The positive relationship between smoking and major general health diseases such as cancer, cardiovascular disease, pulmonary disease and negative pregnancy outcomes is already established. Moreover, the association between smoking as a major environmental risk factor for periodontal disease has also been established. Smoking is considered to have negative adverse effect on periodontal therapy ranging from non-surgical treatment, periodontal surgery, regenerative procedures and implants. This review article will attempt to build on previous studies in the periodontal literature in order to present an evidence-based comprehensive literature review on the effects of smoking on periodontal therapy.

Comparison of microRNA profiles of human periodontal diseased and healthy gingival tissues

MicroRNAs （miRNAs） have been clemonstrated to play an important role in regulation of the immunoinflammatory response; however, the function of miRNAs in periodontal inflammation has not been investigated. The objective of this study was to explore the properties of miRNAs in periodontal inflammation by comparing miRNA profiles of inflamed and healthy gingival tissues. Gingival tissues were obtained from 10 periodontitis patients and 10 healthy subjects. After RNA extraction, miRNA profiles were analyzed by microarray, and expression levels of selected miRNAs were confirmed by real-time quantitative reverse transcription polymerase chain reaction （RT-PCR）. Analyses using two computational methods, Targetscan and ***, were combined to identify common targets of these miRNAs. Finally, the individual miRNA expression levels of three toll-like receptor （TLR）-related miRNAs from inflamed and healthy gingival tissues were evaluated by RT-PCR. Ninety-one miRNAs were found to be upregulated and thirty-four downregulated over two-fold in inflamed gingival tissue compared with those in healthy gingival tissue. Twelve selected inflammatory-related miRNAs, hsa-miR-126＊, hsa-miR-20a, hsa-miR-142-3p, hsa-miR-19a, hsa-let-7f, hsa-miR-203, hsa-miR-17, hsa-miR-223, hsa-miR-146b, hsa-miR-146a, hsa-miR-155, and hsa-miR-205 showed comparable expression levels by microarray and real-time quantitative RT-PCR analyses. In addition, the putative inflammation targets of these miRNAs were predicted, and three that were tested （hsa-miRNA-146a, hsa-miRNA-146b, and hsa-miRNA-155）, showed significant differences between inflamed and healthy gingiva. This remarkable difference in miRNA profiles between periodontal diseased and healthy gingiva implicates a probable close relationship between miRNAs and periodontal inflammation. The data also suggest that the regulation of TLRs in periodontal inflammation may involve miRNA pathways.

Microarray Analysis of the Effects of Amelogenin on U937 Monocytic Cells

Periodontal diseases are chronic inflammation caused by particular types of bacteria and have been recognized as a cause of tooth loss in adults. These bacteria which invade periodontal tissue are phagocytosed mainly by monocytes and macrophages in this immune response, and will be presented to lymphocytes. Recently, therapies for regenerating periodontal tissues have been used extensively to treat periodontal disease, and in particular, enamel matrix derivative (EMD) is commonly used for such therapies in Japan. Amelogenin is a type of the extracellular matrix protein that accounts for 90% of the constituents of EMD. In this study, we carried out a detailed microarray analysis in order to evaluate a gene group involved in amelogenin stimuli in the human monocytic cell line U-937. Microarray analysis revealed that statistically significant changes were apparent in 273 genes (163 up-regulated and 110 down-regulated) subsequent to 4 h of amelogenin stimulation. The most highly enriched categories included “cell cycle”, “DNA replication”, and “DNA repair” in up-regulated annotation terms. On the other hand, “type I diabetes mellitus”, “allograft rejection”, and “graft versus host disease” were observed in down-regulated annotation terms. Specifically, the gene expression of major to compatibility complex (MHC) class I/II and CD80/86 was impaired in U937 cells after stimulation with amelogenin. In addition, the results of heat-map showed that the gene expression of inflammatory cytokine such as tumor necrosis factor (TFN), interleukin-18 (IL-18), and CXCL16 was markedly decreased after stimulation of monocytes with amelogenin. In conclusion, the findings of our study showed that by inducing monocyte growth through the suppression of the antigen-presenting ability of U937 cells, amelogenin may affect the immune responses of periodontal tissues originating from monocytes. The Effects of Different Titanium Surfaces on the Behaviour of Osteoblast-Like Cells

This study investigated the influence of different titanium surfaces on the differentiation of rat osteoblast-like cells (osteo-1). Osteo-1 cells were cultured on the following titanium surfaces: 1) pretreated, smooth surface (PT);2) sandblasted and acid etched surface (SLA);and 3) sandblasted and acid-etched surface rinsed under nitrogen protection to prevent exposure to air and preserved in isotonic saline solution (modSLA). Cell metabolism, total protein content, collagen content and alkaline phosphatase (AP) activity and the formation of calcified nodules were analyzed. The titanium surface did not influence cell metabolism, total protein content and collagen content. The SLA surface influenced cell differentiation, with the observation of a significant reduction of AP activity and formation of calcified nodules after 21 days compared to the PT surface. No difference was observed between the PT and modSLA surfaces. All titanium surfaces tested permitted the full expression of the osteoblast phenotype by osteo-1 cells, including matrix mineralization.

