our study all patients aged 0 to 15 years, presenting with OSAHS of ENT etiology and who had undergone tonsillectomy/adenoidectomy. Results: The frequency of OSAHS was 13%. The mean age of our patients was 5.1 ± 3.8 years. There was a predominance of males (69.6%) with a sex ratio of 2.28. Snoring (98.6%), nasal obstruction (97.1%), and mouth breathing (96.6%) were the main reasons for consultation. Adeno-tonsillectomy (45.4%) was the primary surgical intervention. Almost all children (99.0%) had a simple postoperative course. Conclusion: OSAHS is a condition with a multifactorial etiology. Adeno-tonsillectomy remains the first-line surgical treatment to prevent severe complications and relieve the patient. Multidisciplinary collaboration is essential in the management of OSAHS.
