Rhinoplasty in Chinese： management of lower dorsum and bulbous nasal tip

Background Bulbous nasal tips and lower dorsa are common facial features in Chinese people, and surgery to reshape these is frequently requested. The use of silicone implants in rhinoplasty has been widely used in China for many years, but is not suitable for patients seeking Caucasian tip shapes. The creation of an excessively high tip supported only by a silicone implant inevitably leads to implant extrusion. Although many rhinoplasty techniques have been used in Caucasian patients, there is currently no suitable method for Chinese patients, whose anatomy differs from that of Caucasians. The present study was aimed to investigate the clinical outcome of a novel method of rhinoplasty in Chinese people. Methods Eighty patients underwent rhinoplasty using our method between 2002 and 2006. We classified the patients into three types, according to the distance between tip defining points, and used different techniques accordingly. Furthermore, an innovative cartilage carving method and a tip fibro-fatty tissue flap were designed and combined with traditional techniques, such as insertion of silicone implant, cartilage grafts, suture techniques and cephalic trimming to reshape the nasal contours. The followup period was 10-60 months （average, 21 months）. Results Remarkable modifications in nasal contours were achieved. No complications developed in any of the 80 patients. Seventy-eight patients were satisfied with the results. The outcomes remained unchanged over time. Conclusion Our method is effective and suitable for the treatment of Chinese patients with lower dorsa and bulbous nasal tips.

We describe the prenatal diagnosis and fetal phenotype of partial trisomy 12(p12-pter) transmitted from a maternal reciprocal translocation 6;*** analysis was conducted on umbilical cord blood for a fetus accompanied with tricuspid regurgitation and orbital hypertelorism from a 27-year-old gravida 4,para 1 after sonography at gestation 35 *** karyotype was unusual,with 46,XY,der(6),t(6;12)(p24;p12) *** pregnancy was terminated at 37 gestational *** aborted fetus displayed dysmorphic features of a round flat face with prominent cheeks and high forehead,hypertelorism,short nose,broad and depressed nasal bridge, anteverted nares,deformed philtrum,open mouth,thin upper vermilion and broad everted lower lip,low-set ears and aural atresia,broad hands with simian creases,and a short *** anatomy showed right artery catheter vagus,congenital cataract,no turbinate and external auditory *** karyotype-phenotype analysis of this patient and a review of other reported cases, we believe this is a first report that expands the database of partial trisomy 12p,and is beneficial for future clinical genetic *** study supports that phenotypic variability depends on the type and extent of the associated partial monosomy.

Massive Rhinorrhea as a Complication of COVID-19 Nasopharyngeal Swab

The nasopharyngeal swab for COVID-19 testing is generally considered safe;however, it is primarily performed by staff that may not completely understand the anatomy of the nasal cavity and nasopharynx. We report the treatment of a 48-year-old man who developed unilateral left rhinorrhea after a nasopharyngeal swab sample. It is aimed to draw attention to this complication that can be prevented with the appropriate technique during nasopharyngeal swab sampling.

Accidental Intra-Arterial Injection during Test Dose of Injection of Benzyl Penicillin: Complications and Management—A Case Report

An 18 years old female patient was given a test dose of Benzyl penicillin in the Antecubital Area on the right forearm by a staff nurse of the department. After the injection, the patient complained of severe pain in the right forearm distal to the injection site. This was followed by blanching of the right forearm and hand. She was immediately heparinised followed by other symptomatic treatments. After 12 hours, a bluish discoloration developed on the hand, both on dorsal and palmar aspect, sparing the thumb. This recovered with medication, except the late development of dry gangrene of the tip of distal phalanx of the little finger which got shrivelled later on. Color Doppler flow study, echocardiography, and CT angiography of the right upper limb were done. Understanding of the pathogenesis and prompt action can help in saving a limb. Education of the medical professionals involved in administering injection is necessary so that they become aware of abnormal course or anomalous anatomy of vessels of upper limb. This can help to prevent the complications.

