Early endotracheal intubation is not associated with the rate of return of spontaneous circulation following cardiac arrest at the emergency department:an exploratory analysis

Cardiac arrest poses a significant global public health challenge,manifesting in approximately 550,000 cases annually within the United States.[1]In-hospital cardiac arrest(IHCA)is commonly attributed to airways and respiratory issues.[2]Recommendations emphasize the expertise of responders in airway management.[3]Various options exist,such as chest compression-only cardiopulmonary resuscitation(CPR),bag-mask ventilation(BMV),and advanced airways.The BMV and advanced airways are not deemed equivalent or superior based on previous evidence.[4]Achieving consistency in choosing and timing the optimal airway approach during IHCA is crucial.The current American Heart Association guidelines suggest an advanced airway strategy when endotracheal intubation(ETI)success rates are high,but the optimal time for advanced airway management remains unclear.[5]Wong et al[6]revealed that survival improved by less than 5 min with advanced airway management.According to a subgroup analysis of IHCA patients in emergency departments(EDs),early intubation was associated with a 1.5-fold greater rate of return of spontaneous circulation(ROSC)than in other locations.[7]ED patients’constant monitoring and immediate management,with readily available intubation equipment,enhance early intubation and survival rates.[6]Nonetheless,IHCA patients intubated within the first 15 min had a lower ROSC rate.

The“depict”strategy for discovering new compounds in complex matrices:Lycibarbarspermidines as a case

The comprehensive detection and identification of active ingredients in complex matrices is a crucial challenge.Liquid chromatography coupled with high-resolution mass spectrometry(LC-HRMS)is the most prominent analytical platform for the exploration of novel active compounds from complex matrices.However,the LC-HRMS-based analysis workflow suffers from several bottleneck issues,such as trace content of target compounds,limited acquisition for fragment information,and uncertainty in interpreting relevant MS2 spectra.Lycibarbarspermidines are vital antioxidant active ingredients in Lycii Fructus,while the reported structures are merely focused on dicaffeoylspermidines due to their low content.To comprehensively detect the new structures of lycibarbarspermidine derivatives,a“depict”strategy was developed in this study.First,potential new lycibarbarspermidine derivatives were designed according to the biosynthetic pathway,and a comprehensive database was established,which enlarged the coverage of lycibarbarspermidine derivatives.Second,the polarity-oriented sample preparation of potential new compounds increased the concentration of the target compounds.Third,the construction of the molecular network based on the fragmentation pathway of lycibarbarspermidine derivatives broadened the comprehensiveness of identification.Finally,the weak response signals were captured by data-dependent scanning(DDA)followed by parallel reaction monitoring(PRM),and the efficiency of acquiring MS2 fragment ions of target compounds was significantly improved.Based on the integrated strategy above,210 lycibarbarspermidine derivatives were detected and identified from Lycii Fructus,and in particular,170 potential new compounds were structurally characterized.The integrated strategy improved the sensitivity of detection and the coverage of low-response components,and it is expected to be a promising pipeline for discovering new compounds.

Stereoselectivity in metabolic 3-reduction of tibolone in healthy Chinese female volunteers

Aim:To investigate the stereoselectivity in human metabolic 3-reduction oftibolone.Methods:Twenty healthy Chinese female volunteers were given a singleoral dose of tibolone(2.5 mg),and serial blood samples were collected aftertreatment.The plasma concentrations of the two pharmacologically active 3-hydroxyl metabolites of tibolone,3α-hydroxyl-7-methyl-norethynodrel(3α-HMN)and 3β-hydroxyl-7-methyl-norethynodrel(3β-HMN)in plasma were determinedby using a validated liquid chromatography-mass spectrometry(LC-MS)method.Results:The apparent elimination half-life(Tw)of 3α-HMN was 1.43±0.52 h,andthat of 3β-HMN was 1.53±0.60 h.Maximum plasma concentrations(Cmax)werefound to be 8.75±4.36 μg/L for 3α-HMN and 3.59+ 1.81 μg/L for 3β-HMN.Areasunder the plasma concentration versus time curve(AUC0-t)were 26.30±12.14 μg.h-1·L-1for 3α-HMN and 9.89±4.93 μg·h-1·L-1for 3β-HMN.Conclusion:Stereo-selective differences exist in the pharmacokinetics of tibolone metabolism inhumans.

