Effect of Herb-Partitioned Moxibustion on Structure and Functional Prediction of Gut Microbiota in Rats with Irritable Bowel Syndrome with Diarrhea

Objective:The objective of this study was to observe the effect of herb-partitioned moxibustion(HPM)on the gut microbiota of rats with diarrhea-predominant irritable bowel syndrome(IBS-D).Materials and Methods:A total of 48 male rats were randomly divided into a normal control group and an irritable bowel syndrome(IBS)model *** acetic acid irrigation and constraint stress,an IBS-D rat model was *** the model was made,the IBS rats were divided into IBS,HPM group,and pinaverium bromide(PB)*** HPM received HPM for 20 min every day,while the PB was given gastric perfusion once a day for 14 *** modeling and treatment,the abdominal withdrawal reflex,fecal character score,and fecal water content of rats were scored,and a 16S rRNA sequencing analysis was performed on the gut ***:After treatment,the fecal character score and fecal water content in the HPM increased significantly,while visceral sensitivity *** of 16S rDNA sequencing revealed that α-diversity was reduced in the IBS,and HPM could increase the diversity of *** flora structure of IBS-D rats *** can increase the abundance of probiotics such as Akkermansia and reduce the abundance of opportunistic pathogens such as Bacteroides and *** prediction analysis showed that the HPM was mainly related to the bacillary secret system,tricarboxylic acid cycle,and other ***:HPM can regulate the gut microbiota of rats with IBS-D.

acupotomy alleviates knee osteoarthritis in rabbit by regulating chondrocyte mitophagy via Pink1-Parkin pathway

OBJECTIVE: To investigate the effect of acupotomy, on mitophagy and the Pink1-Parkin pathway in chondrocytes from rabbits with knee osteoarthritis(KOA).METHODS: A KOA model was established via the modified Videman method. Rabbits were randomly divided into a control group(CON), KOA group and KOA + acupotomy group(Acu). Rabbits in the acupotomy group were subjected to acupotomy for 4 weeks after model establishment. The behavior of the rabbits before and after intervention was recorded. Cartilage degeneration was evaluated by optical microscopy and fluorescence microscopy. The level of mitophagy was evaluated by transmission electron microscopy,immunofluorescence and enzyme-linked immunosorbent assay(ELISA). The expression of phosphatase and tensin homolog(PTEN)-induced kinase 1(Pink1)-Parkin mitophagy pathway components was evaluated by immunofluorescence, Western blotting and real-time polymerase chain ***: In rabbits with KOA, joint pain, mobility disorders and cartilage degeneration were observed, the Mankin score was increased, collagen type Ⅱ(Col-Ⅱ)expression was significantly decreased, mitophagy was inhibited, mitochondrial function was impaired, and factors associated with the Pink1-Parkin pathway were inhibited. acupotomy regulated the expression of Pink1-Parkin pathway-related proteins, the mitophagy-related protein microtubule-associated protein-1 light chain-3,the translocase of the outer membrane, and the inner mitochondrial membrane 23; increased the colocalization of mitochondria and autophagosomes; promoted the removal of damaged mitochondria;restored mitochondrial adenosine-triphosphate(ATP) production;and alleviated cartilage degeneration in rabbits with ***: acupotomy played a role in alleviating KOA in rabbits by activating mitophagy in chondrocytes via the regulation of proteins that are related to the Pink1-Parkin pathway.

Effect and safety of acupotomy in treatment of knee osteoarthritis:a systematic review and Meta-analysis

OBJECTIVE: To evaluate the clinical efficacy and safety of acupotomy in treatment of knee osteoarthritis(OA).METHODS: Extensive literature searches were carried out in PubMed, EMBASE, Cochrane Library(Issue 5, 2017), Chinese Biomedical Literature Database, China National Knowledge Infrastructure Database, China Science and Technology Journal Database and Wanfang Database. All databases were retrieved from their inception until May 31, *** controlled trials incorporating acupotomy versus intra-articular sodium hyaluronate for knee osteoarthritis were included. According to Cochrane Reviews’ Handbook(5.2), two reviewers screened each article and extracted data independently and were blinded to the findings of each reviewer. Meta-analysis was performed by the Cochrane Collaboration’s RevMan 5.3 ***: We identified 12 studies involving 1150 patients aged between 40 and 78 years old. The pooled analysis indicated that acupotomy showed a significant improvement for short-term effect[cure rate: odds ratio(OR) = 2.04, 95% confidence interval(CI)(1.46, 2.85), P acupotomy group and control group in adverse events [OR = 2.13, 95% CI(0.14, 32.28), P = 0.58].CONCLUSION: acupotomy is a safe and effective treatment for KOA. However, due to the methodological deficiency of the included studies, well-designed randomized controlled trials are required to further confirm the findings.

