Total Hip Replacement: Short- and Medium-Term Morbidity and Mortality in the department of orthopedic Traumatology of Donka University Hospital, Guinea-Conakry

Introduction: Total hip prostheses are a reliable means of treating hip disorders. It is indicated when pain and reduced mobility of the hip become incompatible with the patient’s daily activities. In low-income countries, the risk factors for hip disease are numerous, but the means for its management, such as total hip prostheses, are not. The aim of this work was to determine the morbi-mortality of total hip prostheses in the short and medium term in our department and to review the literature. Patients and Method: This was a prospective and descriptive study over a period of 9 years. It involved 50 THPs performed on 45 patients. Patients who had received a THP and were followed up for at least two years were included. We excluded patients who were lost to follow-up. The Moore-type posterolateral minimally invasive approach was used. Complications were investigated from the immediate postoperative period and in the medium term. The final functional results were evaluated according to the Postel Merle d’Aubigné score. Results: We performed 50 THPs out of 750 procedures, i.e. , 6.6%. The indications were: primary coxarthrosis 60%, necrosis of the femoral head 26%, post-infectious balloon hip 4%, and pseudarthrosis of the femoral neck 10%. The average time to the consultation was 2.4 years with extremes of 1 and 5 years. According to the terrain, sickle cell disease represented 18%, tuberculosis 12%. Postoperative complications were lameness 12%, dislocation 6%, suppuration 6%, death 4%, peri-prosthetic fracture 8%, loosening 2%, peri-articular ossification 2%, and paralysis of the external popliteal nerve 2%. Conclusion: Whoever performs a THP is obliged to monitor the patient as long as he/she is alive. Complications are possible at any time and can negatively change the assigned goal and force the surgeon to be expensive and sometimes inconclusive secondary interventions.

Regulated cell death is a form of cell death that is actively controlled by *** studies have shown that regulated cell death plays a key role after spinal cord *** and ferroptosis are newly discovered types of regulated cell deaths that have been shown to exacerbate inflammation and lead to cell death in damaged spinal ***,a complex form of cell death that is interconnected with various regulated cell death mechanisms,has garnered significant attention in the study of spinal cord *** injury triggers not only cell death but also cellular survival *** signaling pathways play pivotal roles in influencing the processes of both deterioration and repair in spinal cord injury by regulating pyroptosis,ferroptosis,and ***,this review aims to comprehensively examine the mechanisms underlying regulated cell deaths,the signaling pathways that modulate these mechanisms,and the potential therapeutic targets for spinal cord *** analysis suggests that targeting the common regulatory signaling pathways of different regulated cell deaths could be a promising strategy to promote cell survival and enhance the repair of spinal cord ***,a holistic approach that incorporates multiple regulated cell deaths and their regulatory pathways presents a promising multi-target therapeutic strategy for the management of spinal cord injury.

Evaluation of Intraoperative Iatrogenic Lesions and Postoperative Complications in 1140 Patients Treated for Carpal Tunnel Syndrome in the orthopedic Surgery and Traumatology department of the Moulins-Yzeure Hospital Center in France

Introduction: Carpal tunnel syndrome is a more common form of upper limb canal syndrome, resulting from compression of the median nerve in the carpal tunnel, but is particularly troublesome. Medical treatment is often unsuccessful, and surgical treatment usually involves transection of the annular ligament. The aim of this study was to assess iatrogenic intraoperative and postoperative complications, as well as patient outcomes following the use of conventional and endoscopic surgery in the surgical management of carpal tunnel syndrome. Hypothesis: Are nerve, vascular and tendon injuries of iatrogenic origin always present in the surgical management of carpal tunnel syndrome, even though this surgery is performed on an outpatient basis? Patients and methods: This retrospective series is composed of 1140 patients, 230 men and 910 women, mean age 58.6 ± 16.4 years, operated on between 2010 and 2020 for carpal tunnel syndrome by conventional surgery and under endoscopy. Medical records, operative reports and consultation letters were consulted. All patients were reviewed regularly at one month post-op until recovery. Results: No nerve, vascular or tendon damage was noted, and at a maximum follow-up of 2 years, 20 patients had recurred, i.e. a 2.51% failure rate. Scar disunion was observed in 0.9%, wound infection in 0.9% and scar fibrosis in 0.9%. 92.98% of patients underwent outpatient surgery, irrespective of the type of anesthesia or surgical technique used. Patients who stayed in hospital for a short time were suffering from carpal tunnel syndrome associated with compression of the ulnar nerve in Guyon’s canal, for which both the median and ulnar nerves were freed during the same operation, under general anaesthetic. All patients were able to return to their previous activity within 30 days of surgery. Conclusion: Intraoperative iatrogenic complications, notably nerve, vascular and tendon lesions, were not identified despite the large sample size. On the other hand