Biosynthesis of imipramine glucuronide and characterization of imipramine glucuronidation catalyzed by recombinant UGTIA4

Aim:To study the profile of imipramine N~+-glucuronidation using homogenatesof recombinant uridine-5′-diphosphoglucuronosyltransferase 1A4 (UGT1A4) frombaculovirus-infected sf9 cells.Methods:Recombinant UGT1A4 was obtainedfrom sf9 cells infected with recombinant baculovirus.Imipramine N~+-glucuronidewas biosynthesized by incubating imipramine with recombinant UGT1A4 andthen purified with solid-phase cartridges.A reversed phase-high pressure liquidchromatography (RP-HPLC) assay method was used to directly measure the con-centration of imipramine and its metabolite,imipramine N~+-glucuronide,withp-nitrophenol as the internal standard.The validated method was used to charac-terize the activity of recombinant UGT1A4 and carry out kinetic studies on imi-pramine glucuronidation in vitro.Results:The high concentration of imipramineinhibited glucuronide conjugation,so the formula V=Vmax·S/(Km+S+S~2/Ki) was usedto calculate the parameters,using MATLAB software.The values of apparentKm,Ki,and Vmaxfor imipramine glucuronidation via UGT1A4 were 1.39±0.09mmol/L,6.24±0.45 mmol/L and 453.81±32.12 pmol/min per mg cell homogenate (n=3),respectively.Conclusion:As a specific substrate of UGT1A4,imipramine wasused as a convenient method to characterize the activity of recombinant UGT1A4by using HPLC.Furthermore,the profile of imipramine glucuronidation was evalu-ated by using recombinant UGT1A4 in vitro.

The action mechanism of the work done by the electric field force on moving charges to stimulate the emergence of carrier generation/recombination in a PN junction

It is discovered that the product of the current and the electric field in a PN junction should be regarded as the rate of work(power)done by the electric field force on moving charges(hole current and electron current),which was previously misinterpreted as solely a Joule heating effect.We clarify that it is exactly the work done by the electric field force on the moving charges to stimulate the emergence of non-equilibrium carriers,which triggers the novel physical phenomena.As regards to Joule heat,we point out that it should be calculated from Ohm’s law,rather than simply from the product of the current and the electric field.Based on this understanding,we conduct thorough discussion on the role of the electric field force in the process of carrier recombination and carrier generation.The thermal effects of carrier recombination and carrier generation followed are incorporated into the thermal equation of energy.The present study shows that the exothermic effect of carrier recombination leads to a temperature rise at the PN interface,while the endothermic effect of carrier generation causes a temperature reduction at the interface.These two opposite effects cause opposite heat flow directions in the PN junction under forward and backward bias voltages,highlighting the significance of managing device heating phenomena in design considerations.Therefore,this study possesses referential significance for the design and tuning on the performance of piezotronic devices.

Subpicosecond laser ablation behavior of a magnesium target and crater evolution:Molecular dynamics study and experimental validation

The micro-ablation processes and morphological evolution of ablative craters on single-crystal magnesium under subpicosecond laser irradiation are investigated using molecular dynamics(MD) simulations and experiments.The simulation results exhibit that the main failure mode of single-crystal Mg film irradiated by a low fluence and long pulse width laser is the ejection of surface atoms,which has laser-induced high stress.However,under high fluence and short pulse width laser irradiation,the main damage mechanism is nucleation fracture caused by stress wave reflection and superposition at the bottom of the film.In addition,Mg[0001] has higher pressure sensitivity and is more prone to ablation than Mg[0001].The evolution equation of crater depth is established using multi-pulse laser ablation simulation and verified by experiments.The results show that,under multiple pulsed laser irradiation,not only does the crater depth increase linearly with the pulse number,but also the quadratic term and constant term of the fitted crater profile curve increase linearly.

This is a continuation of the article（Comm.Partial Differential Equations 26（2001）965）.In this article,the authors consider the one-dimensional compressible isentropic Navier-Stokes equations with gravitational force,fixed boundary condition,a general pressure and the density-dependent viscosity coefficient when the viscous gas connects to vacuum state with a jump in density.Precisely,the viscosity coefficient μ is proportional to ρ^θ and 0〈θ〈1/2,where ρ is the density,and the pressure P=P（ρ）is a general pressure.The global existence and the uniqueness of weak solution are proved.

In this article, authors study the Cauch problem for a model of hyperbolic-elliptic coupled system derived from the one-dimensional system of the rudiating gas. By considering the initial data as a small disturbances of rarefaction wave of inviscid Burgers equation, the global existence of the solution to the corresponding Cauchy problem and asymptotic stability of rarefaction wave is proved. The analysis is based on a priori estimates and L^2-energy method.
