AIM: To evaluate the value of MR cholangiopancreatography (MRCP) in patients in whom endoscopic retrograde cholangiopancreatography (ERCP) was unsuccessfully performed by experts in a tertiary center. METHODS: From January 2000 to June 2003, 22 patients fulfilled the inclusion criteria. The indications for ERCP were obstructive jaundice (n = 9), abnormal liver enzymes (n=8),suspected chronic pancreatitis (n = 2), recurrent acute pancreatitis (n = 2), or suspected pancreatic cancer (n=1). The reasons for the ERCP failure were the postsurgical anatomy (n = 7), duodenal stenosis (n = 3), duodenal diverticulum (n = 2), and technical failure (n = 10). MRCP images were evaluated before and 5 and 10 min after i.v. administration of 0.5 IU/kg ***: The MRCP images were diagnosed in all 21patients. Five patients gave normal MR findings and required no further intervention. MRCP revealed abnormalities (primary sclerosing cholangitis, chronic pancreatitis, cholangitis, cholecystolithiasis or common bile duct dilation) in 10 patients, who were followed up clinically. Four patients subsequently underwent laparotomy (hepaticojejunostomy in consequence of common bile duct stenosis caused by unresectable pancreatic cancer; hepaticotomy+Kehr drainage because of insufficient biliary-enteric anastomosis; choledochojejunostomy, gastrojejunostomy and cysto-Wirsungo gastrostomy because of chronic pancreatitis, orcholedo chojejunostomy because of common bile duct stenosis caused by chronic pancreatitis). Three patients participated in therapeutic percutaneous transhepatic drainage. The indications were choledocholithiasis with choledochojejunostomy, insufficient biliary-enteric anastomosis, or cholangiocarcinoma. CONCLUSION: MRCP can assist the diagnosis and management of patients in whom ERCP is not possible.

Anatomical variations of the cystic duct often occur and may be encountered during cholecystectomy. Knowledge of the variable anatomy of the cystic duct and cysticohepatic junction is important to avoid significant ductal injury in biliary surgery. Here, we present two unusual cases with an anomalous cystic duct, namely, low lateral insertion and narrow-winding of the cystic duct. The first case was a 64-year-old man with cholelithiasis and chronic cholecystitis. During surgery, the entrance of the cystic duct was misidentified as being short and leading into the right hepatic duct. Further exploration showed multiple calculi in the right and common hepatic ducts. Cholecystectomy was completed, followed by T-tube drainage of the common and right hepatic ducts. Postoperative T-tube cholangiography demonstrated that the two T tubes were respectively located in the cystic and common hepatic duct. Six weeks later, the retained stones in the distal choledochus were extracted by cholangioscopy through the sinus tract of the T-tube. The second case was a 41-year-old woman, in which, preoperative endoscopic retrograde cholangiopancreatography (ERCP) revealed a long cystic duct, with a narrow and curved-in lumen. The patient underwent open cholecystectomy. Both patients were cured. The authors propose that preoperative ERCP or magnetic resonance cholangiopancreatography (MRCP), and intraoperative cholangiography or cholangioscopy constitute a useful and safe procedure for determining anatomical variations of the cystic duct.

COVID-19 pandemic continues to spread exponentially worldwide, especially in America. At the end of August 2020, Brazil is one of the most affected countries with more than three and a half million cases and up to 114,250 deaths. This study aims to assess the fear and peri-traumatic stress during the COVID-19 pandemics in Brazil, to enhance infection control methods, appropriate interventions, and public health policies. A cross-sectional survey has been conducted from April 12th to 18th using the Peri-Traumatic Distress Scale (CPDI) and the Fear Scale (FCV-19S) aiming to measure the peri-traumatic stress and fear as psychological reactions during the COVID-19 pandemic. For that purpose, an online spreadsheet was used to send the questionnaire and scales to a sample of 1844 participants as a collecting information tool. Significant positive relationship (r = 0.660, p < 0.001) and internal consistencies on CPDI and FCV-195 scales (Cronbach Alfa scores 0.90 and 0.88 respectively) were shown. Results highlight significant gender differences as in both scales women’s mean scores are higher showing that it is paramount that women’s voices were represented in policy spaces as socially constructed gender roles place them in a strategic position to enhance multi-level interventions (primary and secondary effects of COVID-19), equitable policies, and new approaches to control the pandemic.

Background:Gossypium herbaceum(***),a plant commonly found in the wild in Nigeria,is said to possess some therapeutic ***,there is a dearth of information on its chemical ***,there is a need to investigate its therapeutic ***:To investigate the qualitative and quantitative phytochemical components of the leaf extracts of ***,as well as its antioxidant,antimalarial and cytotoxic ***:Gas chromatography-mass spectrometry(GC-MS)analysis was used to determine the components of ethanol and hexane extracts of *** leaf while 2,2-diphenyl-1-picrylhydrazyl(DPPH)and nitric oxide assays were used to determine the antioxidant *** parasites viability was examined using parasite lactate dehydrogenase(pLDH)technique and HeLa cell was used for the cytotoxicity ***:Bioactive compounds identified in ethanol and hexane extracts of *** were 37 and 30 kinds,respectively,with major components as linoleic acid(36.10%and 33.82%),vitamin E(7.15%and 5.98%)and caryophyllene(4.21%and 5.08%).It also has tannins,saponins,alkaloids,flavonoids steroids,phenols and *** antibacterial test,significant inhibitory potentials against multidrug-resistant bacteria strains were *** vitro antioxidant potentials(IC 50)of ethanol and hexane extracts were 3.33 and 4.12μg/mL for DPPH assays,and 3.87 and 5.00μg/mL for nitric oxide assays,*** activities(IC 50)were 9.99 and 9.76μg/mL for ethanol and hexane extracts,***:*** leaf may provide novel plant-derived therapeutic agents,effective in treating infectious diseases arising from multiple drug-resistant bacteria and a target in the management of oxidative stress.