Spinal cord injuries lead to significant loss of motor, sensory, and autonomic functions, presenting major challenges in neural regeneration. Achieving effective therapeutic concentrations at injury sites has been a slow process, partly due to the difficulty of delivering drugs effectively. Nanoparticles, with their targeted delivery capabilities, biocompatibility, and enhanced bioavailability over conventional drugs, are garnering attention for spinal cord injury treatment. This review explores the current mechanisms and shortcomings of existing treatments, highlighting the benefits and progress of nanoparticle-based approaches. We detail nanoparticle delivery methods for spinal cord injury, including local and intravenous injections, oral delivery, and biomaterial-assisted implantation, alongside strategies such as drug loading and surface modification. The discussion extends to how nanoparticles aid in reducing oxidative stress, dampening inflammation, fostering neural regeneration, and promoting angiogenesis. We summarize the use of various types of nanoparticles for treating spinal cord injuries, including metallic, polymeric, protein-based, inorganic non-metallic, and lipid nanoparticles. We also discuss the challenges faced, such as biosafety, effectiveness in humans, precise dosage control, standardization of production and characterization, immune responses, and targeted delivery in vivo. Additionally, we explore future directions, such as improving biosafety, standardizing manufacturing and characterization processes, and advancing human trials. Nanoparticles have shown considerable progress in targeted delivery and enhancing treatment efficacy for spinal cord injuries, presenting significant potential for clinical use and drug development.

Biological Activity and Antibacterial Property of Nano-structured TiO_2 Coating Incorporated with Cu Prepared by Micro-arc Oxidation

Micro-arc oxidized Cu-incorporated TiO2 coatings （Cu-TiO2） were prepared in the Ca, P, Cu-containing electrolyte to obtain an implant material with superior biological activity and antibacterial property. The surface topography, phase, and element composition of the TiO2 and Cu-TiO2 coatings were characterized by scanning electron microscopy （SEM）, X-ray diffraction （XRD）, and energy-dispersive X-ray spectrometry （EDS）, respectively. Staphylococcus aureus （S. aureus） was selected to evaluate the antibacterial property of the Cu-TiO2 coatings, whereas osteoblastic MG63 cells were cultured on the coatings to investigate the biological activity. The obtained results demonstrated that Cu element was successfully incorporated into the porous nano-structured TiO2 coatings, which did not alter apparently the surface topography and phase composition of the coatings as compared to the Cu-free TiO2 coatings. Moreover, the antibacterial studies suggested that the Cu-incorporated TiO2 coatings could significantly inhibit the adhesion of S. aureus. In addition, the in vitro biological evaluation displayed that the adhesion, proliferation and differentiation of MG63 cells on the Cu-incorporated coatings were enhanced as compared to those on the Cu-free coatings and Ti plates. In conclusion, the innovative Cu-incorporated nano-structured TiO2 coatings on Ti substrate with excellent antibacterial property and biological activity are promising candidates for orthopedic implant.

Novel 3D Printed Modular Hemipelvic Prosthesis for Successful Hemipelvic Arthroplasty： A Case Study

In this study, in order to prevent the failure of hemipelvic arthroplasty using a patient-specific Computer-Aided Design （CAD） and Additive Manufacturing （AM） approach, a new design for modular hemipelvic prosthesis was developed. Moreover, the biomechanical properties of the new design were determined. The 3D printed pelvic prosthesis with a sacrum portion that completely matches the back surface of the patient＇s sacrum offers more potential for bone ingrowth between the host bone and prosthesis. The new approach integrated the capabilities of digital medical imaging techniques, CAD and metal AM to realize a modular hemipelvic prosthesis. The patient＇s pelvic Digital Imaging and Communication in Medicine （DICOM） data were imported into Mimics software to construct a digital representative patient model for design of the prosthesis. A physical model was obtained using a Stereolithography （SLA） 3D printer for preoperative planning. The final customized implant was designed by using UG NX 10.0 software. Then a surgically modular hemipelvic prosthesis was fabricated from the Ti6Al4V titanium alloy by electron beam melting technology. The operation was performed according to the preoperative planning. The outcome of the operation was good at the 6-month follow-up. Also, the stress distribution and the relative micromotion revealed positive results based on a finite element model built to detect prosthesis stability. The 3D printed modular hemipelvic prosthesis provided good resolution for the failure of hemipelvic arthroplasty. Personal customization will be important in future surgeries aiming at improving the anatomy and function of the implant.

Mechanical force enhanced bony formation in defect implanted with calcium sulphate cement

To improve the osteogenic property of bone repairing materials and to accelerate bone healing are major tasks in bone biomaterials research. The objective of this study was to investigate if the mechanical force could be used to accelerate bone formation in a bony defect in vivo. The calcium sulfate cement was implanted into the left distal femoral epiphyses surgically in 16 rats. The half of rats were subjected to external mechanical force via treadmill exercise, the exercise started at day 7 postoperatively for 30 consecutive days and at a constant speed 8 m·min-1 for 45 min·day-1, while the rest served as a control. The rats were scanned four times longitudinally after surgery using microcomputed tomography and newly formed bone was evaluated. After sacrificing, the femurs had biomechanical test of three-point bending and histological analysis. The results showed that bone healing under mechanical force were better than the control with residual defect areas of 0.64±0.19 mm2 and 1.78±0.39 mm2（P〈0.001）, and the ultimate loads to failure under mechanical force were 69.56±4.74 N, stronger than the control with ultimate loads to failure of 59.17±7.48 N（P=0.039）. This suggests that the mechanical force might be used to improve new bone formation and potentially offer a clinical strategy to accelerate bone healing.
