Oxcarbazepine oral suspension in young pediatric patients with partial seizures and/or generalized tonic-clonic seizures in routine clinical practice in China: a prospective observational study

Background This study aimed to assess efficacy and safety of oxcarbazepine (OXC) oral suspension in pediatric patients aged 2-5 years with partial seizures (PS) and/or generalized tonic-clonic seizures (GTCS) in real-world clinical practice in China. Methods This 26-week, prospective, single-arm, multicenter, observational study recruited pediatric patients aged 2-5 years with PS or GTCS suitable for OXC oral suspension treatment based on physicians' judgments from 11 medical centers in China. Enrolled subjects started OXC oral suspension treatment as monotherapy or in combination with other antiepileptic drugs. Primary efficacy outcome was the percentage of pediatric subjects achieving ≥ 50% seizure frequency reduction at the end of the 26-week treatment. Secondary efficacy-related parameters and safety parameters such as adverse events (AEs) and serious AEs (SAEs) were also monitored during the 26-week treatment period. Results Six hundred and six pediatric patients were enrolled and 531 (87.6%) completed the study. After 26 weeks of treat-ment, 93.3% subjects achieved ≥ 50% seizure frequency reduction, and 81.8% achieved 100% seizure frequency reduction compared to baseline. Among diff erent seizure types, OXC was eff ective in all subjects with simple PS and in > 90% of subject with other type of seizure present in the study. AEs were observed in 49 (8.1%) subjects. Only three subjects expe-rienced SAE. Rash (n = 18, 2.97%) was the most common AE. Only 17 subjects discontinued due to AEs. Conclusion This study, reporting the real-world data, further confi rms the efficacy and good safety profi le of OXC oral suspension in Chinese pediatric patients aged 2-5 years with PS and/or GTCS.

Autism is a neurodevelopmental disorder characterized by impaired communicati on and social interaction and may be accompanied by mental retardation and epile psy. Its cause remains unknown, despite evidence that genetic, environmental, an d immunological factors may play a role in its pathogenesis. To investigate whet her immune mediated mechanisms are involved in the pathogenesis of autism, we used immunocytochemistry, cytokine protein arrays, and enzyme linked immunosor bent assays to study brain tissues and cerebrospinal fluid (CSF) from autistic p atients and determined the magnitude of neuroglial and inflammatory reactions an d their cytokine expression profiles. Brain tissues from cerebellum, midfrontal, and cingulate gyrus obtained at autopsy from 11 patients with autism were used for morphological studies. Fresh frozen tissues available from seven patients and CSF from six living autistic patients were used for cytokine protein profili ng. We demonstrate an active neuroinflammatory process in the cerebral cortex, w hite matter, and notably in cerebellum of autistic patients. Immunocytochemical studies showed marked activation of microglia and astroglia, and cytokine profil ing indicated that macrophage chemoattractant protein (MCP) 1 and tumor growth factor β 1, derived from neuroglia, were the most prevalent cytokines in bra in tissues. CSF showed a unique proinflammatory profile of cytokines, including a marked increase in MCP 1. Our findings indicate that innate neuroimmune reac tions play a pathogenic role in an undefined proportion of autistic patients, su ggesting that future therapies might involve modifying neuroglial responses in t he brain.

Background: Evidence does not support the view that Parkinson disease (PD) rep resents an accelerated aging process; however, the additional contribution of ag ing to the severity of different motor signs in patients with PD is not known. T his knowledge may have implications for clinical trials of neuroprotective agent s in PD. Objective: To investigate the contribution of aging to the severity of the different motor signs of idiopathic PD. Setting: Center for Parkinson Diseas e and Other Movement Disorders of the Columbia University Medical Center and a n eurology clinic that primarily served individuals from the Washington Heights-I nwood community in New York City. Patients: Sample of patients with a wide range of disease duration and age. Design: Cross-sectional clinic-based study. Pati ents with PD were evaluated using the Unified Parkinson Disease Rating Scale (UP DRS). The total UPDRS motor score was divided into 6 motor domains (tremor, rigi dity, bradykinesia, facial expression, speech, and axial impairment) and 2 subsc ores that represented predominantly dopaminergic (subscore A: tremor, rigidity, bradykinesia, and facial expression) and nondopaminergic (subscore B: speech and axial impairment) deficiency. Analyses were performed using linear regression models with the UPDRS motor domains and subscores as the outcomes. The variation (adjusted R2)of the outcome variables explained by the inclusion o f disease duration in the models, adjusting for sex, years of education, levodop a dosage, and use of other antiparkinsonian medications, was calculated. The add itional variation explained by adding age at examination to the models was used to gauge the contribution of aging to each motor domain and subscore of the UPDR S. Results: A total of 451 patients participated in the study.