Surgical treatment and ESI (epidural steroid injection) are widely used forms of treatment for cervical radiculopathy but they are controversial and burdensome for patients. To relief pain fast without side effects, we devised a new minimally invasive treatment method that widens the facet joints to decompress nerve roots and release the muscle spasm in cervical radiculopathy with acupuncture needles with blunt tip and mini-scalpel, and named it modified acupuncture procedure. MAP (Modified acupuncture procedure) was administered for 37 patients (mean age = 53.1 years, follow-up = 14.2 months) with cervical radiculopathy who did not recover from 4 weeks of nonsurgical treatment. We analyzed clinical outcomes of patients before and after the procedure through VAS (Visual Analogue Scale) and NDI (Neck Disability Index). On average, patients received 1.4 MAP (modified acupuncture procedures). The VAS score difference on the day after procedure and at 1 year follow-up was 36.8 ± 26.5 (from 60.1 ± 25.3 at the baseline to 25.3 ± 17.8 at the reading) (P < 0.01) and 31.0 ± 30.4 (29.0 ± 21.8 at the reading) respectively. The NDI value dropped by 19.9 ± 18.3 (from 37.2 ± 19.7 at the baseline to 17.2 ± 15.0 at the reading) (P < 0.01) on 1 year follow up. MAP was found to have clinical efficacy for cervical radiculopathy.

Effects of acupotomy on the activity of osteoclasts and osteoblasts in the subchondral bone of rabbits with early and mid-stage knee osteoarthritis models

Effect of acupotomy on nitric oxide synthase and beta-endorphin in third lumbar vertebrae transverse process syndrome model rats

OBJECTIVE: To explore the long-term effects and pain relief mechanism of acupotomy by observing changes in nitric oxide synthase(NOS) and beta-endorphin(β-EP) in the hypothalamus, spinal cord,and peripheral blood of rats with third lumbar vertebrae(L3) transverse process ***:Twenty-eight SD rats were randomly assigned to normal, model, electroacupuncture(EA),and acupotomy group. The last three groups were put through an operation to emulate L3transverse process syndrome. Fourteen days after the simulation operation, EA and acupotomy treatments were applied to the respective groups. Fifty-six days afterthe simulation operation, biochemistry tests and enzyme-linked immunosorbent assay were used to measure NOS and β-EP in the hypothalamus, spinal cord, and peripheral ***: Rats with the simulation operation showed significantly higher levels of NOS and β-EP in the hypothalamus, spinal cord, and peripheral blood than those in the normal group. The EA and acupotomy groups had significantly lower levels of NOS and β-EP than those in the model *** was no statistical difference between the EA and acupotomy ***: EA and acupotomy treatments significantly lowered NOS and β-EP levels in the hypothalamus, spinal cord, and peripheral blood and alleviated L3transverse process syndrome.

Clinical Efficacy and Safety of Miniscalpel-Needle Treatment for Tension-Type Headache:A Systematic Review and Meta-Analysis

Objective:To investigate the clinical efficacy and safety of miniscalpel-needle(MSN)treatment for tension-type headache(TTH).Method:Seven medical databases were searched to identify randomized controlled trials(RCTs)evaluating the effect and safety of MSN *** articles published up to November 15,2018 were retrieved.A meta-analysis was conducted for the included studies,and the risk of bias was *** outcomes were visual analogue scale(VAS)or numeric rating scale(NRS)*** outcomes were clinical effective rates including total effective rate(TER),markedly effective rate(MER),and totally cured rate(TCR)determined by improvement in clinical symptoms or VAS scores,the frequency of adverse events(AEs)that occurred during the study,and participant quality of life(QOL).Results:Seven RCTs involving 724 participants were *** treatment showed significantly higher MER and TCR[relative risk(RR)1.27,95%confidence interval(Cl)1.01 to 1.61;RR 1.31,95%Cl 1.09 to 1.57,respectively],but not TER(RR 1.03,95%Cl 0.96 to 1.10)compared to *** treatment plus conventional treatment showed significant lower VAS and higher TER,MER,and TCR(mean difference-3.54,95%Cl-3.80 to-3.28;RR 1.14,95%Cl 1.06 to 1.23;RR 2.31,95%Cl 1.50 to 3.58;RR 3.01,95%Cl 2.25 to 4.02,respectively)compared to conventional ***:According to current evidence,MSN treatment as a monotherapy or as an adjunctive treatment to other existing treatments might have benefits on treating ***,since the number and the sample size of studies included were both small and the methodological quality was poor,the findings of this review should be interpreted with great caution,and our confidence in the results is low.A high quality RCT using objective outcomes should be performed on this topic.