Human dental pulp stem cells(hDPSCs)are easily obtained multipotent cells,however,their potential value in regenerative medicine is hindered by the phenotypic and functional changes after conventional monolayer ***,we employed single-cell RNA sequencing(scRNA-seq)to comprehensively study the transcriptional difference between the freshly isolated and monolayer cultured *** cell cluster analysis based on our scRNA-seq data showed that monolayer culture resulted in a significant cellular composition switch compared to the freshly isolated ***,one subpopulation,characterized as MCAM(+)JAG(+)PDGFRA(−),maintained the most transcriptional characteristics compared to their freshly isolated ***,immunofluorescent staining revealed that the MCAM(+)JAG(+)PDGFRA(−)hDPSCs uniquely located in the perivascular region of human dental pulp ***-cytometry analysis confirmed that their proportion remained relatively stable(~2%)regardless of physiological senescence or dental *** with the annotation of scRNA-seq data,MCAM(+)JAG(+)PDGFRA(−)hDPSCs showed higher proliferation capacity and enhanced in vitro multilineage differentiation potentials(osteogenic,chondrogenic and adipogenic)compared with their counterparts PDGFRA(+)***,the MCAM(+)JAG(+)PDGFRA(−)hDPSCs showed enhanced bone tissue formation and adipose tissue formation after 4-week subcutaneous implantation in nude *** together,our study for the first time revealed the cellular composition switch of monolayer cultured hDPSCs compared to the freshly isolated *** in vitro expansion,the MCAM(+)JAG(+)PDGFRA(−)subpopulation resembled the most transcriptional characteristics of fresh hDPSCs which may be beneficial for further tissue regeneration applications.

The direct link between smoking, lung diseases, cancer, cardiovascular disease and poor pregnancy outcomes, such as miscarriage and low birth weight, has been extensively studied. In the past three decades, there has also been an increasing awareness of the impact of smoking on periodontal disease. Smoking is considered the major environmental risk factor in the prevalence, extent and severity of periodontal disease. This review article will attempt to build on previous studies in the periodontal literature in order to present an evidence-based comprehensive literature review on the effects of smoking on periodontal disease.

Periodontal ligament stem cells(PDLSCs) are a key cell type for restoring/regenerating lost/damaged periodontal tissues, including alveolar bone, periodontal ligament and root cementum, the latter of which is important for regaining tooth function. However,PDLSCs residing in an inflammatory environment generally exhibit compromised functions, as demonstrated by an impaired ability to differentiate into cementoblasts, which are responsible for regrowing the cementum. This study investigated the role of mitochondrial function and downstream long noncoding RNAs(lnc RNAs) in regulating inflammation-induced changes in the cementogenesis of PDLSCs. We found that the inflammatory cytokine-induced impairment of the cementogenesis of PDLSCs was closely correlated with their mitochondrial function, and lnc RNA microarray analysis and gain/loss-of-function studies identified GACAT2 as a regulator of the cellular events involved in inflammation-mediated mitochondrial function and ***, a comprehensive identification of RNA-binding proteins by mass spectrometry(Ch IRP-MS) and parallel reaction monitoring(PRM) assays revealed that GACAT2 could directly bind to pyruvate kinase M1/2(PKM1/2), a protein correlated with mitochondrial function. Further functional studies demonstrated that GACAT2 overexpression increased the cellular protein expression of PKM1/2, the PKM2 tetramer and phosphorylated PKM2, which led to enhanced pyruvate kinase(PK) activity and increased translocation of PKM2 into mitochondria. We then found that GACAT2 overexpression could reverse the damage to mitochondrial function and cementoblastic differentiation of PDLSCs induced by inflammation and that this effect could be abolished by PKM1/2 knockdown. Our data indicated that by binding to PKM1/2 proteins, the lnc RNA GACAT2 plays a critical role in regulating mitochondrial function and cementogenesis in an inflammatory environment.

Replacing Missing Teeth with Dental Implants in Pubescent Patients—A Case Report

Tooth loss due to traumatic dental injury or congenital absence can cause functional and social- psychological consequences in youth. Pubescent children with missing teeth are often targets for school bullying. The treatment modality chosen can impact their well-being during their formative years. Despite the high success rate in adult patients, implant placement in young patients is not common due to its ankylosed nature and concerns with possible infra-occlusion in the future. However, skeletal growth and remodeling is a continuous process throughout life and postponement of dental implant placement does not necessary prevent future complication or need for replacement. Dental implant placement should be considered as a viable treatment option for pubescent patients if all other conventional alternatives fail to alleviate patients’ concerns both functionally and psychologically. This case report evaluates the considerations required to place dental implants in pubescent patients, as well as its advantages and disadvantages.