Laparoscopic cholecystectomy for a left-sided gallbladder

Cholecystectomy is a common *** in the anatomy of the biliary system are common but an abnormal location of the gallbladder is much *** frequent pre-operative imaging,the aberrant location of the gallbladder is commonly discovered at *** article presents a case of a patient with the gallbladder located to the left of the falciform ligament in the absence of situs inversus totalis that presented with right upper quadrant pain.A laparoscopic cholecystectomy was performed and it was noted that the cystic duct originated from the right *** presence of a left sided gall bladder is often associated with various biliary,portal venous and other anomalies that might lead to intra-operative *** spectrum of unusual positions and anatomical gallbladder abnormalities is reviewed in order to facilitate elective and emergent cholecystectomy as well as other hepatobiliary *** proper identification of the anatomy,minimally invasive approaches are still considered safe.

Airway Management for Treacher Collins Syndrome with Limited Mouth Opening

Patients with Treacher Collins Syndrome (TCS) present unique airway management problems for anesthesiologists due to mandibular micrognathia, the small oral aperture, and temporomandibular joint anomalies. We describe the case of a pediatric TCS patient with limited mouth opening who experienced severe airway obstruction during deep inhalation anesthesia (sevoflurane following i.v. midazolam) for routine dental work. When difficult airway management is expected, intubation of conscious patients is a well-recognized technique in adults;however, it is rarely appropriate for pediatric patients who usually do not cooperate. According to general anesthesia algorithms for pediatric patients with difficult airways, in most pediatric patients, tracheal intubation is performed after the induction of general anesthesia and some authors have reported the usefulness of LMA for maintaining airway patency in patients with TCS. However, in our case LMA could not be used because of severe limitation of mouth opening. In addition, the LMA is so bulky that it is impossible to insert a LMA into patients with narrow airway anatomy. We initially planned to carry out fiber-optic intubation while awake and under sedation if the airway patency could not be secured after the induction of anesthesia. The patient was sedated properly with midazolam and sevoflurane, and awake fiberoptic intubation was performed uneventfully. Our experience in this case highlighted that careful planning of backup contingencies is important in achieving fiberoptic intubation and maintaining airway patency in pediatric TCS patients with limited mouth opening, and that awake intubation can be successful even in pediatric patients.

TREATMENT OF EPIGASTRIC PAIN WITH SUBCUTANEOUS NEEDLING

Subcutaneous needling is a new therapeutic method which is based on the features of the meridian-collateral theory of cutaneous regions in traditional Chinese medicine and the neurohumeral theory and skin anatomy of modern medicine and is of the features of “fixed meridian but not fixed acupoints”, “micro stimulation”, and “subcutaneous needling with filiform needles” to achieve the goal of treating diseases. This method is indicated clinically to the analgesia and relaxation of spasm especially the pain caused by the disorders of the internal organs and marked therapeutic effect has been obtained. With method, 150 cases of epigastric pain were treated by our department and the total effective rate was over 90%. It is presented herein below.

Nasal dorsal aesthetic lines and rhinoplasty technical tricks

Rhinoplasty surgery remains one of the most difficult operations of the *** aesthetic appearance and maintaining nasal function are inseparable goals in rhinoplasty surgery,and failure to achieve either of these objectives can be devastating for the *** evaluating a variety of rhinoplasty complications,increased attention was devoted to the surgical technique for reconstruction of the dorsal aesthetic lines and nasal tip projection in the patient with a prominent dorsal *** on the modern concept of cartilage conservation,the autospreader flap rotation technique should be considered when dorsal reduction is *** flaps are a useful tool in the prevention of postoperative nasal obstruction,segmental(inverted V)appearance,midfacial axial asymmetry and an overdone supratip *** addition,they assist in preserving ethnicity of the nose when *** patient with a long nose,prominent dorsal hump,short nasal bones and low lower lateral cartilages are considered to be an ideal candidate for an autospreader flap.
